Objective To evaluate the value of clinical application of sedated gastrocolonoscopy,to release distress of patients,improve curative effect.Methods To select 148 patients with gastrointestinal diseases who received gastrocolonoscopy,148 patients were divided into two groups randomly,therapy group 74 patients,control group 74 patients.Results The patients of therapy group had less distress,higher compliance,no adverse reaction.All of patients completed diagnosis successfully.But the patients of control group appeared adverse reactions,including vomiting,restlessness equal.Furthermore 9 patients end manipulate because of this.Conclusion Sedated gastrocolonoscopy versus conventional gastrocolonoscopy had less adverse reactions,higher security,and it should be spread in clinical application.
2.A study on relationship between juvenile transient synovitis and enterovirus

Objective To study the relationship between juvenile transient synovitis of the hip and enterovirus.Methods EV-IgM antibody was detected with ELISA in blood and arthrocentesis. At same time, sample with coxsackievirus B(CVB) IgM positive was tested by serptype.Results IgM antibody was detected in 38.27% in blood of the patients and in 76.19% in arthrocentesis.CVB-IgM was found positive in 16.05% of blood and in 42.86% of arthrocentesis.AdV-IgM was in 11.73% and in 23.81%,respectively.Most of the CVB-IgM positive individuals were infected with CVB3.Conclusion The results of this study suggest that there may be a significant correlation between juvenile transient synovitis and CVB, especially CVB3.

4.Study on the Preparation and Pharmacokinetics of Baicalin Proliposomes in Rats in vivo

OBJECTIVE:To prepare Baicalin proliposomes (PBA) containing sodium deoxycholate (SD) with optimized for-mulation,and to study pharmacokinetics of it in rats in vivo. METHODS:PBA were prepared by spray drying method. Response surface method based on Box-behnken design was adopted to optimize the formulation of PBA with the amount of HSPC,cholester-ol and SD as factors using entrapment efficiency of PBA as index. The particle size,morphology,leakage rate and stability of the optimal PBA were evaluated along with the pharmacokinetics of it(compared to raw materials)in rats after ig administration of 15 mg/kg. RESULTS:The optimal formulation of PBA was that the amounts of baicalin,HSPC,cholesterol and SD were 100,214, 68 and 53 mg,respectively;the predicted and practical values of entrapment efficiency were 86.42% and 84.32%,respectively, and particle size of the optimal PBA was 358.4 nm. The leakage rate of reconstituted liposomes was low and the stability of PBA was good. Compared with baicalin raw material,t1/2,tmax,MRT,cmax and AUC0-t of PBA were all increased significantly(P<0.05 or P<0.01 or P<0.001). CONCLUSIONS:PBA were prepared successfully using the spray drying method. This method is simple and easy,and the optimized formulation is feasible and can improve the oral bioavailability of baicalin.
5.Ultrasound combined with tri-operators and the blood-oxygen functional image technology in diagnosis of breast cancer in young women

Objective To evaluate ultrasound combined with thri-operators and the blood-oxygen functional image (TOBOFI) technology in diagnosis of breast cancer in young women. Methods 221 breast tumor patients under 45 years old were selected to analyze their TOBOFI and ultrasonographic characteristics respectively. Based on pathological result, the two diagnostic methods were compared to determine the accurate rate. Results Of all the 221 patients, 82 (37.1%) were malignant and 139 benign. The sensitivity of TOBOFI and ultrasonography was 90.24% and 83.57% respectively, while the accurate rate of the two was 89.14% and 84 62% respectively. Accurate rate was improved to 93.90 % (77/82) if the two were combined. Conclusion TOBOFI has unique clinical value in diagnosing breast cancer in young women, especially in combination with ultrasound.
6.Application of transesophageal dynamic three-dimensional echocardiography in surgical treatment of mitral valve prolapse

Objective To study transesophageal three dimensional echocardiography in the postoperative effect evaluation of the mitral valve apparatus.Methods Thirty three patients with mitral valve prolapse were examined by three dimensional echocardiography preoperatively or intraoperatively.Among them 16 patients underwent mitral vavle reconstruction and 17 patients underwent mitral vavle replacement,the location and size of mitral valve prolapse(MVP) were observed in two groups.Postoperative three dimensional echocardiography was performed to show the mechanism of residual mitral regurgitation(MR).Results In the group of mitral valve reconstruction,three dimensional echocardiography showed that all of the patients had posterior mitral leaflet prolapse,the lesion involved two segments in 4 cases,the average area of mitral leaflet prolapse was ( 0.80 ? 1.59 ) cm 2.While in the group of mitral valve replacement,only 3 patients with isolated posterior mitral leaflet prolapse,the lesion involved two segments in 13 cases, the average area of mitral leaflet prolapse was ( 1.39 ? 0.91 ) cm 2 (P
7.Quantitative analysis of fetal RhD genotyping with fetal DNA from RhD-negative maternal plasma

Objective To explore the feasibility of fluorescence quantitative PCR(FQ-PCR)in prenatal diagnosis of the fetal RhD genotyping using free DNA from RhD-negative pregnant women.Methods The fetal RhD gene was amplified from 78 RhD-negative pregnant women with single fetus maternal plasma (gestation from 11 to 40 weeks).Rhe existence of fetal DNA was confirmed by amplification ofnine different polymorphic short tandem repeat loci(STR)and sex-determining region Y chromosome(SRY)gene.Exon5,7,10 and intron 4 were amplified by real-time polymerase chain reaction with TaqMan probe.The results of fetal RhD genotyping were evaluated retrospectively by the serologic analysis of infant cord blood.Results Among the 78 specimens,the SRY positive signals were detected from samples of 41 and were all identified male fetal through 8ex observation after newborn infants delivered from the women enrolled.The mean concentration of SRY gene reached(214.7±120.9)eopies/ml.RhD genotyping results of 70 cases were in complete concordance with the resets through serological detection of fetal cord blood after delivery.In addition,5 cases were false-positive.3 cages were considered inconclusive.The coincidence rate was 90%(70/78).From 5 false-positive cases,4 cases were identified as RhDel phenotype by detecting RHDl227A allele gene.The final accuracy rate of FQ-PCR was 95%(74/78)in the fetal RhD genotyping.Conclusion FQ-PCR analysis for noninvasive prenatal of fetal RhD genotyping could be useful in prevention and diagnosis of hemolytic disease of newborn.
8.Effect of systemic lupus erythematosus on right heart function evaluated by parameters recommended by the American Society of Echocardiography

Objective To evaluate the right heart dysfunction in SLE patients with/ without pulmonary arterial hypertension (PAH) using the parameters recommended by the American Society of Echocardiography (ASE),and to examine whether the right heart dysfunction is directly related to elevated pulmonary arterial systolic pressure(PASP) and pulmonary vascular resistance(PVR).Methods Study population composited of 43 patients with SLE.The patients were divided into two groups according to the PASP measured by echocardiography:Group A was 24 patients with PASP ≤35 mm Hg,Group B was 19patients with PASP>35 mm Hg.Twenty-two healthy subjects with age and gender matched were set as control group.Routine transthoracic echocardiography study was performed on all patients and 26 parameters were measured in order to compare the differences of the parameters among the three groups.Results There were significant differences in 17 parameters among the three groups (P <0.05).Tricuspid annular plane systolic excurtion(TAPSE) and E/A ratio of tricuspid flow velocities in SLE patients without PAH significantly decreased compared to control group (P =0.04 and 0.03).There were significant differences in 11 parameters between SLE with and without PAH group (P <0.05).Multivariate logistic regression analysis indicated that after adjustment for age and gender,the patients with elevated PVR associated with a 6.18-fold increase in right ventricular dysfunction compared to the patients with normal PVR (P =0.02).Conclusions The impairment of right ventricular function in SLE patients was directly related to PVR and PASP.Elevation of PVR was an important predictor for right heart dysfunction.
9.An analysis of the causes of death for patients of peripheral arterial disease during the perioperative period

Objective To investigate the causes of death and the influencing factors in patients with peripheral arterial disease(PAD) during the perioperative period and reduce the perioperative mortality.Methods A retrospective analysis of the causes of death was performed for PAD during the perioperative period in our hospital from July 2005 to July 2010. Results Nine patients died preoperatively.The causes of death were multible organ failure in 5,heart failure in 2,sudden cardiac death in 1,and respiratory failure in 1.Six patients died postoperatively.The causes of death were acute renal failure in 2,heart failure in 2,cerebral infarction in 1,and hemorrhagic shock in 1.The causes of death which were related to heart,kidney and lung were in 10,6 and 2,respectively.The causes of death which were directly due to heart failure,sudden cardiac death,renal failure,respiratory failure,cerebral infarction and hemorrhagic shock were in 4,1,2,1,1 and 1,respectively.The main causes of death were multible organ failure,heart failure and acute renal failure,which accounted for 73.33% of all deaths.In this series the overall perioperative mortality was 3.11% (15/483),Operative mortality was 1.47% (6/407).Conclusions The influencing factors and the function of organs were closely correlated with the causes of death. To reduce the perioperative mortality of PAD,it is necessary both to intervene in the influencing factors and to monitor and treat the dysfunction of vital organs.
10.Early predictive values of P-selectin, thrombus precursor protein and D-dimer for portal vein thrombosis after splenectomy in patients with portal hypertension

Objective To investigate the predictive values of P-selectin (P-sel),thrombus precursor protein (TpP) and D-dimer (D-D) in the early diagnosis of portal vein thrombosis (PVT) in patients submitted to splenectomy (or to devascularization).Methods The clinical data of 48 patients with portal hypertension (the PVT group,n=26; and the non-PVT group,n=22) who received operation in our hospital from 2009 to 2011 were retrospectively analyzed.Detecting the P-sel,TpP and D-D levels in the two groups of patients were done on preoperative day 1 and postoperative day 1,3,5,7,14.The SPSS software was used for statistic analysis.Results There were no significant differences on the preoperative day 1 levels of P-sel,TpP and D-D in the two groups (P>0.05).The postoperative day1 levels of the three indicators in the PVT group were significantly higher than the non-PVT group (P<0.05).Receiver operating characteristic (ROC) curves showed the area under curve (AUC) of P-sel was largest (0.893),followed by D-D,and TpP.The combined detection of the 3 indicators was highest,with the AUC up to 0.977.Conclusions Combined detection of P-sel,D-D and TpP levels were useful in the early diagnosis of PVT after splenectomy in patients with portal hypertension.
