Medical students′ professional spirit cultivation is an important content as well as a basic require-ment of the Preeminent Doctor Education and Training Plan. Currently, there are several problems in medical tal-ents′professional spirit cultivation, such as the unstandardized curriculum, the lack of subjective experience, and the ignorance of the unity of education stages, student differences and coherence. This study intends to explore thedouble teacher method, in which both clinical teaching doctors and medical ethics teachers engage in the medi-cal students′professional spirit cultivation. Its purpose is to build a bridge between medical ethics education in uni-versities and the clinical practice, and further to integrate them through doctors′ and teachers′ joint participation, which will significantly enhance medical students′practice ability of medical moral.

Objective To study the level and clinical significance of serum interleukin-18(IL-18)in patients with acute cerebral infarction.Methods The level of serum IL-18 in patient with acute cerebral infarction(n=90)and normal controls group(n=45)were detected by enzyme linked immunosorbent assay.Results The level of serum IL-18 in large,medium and small lesion group were(288.4±59.3);(237.5±58.0);(186.9±50.6)ng/L respeetlvely,which were significantly higher than those in normal control group.The level of serum IL-18 in control group was(147.2±42.8)ng/L(P＜0.05).large lesion group was significantly higher than that in medium and small lesion group(P＜0.05).medium lesion grouP was significantly higher than that in small lesion group(P＜0.05).The levd of serum IL-18 in heavy,medium and light group were(284.9±57.3);(233.4±55.9);(184.5±48.3)ng/L respectively,which were significantly higher than those in normal control group(P＜0.05);heavy group was slgnificantly higher than that in medium and light group(P＜0.05),medium group was significantly higher than that in light group(P＜0.05).Conclusion The level of serum IL-18 was related to the severity of cerebral infarction,IL-18 may be involved in the pathogenesis of the hypoxic ischemic brain injury.

Method: Two hundreds cases of chronic prostatitis were randomly divided into the treatment group and control group, 100 cases in each group, to observe the therapeutic effects in the clinical symptoms before and after acupuncture treatments and compare the therapeutic effects of the acupoints between the two groups. Results: Clinical subjective symptoms and examination of prostatic fluid were obviously improved in the treatment group (Group Ⅰ) than in the control group (Group Ⅱ), and the effective rate was 100% in the treatment group and was 91% in the control group, in a significant difference (P ＜ 0.01).Conclusion: There is a remarkable therapeutic effect in the treatment of chronic prostatitis by puncturing four acupoints at the root of penis in combination with related acupoints.

This study was aimed to observe effect of combined therapy of acupuncture, tapping and auricular needling on fatty-insulin endocrine axis in obesity complicated with hyperlipidaemia patients withyin-deficiency and blood-stasis pattern. A total of 108 patients with obesity complicated with hyperlipidaemia ofyin-deficiency and blood-stasis pattern were randomly divided into the treatment group of combined therapy of acupuncture, tapping and auricular needling, and the acupuncture and tapping group, with 54 cases in each group. The comparison was made to 52 patients in the normal group. Observations were made on changes of fatty-insulin endocrine axis index, lipid index, obesity index, vegetative nerve function index (Y value) and clinical weight-reducing and lipid-reducing effect. The results showed that the pretreatment obesity index, fasting insulin (FINS), fasting plasma glucose (FPG), insulin resistance index (Homa-IR), fasting leptin (FLP), triglyceride (TG), total cholesterol (TC), low density lipoprotein (LDL-C) and Y value of patients in both groups were significantly higher than those of the normal group (P < 0.01). However, the high density lipoprotein (HDL-C) and insulin sensitivity index (ISI) were significantly lower than those of the normal group (P < 0.01). After treatment, both groups received satisfied effects on weight-reducing and lipid-reducing. The fatty-insulin endocrine axis indexes (FINS, FPG, Homa-IR and FLP) were significantly reduced (P < 0.05); and ISI was significantly increased (P < 0.05). The combined therapy of acupuncture, tapping and auricular needling had better regulation effects on the fatty-insulin endocrine axis level, lipid level and vegetative nerve function level compared with the acupuncture and tapping group (P < 0.05). It was concluded that patients with obesity complicated with hyperlipidaemia ofyin-deficiency and blood-stasis pattern had dysfunction of the fatty-insulin endocrine axis. The combined therapy of acupuncture, tapping and auricular needling, or the therapy of acupuncture and tapping had good regulation effect. However, the combined therapy of acupuncture, tapping and auricular needling had more significant therapeutic effect.

Objective To evaluate the expression of peroxisome proliferator-activated receptor γ(PPARγ)and retinoid x receptorα(RXRα)in cholangiocarcinoma to explore its relation to clinicopathological factors and investigate its impact on prognosis.Methods Expression of PPARγ and RXRα was detected by immunohistochemistry in 69 cases of cholangiocarcinoma and 12 non-tumor cases.The relation of PPARγ and RXRα expression to clinicopathological parameters and prognosis was determined.Results The expression rates of PPARγ in 69 cases of cholangiocarcinoma and 12 non-tumor cases were 59.4% and 0, respectively(P＜0.05).The expression rates of RXRα in 69 cases of cholangiocarcinoma and 12 non-tumor cases were 78.3% and 20%, respectively.There were significant differences between the 2 groups in PPARγ and RXRα expression.PPARγ expression was associated with TNM clinical stages and lymph node metastasis.According to univariate survival analysis,PPARγ expression was correlated with poor prognosis(P＜0.05).There was positive correlation between PPARγ and RXRα.Conclusion The expression of PPARγis significantly correlated with the clinicopathological characteristics and biological behaviors of cholangiocarcinoma.PPARγ and RXRα expression may play an important role during tumorigenesis.

Objective To investigate the survival rate, voiding, sexual function after nerve-sparing surgery with extended systematic extraperitoneal lateral lymphadenectomy in lower rectal cancer. Methods Three hundreds and ninety-two cases with advanced lower rectal cancer who underwent nerve-sparing surgery with radical dissection from 1996 to 2000 were reviewed. Among them,173 cases only cleared in abdominal cavity,219 cases coupled with extraperitoneal lateral lymphadenectomy. Results The metastatic rate of lateral lymph node was 17.8%(39/219),the rate of non-modal foci of metastatic disease in lateral out of the abdominal cavity was 5.9%(13/219).Local recurrence rate, the abdominal cavity group was 16.2%(28/173); the coupled group was 9.6%(21/219), P

Objective This study was to observe the biological effects of eprenone on proliferation ,migration and apoptosis in human gastric epithelial cell line .Methods Human gastric epithelial cells GES‐1 were cultured in vitro .MTT assay were used to e‐valuate the proliferation of GES‐1 cells in different concentrations of teprenone and ensure the appropriate drug concentration .T ran‐swell test and scratch test were used to detect the migration ability of GES‐1 cells treated with appropriate concentration of eprenone .Flow cytometry analysis were used to detect the apoptosis of GES‐1 cells treated by the appropriate concentration of eprenone .Results Treated with eprenone for 24 h ,the proliferation of GES‐1 cells were increased as the concentration of teprenone from 10 to 80 μmol/L ,but from 80 to 320 μmol/L ,the promoting effect showed no staticall significant changes .So the appropriate drug concentration was determined to be 80 μmol/L .Treated with teprenone (80 μmol/L) for 24 h ,the transwell test showed that the migration rate of the teprenone group was 3 .338 ± 0 .293 and the control group was 1 .328 ± 0 .208 .So the number of staining blue cells in eprenone group were more than in control group obviously under membrane of transwell chambers (P<0 .01) .Scratch test showed that the migration rate of the eprenone group was 1 .00 ± 0 .18 and the control group was 0 .72 ± 0 .08 .Similarly ,the migration rate of eprenone group was higher than the control group(P<0 .05) .Treated with teprenone (80 μmol/L) for 48 h ,the apoptosis rate of the teprenone group was (11 .90 ± 1 .53)% and the control group was (25 .61 ± 0 .15)% ,the cellular apoptosis of eprenone group was lower than the control group (P<0 .01) .Conclusion Teprenone can promote the proliferation and migration , inhibit the apoptosis of GES‐1 cells .

To explore the utility of apparent diffusion coefficient(ADC)histogram analysis for differentiating genetic subtypes of diffuse lower-grade gliomas. A total of 55 patients with WHO grade Ⅱ/Ⅲ diffuse lower-grade gliomas who underwent preoperative routine brain magnetic resonance imaging and diffusion weighted imaging in our center were retrospectively evaluated.Among whom there were 14 patients with isocitrate dehydrogenase(IDH)wild-type gliomas(IDH group),19 patients with IDH-mutant 1p19q intact gliomas(IDH 1p19q group),and 22 patients with IDH-mutant 1p19q co-deleted gliomas(IDH 1p19q group).The whole-lesion ADC values derived from histogram analysis(including ADC,ADC,ADC5%,ADC10%,ADC25%,ADC50%,ADC75%,ADC90%,ADC95%,ADC,mode,range,skewness,kurtosis,standard deviation,inhomogeneity,and entrophy)were measured for each patient.All parameters between the different genetic subtypes were compared by using the Student's test or Mann-Whitney test.Receiver operating curve(ROC)analysis was used to assess the diagnostic performance of ADC histogram in distinguishing the different genetic subtypes. Compared with IDH group,the ADC75%(=0.021),ADC90%(=0.015),ADC95%(=0.014),ADC (=0.035),range(=0.009),standard deviation(=0.001)and inhomogeneity(=0.001)were significantly lower in IDH group;in contrast,the ADC (=0.031)and kurtosis(=0.020)of IDH group were significantly higher than those in IDH group.The ADC(=0.010),ADC5%(=0.016),ADC10%(=0.012),ADC25%(=0.007),ADC50%(=0.005),ADC75%(=0.015),and mode(=0.002)were significantly higher in IDH 1p19q group than in IDH 1p19q group.Inhomogeneity achieved the highest area under ROC(AUC)(0.811)in differentiating IDH gliomas and IDH gliomas,with a cutoff value of 0.229;the sensitivity and specificity were 85.7% and 73.2%.The mode achieved the highest AUC(0.744)in differentiating IDH 1p19q gliomas and IDH 1p19q gliomas,with a cutoff value was 1448.75×10 mm /s;the sensitivity and specificity were 57.9% and 90.9%. ADC histograms analysis may be helpful to differentiate genetic subtypes in lower-grade gliomas.

Objective: To develop a new determination method of astrogaloside in s erum by spectrophotofluorimetry. Methods: The fluorescence char acteristics of astragaloside anisic reaction product were used to determine astragaloside in serum. Re sults: The reaction product of astrogaloside and anisic acid had excitatio n and emission maxima were at 320 and 387 nm, respectively. The linear range of determ inati on was between 0.2 and 40.0 mg*L －1. The detection limit was 0.02 mg*L －1.　The recovery of measurement was between 98.8% and 102.4%. Conclusion: The propos ed method has advantages of high sensitivity, low detection limit and no interference. It has been used to determine astragaloside in serum successfully.
