1.Analysis of therapeutic effect of integrated traditional Chinese and Western medicine treatment in the elderly patients with vertebrobasilar insufficiency

Objective To observe the curative effect of integrated traditional Chinese and Western medicine treatment in the elderly patients with vertebrobasilar insufficiency.Methods Totally 120 elderly patients with vertebrobasilar insufficiency were randomly divided into two groups:treatment group and control group(n=60,each).All the patients received Vinpocetine injection by intravenous drip,besides,the patients in treatment group received compound recipe Xueshuantong capsule.Results The total effective rate in treatment group was 95.0%,which was significantly higher than that in control group(78.3%)(P<0.05).The plasm viscosity [(1.2±0.2) vs.(1.6±0.2) mPa/s]and fibrinogen [(3.1±0.6)vs.(4.4±0.5)g/L]were decreased after compound xueshuantong capsule treatment as compared with control group (P < 0.05 ) Conclusions Combination of traditional Chinese and Western medicine has definite therapeutic effect on the old patients with vertebrobasilar insufficiency and can be utilized widely.
2.Advances in application of lipid emulsion in acute poisioning

Lipid emulsion has been used not only as nutrition in clinic practice,but also recently as an adjuvant therapy of lipophilic drugs poisioning,which is most likely to cause cardiopulmonary arrest and brings difficulty to conventional treatments.More and more reports of whether studies in labs or clinic cases suggest that lipid emulsion can be adopted as one of therapies for lipophilic drugs poisioning.The article mainly sums up the development of lipid emulsion remedy against lipophilic drugs poisioning in the hope of providing reference for clinical application of lipid emulsion to detoxification.

4.Dispute and consensus about surgical approaches and lymph nodes excisional area of esophageal carcinoma.

Surgical approaches for esophageal carcinoma are many and varied. Minimally invasive techniques are increasingly used widely in esophagectomy as they not only ensure patients' therapeutic effects, but also reduce trauma and accelerate recovery. The lymph nodes excisional area of esophageal carcinoma is still in controversy. It is necessary to carry out further researches on selective lymph nodes excision, which can decrease complication rate and improve patients' survival in the meantime.

5.Impact of intraoperative hypotension and blood pressure fluctuations on early postoperative delirium after laparoscopic surgery

Objective To investigate whether intraoperative hypotension was associated with postoperative delirium in older patients undergoing laparoscopic surgery with prospective cohort study.Methods Study subjects were patients,> 65 years of age,undergoing laparoscopic surgery,who were enrolled in an ongoing prospective observational study of the pathophysiology of postoperative delirium.Intraoperative blood pressure was measured,and predefined criteria were used to define hypotension.Delirium was measured by the Confusion Assessment Method on the first two postoperative days.Data were analyzed with t-test,two-sample proportion test,and ordered logistic regression muhivariable model,including correction for multiple comparison.Results Data from 213 patients with a mean age of 73.6 years (SD 6.1) were studied.Of these 63 (29.6%) developed delirium on day 1 and 61 (28.6%) on day 2.Relative hypotension (decreases by 20%,30%,or 40%) or absolute hypotension [mean arterial pressure (MAP) < 50mmHg] were not significantly associated with postoperative delirium,nor was the duration of hypotension (MAP < 50 mmHg).Conversely,intraoperative blood pressure variance was significantly associated with postoperative delirium.Conclusions These results showed that increased blood pressure fluctuation,not absolute or relative hypotension,was predictive of postoperative delirium.
6.Exposure Assessment of Vehicle Exhaust Pollution in Wuhan,Hubei

Objective To investigate the exposure level to vehicle exhaust pollution for population by different commuting modes in roadside environment in Wuhan. Methods Eight air monitoring sites were selected in the six main roads in urban area of Wuhan, Hubei province. The concentrations of NO2, CO, total hydro carbon (THC), PM10, benzene, toluene and xylenes (BTX) emitted from the vehicles were monitored and analyzed in winter and summer of 2007, respectively and combined with breathing rate and time spent of the population in traffic environments, exposure doses of different subpopulation were calculated as indicators in order to evaluate their exposure level to traffic exhaust. Results The PM10 was the primary pollutant on the roadside. Compared with the Ambient Air Quality Standard, the rate of exceeding standard limit for PM10 was 8% in summer and 100% in winter. 7.Intake Fraction: an Evaluation of Vehicle Emission Health Effects

Intake fraction is a new concept applied in assessing human health effect of pollutants. In the present paper, the history and practical use of it were reviewed, and then the calculation of it was also analyzed. After illustrating and discussing previous approaches for estimating intake fraction, the calculation can be classified into two sorts: one is taking the spatial area as the object, and the other is taking the people as the object. The latter is more fitful to estimate the intake fraction of mobile emission, in which it is necessary to investigate the related population and then combine the time-activity patterns with the microenvironment concentrations, in addition, modeling microenvironmental concentrations is critical for the calculation of intake fraction.
8.Application of shaping titanium mesh by multi-point forming technique in skull repairing

Objective To select materials with high quality and safety and evaluate the clinical value of multi-point forming technique in skull repairing. Methods 161 patients suffered from skull defect had been cured in our hospital, within them 43 patients were treated with multi-point forming technique in titanium mesh shaping, 19 patients with traditional handwork shaping, 99 patients with bone-like concrete (acrylate). 9.Clinical study on intradermal needle therapy in treating urinary retention after cervical cancer surgery

Objective: To observe the clinical efficacy of intradermal needle therapy for urinary retention after cervical cancer surgery. Methods: A total of 100 patients with urinary retention after cervical cancer surgery were randomized into a control group and an observation group, with 50 cases in each group. The control group was treated with basic nursing only, and the observation group was treated with additional intradermal needle therapy. Both groups were treated for 2 courses of treatment. The main symptom scores and residual urine volume of the two groups were observed before and after treatment, and the inpatient time, catheter indwelling time and the clinical efficacy were compared between the two groups. Results: The total effective rate was 96.0% in the observation group and 88.0% in the control group, and the difference between the two groups was statistically significant (P<0.05). After treatment, the main symptom scores and residual urine volume in both groups decreased significantly (all P<0.05), and the scores and residual urine volume in the observation group were significantly lower than those in the control group (all P<0.05). The inpatient time and catheter indwelling time in the observation group were significantly shorter than those in the control group (both P<0.05). Conclusion: Intradermal needle therapy has an obvious effect in improving symptoms of urinary retention after cervical cancer surgery, and the effect is significantly more persistent than that of simple basic nursing.
10.In vitro balanced sustained-release of Panax notoginseng saponins controlled with various matrix materials.

To explore the influence of matrix materials in complicate ingredients on traditional Chinese medicine and investigate the excipients selection model based on balanced release characteristics of multicomponents, the influence of HPMC (K4M, K15M, K100M) and Carbomer (934P, 971P, 974P) was illustrated by testing in vitro release of ginsenoside-Rg1, ginsenoside-Rb1 and notoginsenoside-R1 in Panax notoginseng saponins (model drug, PNS). According to in vitro release results of PNS matrix tablets in water and artificial intestinal juice, the release curves were analyzed with Peppas equation and simulating factor (f). Significant differences in k value and n value among ginsenoside-Rg1, ginsenoside-Rb1 and notoginsenoside-R1 existed in various formulations. The release behaviors from various excipients could be described with Non-Fickian transport or super Case II transport pattern. The f2 values for ginsenoside-Rg1, ginsenoside-Rb1 and notoginsenoside-R1 in 971P matrix tablet containing 30% Carbomer 971P were 74.91, 53.45, 57.89 in water and 79.35, 55.51, 51.89 in artificial intestinal juice, respectively. The release profiles fit for the regulation of FDA. The result revealed that the balanced release rates of Rg1, Rb1 and R1 in 971P matrix tablet were obtained.
