1.Precision stomatology: current status and challenges.

The completion of human genome project and the progress in medical practice have inevitably lead to the development of precision medicine, which is a medical model that proposes the customization of medical care including medical decisions, practices, and/or medical products with patient's genetic background, environmental factors and life behavior being taken into account. The current work proposed precision stomatology for the first time, and by integrating data reported in recent literature, we described the current practice of precision stomatology in multiple disciplines in modem dentistry. The clinical significance of precision stomatology and its future challenges have also been discussed.

2.Non-premature andpremature ST-elevation myocardial infarction (STEMI) patients with multivessel disease: risk factors and clinical features

Objective To investigate the clinical characteristics and risk factors of non-premature STEMI patients underwentprimaryPCI with multivessel disease.Methods Data of clinic and coronary angiographic features were retrospectively compared between group of 371 younger STEMI patients (male age < 55 years,female < 65 years) and group of 662 older STEMI patients.All patients were admitted to hospital from January 2005 to January 2015 and treated with primary PCI.The patients' gender,smoking history,family history of coronary heart disease (CHD),hypertension,type 2 diabetes mellitus,previous myocardial infarction and revascularization,stroke history,serum uric acid,lipids etc.were documented.The comparison of coronary artery disease characteristics and the incidence of adverse events during hospitalization were also carried out between two groups.Results (1) Prevalence of males (88.4% vs.76.9%),smokers (74.9% vs.51.5%),family history of CHD (21.0% vs.9.7%) and levels of diastolic blood pressure,total cholesterol,low density lipoprotein cholesterol (LDL-c),triglycefides,and low cholesterol were significantly higher in the non-prematuregroup than in the premature group (all P < 0.01),while high density lipoprotein cholesterol (HDL-c) was lower in non-prematuregroup (P < 0.01).(2) The incidence of in-hospital events in both groups were low.There was less ventricular tachycardia in the non-premature group (1.5% vs.0.3%) (P<0.05).(3) There were no statistically significant differences in the number of infarct vessels,site ofinfarctbetween two groups.(4) Logistic regression analysis showed that smoking (OR =2.22,95% CI:1.588-3.108) (P < 0.05),family history of CHD (OR=2.12,95%CI:1.431-3.140) (P<0.05),triglyceride concentration (OR=1.971,95%CI:1.475-2.635) (P<0.05),LDL-c (OR=1.193,95%CI:1.008-1.413) (P=0.04) were independent risk factors fornon-premature STEMI withmultivessel disease.Conclusion Smoking,family history of CHD,triglyceride concentration,LDL are main risk factors of younger age STEMI patients with multiple vessel disease;Compared with younger age patients,older age patients during hospitalization are more likely to occur ventricular tachycardia.Regardless of age difference,the characteristics of coronary artery lesions show no significant difference.
3.Surgical treatment of perforation of gastric carcinoma

Objective To study the outcome of surgical therapy of perforation of gastric cancer. Methods The clinical data of 37 cases with perforation of gastric carcinoma which underwent different surgical procedures were analyzed retrospectively. Among them,24 cases underwent partial gastrectomy,8 cases underwent radical mastectomy,4 cases had repair of perforation. 1 case received non-operative therapy. Results The preoperative diagnotic rate was 24.3%,and misdiagonsis rate was 75.7%. The perioperative mortality rate was 5.4%,including 1 case that underwent repair of perforation. The post-operative complication rate of patients undergoing palliative gastrectomy,radical gastric cancer operation and repair of perforation was 41.7%,25.0% and 75.0%,respectively,and the mean survival time was 25 months,29 months and 3.0 months,respectively.The partial gastrectomy and radical mastectomy patients had significantly better outcomes than those who underwent repair of perforation or received non-operative therapy.Conclusions For patients with perforation of gastric cancer with resectable lesions and who are in good general coaditon,a one-stage radical gastric resection or palliative subtotal gastrectomy can be performed.
4.Expression of MMP-2 and TIMP-2 in the Traumatic PVR Retina of SD Rats.

Objective To investigate the expression of MMP-2 and TIMP-2 during the course of traumatic PVR treated with GM6001 and without GM6001,and to explore the potential role of MMP-2 and TIMP-2 during the course of traumatic PVR and to evaluate the effect of GM6001 on traumatic PVR prevention and treatment.Methods 360 SD rats were divided randomly into three groups: normal control group,the traumatic PVR group,the traumatic PVR treated with GM6001 group.The normal control group was intravitreous injected with normal saline.The traumatic PVR group was intravitreous injected with the PRP.The traumatic PVR treated with GM6001 group was intravitreous injected with the PRP and GM6001.The expression of MMP-2 and TIMP-2 were qualitativly and semiquantitativly analyzed with immunohistochemistry on day 1,3,7,14,21 and 28.Results 1.The results of immunohistochemistry showed that the expression of MMP-2,TIMP-2 was mainly located in the photoreceptor cells layer,out plexiform layer,inner plexiform layer and nerve fiber layer.2.The expression of MMP-2 in the normal group and the traumatic PVR treated with GM6001 group was weak at all time.The differences were statistical significance as compared with the normal group and the traumatic PVR treated with GM6001 group(P
5.In vitro study of the anatomy of 80 maxillary first permanent molars

Objective:To study the anatomy of the maxillary first permanent molars. Methods:Macrography was used to observe the morphology of the mesiobuccal root,the degree of abrasion,the morphology of the floor of pulp cavity, the number and type of apical foramen.Vernier was used to measure the length indexes and the distance from the apical foramen to the tip of the root.Clearing technique was used to investigate the root canal systems. Results:26.9% of mesiobuccal root had type III apical foramen,56.25% of mesiobuccal root had two canals,7.5% teeth had three root canals. Conclusions: The maxillary first permanent molar has a high ratio of more than one root canal in mesiobuccal root(63.75%).
6.Detection and identification of human metapneumovirus infection in Shenzhen children

Objective To detect human metapneumovirus (hMPV)in respiratory intection rapidly and perform molecular analysis of hMPV.Methods Seven respiratory tract virus(11 subtypes)were assessed using multiplex PCR technology and flexible Multi-Analyte Profiling(suspension array).Human metapneumovirus was confirmed by using a real.Time reverse ranscriptase CR(RT-PCR)assay followed by sequencing.The cladogram analysis was performed further.Results The virus were detected in 40.2%(19/47)samples collected from clinicsl respiratory tract infections,including 8(42.1%)HRSV,7(36.8%)influenza virus,1(5.3%)parainfluenza virus,1(5.3%)rhinovirus,1(5.3%) coxsackievirus and 1(5.3%)human etapneumovirus infections.This is the first time that hMPV was deteced from clinical samples in Shenzhen.The sequencing of specific fragment of neucleoprotein of hMPV showed this hMPV shares over 98% homology with Beijing strain.Japan strain and Thailand strain.The cladogram analysis showed that they were in the same cluste.Conclusions Human etapneumovirus is a maior cause of children respiratory tract disease. Multiplex PCR technology and nexible Multi-Analyte Profiling were hish sensitive and high-throughput for detection of human metapneumovirus.They axe very robust and applicable in etiology analysis.
7.Effect of dexmedetomidine on quality of intraoperative wake-up test in patients undergoing balloon occlusion test of the internal carotid artery

Objective To evaluate the effect of dexmedetomidine on the quality of intraoperative wake-up test in the patients undergoing balloon occlusion test of the internal carotid artery.Methods Forty-two patients of either sex with intracranial aneurysm,aged 57-78 yr,weighing 53-86 kg,of American Society of Anesthesiologists physical status Ⅱ or Ⅲ,scheduled for elective balloon occlusion test of the internal carotid artery under general anesthesia,were assigned into 2 groups (n =21 each) using a random number table:propofol conbined with remifentanil group (group PR) and dexmedetomidine combined with propofol and remifentanil group (group DPR).In group DPR,dexmedetomidine was intravenously infused over 15 min in a loading dose of 0.5 μg · kg-1 before induction of anesthesia,followed by an infusion of 0.3 μg · kg-1 · h-1 throughout surgery.Propofol and remifentanil were given by target-controlled infusion (TCI) after infusion of the loading dose.The patients were mechanically ventilated after placement of the laryngeal mask airway.Maintenance of anesthesia was as follows:propofol and remifentanil were given by TCI with the target plasma concentrations of 0.5-1.0 μg/ml and 1-3 ng/ml,respectively,in group DPR;propofol and remifentanil were given by TCI with the target plasma concentrations of 3-5 μg/ml and 3-6 ng/ml,respectively,in group PR.Bispectral index (BIS) value was maintained at 40-60.Before wakeup test,propofol infusion was stopped and the target plasma concentration of remifentanil was decreased to 0.5 ng/ml in two groups,and the infusion rate of dexmedetomidine was decreased to 0.1 pg · kg-1 · h-1 in group DPR.The wake-up time was recorded and the wake-up quality was assessed.After admission to the operating room (T0,baseline),at 10 min before wake-up test (T1),immediately after patients were wakened (T2),at 10 min after patients were wakened (T3) and at the end of wake-up test (T4),the mean blood pressure (MAP),heart rate,respiratory rate (RR),SpO2 and BIS values were recorded.The development of intraoperative awareness,emergence time,postoperative agitation,nausea and vomiting,regurgitation and aspiration and severe pain was recorded.Results MAP,heart rate,SpO2 and RR were all within the normal range during wake-up period in two groups.Compared with the baseline at To,MAP was significantly decreased at Ti,3,4 in group PR,and BIS value was decreased at T1-4 in DPR and PR groups (P<0.05).Compared with group PR,MAP was significantly increased at T1.3,BIS value was decreased at T24,the wake-up time was shortened,Ramsay sedation score and wake-up quality were increased,the emergence time was shortened,and the incidence of agitation was deceased (P<0.05),and no significant change was found in verbal rating scale scores assessed after extubation in group DPR (P > 0.05).No cardiovascular events,respiratory depression,intraoperative awareness,postoperative nausea and voniting,regurgitation and aspiration or severe pain was found in two groups.Conclusion Dexmedetomidine can raise the quality of intraoperative wake-up test in the patients undergoing balloon occlusion test of the internal carotid artery.
8.Construction and identification of siRNA expression vectors for silencing Ca~(2+) transport protein gene

Objective:To construct specific small interfering RNA(siRNA) expressing vectors of intracellular Ca2+ transport protein(CaT1)gene and detect its silencing effects.Methods:The hairpin sequences of siRNAs targeting CaT1 gene were designed,synthesized and cloned into pSlincer 3.1-H1 plasmids after annealing.The vectors were then enriched in E.coli.The recombinant pSlincer 3.1-H1 plasmids were identified by restriction endonuclease cutting and DNA sequencing and then transfected into Human Gastric Carcinoma Cell Line BGC-823.The expression of CaT1 mRNA was examined by RT-PCR.Results:The siRNA oligonucleotides of CaT1 were correctly cloned into the pSlincer 3.1-H1 plasmids and confirmed by restriction endonuclease cutting and DNA sequencing.RT-PCR analysis revealed that the expression of CaT1 mRNA in BGC-823 cells transfected with the pSlincer 3.1-H1 constructs of siRNA was significantly decreased compared with that of the negative control and untransfected group.Conclusion:siRNA expression plasmids for silencing Ca2+ transport protein gene are successfully constructed,and they effectively inhibit the CaT1 gene expression.
9.Competency oriented management: a training pattern of elite stomatology student.

The competency oriented management theory and method, which was proved to be an effective management measure, have been put into practice in many areas including high education. This essay will probe into the possibility of introducing the competency oriented management theory into the education of undergraduate stomatology students, analyze and research its feasibility from a theoretical point of view by combing the features of stomatology students and the patterns of educating stomatology students. This essay proposes a new pattern of training elite students of stomatology major with competency as the orientation and summarizes the practical experiences in training such elite students.

Besides, it plays an important part in the cultivation of medical students. In the face of the urgent need of cultivation for comprehensive humanistic quality of oral medical students, West China School of Stomatology, Sichuan University, led in the cultivation method which is oriented by competency, give some suggestions to deal with students' lack of language ability, humanistic concern to patients and aesthetic accomplishment. And it has already achieved a better teaching effect.
