2.Inhibitory effects of grape seed proanthocyanidin extract on selenite-induced cataract formation and possible mechanism.

This study investigated the inhibitory effect of grape seed proanthocyanidin extract (GSPE) on selenite-induced cataract formation in rats and the possible mechanism. Eighty 8-day-old Sprague-Dawley rats were divided randomly into 5 groups: control group, model group, three GSPE groups (low dose, medium dose and high dose). Control group received subcutaneous injection of physiological saline. Model group was given subcutaneous injection of sodium selenite (20 μmol/kg body weight) on the postpartum day 10, and once every other day for consecutive three times thereafter. GSPE treated groups were respectively administered GSPE at doses of 50, 100, and 200 mg/kg body weight intragastrically 2 days prior to the selenite injection (that was, on the postpartum day 8), and once daily for fourteen consecutive days thereafter. The opacity of lenses was observed, graded and photographed under the slit lamp microscopy and the maximal diameter of the nuclear cataract plaques was measured. The lenses were analyzed for superoxide dismutase (SOD), catalase (CAT), glutathione peroxidase (GSH-PX), malondialdehyde (MDA), calcium (Ca(2+)), nitric oxide (NO) and anti-hydroxyl radical ability (anti-OH(-)). The histomorphology of lenses was observed with HE staining under a light microscope. The levels of calpainII, and iNOS protein and mRNA expression in lenses were detected by using immunohistochemistry and real-time quantitative RT-PCR. The results showed subcutaneous injection of sodium selenite led to severe nuclear cataract in model group, and the achievement ratio of model group was 100%. As compared with model group, the degree of lenses opacity and the maximal diameter of nuclear cataract plaques were significantly reduced in GSPE-treated groups. Moreover, we observed selenite treatment caused a significant decrease in the activities of antioxidative enzymes (SOD, CAT, GSH-PX) and anti-OH(-) ability, accompanied by a significant increase in the levels of MDA, NO, Ca(2+) as well as iNOS, and calpainII protein and mRNA expression. Administration of GSPE could dose-dependently preserve the activities of these antioxidative enzymes and anti-OH(-) ability, accompanied by a significant reduction in the levels of MDA, NO, Ca(2+) as well as iNOS, and calpainII protein and mRNA expression. These results suggested that GSPE markedly prevented selenite-induced cataract formation probably by suppressing the generation of lipid peroxidation and free radicals as well as the activation of iNOS, and calpainII in the lenses.
3.Survey on the reflect problems of nursing administrators in a city public hospitals

Objective To investigate the reflect problems of nursing administrators in a municipal city public hospitals, in order to provide information to improve the management policies in public hospitals. Methods To investigate nursing department director (deputy director), departments, ward head nurses (deputy nursing officer) in the five public hospitals of a city, and analyzee the survey data in the way using of frequency analysis and weighted frequency analysis method. Results The top five of nursing administrators reflect problems of the city's public hospitals was lack of nursing staff (13.66%), head nurses lack of systematic management of knowledge ( 11.08% ), nurses have low wages ( 10.56% ), the low quality of nurses (7.64%) and many matters outside the scope of nursing (7.53%), problem and issue of all categories weighted total frequency of 50.47% ,which will take priority to be solved. Conclusions The top five problem in a weighted total order-bit frequency table will be given priority to be solved, to focus on resolving the primary problem of the lack of nursing staff can promote to solve other problems effectively.Public hospitals should improve the human resource system and the nursing management system construction to improve nursing management level.
5.The clinical effect of butylphthalide in the treatment of acute cerebral infarction

Objective To investigate the clinical value of butylphthalide in patients with acute cerebral infarction.Methods 120 patients with acute cerebral infarction were randomly divided into study group and control group according to the method of random number table,60 cases in each group.The study group used butylphthalide on the basis of routine treatment,while the control group only received routine treatment.Main outcome measures included the National Institutes of Health Stroke Scale (NIHSS)table,activities of daily living Barthel index,blood viscosity,clinical curative effect and health related quality of life (HRQL)score.Results There was no significant difference in NIHSS score,Barthel score,blood viscosity and HRQL between the two groups on admission (P >0.05 ).When compared with the control group after 3 months,NIHSS score of the study group decreased significantly [(948 ±3.84)points vs.(14.38 ±3.29)points,t=5.395,P=0.000],Barthel score increased significantly[(84.44 ± 10.59)points vs.(75.89 ±8.39)points,t=3.854,P=0.015];blood viscosity decreased significantly[(5.09 ± 0.64)mPa.s vs.(648 ±0.71 )mPa.s,t =7.493,P=0.000];HRQL scores increased significantly [(80.47 ± 1 5.39)pointsvs.(69.58 ±14.39)points,t=4.395,P=0.002];clinical curative effect in the study group was significantly higher than control group (χ2 =6.122,P=0.013).Conclusion Butylphthalide is helpful to improve the prognosis and quality of life in patients with acute cerebral infarction.
6.Study of event-related potentials N400 in children with Tic disorders

Objective To investigate the change of event-related potentials(ERPs)N400 and its significances in children with Tic disorders.Methods N400 of 35 children with Tic disorders(case group)and 20 healthy children(control group)were taken.The results of case group and control group were compared with each other.Results N400 latency of the case group was(706.87?198.99)ms,and the amplitude was(26.12?13.64)?V;N400 latency of the control group was(485.75?85.76)ms,the amplitude was(22.35?19.76)?V.There was obvious difference in latency between two groups(P0.05).Conclusion ERPs N400 latency of children with Tic disorders prolong obviously which shows cognitive dysfunction in the patients.
7.Clinical evaluation of mini/VITAL and Bact/Alert, two automated blood culture systems

Objectives To evaluate mini/VITAL and BacT/Alert, two fully automated blood culture systems. Methods Standard mini/VITAL AER and ANA bottles (bioM′erieux, Marcy 1′Etoile, France) and BacT/Alert FAN aerobic and FAN anerobic bottles (organon Netherlands) were used. The bottles were analysed over a period of 7 days. A total of 600 samples of blood, 108 samples of body fluid, and 100 strains of imitation were detected by mini/VITAL and BacT/Alert parallelly. The bottles of flagged positive were smeared and Gram stained before being examined under a microscope. Subcultures were performed in parallel on fresh blood agar and chocolate agar incubated in microaerophilic Conditions with 5% CO 2. All the bottles flagged negative were examined in the same conditions as the positive bottles. Results mini/VITAL: The positive rate for aerobic and anaerobic blood cultures was 10% and 5% respectively, for aerobic body fluid culture was 26 9%. No contaminants were detected. 0.5% false positive and 0.8% false negative were detected. 90% of positive sample were detected in 48 h. The shortest time of report of positive bottles was 1.5 h for blood culture. BacT/Alert: The rate of positive of aerobic and anaerobic blood culture was 9.8% and 5% respectively. The positive rate of body fluid culturs was 26.9%. There were used 3 bottles contaminated. The false positive and false negative rate were 0.5% and 0 98% respective. 95.5% of positive bottles were detected in 48 h. The shortest time of flagged positive was 2.5 h. Conclusion mini/VITAL and BacT/Alert were accurate automated blood culture systems for the detection of bacteremia. There was no significant difference on the speed of detection between BacT/Alert FAN bottles and mini/VITAL standard bottles.
8.Endovascular interventional treatment of bronchial artery aneurysm: case report and review of literature

Objective To present one case of bronchial artery aneurysm successfully treated with endovascular interventional procedure and review literature. Methods A 67-year-old man was diagnosed with mediastinal bronchial artery aneurysm accompanied with tuberculous pleurisy by chest imaging. The aneurysm was treated via transcatheter bronchial artery embolization combined with endovascular stent-graft placement in the thoracic aorta under DSA guidance. Results Postprocedural angiography showed satisfactory exclusion of the aneurysm and no endolead. Conclusion Interventional therapeutic management of bronchial artery aneurysm is feasible and accurate.
9.Intervention of Yangfei Ziyin Decoction on Sjogren's Syndrome Model Mice.

OBJECTIVETo observe the effect of Yangfei Ziyin Decoction (YZD) on symptoms, serum levels of TNF-alpha, IL-6, and aquaporin-5 (AQP-5), and pathology of Sj6gren's syndrome (SS) model mice.
METHODSTotally 60 mice were divided into 6 groups according to random digit table, i.e., the model group, the normal control group, the high, medium, low dose YZD groups (administered with YZD at 36.7, 18.4, 9.2 g/kg, 0.2 mL/10 g), the Chinese patent medicine group [CPM, administered with total glucosides of paeony at 0.6 g/kg], 10 mice in each group. All intervention was performed for six successive days in a week, with an interval of one day, a total of 50 days. Body weight, salivary secretion, food and water intake were measured at day10, 20, 30, 40, and 50, respectively. At day 50 blood was collected. Submandibular gland, thymus, and spleen were weighed. Serum levels of TNF-alpha, IL-6, and AQP-5 were detected by ELISA. Pathological changes of submandibular gland were observed. Results Compared with the normal control group, there was no change in water intake of mice in the model group, but with reduced salivary secretion (P < 0.01, P < 0.05). Thymus/spleen/submandibular gland weight and index increased in the model group (P < 0.01, P < 0.05). Compared with the model group at the same time point, salivary secretion increased in the CPM group and 3 YZD groups (P < 0.01 , P < 0.05). Compared with the model group, thymus/spleen/submandibular gland weight and index decreased in the CPM group (P < 0.01, P < 0.05). Thymus/submandibular gland weight and index decreased in the low dose YZD group (P < 0.01, P < 0.05). Thymus/submandibular gland weight and index, and spleen index decreased in high and medium dose YZD groups (P < 0.01 , P < 0.05). Levels of TNF-alpha and IL-6 decreased, but AQP-5 level increased in the CPM group (P < 0.05). AQP-5 level increased in high and medium dose YZD groups (P < 0.01 , P < 0.05). In the model group alveoli and duct of salivary gland were destroyed, alveoli and duct were irregular, epithelial cells were degenerated, necrotic, and desquamated. Mild-to-moderate lymphocytic infiltration occurred around submandibular gland. Pathological changes were alleviated in the CPM group and 3 YZD groups.
CONCLUSIONYZD could improve clinical symptoms, serum levels of TNF-alpha, IL-6, AQP-5, and pathological changes of SS model mice.

10.Analysis of the complication of transvaginal hysterectomy of patients with non-prolapsed uterus

Objective To explore the advantages and disadvantages of transvaginal hysterectomy for patients with non-prolapsed uterus,and explore the cause and preventive measurements of the complications of transvaginal hysterectomy.Methods The complications of transvaginal hysterectomy ( group TVH ) in 110 patients with non-prolapsed uterus and of transabdominal hysterectomy ( group TAH ) in 120 patients were compared with each other.These patients were chosen randomly from June 2006 to April 2010.Results In the TVH group,rectum hurt occurred in one patient,prolapse of fallopian tube in one patient and celiac internal bleeding in one patient.The preoperative diagnosis were not consistent with the postoperative diagnosis in four patients.Two patients encountered with postoperative residual end bleeding and four patients with vaginal residual end polyps.In the TAH group,three patients had postoperative diagnosis inconsistent with their preoperative diagnosis.In addition,there were four abdominal wall incision liquefaction ( including two phase suture in two patients),three postoperative bronchial lung infection and two residual end polyps.No celiac internal bleeding and visceral injury occurred in this group.Conclusion TVH has a higher risk of visceral injury and postoperative bleeding than TAH.These complications should be avoided when treating the patients with TVH technique.
