1.Protective mechanism of cardiac stem cells against myocardial apoptosis

BACKGROUND:Myocardial apoptosis can lead to many kinds of heart diseases, but most of the conventional treatments cannot get desired effects. The emergence of cardiac stem cel related theory that subverts the previous view of myocardial cel s provides a new idea for the treatment of a variety of heart diseases.
OBJECTIVE:To investigate the protective mechanism of cardiac stem cel s in myocardial apoptosis.
METHODS:Cardiac stem cel s were isolated from five neonatal rats. Another 20 Wistar rats were selected to make myocardial infarction models, and then model rats were equivalently randomized into observation and control groups. One day after modeling, rats in each group were given injection of cardiac stem cel s or culture solution. Fourteen days after injection, myocardial apoptosis index was calculated and expression of apoptosis-related proteins was detected in both groups.
RESULTS AND CONCLUSION:After continuous 3 days of culture, a completely spread growth in the cardiac tissues was visible, and a smal amount of cel s similar to fibrocytes climbed out from the cardiac tissue sample. These cel s isolated using magnetic bead method were sub-cultured for 5 days, and found to be regrouped again. Compared with the control group, the apoptotic index, Fas and Fasl expression were significantly lower (P<0.05), and Bcl-2 expression was significantly higher in the observation group (P<0.05). Al these findings show that cardiac stem cel transplantation can effectively regulate the expression of apoptosis-related proteins and inhibit the apoptosis of myocardial cel s in rats with myocardial infarction.
2.Revision for infected total hip arthroplasty

Objective To discuss operative treatment of the postoperative infection after total hip arthroplasty. Methods 11 cases of infected hip arthroplasty, 6 males and 5 females, with a mean age of 63 years old (from 54 to 71 years), were treated operatively in our department. Their preoperative mean Harris score was 33 (from 25 to 40), and their mean erythrocyte sedimentation rate (ESR) was 61 mm/h (from 34 to 80 mm/h). Bacterial cultures and drug susceptibility tests were carried out before and during operation. One- stage revision was performed for 6 cases, and two- stage revision was performed for 5 cases. Of the 11 cases, 2 were classified as early postoperative infection and 9 as late chronic infection. Preoperative microorganism cultures of joint aspiration or pus in sinus were positive in 8 cases but negative in 3 cases. 5 had staphylococcus epidermidis infection, 2 had escherichia coli infection and 1 had staphylococcus aureus infection. Results No recurrence of infection was found after a mean follow- up period of 17 months (from 12 to 31 months). ESR and blood tests after operation revealed nothing abnormal. 3.Value of Human Rhinovirus Detected in Cerebrospinal Fluid of Children with Viral Encephalitis

Objective To understand the pathogen of viral encephalitis(VE)in children,and establish a rapid and specific method for detecting human rhinovirus(HRV),and investigate the correlation between HRV and viral encephalitis(VE). Methods 169 CSF specimens were collected from children with convulsions and fever,who were admitted to the pediatric intensive care unit (PICU)of Second Affiliated Hospital of Shantou University Medical College between January 2012 and December 2012.Nested RT-PCR was used to detected HRV in CSF specimens,and the positive PCR products were se-quenced,then analyzed and constructed the phylogenetic tree by software.Results 39 (23.1%)out of 169 samples were HRV positive.Among them,148(87.6%)children were below 5 years old.The detection rate of HRV increases from July to September,and reached its highest point in September.Sequence analyzed showed that the 39 HRV positive specimens inclu-ding 18(46.1%,18/39)positive for HRV-A,7(17.9%,7/39)positive for HRV-B,14(35.9%,14/39)positive for HRV-C. There were 8 out of 28 VE cases were detected in HRV,including 3(50%,3/6)positive for HRV-C.Conclusion HRV could be detected in CSF specimens by nested RT-PCR,including three types of HRV,combined with clinical symptoms con-sidered that HRV may be one of the VE pathogen.
4.Phase Ⅱ Clinical Study of Lianhua Qingwen Capsule for Influenza

Objective To evaluate the therapeutic effect and safety of Lianhua Qingwen Capsules (LHQC) in the treatment of influenza. Methods A multi- center, randomized, double- blind, positive- controlled, parallel group trial was designed. A total of 240 cases, which were diagnosed with influenza and classified as the syndrome of toxic heat invading lung, aged from 18 to 50 years old, with body temperature over 38.0 ℃ and disease course with 48 hours, was recruited. The intent- to- treat infected (ITTI) population was 152 cases , among which 76 were treated with LHQC (4 capsules thrice daily for 3 days) and other 76 with Lingyang Ganmao Capsules (2 capsules thrice daily for 3 days). Results In the ITTI population, the effective rate for subsiding fever was 71.1 % and 53.9 % (P=0.029), and that for relieving TCM symptoms was 73.7 % and 55.3 % (P=0.018) in LHQC group and Lingyang Ganmao Capsules group, respectively. A safety analysis was conducted in 233 cases. No adverse action was found in LHQC group while two adverse events (allergic dermatitis and diarrhea) in Lingyang Ganmao Capsules group. Conclusion Early application of LHQC is effective and safe in treating infuenza.
5.Successful cardioversion of fetal persistent supraventricular tachycardia with intrauterine intramuscular injection of cedilanid under ultrasound guidance: a case report

We report the intrauterine treatment of a case of fetal persistent supraventricular tachycardia complicated by heart failure. The ultrasound findings at 32 +1 weeks of gestation showed fetal tachycardia of 242 beats/min, fetal generalized edema, ascites, and visible pulsation spectrum in the intra-abdominal segment of the umbilical vein. Fetal cardiac arrhythmia persisted after maternal oral digoxin for 46 hours. In view of the continuous deterioration of the fetal condition with the persistent umbilical vein pulsation spectrum and worsening edema, fetal intramuscular injection of cedilanid under ultrasound guidance was performed, resulting in successful cardioversion. The pregnant woman gave birth to a baby girl by cesarean section due to premature rupture of membranes and oligohydramnios at 34 +6 weeks of gestation, with normal functions of heart and other organs. During a follow-up to 8 months, no obvious abnormalities were found.
6.Effects of tirofiban, aspirin and clopidogrel on the incidence of major adverse cardiac events after percutaneous coronary intervention in patients with acute myocardial infarction

Objective To observe the effect of tirofiban,aspirin and Clopidogrel Hydrogen triple antiplatelet in patients with acute myocardial infarction undergoing primary percutaneous coronary intervention and the effect on the incidence of major adverse cardiac events.Methods a total of 60 patients with acute myocardial infarction(AMI)in our hospital from January 2015 to-2016 in December were enrolled in this study.All patients were treated with percutaneous coronary intervention(PCI).The patients were randomly divided into control group(n=30)and observation group(n=30).The control group using clopidogrel hydrogen treatment,observation group were treated with tirofiban,aspirin and Clopidogrel Hydrogen triple antiplatelet therapy,the incidence and clinical effects of the two groups of major adverse cardiac events.Results the two groups before treatment,the platelet aggregation rate and platelet count had no statistically significant difference; no significant observation group after treatment compared 30d platelet count difference; observation group 30d after treatment,the platelet aggregation rate was lower than the control group(P<0.05); the observation group after the treatment of 30d MA-ADP on platelet drug reaction the inhibition rate is lower than the control group(P<0.05); the observation group after treatment of AA drugs 30d platelet inhibition rate,the inhibition rate of ADP,higher than that of the control group(P<0.05); the observation group after treatment,the incidence of major adverse cardiac events was 13.33%,33.33%in the control group,two groups of major adverse cardiac events was statistically significant the difference(P<0.05).Conclusion The patients with acute myocardial infarction undergoing primary percutaneous coronary intervention after treatment with tirofiban,aspirin and chlorine Pigre triple antiplatelet ideal treatment effect,can reduce the incidence of major adverse cardiac events,it is worthy of popularization and application.
7.An analysis of the causes of death for patients of peripheral arterial disease during the perioperative period

Objective To investigate the causes of death and the influencing factors in patients with peripheral arterial disease(PAD) during the perioperative period and reduce the perioperative mortality.Methods A retrospective analysis of the causes of death was performed for PAD during the perioperative period in our hospital from July 2005 to July 2010. Results Nine patients died preoperatively.The causes of death were multible organ failure in 5,heart failure in 2,sudden cardiac death in 1,and respiratory failure in 1.Six patients died postoperatively.The causes of death were acute renal failure in 2,heart failure in 2,cerebral infarction in 1,and hemorrhagic shock in 1.The causes of death which were related to heart,kidney and lung were in 10,6 and 2,respectively.The causes of death which were directly due to heart failure,sudden cardiac death,renal failure,respiratory failure,cerebral infarction and hemorrhagic shock were in 4,1,2,1,1 and 1,respectively.The main causes of death were multible organ failure,heart failure and acute renal failure,which accounted for 73.33% of all deaths.In this series the overall perioperative mortality was 3.11% (15/483),Operative mortality was 1.47% (6/407).Conclusions The influencing factors and the function of organs were closely correlated with the causes of death. To reduce the perioperative mortality of PAD,it is necessary both to intervene in the influencing factors and to monitor and treat the dysfunction of vital organs.
8.Research and implementation of gray-scale blood flow imaging system of high frequency ultrasound

Objective To study the method of gray-scale blood flow imaging and image processing in condition of high frequency ultrasound,and the implementation of the system.Methods On the base of research of scattered signals of red blood cells in high frequency ultrasound,20 MHz ultrasound mechanical and linear scanning probe was used to transmit a number of pulses on a scan line.Pulse-echo subtraction method was used to obtain the blood flow information.At the end,simulated blood vascular was used to conduct flow imaging,and the obtained images were analyzed.Results Experiment results showed that clear blood flow images were obtained using this system.The noise from perivascular tissue could be filtered and the signals from blood flow could be enhanced after image processing.Conclusions In the detection of superficial blood vessel,blood flow signals can be obtained even using single pulse emitting via high frequency ultrasound.The blood flow imaging system can be implemented after image processing.
9.Percutaneous mechanical thrombectomy for the treatment of acute limb ischemia

Objective:To analyze the application value of the mechanical thrombectomy system in the treatment of acute limb ischemia.Methods:The clinical data of 50 patients with lower limb ischemia who were treated with the Rotarex mechanical thrombectomy system from Jun 2017 to Sep 2019 were retrospectively analyzed.Results:In 4 cases of popliteal artery rupture occurred during the operation. The success rate of the operation was 92%. Catheter-directed thrombolysis was used in 7 cases, percutaneous transluminal angioplasty was used in 4 cases and percutaneous transluminal angioplasty combined with stent implantation was used in 39 cases. The ankle-brachial index of these 50 patients before and after operation was 0.18±0.24 and 0.64±0.28 respectively ( t=12.87, P<0.001). Treatment was successful in 43 cases. Follow-up ranged from 1 to 24 months, 5 cases were amputated, 2 cases had no improvement of toe ulcer gangrene, 9 cases had thrombus recurrence, and no complications such as bleeding were observed. The primary patency rates at 3, 6 and 12 months were 92%, 84% and 74%, respectively. Conclusion:The mechanical thrombectomy system is safe and effective in the treatment of acute lower limb ischemia with ideal short-term patency.
10.Effect of shensong yangxin capsule on ventricular premature beat and cardiovascular autonomic nervous function in patients with coronary heart disease.

OBJECTIVETo observe the clinical efficacy of shensong yangxin capsule (SYC) on ventricular premature beat (VPB) differentiated in TCM as palpitation of Qi-yin deficiency syndrome or Xin collateral stagnation syndrome, and cardiovascular autonomic nervous function in patients with coronary heart disease (CHD).
METHODSThe randomized, double-blind, parallel contrast method was adopted, patients were randomly assigned by 3:1 ratio into two groups. One hundred and sixty-five patients in SYC treated group and 56 in the control group (treated with Xinlvning tablet), and the therapeutic course for both groups was 4 weeks.
RESULTSThe clinical efficacy on VPB and in improving TCM syndromes was better in SYC group than that in the control group (P < 0.01). After treatment, the heart rate variability (HRV) and QT dispersion in the two groups were improved in a certain degree. The changes of SDNN, SDANN, SDNN Index and PNN50 in the two groups were significantly different (P < 0.05, P < 0.01), the efficacy in the treated group was superior to that in the control group.
CONCLUSIONSYC has definite effect on VPB and TCM Syndromes, it can obviously meliorate the activity of cardiovascular autonomic nervous system in the patients with CHD.
