1.Press fit technique reconstruction of anterior cruciate ligament with different types of tendon transplants

[Objective]To explore the clinical effects of selecting differents type of tendon transplants to reconstruct anterior cruciate ligament(ACL) using press fit technique.[Method]A total of 101 cases of ACL injury were included in this study,62 cases were athletic injuries,22 cases were traffic injuries,and 17 cases were daily-life injuries.According to patient's condition using different types of tendon transplant in which hamstring-tendon-knot 42 cases,bone-bolt-hamstring tendon knot 32 cases,patella bone-quadriceps tendon 27 cases,the tendon-knot was completely fit-into the bottle-necked femoral tunnel,and the tibial tunnel were pulled ont and seperated into two bundles, then tie a knot to fix the tendons tightly onto the bone bridge.[Result]In this group 98 patients were followed-up,According to the knee joint effective evaluate standard,88 were excellent,6 good and 4 fine.Excellent and good rate was 85.7%.[Conclusion]According to patient's condition,different type of tendon transplant to reconstruct ACL by press fit technique should be selected carefully.It is an method of easy to manipulate,reliable fixation,minimized trauma,biological fixation,economical and free expensive implant technique.
2.Lateral retinaculum patellae solution and medial patellofemoral ligament reconstruction using the semitendinosus tendon free grafted for the treatment of recurrent dislocation of the patella

Objective To investigate the outcomes of arthroscopically lateral retinaculum patellae release and medial patellofemoral ligament reconstruction using the semitendinosus tendon autograft for the treatment of habitual or recurrent dislocation of the patella. Methods 18 cases of recurrent dislocation of the patella were included in this group, among which there were 6 males and 12 females. The average age was 20 years old(range from 17 to 24 years). Arthroscopic investigation found tensed lateral retinaculum patellae and damaged cartilage at patellar medial margin and femoral lateral condyle. There were 6 cases of Ⅱdegree cartilage injuries, 5 cases of Ⅲ degrees injuries, and 7 cases of Ⅳ degree injuries caused by patellofemoral joint impact. For cases of Ⅳ degree injuries, the damaged cartilage was shaved and microfractured. For cases of Ⅱ or Ⅲ degree injuries, radiofrequency system was used for debridement. After the lateral retinaculum patellae was extensively released, semitendinosus tendon autograft was used to reconstruct the medial patellofemoral ligament. The knee joint was flexed and extended. Patella's dynamic motion, the tension of the tendon and relationship between the femoral trochlea and patellar bone were arthroscopically observed. Interface screw was used to fix the tendon ends within the bone tunnel on condylus medialis femoris. Results All patients were followed up for 9 to 19 months averaging 13 months. No patellar dislocation occurred postoperatively. Apprehensive test, patella tilt test were negative; and Apley test were negative in 16 patients and positive in 2 patients. One case of inferior pole of patella fractured. Axial view of X-ray and MR images of patella showed 15 cases regained normal anatomical relation of patellofemoral joint. 3 cases had mild semi-dislocation. Lysholm's score averaging (78.0?4.5) points preoperatively and (94.0?3.5) points postoperatively. Conclusion Lateral retinaculum patellae solution and medial part reconstruction under arthroscope can effectively cure recurrent subluxation of the patella and improve knee joint function.
3.Method for Determination of Nicotine in Cigarettes by Fluorescein and Fluorimetry-Basic Possession of Bonuses System

Objective To explore the determination of nicotine in cigarettes by fluorescein and fluorimetry-basic possession of bonuses system.Methods Nicotine could interfere with fluorescence energy transfer between fluorimetry-basic possession of bonuses.A new way of energy transfer was established to determinate nicotine according to the fluorescence increment of fluorimetry.Results The linear range was 0.2-7.0 mg/L for the concentration of nicotine.The correlative coefficient was 0.9992.The detection limit was 0.18 mg/L.The recovery rates were 99.0 %-105 %,and RSDs were 1.1 %-4.8 % respectively.Conclusion This method is simple,fast,accurate and sensitive.It can be used to determinate nicotine in cigarettes.
4.Association between C1GALT1,DEFA polymorphisms and its susceptibility to IgA nephropathy in Chinese Han and Hui population in Gansu Province

Objective:To investigate the association between the C1GALT1 rs1008898,the DEFA rs2738081 polymorphisms and susceptibility to IgA nephropathy in Chinese Han and Hui population in area of Gansu Province and explore molecular markers to predict IgA nephropathy Methods: In this study,there were 146 patients with IgAN and 180 normal controls in Han people and 83 patients with IgAN and 100 normal controls in Hui people.Two SNPs as rs1008898 and rs2738081 were analyzed with polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) and polymerase chain reaction-gene sequencing technology.The genotype and allele frequency of rs1008898 and rs2738081 were compared between patients with IgAN and normal controls.Results: Rs1008898 GG genotypes and G allele were over represented in IgAN patients compared with controls.Distribution of rs1008898 polymorphism in patients with IgAN and normal controls showed no difference in Hui people.Neither Han nor Hui population,rs2738081 polymorphism had difference between IgAN patients and normal controls.Conclusion: The G allele of rs1008898 probably has correlation with the genetic susceptibility of IgAN in Gansu Han people.
5.A clinical study of low frequency electric stimulation on the patients with vertebrobasUar insufficiency

Objective To explore the effects of low frequency electric stimulation on the clinical manifes- tation and its influence on the level of plasma Endothelin (ET),atrial natriuretic peptide (ANP),thromboxane B_2 (TXB_2),angiotensinⅡ(AngⅡ),Calcitonin Gene Related Peptide(CGRP) and Neuron-specific Enolase(NSE) and its clinical significance in the patients with vertebrobasilar insufficiency(VBI). Methods One hundred and tewenty cases of VBI patients were randomly divided into two groups:a treatment group and a control group.Both groups received routine drug treatment.The treatment group (n=60) was also treated with low frequency electric stimulation at bilateral mastoid processfes in addition.The improved rate of the clinical symptom,the plasma level of ET,ANP,TXB_2,AngⅡ,GRP and NSE were observed after treatment. Results The patients treated in the treatment group had significantly better outcome than the control group in terms of symptom relief.The effective rate was 87.93% and 67.80% in the treatment group and control group,respectively(P

To explore the anti-atherosclerotic mechanism of estrogen and especially observe the effect of estradiol on the content of cholesterol in J774a.1 mouse mononuclear/macrophage-derived foam cells which were incubated with oxidized low-density lipoproteins (ox-LDL). J774a.1 mouse mononuclear/macrophages were incubated with ox-LDL or with both ox-LDL and estradiol (1, 0.1 or 0.01 micromol x L(-1)). Oil red O staining was used to observe the formation of foam cells, and cholesterol oxidase fluorometric was used to determine the content of cellular cholesterol content. Western blotting and RTFQ-PCR were used to observe the expressions of scavenger receptor class B type I (SR-B I ) in J774a.1 foam cells. Compared with the control cells, J774a.1 mouse mononuclear/macrophage-derived foam cells showed significantly increased contents of total cholesterol and cholesterol ester (P < 0.001) and decreased SR-B I mRNA expression (P < 0.01). Estradiol treatment significantly lowered the contents of total cholesterol and cholesterol ester (P < 0.05), and increased SR-B I protein and mRNA expression (P < 0.01) in the foam cells in a dose-dependent manner. Estradiol can inhibit the formation of mononuclear/macrophage-derived foam cells by decreasing the contents of total cholesterol and cholesterol ester and up-regulating the expression of SR-B I in the foam cells.
7.Value of virtual touch tissues quantization in the stages of type 2 diabetic nephropathy

Objective To investigate the application value of virtual touch tissues quantization(VTQ)in the stages of type 2 diabetic nephropathy.Methods 120 cases of type 2 diabetes were divided into 3 groups according to urinary albumin excretion rate (UAER):diabetes without urinary albumin,diabetes with trace albuminuria and diabetes with mass albuminuria.The control group consisted 30 normal cases.VTQ was used to measure the renal shear wave velocity (SWV),then compare were made between the 4 groups.Besides,correlation analysis was made between SWV and UAER in diabetic nephropathy group.Results When compared SWV,there was no significant difference between diabetes without urinary albumin and control group (P =0.144),but there was significant difference between each 2 group of diabetes with trace albuminuria,diabetics with mass albuminuria group and control group (P < 0.05).There was also significant difference between each 2 group of diabetes (P < 0.05).The correlation coefficients for SWV and UAER in diabetes were 0.697(P <0.05).ROC analyses showed the area under curves for SWV diagnosis for diabetic nephropathy was 0.93.The best cutoff point,sensitivity and specificity were 2.43 m/s,85.7% and 84.5 %,respectively.Conclusions The renal SWV increases as the renal injury progresses,which also plays an important role in diabetic nephropathy stages.

9.Changes in expression of keratin genes in renal tissues during renal ischemia-reperfusion injury in mice

Objective To investigate the changes in the expression of keratin genes in renal tissues during renal ischemia-reperfusion (I/R) injury in mice.Methods Six wild type male C57/B6 mice,aged 50 days,weighing 20-30 g,were divided into 2 groups (n=3 each) using a random number table:sham operation group (Sham group) and I/R group.Right renal arteries and veins were clamped for 1 h followed by reperfusion,and the left kidneys were removed to establish the model of renal I/R injury.At 24 h of reperfusion,blood samples were collected from the left ventricle for determination of serum creatinine and urea nitrogen concentrations by colorimetric method.The right kidney specimens were obtained for pathologic examination and for determination of the expression of kidney injury molecule-1 and neutrophil gelatinase-associated lipocalin mRNA (by quantitative real-time polymerase chain reaction [qRT-PCR]) and keratin genes (by Affemetrixc DNA microarray).The differentially expressed genes identified were further confirmed by qRT-PCR.Results Compared with Sham group,the serum creatinine and urea nitrogen concentrations were significantly increased,the expression of kidney injury molecule-1 and neutrophil gelatinase-associated lipocalin mRNA was up-regulated (P<0.05),and the damage to the renal tubules was aggravated in I/R group.The results of microarray analysis showed that only keratin 20 gene (the expresion was up-regulated) was the differentially expressed gene (P<0.05),and the results measured by qRT-PCR were consistent with those measured by Affemetrixc DNA microarray.Conclusion Keratin 20 gene expression in renal tissues is up-regulated during renal I/R injury in mice,and the change may be involved in the endogenous protective mechanism during renal I/R injury.
10.The study of protective role of PR-39 on renal ischemia-reperfusion injury in rats

Objective To explore the protective role of PR-39 on the ischemia reperfusion injury renal tissue of rats.Methods 60 male SD rats were enrolled and randomly divided into experimental KP group and negative control KE group.The rats orthotopic renal transplantation model were established.The recombinant adenoviral vectors containing promoter KSP and PR-39 gene (KP) or none (KE) were injected via renal artery.HE staining,immunohistochemistry and TUNEL staining were performed on the rats' renal tissues at 2 days after IRI modeling,and the renal tubular interstitial injury score and peritublar capillary rarefaction was also evaluated.The renal function related factors of rats was monitored before and after IRI,and the blood urea nitrogen and creatinine level was also compared between KP and KE groups.Results Blood urea and creatinine concentration was increased,part of glomerular and tubular injury in renal tissue were also visible in rats underwent renal IRI,which proved the rat orthotopic renal transplantation model were successfully estabilshed.PR-39 gene specifically expressed in renal interstitial tissue,but not in glomerular.The degree of kidney tublar injury and the number of apoptotic cells were lower in KP groups by comparing with negative control KE groups.Renal tubular injury was significantly decreased in group KP than in group KE.Peritubular capillary rarefaction index is smaller in KP than in KE group.The blood urea nitrogen and creatinine level in KP group were significantly lower than those in KE group,and the renal function were also recovered more quickly.Conclusion The specific expressed PR-39 in renal interstitial and tubule,can inhibit the apoptosis of renal tubular epithelial cells and promote the angiogenesis of peritubular capillary,so as to exert its protective role on IRI renal tissue.
