1.The effect of intranasal instillation of insulin on cognitive function in patients with Alzheimer disease

Objective To investigate the effect of intranasal instillation of insulin with different doses on cognitive function in patients with Alzheimer disease (AD).Methods Sixty-three AD patients were collectcd.According to the order of doctor's office visiting,they were divided into insulin 1 group (21 cases,intranasal instillation of insulin 20 U,2 times per day),insulin 2 group (21 cases,intranasal instillation of insulin 40 U,2 times per day) and placebo group (21 cases,intranasal instillation of physiological saline,2 times per day).Mini-mental state examination scale (MMSE) and Rivermead behavioural memory test second edition (RBMT Ⅱ) were used to evaluate the improvement of cognitive function after 3 months and 6 months.Results Before treatment,the scores of MMSE and RBMT Ⅱ in insulin 1 group were (20.2 ± 2.3) and (17.2 ± 1.9) scores,in insulin 2 group were (20.7 ± 2.8) and (16.5 ± 1.9) scores.After treatment for 3 months,the scores of MMSE and RBMT Ⅱ in insulin 1 group were (21.8 ± 3.2) and (19.2 ± 2.0) scores,in insulin 2 group were (21.6 ± 3.5) and (18.1 ± 2.0) scores.The scores of MMSE and RBMT Ⅱ in insulin 1 group and insulin 2 group significantly improved compared with those before treatment (P < 0.05 or < 0.01).But the scores of MMSE and RBMT Ⅱ in placebo group were significantly aggravated,(18.9 ± 3.8) scores vs.(20.9 ± 2.5) scores,(15.2 ± 2.1) scores vs.(16.1 ± 2.0) scores,P < 0.05.After treatment for 6 months,the scores of MMSE and RBMT Ⅱ in insulin 1 group were (22.5 ± 3.3) and (20.5 ± 2.0) scores,in insulin 2 group were (22.7 ± 3.1) and(19.8 ± 1.9) scores,and in placebo group were (17.9 ± 4.5) and(14.7 ± 2.0) scores.The scores of MMSE and RBMT Ⅱ in insulin 1 group and insulin 2 group were significantly better than those in placebo group (P < 0.01).Conclusion Intranasal instillation of insulin can improve cognitive disorders in patients with AD.
2.The Characteristics of Semen and Concentration of Fsh, LH and Testosterone in Serum of Asthenozoospermia Men

Introduction: In Vietnam, the andrology field has been developed in recent years. Some studies have indicated the reasons for male infertility. Amongst them, the most common reason is abnormal semen. Asthenozoospermia rate was found at a higher ratio in abnormal semen analysis. \r\n', u'Objectives: To identify the characteristics of sperm and concentration of FSH, LH and Testosterone in serum; and the association between the sperm morphology and sperm motility as well as the concentration of FSH, LH and Testosterone in serum of asthenozoospermia men. \r\n', u'Subjects and methods: 90 asthenozoospermia male partners of infertile couples. The semen was analyzed at the Laboratory for Tissue and Embryo Preservation, Hanoi Medical University. The hormone concentration was measured by the Automated Chemiluminescence System ACS 180. \r\n', u'Results: The mean semen volume: 3.0 \xb1 1.1 (ml); sperm density: 104.7 \xb1 86.7 million/ml; rapid motility rate: 5.9 \xb1 6.4 (%); sperm vitality rate: 67.4 \xb1 20.1 (%); normal morphology rate: 14.4 \xb1 6.9 (%). In serum, the concentration of FSH was 5.3 \xb1 2.5 lUlL; LH: 4.1 \xb1 1.8 IU/L. Testosterone: 18.3 \xb1 9.4 nmol/L. Conclusions: Among the studied asthenozoospermia men, normal semen volume was found at 91.11 %; normal sperm vitality parameter: 84.4%; normal morphology parameter: 45.56%; normal level of FSH concentration: 47.7%; normal level of LH concentration: 26.66%; normal level of Testosterone concentration: 78.89%. There was a strong association between the sperm morphology and sperm motility as well as serum Testosterone concentration. \r\n', u'

3.Efficacy of intensive treatment by automatic continuous positive airway pressure in elderly patients with obstructive sleep apnea-hypopnea syndrome

Objective To investigate the efficacy of the hospital intensive treatment by auto-CPAP on elderly patients with obstructive sleep apnea-hypopnea syndrome (OSAHS) and to analyse the possible factors related to the effectiveness.Methods Subjects were selected from elderly OSAHS patients over 60 years old.The eligible subjects were divided into intensive treatment group and non-intensive treatment group (non-ITG).Patients in intensive treatment group were intensively treated using auto-CPAP for 3 consecutive days,assisted with targeted health education and guidance,and making proper adjustments to parameters of the auto-CPAP according to treatment outcomes at the previous day.But those in non-intensive treatment group were just treated with auto-CPAP for only 1 day,supplemented by regular health education and guidance.The apnea-hypopnea index (AHI),minimum oxygen saturation (SaO2min),mean oxygen saturation (SaO2mean) and the time spent at SaO2 lower than 90% (tSaO2<90%),the occurrence of clinical symptoms and adverse effect,Epworth sleepiness scale(ESS) were compared between both groups.The correlation analyses were also conducted.Results ESS (3.58±3.76 vs.6.84 ± 3.22),AHI [(7.85±6.53) time/h vs.(10.42±7.27) time/h] and tSaO2<90%[(5.65±15.43) min vs.(15.26±33.14) min] were lower(t=6.902,2.760,2.765,allP<0.05),while SaO2mean [(96.57±1.53)% vs.(94.63±1.38)%] and SaO2min [(88.24±4.43)% vs.(83.28±5.06)%]were higher(t=-9.870,-7.740,both P<0.05)in intensive treatment group than in non intensive treatment group.The occurrences of clinical symptoms and adverse effect (except for skin allergy) were decreased in intensive treatment group versus non-ITG (all P<0.05).The AHI after intensive treatment was decreased in females versus males [(7.85±5.19) times /h vs.(11.27±7.78) times/h,t=2.133,P<0.05].BMI,age,gender and the state of OSAHS were correlated to AHI,SaO2 mean,SaO2 min and tSao2<90% after intensive treatment (all P<0.05).Conclusions The hospital intensive treatment can improve the clinical results and decrease the occurrences of clinical symptoms and adverse effect for elderly OSAHS patients,and is expected to improve therapy compliance,and its curative efficacy is correlated to BMI,age,gender and state of OSAHS before therapy.
4.Clinical and imaging analysis of corpus callosum infarction

Objective To investigate the clinical and imaging characteristics in patients with corpus callosum infarction.Methods The clinical data of 416 patients meetingthe diagnosis of cerebral infarction Were collected,in which,8 patients Were confirmed as corpus callosum infarction by MRI.Results Corpus callosum infarction accounted for 1.9% of all patients with cerebral infarction.CT scan did not show the corpus callosum infaretiom in 7 patients.The nonenhanced MRI revealed the lesions.The enhanced MRI revealed the lesion in another patient.The infarction foci were not only involved in the corpus callosum(knee,body or splenium),but also cornplicated with frontal lobe,occipital lobe and thalamus infractions.The clinical manifesta tions of the corpus callosum infarction were different due to the specific lesion sites.The simple infarction in the body of the corpus callosum mainly presented as contralateral paraparesis.atria,and left limb apra.xia;the infarction in the knee of the corpus callosum mainly presented as lower limb paralysis or contralateral paraparesis;the infarction in the splenium of the corpus callosum presented as limb paralysis and dizziness;the lacunar infarct in the corpus callosum had no obvious clinical manifestations.The patients who complicated with frontal lobe and thalamus infarction had behavioral and psychological syrnptonm,including mental retardation,language abnormalities.and incontinence.Conelusions Corpus callosum infarction is not common.MRI is the basis of diagaosis.The clinical manifestation is lack of specificity.and it is agsociated with the location of corpus callosum infarction and whether it comolicates with the infarction on other part.
5.Promoting effect of p75NTR receptor overexpression on oxidative stress injury in human retinal pigment epithelium cells

Background Choroidal neovascularization (CNV) is the primary pathogenic cause of many fundus diseases.Oxidative stress injury of retinal pigment epithelial (RPE) cells plays important role in angiogenesis of choroid new blood vessels.Oxidative stress injury can active p75NTR receptor, a member of tumor necrosis factors family,resulting in the proliferation of vascular endothelial cells.However, the mechanisms of vascular endothelial cell proliferation remain unclear.Objective This study was conducted to investigate the effect of p75NTR overexpression on CNV and the relative mechanism.Methods The ARPE-19 cell line was used in this study.RPE cells were transfected with p75NTR receptor overexpressed plasmid, and untransfected cells served as the control group.The transfected results were verified by reverse transcription-PCR and Western blot assay.Viability of the cells over time was determined in the p75NTR receptor plasmid transfected group by using BrdU assay.The percentage of apoptotic cells was detected by flow cytometry using Annexin V-FITC/PI fluorescence staining.The percentage of reactive oxygen species (ROS) expression in the cells was detected by using H2 DCFDA fluorescence and flow cytometry.Mitochondrial membrane potential and cytochrome C expression were examined under the confocal microscope.The protein expressions of cleaved caspase-3, Fas and VEGF were determined by Western blot assay.Results The relative expression level of p75 NTR receptor mRNA was (6.11 ±0.77) times higher than that of the control group, and relative expression level of p75NTR receptor protein in the cells in the p75NTR receptor plasmid transfected group was (7.42±0.48) times higher than that in the control group (t=11.49 and 23.17 ,both at P<0.01).The absorbency values of the p75NTR receptor plasmid transfected group were (93.12±0.56) % , (86.30±0.66) % , (72.53-±0.86) % and (60.77 ±2.81) % in 12,24,36 and 48 hours after plasmid transfection, which were significantly lower than 100% in the control group, and the apoptotic percentages were evidently higher than that in the control group (all at P<0.05).The relative fluorescence intensity of ROS fluorescence in the p75NTR receptor plasmid transfected group was 2.4 times higher than that in the control group,showing significant difference (t=16.45, P<0.01).The positive expressing rate of mitomarker (mitochondrial membrane potentials) was 100% in the control group and (37.30± 2.06)% in the p75NTR receptor plasmid transfected group, with significant difference between them (t =57.71,P<0.01).The fluorescence intensity of cytochrome C expression was elevated in the p75NTR receptor plasmid transfected group compared with the control group.Compared with the control group,the expressing levels of cleaved caspase-3 ,Fas and VEGF165 proteins in the cells were significantly raised in the p75NTR receptor plasmid transfected group (all at P<0.01).Conclusions Overexpression of p75NTR receptors in RPE cells leads to mitochondrial damage and cellular apoptosis and the secretion of VEGF protein, which sequentially promote CNV.P75NTR receptor may be another important regulation pathway in RPE oxygen damage.
6.Relationship Between Gastric Polyps and Helicobacter pylori Infection

OBJECTIVE To investigate the relationship between gastric polyps and Helicobacter pylori infection.METHODS The patients with gastric polyps were taken by gastroendoscopy in 2005.The tissues from their antrums were examined for presence of H.pylori.We collected and analyzed all of their general information and the data about their gastric polyps and H.pylori infection condition.RESULTS In the 95 gastric polyps patients,76 cases(80.0%) had inflammatory polyps and 19 cases(20.0%) had H.polyps.The total H.pylori infection rate was 33.7%.The H.pylori infection rate in the inflammatory polyps patients and H.pylori patients were 38.2% and 15.8%,respectively.CONCLUSIONS H.pylori infection promotes the formation of gastric inflammatory polyps.The examination and treatment for H.pylori is necessary for the gastric polyps patients.
7.Effect of enteral nutrition support on nutritional status of patients with esophageal cancer complicated with dysphagia after CT guided percutaneous gastrostomy

Objective:To investigate the effect of enteral nutrition support by CT-guided percutaneous gastrostomyon the nutritional status of patients with esophageal cancer complicated with dysphagia during radiotherapy.Methods:Therewere46 cases of esophageal cancer patients with dysphagia treated with CT-guided percutaneousgastrostomy.Others 43 cases of esophageal cancer by oral feeding in patients with dysphagia as control groupduringthe sametimein our hospital radiotherapy center.Patients in the observation group were ingested daily through the gastrostomy,and the nutritional intake of the control group included oral ingestion and intravenous infusion.All patients were measuredthe body height,body weight (BW).body mass index (BMI),Serum levels of serum albumin (ALB),pre-albumin (PA) and hemoglobin (HB) before and after radiotherapy.We also observed the incidence of acute radiation esophagitis and the completion of the treatment plan during radiotherapy in both groups,and to observe the two groups of patients the incidence rate of radiotherapy and treatment plan during the completion of acute radiation esophagitis.Results:There was no significant difference in BW,BMI,ALB,PA,HB before radiotherapy between the two groups (t =0.84,0.63,-1.07,-0.81,1.48,P > 0.05).The BW,BMI,ALB,PA and HB of the observation group were significantly higher than those of the control group at the end of radiotherapy,which werestatistically significant (t=3.30,4.65,6.82,43.56,31.91,P < 0.01).During the radiotherapy,the total incidence of acute radiation esophagitis in the observation group was significantly lower than that in the control group,(x2=3.971,P< 0.05).In addition,the completion rate of the observationgroup was significantly higher than that of the control group (x2 =6.811,P < 0.01).Conclusion:To the Patients with dysphagia of esophageal cancer,enteral nutrition byCT guided percutaneous gastrostomy,can improve the malnutrition,the immune function of the patients and reduce the acute radiation esophagitis during radiotherapy and ensure the successful completion of the treatment plan.

Object To study the chemical structures and DNA cleavage activity of the water-soluble constituents from Polygonum bistorta L.Methods To isolate the constituents by reverse phase chromatography, and characterize their structures by the analysis of chemical property and spectral data.Results Ten compounds were isolated from the 60% acetone extract of the rhizoma from P.bistorta.Their structures were elucidated as gallic acid (Ⅰ), tryptophan (Ⅱ), 2, 6-dihydroy-bezoic acid (Ⅲ), (+)-catechin (Ⅳ), chlorogenic acid (Ⅴ), (-)-epicatechin-5-O-?-D-glucopyranoside (Ⅵ), (+)-catechin-7-O-?-D-glucopyranoside (Ⅶ), 1-(3-O-?-D-glucopyranosyl-4, 5-dihydroxy-phenyl)-ethanone (Ⅷ), (+)-catechin-5-O-?-D-glucopyranoside (Ⅸ) and (-)-epicatechin (Ⅹ), respectively.Conclusion Compounds Ⅱ, Ⅲ, Ⅴ-Ⅹ were isolated from the plant for the first time.Compounds Ⅰ, Ⅳ, Ⅵ, Ⅶ, Ⅸ, Ⅹ showed significant DNA cleavage activity.

Methods To isolate the constituents by reverse phase methods, and characterize their structures by the analysis of chemical property and spectral data. Results Six compounds were isolated from the 60% aqueous acetone extract from the rhizome of P. cuspidatum. Their structures were elucidated as reveratrol (Ⅰ); piceid (Ⅱ); 2, 3-dihydro-2-(4′-O-?-D-glucopyranosyl-3′-methoxy-phenyl)-3-hydroxymethyl-5-(3-hydroxypropyl)-7-methoxybenzofuran (Ⅲ); 2, 6-dimethoxy-p-hydroquinone-1-O-?-D-glucopyranoside (Ⅳ); 5, 7-dihydroxy-isobenzofuran (Ⅴ) and 5, 7-dihydroxy-isobenzofuran-7-O-?-D-glucopyranoside(Ⅵ), respectively. Conclusion Compounds Ⅲ-Ⅵ are isolated from the plant for the first time. Compounds Ⅰ-Ⅵ show no DNA cleavage activity. Compound Ⅱ exhibits weak cytotoxicity against two human cancer cell lines (KB and MCF-7) in vitro.
10.Nursing effect of personalized traditional Chinese medicine nursing mode for patients with subtotal ;gastrectomy

Objective To explore the application effect of individualized traditional Chinese medicine nursing mode in the nursing of patients with subtotal gastrectomy. Methods A total of 60 eligible patients who met the standard were divided into the control group and the intervention group with 30 cases in each group according to the random number table method, the control group received only normal nursing while the intervention group was subjected to the individualized TCM nursing based on the normal nursing. The nursing effect was compared between two groups. Results Scores of negative mood of patients of the intervention group before surgery, electrogastrogram slow wave 3 h, the first day and the second day after the operation, first feeding time after operation, early ambulation time, average length of stay was (16.50 ± 3.20) points, 2.58 ± 0.46, 2.88 ± 0.08, 2.99 ± 0.54, (28.70 ± 4.43) h, (29.09 ± 4.55) h, (8.24 ± 1.38) d. The control group was(16.98 ± 2.61)points, 2.40 ± 0.53, 2.79 ± 0.14, 2.71 ± 0.30,(31.45 ± 3.80)h,(32.91±2.28)h,(10.40±1.99)d, there were significant differences between the two groups (t=2.53-25.19, P < 0.05). In the third day after operation, the electrogastrogram slow wave in the intervention group and the control group were 2.94 ± 0.53, 2.93 ± 0.12, there was no significant difference between the two groups (t=0.40, P>0.05). Conclusions Individualized TCM nursing can effectively alleviate the patients′ negative mood, shorten the recovery time of postoperative electrogastrogram slow wave, first feeding time and early ambulation time, to restore the gastrointestinal function of patients earlier, effectively promote the rehabilitation of patients;at the same time, shorten the hospitalization time.
