1.Dynamic observation of ossification process in a rat femur distraction model by living small animal Micro-CT

[Objective]In order to dynamically observe the ossification process by living small animal Micro-CT in a rat femur distraction osteogenesis (DO) model,the author applied a special designed external fixiator system. [Methods]A femur DO model was made on 12 SD rats.After 7 days of latency,the femurs were distracted at a speed of 0.25 mm every 12 hours for 14 days.At the first day of consolidation period,the steel external fixiators were substituted by radio-transparent polymer splint material made fixiators.At 7 and 21 days of consolidation period,two randomly selected rats were sacrificed for histological examination.The other 8 rats were selected for X-ray and living Micro-CT examination at 0,7,21,35 days of consolidation period.All animals were sacrificed at 35 days of consolidation period,5 for biomechanical test,3 for histological examination.[Results]There was no significant new bone formed in distracted zone at latency and distraction periods.The volume of distracted zone increased little in the following 5 weeks of consolidation period.Bone volume,bone mineral content,bone volume fraction,and the number of trabeculae all increased over the consolidation period,while mineral content of mineralized tissue and thickness of traleculae did not increased until 3 weeks after consolidation began.At final observation (56 days post operation),there still exist un-mineralized cartilage tissues in the central of distracted zone,and the distracted femurs had lower biomechanical property when compared with corresponding normal femurs.[Conclusion]1) Radio-transparent polymer splint is a satisfactory substitute of steel external fixator.2) Micro-CT is a useful tool for dynamical observing and evaluating bone formation status in DO model.3) The increasing of bone volume began at the end of distraction,lasting at least 5 weeks,while the remodeling of new formed bone began 3 weeks after distraction ended.The increasing and remodeling of new formed bone was not concord.This result indicates that,in order to obtain proper results,the corresponding interference should be taken at different time points.
2.The value of DWI and ADC of 1.5T magnetic resonance in differential diagnosis for localized prostate cancer, chronic inflammatory response and benign hyperplasia

Objective:To compare and analyze the value of diffusion weighted imaging (DWI) and apparent diffusion coefficient (ADC) of 1.5T magnetic resonance (MR) in differential diagnosis for localized prostate cancer, chronic inflammatory response and benign hyperplasia.Methods: 80 patients with localized prostate cancer were enrolled in the research. The signal to noise ratio (SNR) of DWI, ADC value and semi-quantitative classification of DWI on lesions were measured, and the diagnostic efficiencies of them were compared by using ROC curve.Results: In the 80 patients with localized prostate cancer patients, there were 52 malignant lesions and 43 benign lesions in peripheral band, and there were 31 malignant lesions and 46 benign lesions in central gland. The ADC values of prostate cancer in peripheral band and central gland were 0.91±0.12 and 0.86±0.15, respectively, and they were significantly lower than that of normal tissue (1.68±0.23 and 1.28±0.31) and benign lesions (1.24±0.21 and 1.12±0.16). The semi-quantitative classifications of DWI for benign lesions were significantly higher than that for malignant lesions in peripheral band and central gland, respectively (x2=20.88,x2=12.14;P<0.05). For the diagnostic efficiency of ADC, the sensitivities of benign and malignant lesions in peripheral band and malignant gland were 91.3% and 79.1%, respectively, and the specificities of them were 89.6% and 70.2%. And they was significant higher than the corresponding sensitivities (71.2% and 51.3%)and specificities (78.4% and 65.8%) of DWI imaging.Conclusion: Both of DWI and ADC of 1.5T magnetic resonance are the important indexes in differential diagnosis for prostate cancer, chronic inflammatory response and benign hyperplasia, while ADC value is better than DWI image in the clinical efficiency.

Ribosome inactivating protein(RIP) is a kind of toxin plant pr ot ein in which extensively lives in the body of higher plants and controls ribosom e's function. Beside it can control protein's combination,it has lots of biolog ical reactivity as resisting giving birth and tumor and controlling HIV. At fir st, RIP is isolated from seeds of bitter melon.The result of SDS-PAGE indicate s that there are lots of RIP in the abstraction liquid.Then we study the antivi ral action of RIP through the cell of CEF and SPF chicken embryos.The results s how that RIP can resist NDVF_48E_8,MDVCVI_988 and FPV-SD4 to so me extent.
4.Analysis of Gene Mutation of Voltaged-Gated Sodium Channel ?1 Subunit in 2 Generalized Epilepsy with Febrile Seizures Plus Families

Objective To collect the families with generalized epilepsy with febrile seizures plus(GEFS+) and analyze the clinical status and heredity features of Chinese GEFS+.Voltaged-gated sodium channel ?1 subunit(SCN1B) gene of 2 families were detected,and expect to find new mutation sites.Methods All participant in the study of 2 families members were informed of voluntary participate in this investigation,health examination and blood sampling.All 6 gene exons of proband,patients and healthy control group were sequenced.The sequencing result was compared and analyzed with the normal sequence of genomic exon fragment and exon fragment sequencing result of control group through internet(BLAST).Results 1.A new G/A heterozygous polymorphism (G181A)was found in the 181th basyl of SCN1B gene exon 3,and codon was changed from TCG to TCA,both encoding serine (Ser,S).It was synonymous mutation.2.A new G/A heterozygous polymophism(G15A)was found in the 15th basyl of SCN1B gene exon 3,and codon was changed from GAG to GAA,both encoding glutamic acid(Glu,E).It belonged to synonymous mutation.3.A new T/C heterozygous polymorphism (T37C)was found in the 37th basyl of SCN1B gene exon 6.The patients genetype were:5 cases with T/C heterozygote,3 cases with T/T homozygote,2 cases with C/C homozygote.Healthy control group were all T/T homozygote.Allele frequency distribution for T was 55.0%,and 45.0% for C.4.A new A/C heterozygous polymorphism (A81C)was found in the 81th basyl of SCN1B gene exon 6.The patients genetype were:5 cases with A/C heterozygote,3 cases with A/A homozygote,2 cases with C/C homozygote.Healthy control group were all A/A homozygote.Allele frequency distribution for A was 55.0%,and 45.0% for C.Conclusions Two new heterozygous polymorphism (G181A),(G15A) were found in SCN1B gene exon 3.Two new heterozygous polymorphism (T37C),(A81C) were found in SCN1B gene exon 6.These 4 polymorphism enriched single nucleotide polymorphism(SPN) database and provided candidate sites for the research of epilepsy susceptbility polymorphisms.
5.Study Progress of Voltaged-Gated Sodium Channel Gene in Generalized Epilepsy with Febrile Seizures Plus

Generalized epilepsy with febrile seizures plus(GEFS+) is a new epilepsy syndrome proposed by International League Against Epilepsy.At present,the progress of genetic studies of GEFS+ focus on gene mapping based on family analysis,many researches indicate that GEFS+ is associated with voltaged-gated sodium channel(SCN) gene mutation.This paper intends to discuss the relationship beween GEFS+ and SCN1B,SCN2B,SCN1A,SCN2A genes,mutations in order to improve the cognition about GEFS+.
6.An evaluation on effect and safety of morphine sulfate controlled-release tablet combined with celecoxib for the treatment of moderate to severe cancer-induced pain

Background and purpose:Pain is one of the most common symptoms in advanced cancer patients, and morphine is a representative drug in controlling moderate to severe cancer-induced pain, but some unacceptable adverse effects limited its use in part of patients.We evaluated the effect and safety of morphine sulfate controlled-release tablet (MS-CRT) combined with celecoxib for the treatment of moderate to severe cancer pain, and assessed the life quality of patients. Methods:Retrospective analysis of 125 cancer patients with moderate to severe cancer pain who were divided into two groups, one(including 67 patients) was treated by single drug of MSCRT whose initial dosage was 20 mg/12 hrs, then evaluated by verbal rating scale within 24 to 48 hrs, dosage was adjusted personally according to the state of pain (increasing rate was 50% and declining rate was 25%) until the maintenance dosage was reached; the other(including 58 patients) was treated by MS-CRT with the same initial dosage and combined with celecoxib whose dosage was 200 mg/12 hrs at first, and increased to 400 mg/12 hrs if the pain was not relieved well, then gradually increased the dosage of MS-CRT to the maintenance dosage, and analyzed the effect, dose adjustment,side effect of drug combination and improvement of quality of life for the patients. Results:The mean of maintenance dosage for MS-CRT alone was 67.3 mg/day, and for the combination of MS-CRT and celecoxib was 51.3 mg/day, the reduction rate of MS-CRT in the drug combination group compared with MS-CRT alone was 23.77% with the same analgesia effect, and the incidence of side effects such as constipation and nausea/vomiting was statistically reduced compared with the single drug group. The quality of life in both groups was improved aftertreatment. Conclusion:The combination of MS-CRT and celecoxib can effectively control moderate to severe cancer pain, , improve the quality of life in advanced cancer patients, and reduce the consumption of MS-CRT with similar side effects as morphine alone.
9.Pharmacokinetics Effects of Naloxone on Ginsenosides Rg1,Re and Rb1 of Shenmai Injection in Rats in vivo

OBJECTIVE:To study the pharmacokinetics effects of naloxone combination on Shenmai injection in rats in vivo. METHODS:12 rats were randomly divided into monotherapy group (Shenmai injection 9.00 ml/kg,iv) and combination group (Shenmai injection 9.00 ml/kg+naloxone 1.80 ml/kg,iv). The blood samples were collected before administration and 0.083,0.25, 0.5,0.75,1,1.5,2,3,6,12,24,48,96 and 144 h after administration. HPLC was adopted to determine the plasma concentra-tions of ginsenosides Rg1,Re and Rb1,and DAS 2.0 software was used to calculate the pharmacokinetic parameters. RESULTS:Compared with monotherapy group,the plasma concentration of ginsenosides Rg1 in combination group was increased,CL was de-creased,t1/2 and MRT were prolonged,and AUC0-144 h was increased;the plasma concentration of ginsenosides Re was increased,Ke was decreased,t1/2 was prolonged,MRT was shortened,and AUC0-144 h was increased;the plasma concentration of ginsenosides Rb1 was decreased,Ke was increased,t1/2 and MRT were shortened,and AUC0-144 h was decreased,with significant differences(P<0.01 or P<0.05). CONCLUSIONS:Shenmai injection combined with naloxone can slow down the removing of ginsenosides Rg1 and Re in vivo,and obviously the plasma concentration of Shenmai injection is higher than monotherapy group;speed up the removing of ginsenosides Rb1,and the plasma concentration of Shenmai injection is lower than monotherapy group obviously.
10.Total knee arthroplasty with Gemini MKII rotating-platform prosthesis

BACKGROUND:Total knee arthroplasty mainly used fixed platform, but more and more movable platform was selected year by year in the clinic. Rotary platform theoretical y can apparently improve the curative effects, but it has not been verified in the clinic.
OBJECTIVE:To summarize the design characteristics and clinical results of Gemini MKII (Link, Germany) rotary platform.
METHODS:From January 2004 to January 2009, 108 patients (119 knees) including 31 males and 77 females were treated with total knee arthroplasty by using Gemini MKII system. There were 97 unilateral cases and 11 bilateral cases at the first stage of replacement. Preoperative diagnosis:1 case of hemophilic arthritis, 1 case of venereal Charcot’s disease, 8 cases of post-traumatic arthritis, 11 cases of rheumatoid arthritis, and 87 cases of degenerative osteoarthritis. The prosthesis was fixed with bone cement. None received patel ar replacement. RESULTS AND CONCLUSION:None experienced infection, femoral condyle fracture or nerve and vascular damage. Except bilateral replacement, the average operation time of unilateral replacement was (56±13) minutes. Average postoperative hemoglobin decreased (25±5) g/L. After replacement, hemoglobin maintained over 100 g/L in 83 patients (86%), without blood transfusion. Knee joint score and function score recovered to (92.0±4.7) points and (90.3±6.8) points at half years after replacement, and became (94.4±3.2) points and (91.6±5.9) points during final fol ow-up, which was significantly improved as compared with that pre-replacement (P<0.01). No revision surgery should be done for lining prolapse or spin out, osteolysis and prosthesis loosening. These results indicated that the early and midterm results of Gemini MKII rotating-platform prosthesis were satisfactory, but the long-term results remain to be confirmed.
