1.Nursing of respiratory tract of patients with brain trauma undergoing mechanical ventilation

Objective To discuss the nursing measures for respiratory tract of patients with brain trauma undergoing mechanical ventilation in order to prevent ventilator associated pneumonia. Methods Patients with brain trauma undergoing mechanical ventilation (45 cases) were given nursing of respiratory tract. The nursing effect was observed. Results There were 28 patients went through respiratory failure and discharged from hospital after completely cured. Other 17 patients died because of the severity of brain injury. Conclusion Nursing of respiratory tract of patients undergoing mechanical ventilation had important significance for the prevention of complications and improvement of rehabilitation.
2.Early nutrition support therapy for patients with severe acute pancreatitis

Nutrition support therapy plays an important role in the treatment of severe acute pancreatitis (SAP).This article reviews the hot topics and new ideas in nutrition support therapy for SAP in recent years.Damage of intestinal mucosal barrier plays an important role in the progression of pancreatitis.Enteral nutrition can protect the function of intestinal mucosal barrier and early enteral nutrition can significantly reduce patients′mortality rate and incidence rate of complications.Nutrition support for SAP patients via a nasogastric tube has comparable safety and effect to the nutrition support via a nasojejunal tube.At present,there are no enough experience and clinical studies on semi -el-emental or elemental enteral nutrition preparations,and immune -enhancing enteral nutrition preparations and probiotics are not recommen-ded as conventional means.
3.Application and research status of glove retrieval bag in thoracoscopic surgery

The rapid development of minimally invasive surgery in thoracic surgery has significant advantages over traditional surgery.At the same time, problems such as the difficulty in obtaining specimens from small incisions and the increase in operating costs due to special medical consumables have also emerged. Therefore, how to remove surgical specimens safely, efficiently and cost-effectively is an important issue faced by thoracic surgeons during surgery. Operating room sterile gloves are often used as specimen collection bags and widely used in thoracic surgery and many other departments because of their easy availability, low cost, and good flexibility. At present, most of the glove specimen collection bags are based on sterile gloves, and various manufacturing methods have evolved by the combination with instruments and sutures.This article will briefly review the application of various forms of glove specimen retrieval bags in thoracoscopic surgery reported in existing research, analyze their advantages, disadvantages and scope of application, so as to provide references for future clinical applications and technological innovation researches.
4.Efficacy of ultrasound-guided transversus abdominis plane block for postoperative analgesia in patients undergoing inguinal hernia repair

Objective To evaluate the efficacy of ultrasound-guided transversus abdominis plane (TAP) block for postoperative analgesia in patients undergoing inguinal hernia repair.Methods Forty ASA Ⅰ or Ⅱ patients,aged 18-79 yr,with body mass index < 30 kg/m2,scheduled for elective unilateral inguinal hernia repair combined spinal-epidural anesthesia,were randomly divided into 2 groups (n =20 each):control group (group C) and ultrasound-guided TAP block group (group B).The ultrasound-guided TAP block was performed at the end of surgery and 20 ml of 0.375 % ropivacaine was injected in group B,while the equal volume of normal saline was given in group C.Tramadol was injected intravenously when VAS score ≥ 4 after surgery.VAS scores at rest and during activity were recorded at 4,6,24 and 48 h after surgery.The warm block plane on the blocked side was measured at 24 and 48 h after surgery.The overall satisfaction on analgesia was scored and the time when the patients passed the flatus was recorded.TAP block-related side effects were recorded.Results Four patients required tramadol in group C,while no patients required rescue analgesic in group B.Compared with group C,VAS scores were significantly decreased,the overall satisfaction scores were increased (P < 0.05 or 0.01),and no significant change was found in the time when the patients passed the flatus in group B (P > 0.05).The rate of warm plane block on the blocked side was 80% at 24 h after surgery and there was not warm block plane in patients at 48 h after surgery in group B.There was not warm block plane in patients at 24 and 48 h after surgery in group C.TAP block-related side effects were not found in group B.Conclusion The efficacy of ultrasound-guided TAP block for postoperative analgesia is better in patients undergoing inguinal hernia repair and the safety is higher.
5.Knockdown of NF-?B signaling pathway by siRNA inhibits the proliferation and invasiveness of HeLa229 cells

Objective To investigate cell proliferation and invasiveness of cervical cancer HeLa229 cell after knockdown of NF-?B signaling pathway by P65 siRNA.Methods RNA interference was employed for specific inhibition of the expression of P65.HeLa229 cell was divided into transfected group and untransfected group.Cell viability was detected by MTT after the HeLa229 cells were transfected with or without P65 siRNA for 24,48,72 h.The sensitivity to 5-Fu of the HeLa229 cell,transfected with or without P65 siRNA,was evaluated also by MTT.Boyden chamber experiment in vitro was used to detect the invasion of HeLa229 cell.Results P65 siRNA inhibited the cell proliferation as compared with the untransfected cells.Proliferations of both cells transfected with and without P65 siRNA were inhibited in a concentration-dependent manner,while at the same concentration of 5-Fu the viability of transfected HeLa229 cells was significantly suppressed(P
6.Construction of an immortalized rat astrocyte strain with inducible enkephalin expression

Objective To construct an immortalized rat astrocyte strain expressing enkephalin which can be regulated by doxycycline.Methods pRevTet-On and recombinant plasmid pRevTRE/hPPE were transfected into packaging cell PT67 respectively by standard lipofectamine methods.The supernatant containing RevTet-On virus was used to infect immortalized rat astrocyte strain(IAST)and RevTRE/hPPE virus was used to infect the IAST/Tet-On cell.Immortalized rat astrocyte strain that stably expressed Tet-regulated enkephalin was established. hPPE gene expression level of IAST/Tet-On/hPPE cell strain was detected by real time-PCR,immuno- cytochemistry and radio-immunoassay.Results Real time-PCR,immuno-cytochemistry and radio-immunoassay confirmed the level of enkephalin expression was regulated by doxycycline in a dose-dependent manner.Conclusion An immortalized rat astrocyte strain which secrets enkephalin as controlled by doxycycline has been constructed successfully.
7.Clinical experience of treating accelerated rejection of cadaver renal transplantation

Objective To discuss the way of treating accelerated rejection. Methods Seven patients of accelerated rejection were treated by efficient anti-rejection treatment. ResultsSix patients of accelerated rejection were reversed by efficient treatment of anti-rejection. One allograft was removed because treatment was invalid. And six patients were still alive, the longest survival one has reached to 3 years. ConclusionThe treatment emphasis of accelerated rejection should be focused on 3 aspects, including early diagnosis, efficient treatment in time, and paying more attention to any possible complications during the process of treatment.
8.Changes in life quality of patients with common bile duct stones after endoscopic retrograde cholangiopancreatography

Objective To investigate the influence of endoscopic retrograde cholangiopancreatography (ERCP) on life quality of patients with common bile duct stones. Methods The life quality of 35 patients, who underwent ERCP because of common bile duct stones, were assessed by GIQLI questionnaires before, 2 and 6 weeks after the procedure. Results The mean GIQLI score of patients before ERCP was 99. 9, which was significantly increased to 112. 2 and 121.9 at 2 and 6 weeks after ERCP (P <0. 01 ). At 6weeks after ERCP, the GIQLI score was similar to that of normal population. Conclusion ERCP can improve the life quality of patients with common bile duct stones.
9.Management strategy of implantable medical device in clinical department
