1.The determinants of the patient readiness for hospital discharge

As the development of fast-tracking, the patient perception of readiness for hospital discharge is becoming more and more popular in discharge decision making. A lot of research indicates that the patient readiness of discharge can predict the satisfaction, the safety, readmission, death and compliance of the patients. This study is about to review the conception, research significances, measurements, and determinants of the discharge readiness. The primary objective was to analyze the determinants of hospital discharge, according to three aspects of patient factors, hospital factors and social support. This study is expected to offer theoretical support for the improvement of discharge readiness.
2.Analysis and countermeasures of utilization rate of large equipment in medical university

Objective To enhance the utilization rate of large medical equipment in medical university.Methods The running and utilization rates of 36 pieces of large medical equipment were statistically analyzed during the past 5 years in the Scientific Experiment Center of some university,and the histogram was established for the effective machine hours to explore the utilization of large medical equipment.Results The statistical analysis and histogram showed that the utilization rate in the past 5 years was low and idleness occurred in some equipment sometimes.Conclusion Some measures are proposed to enhance the utilization rate of large medical equipment from the aspects of equipment maintenance and calibration,propaganda,new function development,personnel and etc.
3.Efficacy of vitrectomy for vitreous amyloidosis

Background Vitreous amyloidosis is an autosomal dominant hereditary disease,often leads to reduction of visual acuity and affects life quality of patients.Vitrectomy is the main treatment for vitreous amyloidosis,but the analysis of curative efficacy in China is rarely reported.Objective The study was to observe and analyze the efficacy of vitrectomy for vitreous amyloidosis.Methods The clinical data of 11 eyes from 6 patients with vitreous amyloidosis who received vitrectomy from March 2011 to February 2013 in Affiliated Hospital of Guizhou Medical University.The best corrected visual acuity (BCVA),intraocular pressure (IOP),manifestations under the slit lamp microscope,findings of ocular fundus and results of fundus fluorescence angiography (FFA) were observed after surgery.Pathological examination for vitreous samples was carried by Congo red staining.Results Before surgery,the visual acuity was counting fingers for 30 cm in 5 eyes and hand movement for 30 cm in 6 eyes,and the mean IOP was (13.09±2.12)mmHg (1 mmHg=0.133 kPa).Fixed white flocks in vitreous and white membrane on the posterior lens capsule were seen under the slit lamp microscope.The BCVA remained 1.0 in 7 eyes and 0.8 in 4 eyes 1 week,6 months and 2 years after vitrectomy,and the mean IOP was (13.32±2.07),(14.09±2.47),(12.32±1.87)mmHg at 1 week,6 months and 2 years after vitrectomy,respectively.FFA showed a little fluorescein leakage at peripheral retinal vessel in all eyes at 1 week after vitrectomy but the leakage disappeared at 6 months after vitrectomy.The white flocks increased at peripheral retina in 4 eyes at 2 years after vitrectomy,but the BCVA remained the same in the eyes.Pathological examination of vitreous specimens comfirmed the diagnosis of vitreous amyloidosis.Conclusions Vitrectomy is an effective method for the treatment of vitreous amyloidosis,which can effectively improve the visual acuity of patients.Postoperative recurrence of vitreous amyloidosis appear in parts of patients,but the BCVA is unaffected.
4.THE EXPRESSION AND SIGNIFICANCE OF LEPTIN AND LEPTIN RECEPTOR IN RAT SUBMANDIBULAR GLAND

Objective To study the location and significance of leptin and leptin receptor in rat submanibular gland. Methods Submandibular gland from SD rats were observed by hematoxylin-eosin, leptin and leptin receptor immunohistochemical staining. Result The granular convoluted tube and striated duct were leptin and leptin receptor positive. The density of positive cells in granular convoluted tube were much high than those of in striated duct. The acini were negative.Conclusion leptin and leptin receptor were widly expressed in granular convoluted tube and striated duct. These results might indicate that submandibular gland participated in adjusting the balance of energy.;
5.Effects of community nursing interventions on rehabilitation of stroke patients with attention disorders

Objective To study the effects of community nursing interventions on stroke patients with attention disorders.Methods A total of 60 stroke patients fulfilling eligibility were divided randomly into two groups.Their data were collected between December 2010 and February 2012.The intervention group was treated by community nurses while the control group trained by family members.Attention was assessed by simple reaction time (SRT) and number cancellation test (NCT).Modified Barthel index was used to assess the activities of daily living (ADL).Both groups were evaluated before and after a 2-month intervention.Results Before treatment,the control group had(0.71 ± 0.25),(662.8 ± 152.7) and(31 ±8)scores at SRT,NCT and MBI respectively.And the intervention group yielded(0.69 ± 0.19),(652.7 ±131.0) and (30 ± 8)scores at SRT,NCT and MBI respectively.There were no significant differences in average value of SRT,NCT and MBI between training and control groups at pre-treatment (P > 0.05).After 2 months,the control group had (0.68 ± 0.20),(637.4 ± 151.7) and (33 ± 8) scores at SRT,NCT and MBI respectively.And the intervention group yielded (0.56 ± 0.16),(540.3 ± 125.9) and(37 ± 8) scores at SRT,NCT and MBI respectively.Very significant difference existed in attention scale scores at Month 2 (P < 0.01) and the difference in ADL scale scores was also significant (P < 0.05).Conclusion For stroke patients with attention disorders,strengthening community nursing interventions may help to improve attention and boost activity of daily living.
6.Expression of dystrophin associated glycoprotein complex in Duchenne muscular dystrophy at different age groups

Objective To analyze the expression of dystrophin associated glycoprotein complex (DGC) in Duchnne muscular dystrophy (DMD) in different age groups. Methods The confirmed 24 DMD patients were divided into 3 groups according to their chronological age (children whose age between 1 to 3 were assigned to early childhood group; children whose age between 3 to 6 were assigned to preschool group; children whose age between 6 to 13 were assigned to school-age group). Several proteins from sarcolemmal muscle membrane were analyzed by immunohistocbemistry. Results The primary loss of dystrophin may lead to a secondary or completely deficiency of α-SG, β-SG, γ-SG,δ-SG, β-DG and dysferlin from sarcolemmal muscle membrane in different age groups. The expression of nNOS is completely deficient in the 3 groups, while the expression of caveolin-3 increased. Conclusions The expression of dystrophin related DGC proteins from DMD patients in different age groups showed diversed degrees of deficiency or complete deficiency. The deficiency of these proteins may occur during the embryonic period, while the increased expression of eaveolin-3 may have a relation with the regeneration of skeletal muscle fibers.
7.Pathogen Spectrum Distribution in Respiratory Tract Infection: A Retrospective Investigation During Past Twelve Years

OBJECTIVE To investigate the pathogen spectrum distribution in respiratory tract infection during the past twelve years.METHODS The method was retrospectively analyzed.RESULTS A total of 7596 strains in(clinical) specimens were isolated.It showed that the isolating rate of Gram positive cocci,Gram negative bacilli and fungis was 34.56%,48.62% and 10.10%,respectively.The isolating rate of Gram positive cocci was 25.96% between Jan 1991 to Dec 1996,the most commonly isolated bacteria of which were Staphylococcus(aureus),Streptococcus pneumoniae and ?-hemolytic streptococci,with isolating rates being 6.85%,4.84 % and(4.26%),(respectively);the isolating rate of Gram negative bacilli was 63.33%,the most commonly isolated bacteria of which were Pseudomonas aeruginosa,Escherichia coli and Klebsiella pneumoniae,with isolating rates being(8.46%),7.97% and 7.09%.The isolating rate of Gram positive cocci was 37.91% between Jan 1997 to Dec 2002,the most commonly isolated bacteria of which were S.aureus,?-hemolytic streptococci and Str.pneumoniae,with isolating rates being 10.28%,7.02% and 6.68%,respectively;the isolating rate of Gram negative bacilli was 47.64%,the most commonly isolated bacteria of which were P.(aeruginosa),E.coli and K.pneumoniae,with isolating rates being 8.19%,7.07% and 5.21%,respectively.The isolating rate of fungi was 6.06% and 11.43%,respectively during the two stages,the most commonly pathogen of which was Candida albicans.(CONCLUSIONS) The pathogen spectrum distribution in respiratory tract infection during the past twelve years has changed significantly.The isolating rate of Gram positive cocci and fungi,respectively is gradually increasing,but that of Gram negative bacilli is decreasing.

Objective To improve work efficiency and safety of blood transfusion in the battlefield.Methods Polyurethanes material was adopted to construct the insulation layer.Low density polyethylene,glass reinforced plastics and low-temperature bio-chemical method also got involved in.Results The blood warmer was passive,small,easy-to-carry and mobile,whose temperature regulation was realized by the input of cold or warm water.Conclusion Being advanced and easy-to-operate,the blood warmer has a brilliant future.
9.Research and Development of Multifunctional Field Blood-Warming Apparatus

Objective To improve the work efficiency and safety quality of blood transfusion remedy in field battle.Methods Low density polyethylente or glass reinforced plastics was used as raw and processed materials.The polyurethane foam was used as insulation layer.U-shape Mipor bi-pass blood-transfusion tube was used in heating.Results The apparatus is small,easily-carried,flexible and free from power supply.The temperature is regulated by the infusion of cold or hot water.Conclusion Being advanced and convenient,the apparatus has good application value.
10.Correlation analysis between the readiness for hospital discharge and the pain degree in patients with thoracolumbar osteoporotic fractures

Objective To investigate the status of the readiness for hospital discharge and pain degree in inpatients with thoracolumbar osteoporotic fractures, and the correlation between them.Methods A cross-sectional analysis of survey data from a sample of 252 patients with thoracolumbar osteoporotic fractures was conducted in a grade A tertiary hospital in Xi′an from January 1st, 2016 to June 30th, 2016. The status of the readiness for hospital discharge and pain level were investigated through the Readiness for Hospital Discharge Scale and Numerical Rating Scale (NRS) respectively. Pearson correlation coefficient method was used to detect the correlation between the two target factors. Results The average score of readiness for hospital discharge was 7.71±1.55. The mean NRS score decreased from 7.8 ± 0.8 at baseline to 2.7 ± 0.6 before discharge. The scores of each dimensions of readiness for hospital discharge from high to low were expected support, personal status, and coping capacity. There was a negative correlation between discharge readiness and pain degree in patients with thoracolumbar osteoporotic fractures (r =-0.537, P<0.05). Conclusions The status of the readiness for hospital discharge among the patients with thoracolumbar osteoporotic fractures is in a medium to high level before discharge. The pain degree is significantly decreased to a lower level. There is a negative correlation between the readiness for hospital discharge and the pain degree.
