1.Natural oyster shell nanocomposite material for bone defects

BACKGROUND:Oyster shels and other shels of marine organisms can transfer into mineral salts, which is closer to the actual situation of human being. OBJECTIVE:To explore the effect of natural oyster shel nanocomposite material for bone defect repair. METHODS:Thirty adult big-eared white rabbits were randomly divided into two groups after establishment of bilateral radial bone defect models: experimental group and control group were given implantation of natural oyster shel nanocomposite material and injectable calcium sulfate bone graft, respectively. X-ray examination was done at weeks 2, 8, 12 after implantation to understand the osseointegration of bone grafts. Bilateral radial bone specimens were taken at 12 weeks to detect bending strength using biomechanical testing system and quantitatively analyze the osteogenesis using a color image analyzer. RESULTS AND CONCLUSION:At 2 weeks after implantation, the bone mineral density of bone grafts was lower in the two groups than the normal value, there was a clear boundary between the defect and graft, and no bone repair was found. At 8 weeks after implantation, the bone grafts were covered with thick soft tissues in the two groups, and the vascular component was reduced dramaticaly in the experimental group. At 12 weeks after implantation, the bone grafts were closely connected to the adjacent tissues in the two groups, with no boundary; the radius surface in the experimental group recovered to the normal level, and exhibited no difference from the normal tissues in aspects of morphology, texture and structure, but in the control group, there was stil a clear projected boundary. The bending strength and osteogenic amount of the radius were significantly higher in the experimental group than the control group (P < 0.05). These findings indicate that the natural oyster shel nanocomposite material for bone defect repair can have a good blending strength of the radius that can promotenew bone formation.
2.Analysis of hospitalization expenses of malignancy in Shanxi Cancer Hospital from 2003 to 2010

Objective To understand the malignancy hospitalization expense of malignancy in Shanxi Cancer Hospital and provide a reference for the effective control of hospitalization expense.Methods 89 716 malignancy hospitalization cases in Shanxi Cancer Hospital from 2003 to 2010 were reviewed.Hospital costs of top six malignant tumors were analyzed.Results The hospitalization expense of 6 kinds of malignancy increased in varying degrees.In all cancer patients,the top six number of cases were lung cancer,cervical cancer,stomach cancer,breast cancer,esophageal cancer and colon cancer,there were a total of 89 716 passengers.The growth rate of cervix malignancy' s expense was the fastest.Conclusion Malignancy hospitalization is expensive,comprehensive measures need to be used to control the expense and make full use of health resources.
3.A study of tracking the superparamagnetic iron oxide and enhanced green fluorescent protein labeled miniature porcine bone marrow stem cells by in vitro MRI

Objectives To track bone marrow stem cells (BMSCs) labeled by enhanced green fluorescent protein (EGFP) and superparamagnetic iron oxide ( SPIO ) -poly-L-lysine (PLL) compound by MRI in vitro for autotransplantation into pancreas of type 1 diabetes miniature pigs. Methods The BMSCs were isolated by density gradient centrifugation and attachment culture from type 1 diabetes minipigs' bone marrow. Expressional intensity of EGFP in BMSCs transfected lentivirus-EGFP with a multiplicity of infection Different magnetic resonance scanning protocols were carried out on various density BMSCs labeled by different concentration of SPIO in various time-point in vitro. Results When SPIO concentration was 25mg/L (count in Fe3 + ), the positive Fe3+ -labeling rate of BMSCs was 93. 1%. Most of SPIO particles in BMSCs' cytoplasm were observed in secondary lysosomes, but they were not detected in important organelle as cell nucleus. Comparing with gelatin the MRI of BMSCs labeled with SPIO in the condition with 1 ×104/ml cells density and 25 mg/L Fe3+ concentration in vitro, the signal intensity changes (△SI) after BMSCs labeled with SPIO 3 weeks and 6 weeks in TSE T1WI, TSE T2WI and FLASH T2 * WI sequences were 12%, 41%, 63% and 7%, 28%, 46% respectively (P < 0.01 and P < 0.05, respectively).Conclusions The data showed that the porcine BMSCs labeled with SPIO and EGFP could be traced successfully in vitro by MRI in the suitable sequences.
4.Study on the factors influencing the quality of life among patients with breast cancer

Objective To describe the quality of life of breast cancer patients and explore the risk factors for their quality of life (QOL). Methods QOL of 260 patients with breast cancer were studied using FACT-B, SCL-90, Social Support Rating Scale and multiple linear regression models to study the main factors influencing the QOL of patients with breast cancer on functional scales (physical, social, emotional, role). Results Economic conditions (β =2.024, P =0.003), emotion regulation ability (β =4.993, P =0.000) and self-evaluation of marital quality (β =-3.954, P =0.001) were associated with total QOL scores, emotion regulation ability (β =0.987, P =0.008), the number of emergency incidents (β=-0.866, P =0.011) were associated with physical domain scores, economic conditions (β =1.016, P =0.000), marital status (β =-1.018, P =0.041), emotion regulation ability (β =0.971, P =0.008), self-evaluation of marital quality (β =-1.382, P =0.001) were associated with social and family domain scores, emotion regulation ability (β =1.094, P =0.000), self-evaluation of marital quality (β =-0.892, P =0.006) were associated with emotional domain scores, regional factors(β =1.653, P =0.008), emotion regulation ability (β =0.962, P=0.01), self-evaluation of the QOL (β =-1.705, P =0.000) were associated with function domain scores. Conclusion Guiding the patients to have a correct understanding of the breast cancer, strengthening their confidence of life, increasing physical activity, having a good family environment and necessary social welfare so that improving the QOL of the patients.
5.Quantitative perfusion parameters of dynamic contrast-enhanced magne-tic resonance imaging in patients with rectal cancer:estimation of the mi-crovascular perfusion and permeability

AIM:To investigate the perfusion parameters using dynamic contrast-enhanced magnetic resonance imaging ( DCE-MRI) in rectal cancer patients so as to explore its potential value in estimating the microvascular condition including perfusion and permeability .METHODS:The data of 38 rectal cancer patients examined with DCE-MRI was ret-rospectively analyzed .The perfusion parameters of carcinoma and normal rectal wall in each case were calculated , inclu-ding volume transfer constant (Ktrans), rate constant of back flux (Kep), extravascular extracellular space fractional volume (Ve) and initial area under curve (iAUC).The mean values of tumor and normal rectal wall , mucinous and nonmucinous carcinoma, poorly and moderately-to-well differentiated carcinoma , case with or without lymph node metastasis were com-pared.RESULTS:All the parameters of rectal cancer were higher than normal rectal wall (P<0.01).No significant difference was found between poorly and moderately-to-well differentiated carcinoma in terms of K trans, Kep and Ve, neither was the case with or without lymph node metastasis .The cases with lymph node metastasis had lower iAUC than those with-out (P<0.05).CONCLUSION:Quantitative perfusion DCE-MRI answered the microvascular perfusion and permeability change of rectal cancer compared with normal rectal wall , besides it could be used to distinguish between mucinous and nonmucinous carcinoma , which demonstrated its value in the evaluation of rectal cancer .However , it should not be recom-mended to predict the degrees of tumor cell differentiation and lymph node metastasis just according to the perfusion param -eters.
6.Quantitative comparison of parathyroid hormone 1 receptor between human periodontal ligament stem cells and human periodontal ligament cells

BACKGROUND:Human periodontal ligament stem cels are a kind of mesenchymal stem cels that have self-renewal and multidifferentiation potential. Previous studies have showed that human periodontal ligament stem cels can differentiate into osteoblast-like cels or adipocyte-like cels under appropriate induction. Yet few studies have focused on the expression level of parathyroid hormone 1 receptor which wil affect the osteogenic potential of Human periodontal ligament stem cels. OBJECTIVE:To examine the expression level of parathyroid hormone 1 receptor between human periodontal ligament stem cels and human periodontal ligament cels and to discuss the role of parathyroid hormone 1 receptor in osteogenic differentiation. METHODS:By using magnetic-bead cel sorting, we separated and identified the human periodontal ligament stem cels and human periodontal ligament cels. We examined and compared the mRNA expression level of parathyroid hormone 1 receptor in human periodontal ligament stem cels and human periodontal ligament cels by Real-Time PCR. Osteoblastic differentiation was examined throughin vitro matrix mineralization by alizarin red staining and alkaline phosphatase assay. RESULTS AND CONCLUSION: Positive immunomagetic sorted cels were positive for STRO-1, CD146, Vimentin, indicating that they were periodontal ligament stem cels. Parathyroid hormone 1 receptor was expressed in human periodontal ligament stem cels and mainly located in cel membrane and cytoplasm which were similar to human periodontal ligament cels and MG63 cels. The expression of parathyroid hormone 1 receptor in human periodontal ligament stem cels was 3.7 times higher than that in human periodontal ligament cels, which was similar to that in MG63 cels. After osteogenic induction, human periodontal ligament stem cels showed a higher expression of parathyroid hormone 1 receptor and osteoblast-related genes as wel as the activity of osteoblast alkaline phosphatase and mineralization compared to human periodontal ligament cels. Our data showed that parathyroid hormone 1 receptor was higher in human periodontal ligament stem cels than human periodontal ligament cels and the expression was related with osteogenic differentiation, suggesting that human periodontal ligament stem cels display a higher potency of osteogenic differentiation and act as seed cels with a vast application prospect in oral tissue engineering.
7.Factors affecting mean blood glucose level of newly diagnosed type 2 diabetes pa-tients

AIM:TodetecthemoglobinA1c(HbA1c)andparametersofbloodglucosefluctuationinChinesenewlydiag-nosed type 2 diabetes mellitus (T2DM) patients, and further to specify the factors that were related to mean blood glucose (MBG) in this population.METHODS:Newly diagnosed T2DM patients (n=90) from 4 hospitals in Guangdong province were enrolled, and subjected to 3 d continuous glucose monitoring (CGM) after testing for HbA1c and other laboratory tests.Blood glucose data collected during CGM were used to calculate MBG and parameters of blood glucose fluctuation.RESULTS: Correlation analysis revealed that MBG was significantly related to all parameters of blood glucose fluctuation, HbA1c, fast plasma glucose ( FPG) and 2 h postprandial glucose (P<0.01), but not to sex, age or blood lipid profile.Further analysis utilizing step-wise general linear model showed that HbA1c, absolute means of daily difference ( MODD) , difference between maximal and minimal glucose ( DMMG) and FPG had the strongest relation to MBG.CONCLUSION: Factors affecting MBG of the newly diagnosed T2DMpatients in our country include HbA1c, FPG, DMMG and MODD, and thus it may be prone to misleading results that only HbA1c is applied to estimate MBG in this population.
8.Low altitude assessment of arterial blood pressure predicts susceptibility to acute mountain sickness at high altitude

Objective To explore that whether the normoxic low altitude measurement of arterial blood pressure would predict subsequent susceptibility to acute mountain sickness ( AMS) during rapid ascent to high altitude .Methods Arterial blood pressure ( using a wrist sphygmomanometer ) was determined in two hundred and four healthy lowlanders first exposed to 3700 m-altitude (Lhasa) from plain (500 m) by air, and the Lake Louise self-report questionnaire(LLS) was used to assess AMS.We compared the low altitude blood pressure related indicators of two groups ( AMS and non-AMS ) and analyzed the relationship of diagnostic score and blood pressure related indicators .Results ①The incidence of AMS in the selected subjects was 53.92% by LLS.AMS scores increased markedly at high-altitude (P<0.05 versus low altitude).②Diastolic blood pressure (DBP) and mean arterial BP (MABP) in the AMS group were higher than those in the non-AMS group(P<0.05).The low altitude DBP levels for diagnosis of AMS at high-altitude (3700 m) had an area under curve(AUC) =0.598, P<0.05, with sensitivity of 56.3%, specificity of 63.2%, and cut-off point of 72.5 mmHg. Conclusion ①After acute exposure to high altitude , the incidence of AMS increases significantly .②A higher baseline DBP may be considered a potential risk factor for AMS , and is positively associated with LLS .DBP may serve as a predic-tive parameter for diagnosis of AMS .However , the clinical application of DBP as a predictive criterion is limited because of its poor specificity or sensitivity .The use of DBP as a predictive criterion should be combined with other indicators for the better predictive value of AMS .
9.Application of Case-based Learning (CBL) Method in Clinical Practice of Internal Medicine

Objective To investigate the effect of Case Based Learning (CBL) method in clinical practice of internal medicine. Methods 88 clinical medicine students were divided into CBL teaching group and traditional teaching group. Traditional teaching method was introduced to traditional teaching group while CBL teaching method was carried out in the other. The test papers and questionnaires were analyzed in both groups. Result The students in CBL teaching group mastered internal medicine knowledge better than the other group. Conclusion CBL teaching mode helps improve the overall teaching quality of internal medical in clinical practice,and the interns' interest in learning and their clinical practice ability can be greatly enhanced.
10.Preliminary mechanism study of HCoV-OC43 escape from human dendritic cell immune elimination

Objective:To study the possible immune escape mechanisms of HCoV-OC43 from human dendritic cells(DC).Methods:HCoV-OC43 was isolated from clinical specimen using BSC-1 cells and identified by Real-time PCR,and the cytopathic effect was observed by phase contrast microscope.DCs were induced in vivo using hu-GM-CSF and IL-4 cytokines,and after 7 days of differentiation,DCs were infected by HCoV-OC43.The morphology of HCoV-OC43 infected DC was observed by transmission electron microscope,and the cytokines related to DC functions were detected by Real-time PCR after infection.DC proportion and function related co-stimulatory molecules were analyzed by flow cytometry.Results:In vitro HCoV-OC43 infected human DC model was successfully built.HCoV-OC43 can infect DC and generate immune response of DC in vitro,but no virus nucleonic acid could be detected in culture supernatant.The DC expression of IFN-α,IFN-β,CCL3 and CCL5 were significant decreased when infected with HCoV-OC43,but the expression of costimulatory molecules including HLA-DR,CD1c and CD86 were not affected by HCoV-OC43 infection.Conclusion:Human DC could be infected by HCoV-OC43 and generate immune response,but could not produce progeny virus.HCoV-OC43 may escape from immune response by suppressing the expression of IFN-α and other inflammatory cytokines and chemokines in DC.
