1.Variations of brain edema and neurological function of rat models of cerebral infarction after hyperbaric oxygen therapy

BACKGROUND:Several studies have suggested that hyperbaric oxygen could better protect cranial nerve and brain cels. Hyperbaric oxygen therapy can make oxygen partial pressure rapidly diffusing toward relatively hypoxic brain tissue, so as to increase blood oxygen content in the brain tissue, reduce brain edema and promote the recovery of brain function. OBJECTIVE: To observe the effects of hyperbaric oxygen therapy on brain tissue sweling in rats after middle cerebral artery occlusion, and discuss the possible mechanism of action underlying the neuroprotective effects of hyperbaric oxygen therapy in rats with cerebral infraction. METHODS:Sixty adult female rat models of cerebral infarction were successfuly established by middle cerebral artery occlusion using suture method and then randomly divided into the sham, cerebral infarction and hyperbaric oxygen therapy groups (n=20 rats/group). At 3 days after middle cerebral artery occlusion, apoptosis of nerve cels in the infract area of rats in each group was detected by TUNEL method. At 72 hours after middle cerebral artery occlusion, the gene transcription and protein expression of aquaporin 4/9 and matrix metaloproteinases 9/2 in the peri-infarct area were detected by RT-PCR and western blot analysis. The pathomorphological change in the infract area was observed by hematoxylin-eosin staining. The expression level of glial fibrilary acidic protein was detected by immunohistochemistry. At 24 hours and 3 days after hyperbaric oxygen therapy and at 1 and 2 weeks after middle cerebral artery occlusion, neurological behaviors were evaluated using Longa behavioral scores. RESULTS AND CONCLUSION:After 1, 2 days of hyperbaric oxygen therapy, Longa behavioral scores in the hyperbaric oxygen therapy group were significantly lower than those in the cerebral infarction group (P < 0.05). At 3 days after middle cerebral artery occlusion, cel apoptosis index in the hyperbaric oxygen therapy group was significantly lower than that in the cerebral infarction group (P < 0.05). At 72 hours after middle cerebral artery occlusion, the aquaporin 4/9, matrix metaloproteinases 9/2 gene and protein expression in the hyperbaric oxygen group were significantly lower than those in the cerebral infarction group (P < 0.05). These results suggest that hyperbaric oxygen therapy can play its protective role by decreasing apoptosis of nerve cels in the infarct area and lessening the edema of brain tissue in rats with cerebral infarction.

3.Study on the peri-puerperium outcome and associated factors of patients with primary Sj?grenˊs syndrome

Objective To analyze the peripartum complications,frequency of neonatal abnormalities and the associated factors in patients with primary Sj?grenˊs syndrome (SS). Methods The chart of 39 patients with primary SS who were admitted to the hospital for delivery were retrospectively reviewed. The clinical data, obstetrical outcome and the frequency of neonatal abnormalities as well as the possible associated factors were analyzed. Results There were 76 pregnancies and 41 deliveries among these 39 patients. Two patients (5%) had pregnancy-related hypertension, 1 (3%) had gestational diabetes and 1 (3%) had eclampsia. Twenty-eight(72%) patients had at least one episode of complication. In which, 27(68%) were induced abortion, 2 (5%) were premature birth, 6 (21%) were amniotic fluid volume abnormalities, 2 (5%) were post-partum bleeding, 5 (13%) were premature rupture of membrane, 6 (21%) were intra-uterine distress and 3 (8%) had intra-uterine growth retardation. Fetal abnormality was detected in 7(18%) patients, in which 3 fetus (8%) died before delivery, 2 fetus (5%) were small than gestational age and 2 (5%) had fetal deformity. Compared to the reports in the literature, the rate of abortion and fetal death was higher, but the rate of growth retardation , induced abortion was lower. Conclusion The frequency of peri-partum complication and fetal abnormality is increased in patients with primary SS. Pregnancies in patients with primary SS should beclosely monitored by rheumatologists.
4.Single-dose intra-arterial infusion chemotherapy for refractory bladder cancer after transurethral resection of bladder tumor: Report of 12 cases

Objective To evaluate clinical effects of single-dose intra-arterial infusion chemotherapy for the treatment of refractory bladder cancer after transurethral resection of bladder tumor (TURBt). Methods A retrospective analysis was made on clinical data of 12 cases of refractory bladder cancer treated by single-dose intra-arterial infusion chemotherapy after TURBt from November 1999 to June 2005. The bladder tumor was resected as thoroughly as possible. Postoperatively, a bilateral internal iliac arteriography was made by using the Seldinger technique. The tumor vessels and normal blood supply were identified through the intubation of the right femoral artery. Half dose of chemotherapeutics (epirubicin 25 mg) was infused into both internal iliac artery, then tumor vessels of bladder was selected and infused with peripheral embolization agent (a mix of fragmented gelatin sponge and cisplatin 200 mg). The embolization agent was used repeatedly until all tumor vessels were embolized. Intravesical instillation with epirubicin was carried out postoperatively, and cystoscopy was performed every 3 months after operation. Results After intra-arterial infusion chemotherapy, there were 12 cases of nausea and anepithymia, 3 cases of vomiting, and 2 cases of fever, all of which were symptomatically relieved with expectant treatment. Mild hip pain occurred in 6 cases and subsided in 3~5 days. Decreased erythrocyte and leucocyte were restored to normal levels in 2 weeks. Liver and renal functions did not present marked changes. Follow-up was conducted for 4~55 months (mean, 34 months). There were 1 case of recurrence at 32 postoperative month and 11 cases of progression free survival. Conclusions This technique lowers the recurrent rate of refractory bladder cancer and the incidence of side effects, being a new alternative for patients who are not willing to receive total cystectomy.
5.Comparison of Cisplatinum/Paclitaxe concurrent radiotherapy for advanced nasopharyngeal carcinoma

Objective To observe the acute side effects and recent therapeutical effect of cisplatinum/paclitaxe concurrent radiotherapy for patients with advanced nasopharyngeal carcinoma.Methods 87 patients with advanced nasopharygeal carcinoma were enrolled.According to the treatment,87 patients were divided into paclitaxe group (groupA,n =43) and cisplatinum group (group B,n =44).The two groups were treated with 6MV-X ray for radiotherapy.The prescription dose of radiation therapy was PGTV,PGTVnd 6 996 cGy/33F,PTV1 6 006 cGy/33F,PTV2 5 096 cGy/28F.The two groups began concurrent chemotherapy in radiotherapy,specific method:group A paclitaxe 30mg/m2,1 time a week,until the end of radiotherapy,group B cisplatinum 80 ~100 mg/m2,every 21 days.Results The recent therapeutical effect of nasopharyngeal lesions and lymph nodes of neck for group A and group B were 88.4% and 86.4% (x2 =0.079,P =0.778),88.4% and 88.6% (x2 =0.000,P =1.000),respectively.But the occurrence rate of group B ' oral mucosa reaction (x2 =4.295,P =0.038) and gastrointestinal reaction (x2 =4.482,P =0.028) were higher than that of group A.Conclusion Treated with cisplatin in advanced nasopharyngeal carcinoma after radical radiotherapy combined with docetaxel is similar,and the side effect is low,the tolerance of patients is good,curative effect is satisfied.
6.Intervention of Yangfei Ziyin Decoction on Sjogren's Syndrome Model Mice.

OBJECTIVETo observe the effect of Yangfei Ziyin Decoction (YZD) on symptoms, serum levels of TNF-alpha, IL-6, and aquaporin-5 (AQP-5), and pathology of Sj6gren's syndrome (SS) model mice.
METHODSTotally 60 mice were divided into 6 groups according to random digit table, i.e., the model group, the normal control group, the high, medium, low dose YZD groups (administered with YZD at 36.7, 18.4, 9.2 g/kg, 0.2 mL/10 g), the Chinese patent medicine group [CPM, administered with total glucosides of paeony at 0.6 g/kg], 10 mice in each group. All intervention was performed for six successive days in a week, with an interval of one day, a total of 50 days. Body weight, salivary secretion, food and water intake were measured at day10, 20, 30, 40, and 50, respectively. At day 50 blood was collected. Submandibular gland, thymus, and spleen were weighed. Serum levels of TNF-alpha, IL-6, and AQP-5 were detected by ELISA. Pathological changes of submandibular gland were observed. Results Compared with the normal control group, there was no change in water intake of mice in the model group, but with reduced salivary secretion (P < 0.01, P < 0.05). Thymus/spleen/submandibular gland weight and index increased in the model group (P < 0.01, P < 0.05). Compared with the model group at the same time point, salivary secretion increased in the CPM group and 3 YZD groups (P < 0.01 , P < 0.05). Compared with the model group, thymus/spleen/submandibular gland weight and index decreased in the CPM group (P < 0.01, P < 0.05). Thymus/submandibular gland weight and index decreased in the low dose YZD group (P < 0.01, P < 0.05). Thymus/submandibular gland weight and index, and spleen index decreased in high and medium dose YZD groups (P < 0.01 , P < 0.05). Levels of TNF-alpha and IL-6 decreased, but AQP-5 level increased in the CPM group (P < 0.05). AQP-5 level increased in high and medium dose YZD groups (P < 0.01 , P < 0.05). In the model group alveoli and duct of salivary gland were destroyed, alveoli and duct were irregular, epithelial cells were degenerated, necrotic, and desquamated. Mild-to-moderate lymphocytic infiltration occurred around submandibular gland. Pathological changes were alleviated in the CPM group and 3 YZD groups.
CONCLUSIONYZD could improve clinical symptoms, serum levels of TNF-alpha, IL-6, AQP-5, and pathological changes of SS model mice.

7.Effects of ?-catenin-dependent LEF-1 isoforms on the biological behavior of HeLa cells

Objective To study the effects of ?-catenin-dependent lymphoid enhancer factor(LEF-1) isoforms on biological behavior of HeLa cells.Methods ?-catenin-dependent LEF-1 genes were obtained by PCR from human lymphoid node cDNA library and inserted into pcDNA3.1/V5-His vector to construct the eukaryotic expression plasmid pcDNA3.1-F-LEF-1.Using lipofectamineTM 2000,the plasmid pcDNA3.1-F-LEF-1 was transfected into Hela cells.Then we screened the stable cell lines that expressed the truncated LEF-1 isoforms by G418 and identified the expression of target gene with Western blot.Then we analyzed the proliferation,apoptosis,cell clone formation and capability of tumor formation in vivo of transfected cell lines.Results We successfully constructed the ?-catenin-dependent LEF-1 eukaryotic expression plasmid and obtained the stable HeLa cell lines that expressed the full-length LEF-1 isoforms.The proliferation and capability of tumor formation in vivo of transfected cells were increased while apoptosis was decreased.Conclusion The overexpression of ?-catenin-dependent isoforms can stimulate the malignant biological behavior of HeLa cells.
8.Effect of pristimerin on experimental inflammation in mice and rats

AIM To study the anti-inflammatory effects and mechanisms of pristimerin. METHODS Several inflammatory models were established, such as ear edema induced by croton oil, hind paw swelling by carrageenan, elevation of capillary permeability by acetic acid in mice and a-cute peritonitis induced by carrageenan in rats. Protein content was measured by Coomassie brilliant blue method, nitric oxide (NO) content by Griess reaction assay, N-acetyl-?-D-glucosamini-dase (NAG) activity by colorimetry, superoxide dismutase (SOD) activity by hydroxylamine method, catalase (CAT) activity by ultraviolet spectro-photometry, and malondialdehyde (MDA) content by fluorescence method in peritoneal exudate in rats. RESULTS Pristimerin ip 0. 156 - 0. 625 mg ? kg-1 or im 1-4 mg - kg-1 inhibited ear edema, hind paw swelling, and elevation of capillary permeability in mice. In the rat peritonitis induced by carrageenan, pristimerin im 1 - 2 mg ?kg-1 reduced neutrophil counts, lessened protein and NO content, inhibited the production of MDA and decreased NAG activity, while augmented the SOD and CAT activity in exudate. CONCLUSION Pristimerin has a significant anti-inflammatory effect which may be related to the inhibition of NO production, scavenging oxygen free radicals, anti-lipoperoxidation and stabilizing lysosome membrane.
9.Spectral-domain OCT features of macular edema induced by central retinal vein occlusion

Background Cystoid macular edema(CME) is an important cause of visual impairment of central retinal vein occlusion(CRVO).Spectral-Domain optical coherence tomography(SD-OCT) has increased speed and higher resolution,offering a better chance of understanding the morphological changes and pathogenesis of CME. Objective This study was to survey the morphologic features of macular edema associated with CRVO by SD-OCT. Methods Clinical data of the patients with CRVO diagnosed in Department of Ophthalmology,Peking Union Medical College Hospital from March 2008 to August 2009 were retrospectively analyzed.SD-OCT features of macular edema induced by CRVO were analyzed and recorded.Results The average macular foveal thickness was(527.5±218.2) μm in macular edemas eyes.Main morphological changes included 55 cases(84.6%) of CME,15 cases of(23.1%) serous macular detachment(SMD),and 10 cases(15.4%) of simple macular edema,and these findings occurred at the same time in some eyes.Cystoid spaces in the parafoveal region were seen in the inner nuclear layer,outer plexiform layer and outer nuclear layer,and discontinuous or weak inner segment/outer segment(IS/OS) line was often seen in CME.The incidence of CME associated with incomplete posterior vitreous detachment(PVD) was 14.5%,and that of neural epithelial edema associated with incomplete PVD was 10.0%,showing an insignificant difference between them(χ2=0.000,P=1.000).The average area of SMD was 1838.4μm ×1428.1μm×190.1μm,and the incidence of partial PVD was higher(χ2=4.266,P=0.039).Conclusion SD-OCT can reveal the micro-morphological change of macular zone in macular edema eye.SD-OCT enabled visualization of its spatial extent in each retinal layer and the condition of IS/OS layer.Serous macular edema is related with partial PVD.
10.Mechanisms of rosuvastatin in treatment of elderly patients with coronary heart disease complicating hyperlipidemia

Objective To observe the changes of serum lipids ,hs‐CRP ,MFG‐E8 and Klotho gene levels after rosuvatatin treatment in elderly patients with coronary heart disease complicating hyperlipidemia for investigating the action mechanisms of ro‐suvatatin and its application value .Methods Totally 129 elderly patients with coronary artery disease complicating hyperlipidemia in our hospital were randomly divided into two groups .The control group received only conventional treatment ,while on this basis the rosuvatatin group was given rosuvastatin 10 mg everyday ,with 2 months as a treatment cycle .Blood lipids ,hs‐CRP ,MFG‐E8 and Klotho gene levels before and after treatment were compared between the two groups .The regulation effect of rosuvatatin was investigated Results The blood lipod ,hs‐CRP ,MFG‐E8 and Klotho before treatment had no obvious difference between the two groups .The levels of TC ,LDL‐C and TG after treatment in the rosuvatatin group were significantly decreased compared with the control group ,while the HDL‐C level was significantly increased ,the differences were statistically significant (P<0 .05) .In addi‐tion ,the hs‐CRP level after treatment in the rosuvatatin group was significantly lower than that in the control group (P<0 .05) . Compared with the control group ,the levels of MFG‐E8 and Klotho after treatment in the rosuvatatin group were increased ,the difference was statistically significant (P<0 .05) .Conclusion Rosuvastatin could decrease the blood lipid and hs‐CRP levels ,up‐regulates the MFG‐E8 and Klotho levels ,alleviates the inflammatory reaction and has the anti‐vascular aging effect ,thus effectively treats the patients with coronary heart disease complicating hyperlipidemia .
