1.Status quo and countermeasures analysis of the construction of health personnel in township health centers in Jiangsu Province

Objective To learn the status of health care personnel construction of primary health institutions in Jiangsu province. Methods In July 2015, cluster sampling method was adopted in which 14 areas from Jiangsu province were selected, then one or more township health centers from each area were selected , and a total of 23 hospitals were selected. Personal interview were applied to administrators, the medical staff and residents respectively. A self-made questionnaire was performed in medical personnel from different departments. The method of descriptive statistics was used to collect data of rural health care personnel in primary health institutions. Results A total of 521 questionnaires were collected. In the town hospitals, the average age of the health professional staff was 37 years old. 68.33% (356 people) of the staff had college diploma or technical secondary school diploma and 35.12%(183 people) of them had junior title. There were 10.94%(57 people) and 14.01%(73 people) of the staff had no title and no physician practice qualification respectively. Conclusion Grass-roots health personnel construction task is still formidable. Further efforts are needed to improve the educa-tional system of health talents. And the incentive mechanism should be improved to strengthen the at-traction and stability of grass-roots health personnel. Strengthening education and training for the exist-ing staff is also needed to intensify the ability to primary health care.
2.Effect of Massage on M-Waves and F-Responses from Muscle Flexor Hallucis to Tibial Nerve Stimulation in Children with Cerebral Palsy

M-waves and F-responses were investigated before and after massage using the bipolar surface electrodes to stimulate the tibial nerve at the ankle recorded by bipolar surface electrodes from the muscle flexor hallucis in 30 children with cerebral palsy. After treatment, the amplitudes of the M-wave increased (P<0.05 ); F-response reduced (P<0.05 )in the lesion side. The observations were made bilaterally and the results revealed no significant differences(P>0.05 )by comparing side to side. The latencies of Mwaves prolonged to 6.09~8. 28ms(P<0.05);F-responses 29.07~3l. 42ms(only Fn wave was P<0.05in the lesion side). The significant efficiency was 25.93 %,the efficiency was 88.89 % except that 2 objects did not change and one continued to deteriorate. The experimental results suggest that the massage can improve the functional disorder of the skeletal muscle and the ventral horn of the spinal cord.
3.Analysis of distribution and antimicrobial resistance of enteric pathogens among children

Objective To investigate the distribution,epidemiologic features and antibiotic resistance of the enteric pathogens i-solated from children with diarrhea.Methods Enteric pathogens were isolated from children’s stool samples.The children with diarrhea were treated in the outpatient and inpatient of Shanghai Children’s Hospital between 2008 and 2013.Antimicrobial susceptibility testing was conducted by disk diffusion method for Salmonella and Shigella with 6 antimicrobial agents.Results A total of 545 enteric pathogens were collected.Salmonella was the dominant pathogen,accounting for 67.2%,followed by Shigella (20.7%),S.aureus (4.6%),C.jejuni (3.7%),Aeromonas (2.4%),and enteropathogenic E.coli (0.9%).The main serotypes of Salmonella were S.typhimurium and S.enteritidis.Approximately 56.3% of the patients were boys.A-bout 72.7% of the patients were infants under 2 years.The prevalence of diarrhea peaked in summer and autumn (72.9%). The susceptibility of these isolates was only tested with seven antibiotics.Shigella showed higher level of resistance to ampicil-lin and trimethoprim-sulfamethoxazole than Salmonella (P<0.05).Significantly higher percentage of S.flexneri isolates were resistant to sulbactam-ampicillin,ceftriaxone,ciprofloxacin,and chloramphenicol than S.sonnei (P<0.001).Further-more,the prevalence of multidrug resistant strains in Shigella (68.3%)was much higher than that in Salmonella (44.7%,P<0.001).Conclusions A variety of diarrhea-causing enteric pathogens are isolated from the children in Shanghai Children’s Hospital.The isolates are predominantly Salmonella and Shigella species.The epidemiological features of Salmonella and Shigella species are different in terms of gender,age,season and geographical distribution.The resistance to antibiotics is a serious problem and varies with different types of pathogens. Intensive and ongoing surveillance of enteric pathogens and their changing resistant pattern is required to control diar-rhea in children.
4.Effect of massage therapy on cerebral palsy children with strabismus

ObjectiveTo investigate the effect of massage therapy on cerebral palsy children with strabismus.Methods60 cerebral palsy children with strabismus were divided into the treatment group (30 cases) and control group (30 cases). Children of the treatment group were treated with massage therapy "YangGan RouJin Fa", those of the control group were not given any treatment, 30 strabismus children without cerebral palsy were treated with massage therapy "YangGan RouJin Fa" as the observation group.ResultsEffect of the treatment group was significantly different from the control group (P<0.01), and observation group (P<0.05).Conclusionmassage therapy "YangGan RouJin Fa" is a efficient method to strabismus in cerebral palsy children.
5.The short-term observation of Shenqifuzheng injection combined with NP chemotherapy in treating elder patients with advanced non-small cell lung cancer.

BACKGROUNDAbout 80% lung cancer is non-small cell lung cancer (NSCLC) and 70%-80% are in advanced stage. Chemotherapy is main treatment method. The aim of this study is to compare the therapeutic effects and toxicity of NP regimen combined with Shenqifuzheng injection on elder patients with advanced NSCLC.
METHODSTotally 69 patients enrolled into this study and were randomized into two groups: treatment group (35 patients) and control group (34 patients). Each patient received NVB 25mg/m² intravenously at days 1 and 8 and DDP 30mg intravenously from 1st day to 4th day. Shenqifuzheng injection was used in the treatment group by 250mL per day for 10 days.
RESULTSThere was no significant difference of the response rate between two groups (45.7% vs 41.2%, P > 0.05). The hematological toxicity, nausea and vomiting in the treatment group were lower than those in the control group with significant difference (P < 0.05). The adverse effects were well tolerable.
CONCLUSIONSNP regimen combined with Shenqifuzheng injection on elder patients with advanced NSCLC is effective and safe. Shenqifuzheng injection has definite toxicity relieving effect on treating elder patients with advanced NSCLC.
6.Effect of 3D-Slicer combined with sina software assisted minimally invasive neuroendoscopic surgery for hypertensive intracerebral hemorrhage

Objective To observe the effect of 3D-Slicer combined with sina software assisted minimally invasive neuroendoscopic surgery for hypertensive intracerebral hemorrhage,and investigate the application value of 3D-Slicer combined with sina software in the preoperative localization of minimally invasive neuroendoscopic surgery.Methods From July 2015 to October 2017,38 consecutive patients with supratentorial hypertensive intracerebral hemorrhage admitted to Shunde Hospital,Guangzhou University of Chinese Medicine were enrolled retrospectively.According to the different treatment methods,they were divided into an endoscopic group and a puncture group (n =19 in each group).The endoscopic group was treated with 3D-Slicer combined with sina software for neuroendoscopic hematoma removal,and the puncture group was treated with the hematoma minimally invasive soft-channel puncture and drainage under the CT localization.The effect of 3D-Slicer combined with sina software in the preoperative localization of hypertensive intracerebral hemorrhage by the minimally invasive surgery was evaluated by comparing the hematoma clearance of the first and third day of the two groups of patients after procedure,puncture to the preset position success,postoperative rebleeding,postoperative complications,and good prognosis at 3 months after procedure.Results The clearance rate of hematoma at the first day after operation in the endoscopic group was significantly higher than that in the puncture group ([90 ± 10]% vs.[46 ± 16]%;t =2.348,P < 0.05).The success of the puncture to the preset position was better than that in the puncture group (19/19 vs.14/19;x2 =5.758,P =0.016),and postoperative rebleeding rate was lower than that in the puncture group (0 vs.4/19;x2 =4.471,P =0.034).There were significant differences.There was no significant difference in postoperative infection complications between the two groups (all P > 0.05).The prognosis of the endoscopic group was good in 17 patients within 3 months after procedure,and the prognosis was good in 11 patients in the puncture group.The good prognosis in the endoscopic group was better than that in the puncture group (x2 =4.866,P =0.027).Conclusion The effect of 3D-Slicer combined with sina software assisted minimally invasive neuroendoscopic surgery for hypertensive intracerebral hemorrhage was better than the hematoma minimally invasive soft-channel puncture and drainage under the CT localization,and the 3D-slicer combined with sina software can provide rapid and accurate preoperative localization for minimally invasive neuroendoscopic surgery of hypertensive intracerebral hemorrhage.

A group of 19 referred hypertensive patients were diagnosed to have primary aldosteronism(PA) with inconclusive computed tomography scan results.Adrenal vein sarmpling (AVS) was performed in all patients.AVS was successful in 16 cases but failed in 3 cases.According to the results of AVS and postoperative pathology,8 cases were diagnosed as aldosterone-producing adenoma (APA) and unilateral adrenal hyperplasia (UAH),and the other 8 cases were diagnosed as idiopathic hyperaldosteronism (IHA).In conclusion,AVS is one of the most crucial methods in typing diagnosis of PA.
8.Systemic sclerosis with co-infection by Salmonella Michigan ST2065: a case report

A 57-year-old male patient was admitted to Shanghai Huashan Hospital due to recurrent systemic sclerosis in May 2023. The patient presented with mild diarrhea (2-3 times per day) and was suspected of having concurrent infectious diarrhea. Fecal samples, after selective enrichment and isolation, identified a'non-A-F serogroup Salmonella′. Salmonella Michigan ST2065 was identified in the reference laboratory by serotyping, drug resistance phenotyping, whole genome sequencing and cross-referencing with the regional serotype database of Salmonella in China. Combined with drug resistance and virulence results, the strain was determined to exhibit a phenotype characterized by high drug resistance and low enterotoxin production. The patient was prescribed oral tigecycline for 7 days and was discharged with improved symptoms.
