1.A biomechanical comparison of parallel plating and vertical plating for humeral intercondylar comminuted fractures

Objective To compare biomechanical performances of parallel plate fixation and vertical plate fixation for better clinical treatment of humeral intercondylar comminuted fractures (HICF).Methods Models of T-shaped fracture were created using an osteotome at the distal end of humerus in 6 specimens of upper limb in 3 human adult cadavers.The 2 fracture models of left and right humeri in the same cadaver were respectively fixated with parallel plating and vertical plating.On a biomaterial-testing machine,a vertical load was applied onto the fixated distal humerus at a speed of 5 N/s till it reached 2000 N.The loading was repeated 3 times.Vertical compression displacement,intercondylar separating displacement,deformation of medial and lateral condyles and vertical compression stiffness were documented for statistic analyses. Results Under the same load,parallel plating led to significantly less compression displacement,separating displacement and deformation of condyles( P < 0.05) but significantly greater axial compression stiffness( P < 0.05) than vertical plating. Conclusion In biomechanics,parallel plate fixation has better performance than vertical plate fixation for HICF.
2.Effects of riluzole on retinal neovascularization and in vitro cultured human umbilical vein endothelial cells

BACKGROUND: Studies show that riluzole can greatly inhibit protein kinase C-?Ⅱ in mediating vascular endothelial cell growth factor effects on endothelial cells, so we presume it can be used in inhibiting retinal neovascularization. OBJECTIVE: To explore the inhibitory effects of riluzole on the proliferation of human umbilical vein endothelial cells and retinal neovascularization, and investigate potential mechanism of its effects. DESIGN, TIME AND SETTING: Single-sample observation and randomized controlled animal trial were performed at the Animal Laboratory and Basic Study Laboratory of Chongqing University of Medical Sciences from August to December 2007. MATERIALS: Thirty 3-day-old healthy C57BL/6 mice (60 eyes), irrespective of gender, were selected to model proliferative retinopathy induced by hyperoxia. Raw material of riluzole was product of Venturepharm Laboratories Limited, Beijing (No. 060630, content ≥98.5%). METHODS: Human umbilical vein endothelial cells were cultured, and randomly divided into 3 groups. The mice in hyperoxia model group and riluzole group were modeled, and riluzole group was additionally intraperitoneally injected 10 mg/kg per day riluzole; control group was intraperitoneally injected normal saline. MAIN OUTCOME MEASURES: Effect of riluzole on proliferation of endothelial cell proliferation induced by vascular endothelial growth factor was observed using MTT assay; The inhibitory effects of riluzole on retinal neovascularization were evaluated by counting the endotheliocyte nuclei of new vessels beyond the inner limiting membrane in sections with HE staining. The expression of retinal protein kinase C-?Ⅱand vascular endothelial growth factor were detected with SABC immunohistochemistry. RESULTS: The riluzole of 0.1-10 ?mol/L inhibited proliferation of human umbilical vein endothelial cells in a dose-effect dependent manner. TOF-MS has been more and more widely used in clinical and scientific research due to its advantages such as fast and easily operated.Diabetic nephropathy is the most common complications of diabetes.But clinical existing detection meth-ods to some extent have some hysteresis.The applications and developments of TOF-MS coupled with other proteomics technologies provided new ideas and methods for early diagnosis and prognosis of diabetic nephropathy.In this paper,the lat-est research results in nearest years about the application of TOF-MS to diabetic nephropathy will be reviewed.
4.Revision for infected total hip arthroplasty

Objective To discuss operative treatment of the postoperative infection after total hip arthroplasty. Methods 11 cases of infected hip arthroplasty, 6 males and 5 females, with a mean age of 63 years old (from 54 to 71 years), were treated operatively in our department. Their preoperative mean Harris score was 33 (from 25 to 40), and their mean erythrocyte sedimentation rate (ESR) was 61 mm/h (from 34 to 80 mm/h). Bacterial cultures and drug susceptibility tests were carried out before and during operation. One- stage revision was performed for 6 cases, and two- stage revision was performed for 5 cases. Of the 11 cases, 2 were classified as early postoperative infection and 9 as late chronic infection. Preoperative microorganism cultures of joint aspiration or pus in sinus were positive in 8 cases but negative in 3 cases. 5 had staphylococcus epidermidis infection, 2 had escherichia coli infection and 1 had staphylococcus aureus infection. Results No recurrence of infection was found after a mean follow- up period of 17 months (from 12 to 31 months). ESR and blood tests after operation revealed nothing abnormal. 5.Transluminal angioplasty and stenting in treating iliac arterial occlusive disease

Objective To evaluate therapeutic effect of transluminal angioplasty and stenting on arteriosclerosis related iliac arterial occlusive disease. Methods This retrospective study included a total of 61 cases (76 limbs) with iliac arterial atherosclerotic occlusive disease, grading as TASC A (n =29),B (n = 16), C (n = 11) and D (n = 5). Percutaneous interventional reconstruction and stent implantation were carried out in our hospital from December 2002 to December 2008. Results In 61 patients (76 lesions) 63 stents were implanted successfully with the success rate of 93% (71/76). The rate of clinical improvement was 100% among the patients who had primary technical success. The ankle-branchial index (ABI) improved from an average of 0. 33 ± 0. 17 before intervention to 0. 72 ± 0. 20 on the day following intervention (P < 0. 05). All cases were followed up between 6 month and 60 month. One year patency rate in all treated lesions was 90% (92% in the TASC A and B, 84% in the TASC C and D).Three year patency rate in all was 75% (80% in the TASC A and B, 63% in the TASC C and D). Five year patency rate was 72%. Conclusion There is a tendency towards utilizing transluminal angioplasty and stenting to treat iliac arterial occlusive disease as a therapy instead of traditional vascular surgery.

8.Study on the Relationship between Patients with Hepatitis B Viral Loads and Immunoglobulin A,G,M and Complement C3,C4

Objective To investigate the relationship between Hepatitis B patients with different viral loads and immunoglob-ulin A,G,M and complement C3,C4.Methods Firstly,followed by real-time fluorescence quantitative PCR detection 210 cases of hepatitis B patients with HBV-DNA levels,according to 10n copies/ml different viral load detection results,it was divided into 102 ~108 copies/ml of the experimental groups.Then the experimental groups and control group were simulta-neously detected in immunoglobulin A,G,M and complement C3,C4.Analysed the correlation between HBV loads and im-munoglobulin A,G,M and complement C3,C4.Results When the viral loads of hepatitis B patients were 105 ~108 copies/ml,the testing results of IgA,IgG and IgM were both increasing (U =12.43,10.96,6.42,P <0.01),while C3 and C4 were both decreasing (U =8.37,6.0,P <0.01).When the viral loads of hepatitis B patients were 102 ~ 104 copies/ml,only IgA and IgM were increasing (U =2.36,2.04,P <0.05),the other testing results had no statistical significance.Between the test of 7 experimental groups compared with each other,only 104 group and 105 group had significantly changed (IgA and IgM were increasing,C4 was decreasing,U =2.39,2.46,2.09,P <0.05,IgG was increasing,U = 3.25,P <0.01),but between other low viral loads or high viral loads were not significantly differences.Conclusion The different viral loads of hepatitis B patients could cause the different changes of immunoglobulin A,G,M and complement C3,C4,especially in the 4 groups from 105 to 108 copies/ml.Followed by increasing in viral loads,there were immunoglobulin A,G,M increasing and comple-ment C3,C4 decreasing,and also serious impaction on the immune function of organism.There was a positive correlation be-tween viral loads in vivo and immune damages,correlation coefficient (γ =0.967,P <0.01).When the viral loads from 104 to 105 copies/ml,the testing results had changed significantly.It suggest that should control viral loads under 104 copies/ml in the hepatitis B antiviral treatment,so the effect of immune function damage will be the minimum.
9.The prothesis of the immense abdominal wall defect after resection of malignant tumors

Objective To evaluate the repair of the immense abdominal wall defect after resection of malignant tumours occurred primarily in the abdominal wall or that invaded the abdominal wall secondarily. Methods Among the 20 eases, 12 eases were of malignant tumours within abdominal wall (9 eases of rhabdomyosareoma, 3 eases of malignant fibrous histocytoma), 8 eases with the invasion of abdominal wall from malignant turnouts were of retroperitoneal or intraabdominal malignant tumors (3 eases with retroperitoneal malignant tumours, 1 ease of transverse eolon carcinoma, 3 eases with local reeurrenee in abdominal wall of ascending colon carcinoma undergoing fight hemieoleetomy 2~3 years ago, 1 ease with abdominal wall implantation metastasis of renal carcinoma after resection 5 years ago ). Abdominal wall defects left over by resection were repaired with polypropylene and expanded polytetrafluoroethylene (PPePTFE) mesh. The postoperative eomplieation and healing rate were observed. Results Wounds healed by primary intension, with no subeutaneous hydrops, no incision infeetion, dehiscence and hernia, no intestinal adhesion in all eases. The rate of successful repairing for the abdominal defect was 100%. All patients were followed up, the meshes with no rejection, inflammation and incision hernia, no reeurrenee of tumour in the repaired abdominal wall. Conclusion The PP-ePTFE mesh is a favourable repairing material for the abdominal defect after tumour resection, with the advantages of strong tensility, good bioeompatibility.
10.The effect of intravenous administration of gadolinium contrast medium on diffusion-weighted MR of breast

Objective To evaluate the effect of intravenous Gd-DTPA on DWI of breast. Methods DWI was performed before and after Gd-DTPA injection on 25 patients with breast cancer confirmed by core or fine-needle aspiration biopsy. The signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR) and ADC values were measured on DWI when b = 0 and 1000 s/mm2 were selected. A paired t-test and non-parametric test were used to compare the differences between SNR, CNR and ADC values before and after enhancement in breast cancer and normal breast. Results There were no significant differences between the SNR(34. 56 ± 11.34 and 33.60 ± 14. 34 for cancer, 9. 88 ± 3.16 and 10. 42 ± 4. 18 for normal breast)and CNR( 24. 16 ± 9. 05 and 22. 26 ± 10. 05 ) before and after administration of contrast medium in breast cancer and normal breast, and no significant differences between the ADC values before and after administration of contrast medium in breast cancer [(0.96±0. 13) × 10-3 vs (0.95±0. 14) × 10-3 mm2/s] and normal breast[(1.90±0.47) × 10-3 vs (1.91±0.61) × 10-3 mm2/s], either (P =0.20 and 0.97). Conclusion It is possible to repeat DWI acquisition after administration of contrast medium in breast MRI.
