Objective To investigate the significance of serum Omentin-1 and matrix metalloproteinases-9 (MMP 9) level detec-tion in the patients with ischemic stroke complicating multiple sclerosis (MS) .Methods A total of 169 patients with ischemic stroke in our Hospitals from September 2013 to September 2016 were selected as the diseases group and divided into the combined group (n=57 cases) and single disease group (n=112) according to whether complicating DS .Contemporaneous 30 health persons in the physical examination center were chosen as the health group .The enzyme-linked immunosorbent assay (ELISA) was used to detect serum Omentin-1 and MMP-9 levels in all subjects .Then the serum Omentin-1 and MMP-9 levels and neural function were statisti-cally analyzed .Results Serum MMP-9 level and EDSS score in the disease group were significantly higher than those in the healthy group ,the serum Omentin-1 levels of the former was significantly lower than that of the latter ;serum MMP-9 level and expaned disability status Scale(EDSS) score in the combined group were significantly higher than those in the single disease group ,the ser-um Omentin-1 levels of the former was significantly lower than that of the latter ,the difference was statistically significant (P<0 .05);The Pearson correlation analysis results showed that the serum Omentin-1 level was negatively correlated with the EDSS score (r= -0 .724 ,P<0 .05) ,the serum MMP-9 level was positively correlated with the EDSS score (r=0 .763 ,P<0 .05);the Logistic regression analysis results showed that low serum Omentin-1 level ,high MMP-9 level of and high EDSS score were the in-dependent risk factors for ischemic stroke complicating MS (P<0 .05);the ROC curve analysis results showed that serum Omentin-1 level of 180 ng/mL serving as the cutoff value and serum M M P-9 level of 260 ng/mL as the cutoff value ,the sensitivity ,specific , accuracy of their combination for predicting ischemic stroke complicating MS were 94 .74% ,96 .43% and 95 .86% respectively . Conclusion Serum Omentin-1 and MMP-9 levels are related with the occurrence and development of ischemic stroke complicating MS ,are the independent risk factors for the latter occurrence ,moreover can effectively assess the patient′s neurologic disability situ-ation ,their joint detection can effectively assist doctors to assess the patient′s condition change ,and can effectively predict the oc-currence of ischemic stroke complicating MS ,which is worth further clinical promotion .

Objective To explore the expression levels of HIF-1 α and MUC5AC,MUC5B mRNA in chronic rhinosinusitis(CRS) and their correlation.Methods Eighty cases of CRS with nasal polyps(CRSwNP) treated in our hospital from May 2014 to May 2015,80 cases of CRS without NP(CRSsNP) and 80 healthy people without CRS(control group) were chosen.Nasal sinus mucosa in 3 groups were collected and treated.The expressions of HIF-1 α,MUC5AC and MUC5B mRNA were measured by semiquantitative reverse transcription polymerase chain reaction(RT-PCR),and their correlations were analyzed.Results The relative expression levels of HIF-1 α,MUC5AC and MUC5B mRNA in the CRSwNP group or CRSsNP group were 1.35±0.85,1.35±0.91;0.80±0.55,0.79±0.49 and 1.18±1.01,1.21±1.02 respectively,while which in the control group were 0.42±0.33,0.43±0.36 and 0.47±0.43 respectively,the difference between the CRS groups and control group was statistically significant(P<0.05).The expression of HIF-1αin the CRSwNP group and CRSsNP group was positively correlated with MUC5AC,MUC 5B mRNA(r=0.476,P=0.023,r=0.476,P=0.026,r=0.478,P=0.035,r=0.508,P=0.021).The ROC analysis results showed AUC=0.956 4.Conclusion The expression levels of HIF-1α and MUC5AC,MUC5B mRNA in chronic rhinosinusitis are significantly upregulated,moreover HIF-1α is closely related with MUC5AC and MUC5B mRNA.

Objective To compare the gas and heat exchange performance of membrane oxygenator DDMO-100S and membrane oxygenator Terumo SX-10 in cardiopulmonary bypass(CPB)in children.Methods 39 children aged 3-12 yrs who suffered from congenital heart disease were selected,and the repair of ventricle septal defect or/and atrial septal defect was performed under cardiopulmonary bypass.The 39 children were randomly divided into Group DD(n =21)and Group SX(n = 18).Membrane oxygenator DDMO-100S(Made by Xi'an Daidai,China)was used in Group DD and Membrane oxygenator Terumo SX-10(Made by Terumo,Japan)in Group SX,respectively.The hypothermic cardiopulmonary bypass with high perfusion flow and the cool crystalloid cardioplegia were applied in all children.The artery blood samples were taken for measuring PO2 and PCO2 and the oxygenating index(OI)was calculated by the formulae(OI = PaO2/FiO2).The temperature at nasal pharynx was monitored and the rewarming time was recorded.Results The total priming volume in Group DD was bigger than in Group SX [(742.5 ± 107.3)ml vs(531.3 ± 84.3)ml,P ＜ 0.05].The PO2,OI and PCO2 at 15 min after CPB or at 5 min before cease of CPB had no differences in both group(P ＞0.05).The rewarming time had no difference in both group [(31.5 ± 10.2)min vs(30.61 ± 8.2)min,P ＞0.05].Conclusions Under our observation condition,the gas and heat exchange performance of membrane oxygenator DDMO-100S were the same as membrane oxygenator Terumo SX-10,when it was used in cardiopulmonary bypass in children,but it had a little bit more priming volume.

Objective To investigate the clinical application value of microendoscope in the treatment of laryngeal,tracheal,and bronchial diseases.Methods Under microendoscope,laryngological operations or tracheal or bronchial foreign body removal were carried out in 166 cases,including 19 cases of vocal nodule,75 cases of polyp of the vocal cord,16 cases of papilloma of the vocal cord,6 cases of cyst of the vocal cord,9 cases of cyst of the epiglottis,5 cases of laryngeal hemangioma,19 cases of laryngeal amyloidosis,3 cases of keratosis of the vocal cord,2 cases of leukoplakia of the vocal cord,6 cases of glottic cancer,and 6 cases of tracheal or bronchial foreign body.Results Foreign bodies in the trachea or the bronchus were removed successfully in all the 6 cases.Of the rest of 160 cases,laryngological operations were accomplished smoothly in 157 cases while a relatively large amount of hemorrhage was encountered in 3 cases and treated with microwave hemostasis.The operation time was 30~60 min(mean,45 min).No postoperative complications occurred.Follow-up observations for 2~12 months(mean,6 months) showed 148 cases of full recovery of vocal function and 12 cases of improvement.No recurrence was found in the 6 cases of glottic cancer during 1 year of(follow-up).Conclusions The application of microendoscope in the treatment of laryngeal,tracheal,and bronchial diseases has advantages of broad surgical view and direct and clear visualization.This minimally invasive procedure can remove the lesion thoroughly and reserve functions to the greatest advantages.

Objective To develop a method for localizing a detecting capsule in the gastrointestinal tract.Methods A three-axis magnetic sensor was sealed in the detecting capsule.Three flat coils were fixed on patients' back.The coils were excited one by one to generate electromagnetic fields.The position and the orientation of the capsule could be determined by telemetering the signals from the magnetic sensor incorporated in the capsule.Results The telemetric localization system worked successfully.A localization model describing the relationship between the magnetic field strength and the position and orientation of the capsule,was correctly established basing on the principle of magnetic dipoles,and neural network algorithm was employed to solve a non-linear equation set.Conclusion The results showed that the localization method is feasible and the precision is higher than existing methods.The localization device can perform consecutive tracking and can be made into a portable type.After putting into practice,the device can also be used in food selection for astronauts by monitoring their gastrointestinal parameters in space.

Objective To investigate whether lamivudine antiviral treatment after radical surgery can prolong disease-free survival of patients with hepatitis B virus-related liver cancer.Methods A total of 120 patients with hepatitis B virus-related liver cancer who underwent conventional radical surgery in Dongfeng Hospital Affiliated to Hubei Medical College from March 2014 to March 2016 were enrolled,and among these patients,60 were given conventional treatment (group A) and 60 were given lamivudine antiviral treatment (group B).ELISA was used to measure serum HBV DNA level.The t-test was used for comparison of continuous data between groups,the chi-square test was used for comparison of categorical data between groups,the Kaplan-Meier method was used to compare disease-free survival rate and recurrence rate between groups,and the Spearman method was used to investigate the correlation between serum HBV DNA level and survival time.Results According to the results of the 3-year follow-up,71 patients (59.17％) died (group A:46 patients died of tumor and 4 died of hepatic encephalopathy;group B:20 died of tumor and 1 died of hepatic encephalopathy).Compared with group A,group B had significantly lower recurrence rate (48.33％ vs 90.00％,x2 =16.98,P ＜0.001) and mortality rate within 3 years (35.00％ vs 83.33％,x2 =10.34,P ＜ 0.001).Group B had significantly higher mean disease-free survival time and 1-,2-,and 3-year dis-ease-free survival rates than group A(t =9.82,x2 =7.87,11.43,and 7.98,all P ＜ 0.001).After surgery,group B had a significantly lower serum HBV DNA load than group A [(0.008 1 ±0.003 2) × 105 copies/ml vs (0.014 3 ±0.008 9) × 105 copies/ml,t =18.54,P ＜ 0.001].In group A,there were significant differences in 1-,2-,and 3-year disease-free survival rates between patients with serum HBV DNA load ≥ 1.0 × 105 copies/ml and those with serum HBV DNA load ＜ 1.0 × 105 copies/ml (x2 =8.57,P ＜ 0.05),and further analysis showed that there were significant differences in 1-,2-,and 3-year disease-free survival rates (x2 =4.36,5.36,and 9.53,P ＜ 0.05);in group B,there were no significant differences in 1-,2-,and 3-year disease-free survival rates between these two groups of patients (P ＞ 0.05).Serum HBV DNA expression was positively correlated with patients' survival time (r =0.67,P ＜ 0.001).Conclusion Lamivudine antiviral therapy after radical surgery can effectively reduce serum HBV DNA level,and helps to prolong the disease-free survival time of patients with hepatitis B virus-related liver cancer.

OBJECTIVE: To observe the application of the tragus cartilage in tympanoplasty. METHOD: Thirty-eight patients with tympanoplasty carried out tragus cartilage-perichondrium repairing tympanic membrane. The air-bone gap changes and the recovery of patients'tympanic membrane were evaluated at pre-operative and two months after the operation. RESULT: Thirty-one patients finishing follow-up visit showed tympanic membrane healed and looked like normal appearance in 1 to 3 months after the operation. The postoperation narrow of the air-bone gap >18 dB, tragus looked normal. CONCLUSION Tympanic membrane reconstruction using the tragus cartilage-perichondrium is feasible for tympanoplasty, and the survival rate and postoperative hearing would be acceptable.

Objective To investigate the effect of pretreatment with parecoxib for postoperative analgesia in patients undergoing laparoscopic hernia repair. Methods Sixty patients undergoing laparoscopic hernia repair were assigned in two groups by random digits table with 30 cases each. The patients in pretreating group received parecoxib 40 mg intravenously before anesthesia, and in control group with the same capacity of normal saline. All patients were anesthetized with combined spinal epidural blockage and tranquilized with pethidine, droperidol and diazepam. Postoperative pain of the abdomen incision was assessed by visual analogue scale (VAS) and the pain of shoulder or back was judged by a 4 grade scale. Side-effects, supplement analgesic, passage of gas by anus 24 h after surgery were observed as well. Results The abdomen incision VAS at 4,8,12,24 h after surgery in pretreating group were lower than those in control group (P ＜ 0.05). And the incidence of pain of shoulder or back was lower in pretreating group (13.3% ,4/30) than that in control group (33.3% ,10/30)(P ＜0.05). While the cases needing supplement analgesic also decreased in pretreating group (13.3% ,4/30) compared with control group (40.0%, 12/30)(P ＜ 0.05). And the side effects and passage of gas by anus 24 h after surgery had no significant difference in two groups. Conclusion Pretreatment with parecoxib in patients undergoing laparoscopic hernia repair may relieve the postoperative incision pain and the pain of shoulder or back while reducing the analgesic supplement without side effects.
