1.Clinical analysis of 68 young patients with gastric cancer

Objective To analyze the incidence and pathological characteristic of gastric cancer in young patients and to explore the possibility of screening and preventing hereditary gastric cancer. Methods 68 young patients with gastric cancer under 35 years of age were retrospectively analyzed about their sex, endoscopes datum, clinic representation,pathology and expression of c-erbB-2 protein. Results The hemoglobin was lower in the female group (94. 5 ±21.7) g/L than male group(126. 2 ± 12. 8) g/L(t =4. 502 ,P <0.05) ;The malignant pathology was significant difference(P <0.05) in young female group than the male' s;The expression of c-erbB-2 protein has no significant difference(P > 0.05). Conclusions Gastric cancer in young patients was diagnosed later, misdignosed more,and move highly malignant. The prognosis is poor. Those phenomena was obviously in female.
2.Clinical observation of recombinant human epidermal growth factor in treatment of recurrent oral ulcer in elderly patients

Objective To investigate the clinical efficacy of recombinant human epidermal growth factor in the treatment of recurrent oral ulcers in elderly patients.Methods 82 cases of elderly patients in our hospital in the treatment of recurrent oral ulcer were selected,41 cases in the control group,the conventional western medicine treatment,41 cases of the study group,recombinant human epidermal growth factor in treatment before and after treatment,blood determination of immune status,clinical efficacy and adverse reactions of comparison were performed at one-year follow-up.Results the effective rate of treatment group is lower than 78.05%of the effective rate of treatment group 95.12%,study group 3,6 and 12 months recurrence rate(4.88%,7.32%,7.32%)than in the control group the recurrence rate(19.51%,24.39%,29.27%),the study group after the treatment of CD3+ and CD4+/CD8+ and NK were higher than the control group,the CD8+ level is lower than the control group(P<0.05),the two groups had no serious adverse reaction.Conclusion recombinant human epidermal growth factor is effective in the treatment of oral ulcer.
3.Effect of low-dose dexmedetomidine on hemodynamics in general anesthesia in patients with essential hypertension

Objective To explore the effect of low-dose dexmedetomidine on hemodynamics of general anesthesia in hypertensive patients.Methods A total of 92 patients with hypertension were enrolled from May 2015 to February 2016 in People's Hospital of Fenghua District, Ningbo City were randomly divided into observation group and control group,with 46 cases in each group.All patients were treated with static and general anesthesia, the observation group before induction of general anesthesia 10 min received continuous intravenous micro-pump injection of dexmedetomidine 0.3 μg/kg(diluted with saline 20 mL);The control group received intravenous micro-pump injection physiology saline 20 mL.The mean arterial pressure(MAP)and heart rate(HR)before and after treatment, and the operation time, recovery time, extubation time, RSS sedation score and adverse reaction occurred were compared between the two groups.Results There was no significant difference in MAP and HR between the two groups before treatment.After treatment, the serum MAP of the two groups was increased, but the MAP in the observation group was(109.2±10.2)mmHg significantly lower than that in the control group(116.1±11.3)mm Hg,and HR in the observation group was(71.9±3.1)mmHg significantly lower than that in the control group(92.1±2.8)mmHg.The difference between the two groups was statistically significant(P<0.05).There was no statistically significant difference between the two groups in terms of operation time, recovery time and extubation time.The RSS sedation scores of the observation group before extubation(T1), extubation(T2)and extubation 1 min(T3)were higher than those of the control group.At T1, the RSS sedation score in the observation group was(2.3±0.4)points higher than that of the control group(1.8±0.3)points, At T2, the RSS sedation score was(2.9±0.4)points higher than that of the control group(2.1±0.3),At T3, the RSS sedation score was(2.5±0.3)points higher than that of the control group(1.9±0.3).The difference between the 2 groups was statistically significant(P<0.05).There was no significant difference between the two groups in adverse reactions.Conclusion Small doses of dexmedetomidine can reduce the patient's pumping, extubation and other stress response, providing the appropriate sedative effect, help to stabilize the patient's hemodynamic parameters, to ensure the effect of anesthesia and surgery to improve surgical safety.
4.The curative effect of combizym combined with esomeprazole in the treatment of gastroesophageal reflux disease with dyspepsia

Objective To investigate the curative effect of Oryz-Aspergillus Enzyme and Pancreatin Tablet (Combizym) combined with esomeprazole Magnesium Enteric-coated Tables (Esomeprazole) in the treatment of gastroesophageal reflux disease with dyspepsia.Methods 133 patients with gastroesophageal reflux disease and dyspepsia symptoms were randomly divided into two groups.Group A received esomeprazole 20mg once a day and Combizym 1 pill three times a day,while group B only received esomeprazole 20mg once a day.The symptoms integral,curative effect and adverse reaction in the two groups after treatment for 2 weeks and 4 weeks were compared.Results Compared with group B,the symptoms of abdominal distension and satiety significantly relieved after 2 weeks treatment in group A (abdominal distension x2 =4.516,P =0.038 ;satiety x2 =4.630,P =0.041),and one more symptom:loose stools relieved after treatment for 4 weeks in group A (abdominal distension x2 =5.317,P =0.025 ;satiety x2 =5.036,P =0.027 ; Loose stools x2 =4.989,P =0.030),while other symptoms didn t relieve (P > 0.05).The total effective rate was 85.2% vs 68.1% (x2 =8.917,P =0.030) and 93.4% vs 69.4% (x2 =17.526,P =0.001)in gourp A and B after treatment for 2 weeks and 4 weeks respectively.The symptoms integral reduced notably no matter after 2 or 4 weeks treatment and decreased more significantly in group A (2 weeks t =6.355,P =0.000 ;4 weeks t =5.450,P =0.000).The total effective rate increased more significantly in group A,without serious adverse reaction in both two groups.Conclusion Combined combizym with esomeprazole can better improve the symptoms of gastroesophageal reflux disease with dyspepsia without obvious adverse reaction.
5.The clinical efficacy of ilaprazole in the treatment of duodenal bulb ulcer

Objective To compare the clinical efficacy of ilaprazole standard triple therapy with omeprazole standard triple therapy in the treatment of patients with Helicobacter pylori ( Hp ) positive duodenal bulb ulcer. Methods 100 patients with Hp positive duodenal bulb ulcer were randomly divided into the two groups by lottery, given ilaprazole standard triple therapy and omeprazole standard triple therapy respectively.After 5 weeks of the course,take gastroscopy and the enzyme 13C-urea breath test,and the Hp eradication rate,clinical efficacy,ulcer heal-ing rate and adverse reactions were observed.Results The Hp eradication rate is better than the omeprazole group (92%vs 78%,χ2 =3.853,P<0.05),while the ulcer healing rate were no significant (96%vs 90%,χ2 =1.561, P>0.05).Conclusion It is a good method in the treatment of Hp positive duodenal bulb ulcer for ilaprazole stand-ard triple therapy.
6.Clinical significance of c-erbB-2 and nm 23 expression in gastric cancer

Objective To investigate the relationship between the expression of c-erbB-2 and rim23, and their clinical significance in gastric cancer. Methods The expression of c-erbB-2 and nm23 were detected by using immunohistochemical (SP) method in 85 surgi-cally removed specimens from patients with gastric cancer. Results The expression rate of c-erbB-2 and nm23 were 74.1% (63/85),52.9% (45/85). c-erbB-2 was positively correlated with the location, size and depth of invasion of gastric cancer, but not related with the histological types and lymph node metastasis. The expression rate of nm23 in patients without lymph node metastasis (85.7%) and in stage Ⅰ+Ⅱ (72.7%) was higher than that in patients with lymph node metastasis (42.2%) and in stage Ⅲ + Ⅳ (46.0%). The depth of inva-sion and clinical stage between the patients with c-erbB-2 over-expression and nm23 low-expression were positively related(P<0.01 or P<0.01). Conclusion c-erbB-2 promoted growth and invasion of gastric cancer, and nm23 restrained metastasis of gastric cancer. Both c-erbB-2 and nm23 could be served as predictors for biological behavior of gastric cancer.
7.Therapeutic Evaluation of Acupuncture for Female Stress Urinary Incontinence

Objective Taking sham electroacupuncture as control, to observe the clinical efficacy of electroacupuncture in treating female stress urinary incontinence (SUI). Method By using central random design, the eligible subjects were randomized into an electroacupuncture group of 42 cases [electroacupuncture at Zhongliao (BL33) and Huiyang (BL35)] and a sham group of 42 cases [sham electroacupuncture at points beside Zhongliao and Huiyang]. The therapeutic efficacy was evaluated by observing the urine leakage volume in the 1-h pad test and International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF). Result In the electroacupuncture group, the urine leakage volumes in the 4th, 6th, 20th, and 32nd treatment week were significantly lower than that before treatment (P<0.05);the decreases of urine leakage volume in the electroacupuncture group were more significant than that in the sham group in the 4th, 6th, 20th, and 32nd treatment week (P<0.05). In the electroacupuncture group, the ICIQ-SF scores in the 4th, 6th, 20th, and 32nd treatment week were significantly lower than that before treatment (P<0.05); the ICIQ-SF scores were significantly lower in the electroacupuncture group than in the sham group in the 4th, 6th, 20th, and 32nd treatment week (P<0.05). Conclusion Electroacupuncture can effectively improve urine leakage and other urinary symptoms in SUI.
8.The influence of low concentration iodinated contrast agent and low-dose CT scanning technique combined with body mass index on radiation dose and image quality of upper abdominal CT examinations

Objective To investigate the influence of“double low”technology(low concentration iodinated contrast agent and low-dose scan) combined with body mass index(BMI) on radiation dose and image quality of contrast-enhanced upper abdominal CT examination. Methods One hundred and twenty patients who received upper abdominal enhanced CT examination were randomly divided into 4 groups:group A1, the iodinated contrast agent iodixanol(270 mg/ml), BMI<18.5 kg/m2 and 80 kVp;group A2, the iodinated contrast agent iodixanol(270 mg/ml), 18.5 kg/m2≤BMI≤24.9 kg/m2 and 100 kVp; group B1, the iodinated contrast agent ioversol(320 mg/ml),<18.5 kg/m2 and 120 kVp; group B2, the iodinated contrast agent ioversol(320 mg/ml), 18.5 kg/m2≤BMI≤24.9 kg/m2 and 120 kVp. Image quality was subjectively scored, the objective parameters(noise, CT values of abdominal aorta and liver parenchyma, contrast noise ratio of abdominal aorta and liver parenchyma) were evaluated and radiation dose was recorded. The differences of the indexes between A1 and B1 groups, A2 and B2 groups were compared with Mann-Whitney U test and pared-samples t test. Results All CT images were good. No images with 4 scores were obtained. No significant difference was found between group A1 and B1, between group A2 and B2(P>0.05). There was no significant difference in contrast noise ratio of liver parenchyma(P>0.05), while significant differences existed in CT values of abdominal aorta and liver parenchyma, contrast noise ratio of abdominal aorta between group A1 and B1(P<0.05). Significant differences existed in the parameters above mentioned between group A2 and B2, respectively(P<0.05). Radiation dose was lower in group A1 than in group B1 and in group A2 than in group B2(P<0.05), respectively. Radiation dose was decreased by 40.1%(0.89/2.22) in group A1 than group B1 while radiation dose decreased by 56.9%(3.02/5.31) in group A2 than group B2. Conclusion According to BMI, the low concentration iodinated contrast agent and low-dose scan CT scanning technology could effectively reduce radiation dose and generate ideal images during the contrast-enhanced upper abdominal CT examination.
9.Quantitative comparison of parathyroid hormone 1 receptor between human periodontal ligament stem cells and human periodontal ligament cells

BACKGROUND:Human periodontal ligament stem cels are a kind of mesenchymal stem cels that have self-renewal and multidifferentiation potential. Previous studies have showed that human periodontal ligament stem cels can differentiate into osteoblast-like cels or adipocyte-like cels under appropriate induction. Yet few studies have focused on the expression level of parathyroid hormone 1 receptor which wil affect the osteogenic potential of Human periodontal ligament stem cels. OBJECTIVE:To examine the expression level of parathyroid hormone 1 receptor between human periodontal ligament stem cels and human periodontal ligament cels and to discuss the role of parathyroid hormone 1 receptor in osteogenic differentiation. METHODS:By using magnetic-bead cel sorting, we separated and identified the human periodontal ligament stem cels and human periodontal ligament cels. We examined and compared the mRNA expression level of parathyroid hormone 1 receptor in human periodontal ligament stem cels and human periodontal ligament cels by Real-Time PCR. Osteoblastic differentiation was examined throughin vitro matrix mineralization by alizarin red staining and alkaline phosphatase assay. RESULTS AND CONCLUSION: Positive immunomagetic sorted cels were positive for STRO-1, CD146, Vimentin, indicating that they were periodontal ligament stem cels. Parathyroid hormone 1 receptor was expressed in human periodontal ligament stem cels and mainly located in cel membrane and cytoplasm which were similar to human periodontal ligament cels and MG63 cels. The expression of parathyroid hormone 1 receptor in human periodontal ligament stem cels was 3.7 times higher than that in human periodontal ligament cels, which was similar to that in MG63 cels. After osteogenic induction, human periodontal ligament stem cels showed a higher expression of parathyroid hormone 1 receptor and osteoblast-related genes as wel as the activity of osteoblast alkaline phosphatase and mineralization compared to human periodontal ligament cels. Our data showed that parathyroid hormone 1 receptor was higher in human periodontal ligament stem cels than human periodontal ligament cels and the expression was related with osteogenic differentiation, suggesting that human periodontal ligament stem cels display a higher potency of osteogenic differentiation and act as seed cels with a vast application prospect in oral tissue engineering.
10.Effect of acute high altitude exposure on lung functions and relationship between lung function and AMS

Objective To investigate the effect of acute high altitude exposure on lung functions and the relationship between lung functions and acute mountain sickness ( AMS) .Method We collected the lung function and Lewis Lake data of 73 subjects (aged 18 to 26,male) at 400 m above sea-leve and those at high altitude(exposure to 3900 m, 5 d).Results Compared with sea-level, lung functions decreased in forced vital capucity (FVC), maximum midexpiratory flow(MMF), V50, V25 while forced expiratory volume in 1 second(FEV1), peak expiratory flow(PEF), V75 did not change.FVC, FEV1, PEF, MMF were used to analyze the relationship between lung functions and AMS .There was no difference in lung functions between AMS group and NON AMS group at sea-level, but lung functions of AMS group were significantly lower than those of NON AMS group in FVC , MMF at high altitude .There was difference between AMS group and NON AMS group in the rate of change of FVC and MMF .Logistic regression analysis showed that the rate of change of FVC was an independent risk factor , while correlation analysis showed that the change of FVC and the change of oxygen saturation were relevant.Conclusion Lung functions showed restrictive decrease after acute high altitude exposure .Changes of lung func-tions will increase hypoxia and susceptiblity to AMS .
