1.Treatment of Severe Displaced Supracondylar Fracture of Humerus by Combined Ameliorative External Fixation

【Objective】External fixation for supracondylar fracture of humerus is improved for the effective prevention of cubitus varus.【Methods】Sixty-eight patients with severe displaced supracondylar fracture of humerus was fixed by medial-lateral splint and elastic rubber ring,with elbow fixed in flexion and supination and near the chest by plaster support.【Results】 The results of follow-up showed that 68 patients were clinically cured,and the rate of cubitus varus was 2.9%,which was lower than that by other methods reported in domestic literatures.【Conclusion】This kind of fixation has the advantages of fixing the fracture firmly,keeping good para-position,preventing rotation displacement,counteracting gravitation-induced ulnar deviation and reducing the occurrence of cubitus varus.
2.Epidemiological analysis and countermeasures discussion on imported ma-laria in Zhangjiagang City

Objective To analyze the epidemiological characteristics of imported malaria in Zhangjiagang City. Methods The epidemiological data were collected and retrospectively analyzed for the distribution,cost,and exit-entry mode and port of imported malaria cases in Zhangjiagang City from 2005 to 2015. Results There were 25 imported malaria cases in Zhangjia-gang City from 2005 to 2015,and among them,there were 16 cases of falciparum malaria(64%),6 cases of vivax malaria (24%),and 3 cases of ovale malaria(12%);there was 1 cases of critically ill(4%),there were 8 cases of serious ill(32%) and 16 cases of mild ill(64%). The time of onset was in accordance with the circular distribution. The peak of the incidence of the imported malaria was one month earlier than that of the domestic infection. The seasonal peak was gentle ,and there was also the occurrence in the non-epidemic season in the city. The imported malaria patients were mainly from Africa ,followed by South-east Asia and Oceania. Conclusion The information technology should be applied to improve the key population coverage on the basis of improving the ability of malaria diagnosis and treatment of medical staff and the multi-sector 's cooperation for the im-ported malaria prevention and control in Zhangjiagang City.
3.Role of SLB elements presented at 5 UTR of dengue virus on translation and RNA synthesis

Objective To investigate the effects of SLB elements presented at 5' UTR of dengue virus genome on viral translation and RNA replication.Methods The 5 end of RNA secondary structures of dengue virus genome were predicted using mfold 3.2,and three mutants with different modifications in SLB elements were designed:SLB1,part deletion of the 5' UAR sequence(82-87nt);SLB2,mutation of the sequence on stem(Mut 77G/C);SLB3,mutation of the sequence on stem(M77-78AG/GA),respectively.The mutants described above were constructed by OL-PCR based on DEN-R.luc2A-RP,respectively.Replicon RNAs corresponding to DEN-R.luc2A-RP,DEN-R.luc2AGDD-RP and the 3 mutants mentioned above were in vitro transcribed and equal amounts of RNA were transfected into BHK cells with Lipofectamine 2000.After RNA transfection,the replicons were detected and characterized by RT-PCR,IFA,Renilla Luciferase assay system and real time RT-PCR,respectively.Results It was shown that SLB1 mutant did not significantly affect the translation of the input RNA,but seriously compromised RNA synthesis;SLB2 mutant did not significantly affect the translation of the input RNA either,but its RNA replication was abolished;both the translation and replication of SLB3 mutant were abolished.Conclusion Both the nucleotide sequence and the RNA secondary structure of the second loop and short stem of SLB element are highly conserved.SLB element may play an essential role on viral translation and RNA replication.The findings of present study may set a foundation for elucidating the role of SLB during viral translation and replication.
4.Research status and prospect of SPOP in renal cell carcinoma

Renal cell carcinoma is one of the most common urinary tract cancers, accounting for 90% of renal cancer cases, and the clear cell renal cell carcinoma is its most common subtype. Because of poor response to conventional anti-cancer treatment, metastasis may occur in the early stage of renal cell carcinoma. The high degree of metastasis of renal cell carcinoma determines the necessity and importance of early diagnosis. Although a lot of researches about renal cell carcinoma have been carried out, its underlying mechanisms still remain unclear. Recent studies have shown that spot-type POZ protein (SPOP) can perform biological functions in a cancer type-specific way. Based on the latest findings, this article reviewed the current research status and prospectives of SPOP in renal cancer cells.
5.Quantitative Determination of Saponins in the Root of Panax pseudo-ginseng var.notoginseng by HPLC-ELSD and UV Spectrophotometry

A reverse phase HPLC-ELSD method for the determination of ginsenoside Rg1 in the rootof Panaa pseudo-ginseng var. notoginseng (Burkill) Hoo et Tseng was reported. Chromatographic condi-tions: Shim-pack CLC-ODS column (6.0 mm×150 mm); acetonitrile-water (30: 70) as the mobilephase; Shimadzu LC-6A with SEDEX-55 ELSD detector. The method was found to be simple and accuratewith recovery rate of 100. 50% and RSD= 1.82 %. The established UV spectrophotometric determinationof total saponins in P. pseudo-ginseng var. notoginseng was also tried and gave an accurate result coinci-dental with that of the HPLC results. The recovery rate was 101.50%, and RSD=1. 44%. It seemed thatboth methods can be used reliably for the quality control of P. pseudo-ginseng var. notoginseng.
6.Evaluation the short term effects on serum creatinine concentration in patients with normal renal function, mild and moderate renal insufficiency after intravenous injection of gadopentetate dimeglumine

Objective To explore the effects of gadopentetate dimeglumine injection on renal function. Methods The study included 623 consecutive patients. Their serum creatinine concentrations before and within 3 days after injection of gadopentetate dimeglumine were analyzed. Their eGFR ( estimated glomerular filtration rate) were calculated. Patients were divided into two groups according to their injection dose: group received single dose and group received double dose. Patients in each group were subdivided into three subgroups according to their eGFR: subgroup of normal renal function, subgroup of mild renal insufficiency and subgroup of moderate renal insufficiency. Paried sample t test and group design paried sample Rank Sum test were used for statistical analysis. Results There was no adverse reaction oroccurrence of acute renal insufficiency. The mean level of serum creatinine dropped from (74. 0 ±17. 2) μ mol/L to (71.5 ± 19. 0) μ mol/L (t = 5. 39, P < 0. 05 ) in 623 cases after injection of gadopentetate dimeglumine. However, the mean level of serum creatinine increased from (89.6 ± 12. 2 ) μmol/L to (92. 1 ± 14. 6) μmol/L ( t = 0. 68, P > 0. 05 ) in subgroup of mild renal insufficiency under group received double dose and increased from ( 118. 3 ± 15. 3 ) μmol/L to ( 135.7 ± 8. 5 ) μ mol/L ( t = 2. 02, P < 0. 05 ) in subgroup of moderate renal insufficiency under group received double dose, the mean level of serum creatinine dropped in all other subgroups. Conclusions Single dose gadopentetate dimeglumine is safe for patients with normal renal function and mild, moderate renal insufficiency in short term, but patients with abnormal renal function should be followed up after double dose injection.
7.ADF/cofilins proteinic family constellation in cytoskeletal protein:How does depolymerization occur through combination of ADF/cofilins with actin?

The actin depolymerizing factor/cofilin (ADF/cofilins) are a family of actin-binding proteins expressed in all eukaryotic cells. The ADF/cofilins appear to have multiple functions, and this is reflected in their very complex association with both monomeric and filamentous actin. Phosphorylation by some kinases and other factors such as LIM kinases 1 and 2, TESK 1 and TESK 2 kinase, Insulin, etc, prevents ADF/cofllins from binding actin. The serial researchs of ADF/cofilins are increasingly becoming study hot spots, especially on the relationship between homo-sapiens disease and mechanism of action of ADF/cofilins.Now in this domain wilderness details are still far from clear, such as the mechanism by which actin filaments are depolymerized by ADF/cofilins has been controversial.

9.Effect analysis of three-dimensional and two-dimensional imaging systems in laparoscopic radical resection of rectal cancer

Objective To compare the clinical effect of three-dimensional(3D)and two-dimensional(2D)imaging systems in laparoscopic radical resection of rectal cancer.Methods The retrospective cohort study was adopted.The clinical data of the 97 patients who underwent laparoscopic radical resection of rectal cancer at the Xinmin Branch of the China-Japan Union Hospital of Jilin University between May 2012 and December 2014 were collected.Of 97 patients,47 undergoing 3D laparoscopic radical resection of rectal cancer were allocated into the 3D group and 50 undergoing 2D laparoscopic radical resection of rectal cancer were allocated into the 2D group.The operation followed strictly tumor-free and total mesorectal excision principles.Observation indicators included:(1)surgical situations:operation time,time of deep lymph nodes dissected,volume of intraoperative blood loss,bleeding volume of obturator lymph nodes dissected,number of lymph nodes dissected,postoperative complications and duration of hospital stay.(2)Follow-up situations:follow-up using outpatient examination and telephone interview was performed to detect the survival of patients and tumor recurrence and metastasis up to December 2015.Measurement data with normal distribution were presented as-x±s and comparison between groups was analyzed using the t test.Count data were analyzed using the chi-square test.Results(1)Surgical situations:all the patients underwent successful laparoscopic radical resection of rectal cancer,without conversion to open surgery.Operation time,time of deep lymph nodes dissected,volume of intraoperative blood loss,bleeding volume of obturator lymph nodes dissected,number of lymph nodes dissected and duration of postoperative hospital stay were(134.6±18.5)minutes,(21.2±2.7)minutes,(65±20)mL,(16±3)mL,23.6±3.5,(8.2±2.3)days in the 3D group and(157.4±17.8)minutes,(25.2±2.5)minutes,(89±27)mL,(23±5)mL,20.5±2.8,(9.4±2.1)days in the 2D group,respectively,with statistically significant differencesbetween the 2 groups(t=2.999,3.739,2.327,4.221,2.337,1.274,P<0.05).The postoperative complications were detected in 7 patients in the 3D group and 9 patients in the 2D group,with no statisfically significant difference between the 2 groups(x2=0.170,P>0.05).(2)Follow-up situations:75 of 97 patients were followed up for 12-36 months with a median time of 24 months,including 35 in the 3D group and 40 in the 2D group,and they were tumor-free survival.Conclusion Compared with 2D laparoscopic radical resection of rectal cancer,3D laparoscopic radical resection of rectal cancer can shorten the operation time and reduce tissue damage and intraoperative blood loss,and it is beneficial to deep lymph node dissection.
10.Effects of bupivacaine on short-circuit currents in human alveolar epithelial cells

Aim Bupivacaine is a kind of long-acting amide local anesthetics.This paper aims to explore the effects of bupivacaine on the short-circuit currents in human alveolar epithelial monolayers and study the possible mechanisms.Methods Short-circuit currents were recorded by ussing-chamber setup.Amiloride-sensitive currents were defined as the difference be-tween the total current and the amiloride-resistant cur-rent.ERK1 /2 phosphorylation protein levels were ana-lyzed by Western blot at 0,1 5,30 and 60 min after administration of 1 00 μmol·L -1 bupivacaine.Results Bupivacaine could inhibit the short-circuit currents in H441 monolayers dose-dependently,which could be inhibited by amiloride.Western blot analysis showed that bupivacaine increased the level of ERK1 /2 phos-phorylation.Conclusion These data demonstrate that bupivacaine can reduce the alveolar ion transport by in-hibiting the amiloride-sensitive currents,possibly by the enhancement of ERK1 /2 phosphorylation. The effects of alveolar fluid clearance following application of bupivacaine should be considered clinically when the patient is complicated with lung injury.
