2.A comparative study of radiographic lumbar Modic changes

Objective To analyze the imaging performances and significance of Modic changes on X-ray,CT and MRI.Methods The imaging data (MR,CT and X-ray)of patients with low back pain were retrospectively analyzed.The selected patients’images were studied on the same work station by the two physician experts in muscle and bone imaging.A double-blind method was used. Firstly,MRI was used to sort the MC.Then,the alterations of the density in the lesions on X-ray plain film and CT image were measured.if the judgments of the two readers were inconsistent,another professor of radiology will join the discussion and consist-ent results will be got finally.The data were recorded using Excel 2010,and processed for statistical analysis using SPSS 1 6.0.Re-sults A total of 47 cases that met the requirements were collected which included 1 6 males and 31 females,aged from 32 to 76 years,with a median age of 56.0 years.A total of 93 on MRI endplate were changed.However,a part of lumbar endplates with MC on MRI images were displayed as higher density than normal vertebral body on the X-ray and CT reconstruction images (total of 36).The occurrence rates of Type Ⅰ,Type Ⅱ,Type Ⅰ/Ⅱ were 40.0%,14.8%,58.3%,respectively.But the occurrence rate of Type Ⅲ (100%)and hybrid Type which contained type Ⅲ were 100%.Conclusion Some Modic typeⅠ,Ⅱ,Ⅰ/Ⅱchanges in X-ray plain film, CT reconstruction images had a higher density,speculating that there is reactive new bone formation,which reflects the internal ver-tebral body bone mineralized rather than the change of the bone marrow.

Objective To investigate the changes in the expression of inflammatory cytokines in serum and aorta of rats with chronic renal failure.Methods Animal model of chronic renal failure was established by single nephrectomy and partial ligation of the contralateral renal artery.The rats were randomly divided into three groups(10 each):normal control group(Con),chronic renal failure group(CRF),rats with chronic renal failure and treated with proteasome inhibitor MG-132 group(CRF+M).The protein expression of TNF? was detected by immunohistochemistry and image analysis,the levels of TNF? and IL-1? of peripheral blood mononuclear cells(PBMC) in blood plasma and culture supernatant were detected by ELISA.Results The protein expression of TNF? 4 and 6 months after operation significantly increased in aorta of CRF group compared with that in CON group(10.7?1.3 vs 4.3?1.7,12.6?2.2 vs 4.3?1.1,P
5.Determination of Fuyinjie Compound Huangsong Lotion by GC-MS

AIM: To analyze the chemical composition and their relative contents of Fuyinjie Compound Huangsong Lotion. METHODS: A capillary column HP-5 MS was used. The column temperature was controlled by a program and the MS analysis was performed with EI and quadrupole mass analyzer. The chemical composition was identified by NIST98 searching and mass spectra comparing, and their relative contents were determined by using normalization method of chromatographic peak areas. RESULTS: There were 39 components found and 31 composed of more than 90% separated components were identified. CONCLUSION: The GC-MS is a simple, rapid and sensitive method for the chemical composition analysis of Fuyinjie Compound Huangsong Lotion.
6.Pharmacodynamics and pharmacokinetics of epidural ropivacaine in patients undergoing renal transplantion

Objective To investigate the pharmacodynamics and pharmacokinetics of ropivacaine for epidural anesthesia in renal transplantion. Methods Eight patients undergoing renal transplantation (group R) and eight patients with normal renal function (group C) were included in this study. Nine patients were male and seven female. Age ranged from 27-52 years and body weight from 46-89 kg. Radial artery was cannulated under local anesthesia for blood pressure monitoring and blood sampling. An epidural catheter was placed at L1-2. An epidural bolus of 0.75% ropivacaine 16 ml was given. The level of sensory block was tested every 3 min within 30min after epidural ropivacaine and then every 15 min.The degree of motor block was assessed using Bromage score every minute (0 = the patient can move his/her lower limb freely, 3 = the patient can not flex his/her hip, knee and ankle) . Arterial blood samples were taken before, during and after surgery for determination of plasma concentration of Alphal acid glycoprotein (?1-AGP) using an ultrafiltration technique. Results Demographic data including gender, age and body weight were comparable between the two groups. The onset time of sensory block in group R (20.00?4.63)min was significantly shorter than that in group C (32.14?8.09) min, while duration of sensory block in group R (60.00?27.69) min was significantly tonger than that in group C (43.13 ? 27.64) min. Objective To understand the basic circumstance of the hospital infection,and to improve the knowledge of medical staff on the management of hospital infection.Methods With bedside observation and examination combined with checking clinical case history,462 inpatients were investigated on current rate of hospital infection and its related factors.Results There were 30 inpatients suffering from hospital infection,the current rate of the hospital infection was 6.49 %;among them,the rate of the lower respiratory tract accounted for 33.00%.The current rates of hospital infection in inpatients with antibiotics(16.56%),or with treatment of urinary catheters(18.52%),or with operation(16.30%),with more than one month hospitalization(19.05%) were significantly higher than those of inpatients without antibiotics(1.00%),without treatment of urinary catheters(5.75%),without operation(4.05%) or with less than one month hospitalization(4.55%)(all P

Objective To explore the effect of psychological intervention on patients with advanced ovarian cancer. Methods A total of 80 patients with advanced ovarian cancer patients combined with carboplatin or Western him paclitaxel combined with cisplatin in the treatment of patients, the implementation of grouping according to nursing, patients in the control group for routine care, patients in the observation group in the control group on the basis of psychological nursing. The two groups were observed and compared the clinical effect of intervention. Results The observation group the effective rate of the treatment and nursing satisfaction rate were significantly higher than the control group; the implementation of group nursing, observation group patients psychological status and improve the psychological status of patients in the observation group after treatment were better, and the control group have obvious difference. Conclusion For patients with advanced ovarian cancer chemotherapy, combined with psychological intervention on the basis of routine nursing, can significantly improve the treatment efficiency, improve the psychological status of patients, is worthy of clinical application
9.Multi-mode analgesia of celecoxib after hip and knee replacement

BACKGROUND:Hip and knee replacement is a common surgery in the clinic; the procedure is relatively complex; and the risk of surgery is relatively high, so that postoperative analgesia is not satisfactory. Perioperative pain management has been a clinical concern. To find safe and effective analgesia has become one of the important tasks of joint surgeons. OBJECTIVE:To investigate the effect of celecoxib on multi-mode analgesia after hip and knee replacement. METHODS: A total of 80 cases undergoing hip and knee replacement in the Chongqing Dongnan Hospital from September 2012 to September 2013 were enroled in this study. These patients were randomly divided into two groups. In the control group, celecoxib was not used for analgesia after replacement. In the experimental group, celecoxib was used after replacement. The pain was observed at 1-5 days after surgery in the two groups. When the analgesic pump was removed, the drug dosage and opioid analgesics use were recorded. Side effects of drug use were also recorded. RESULTS AND CONCLUSION:In terms of analgesic efficacy, the analgesic effect was better in the experimental group than in the control group (95%, 85%, P < 0.05). 95% patients in the experimental group were satisfied with the analgesia, which was significantly higher than in the control group (65%;P< 0.05). No significant difference in pain visual analogue scale score was detected between the two groups immediately, 4 and 5 days after surgery (P > 0.05). Pain visual analogue scale score was significantly lower in the experimental group than in the control group at 1, 2 and 3 days post surgery (P< 0.05). The drug dosage was significantly more in the experimental group than in the control group after surgery (P < 0.05). The frequency of opioid use in the experimental group was significantly lower than in the control group (P < 0.05). The complication rate was significantly lower in the experimental group (8%) than in the control group (18%) (P < 0.05). These findings demonstrate that the analgesic effect of celecoxib was ideal after hip and knee replacement using multi-mode analgesia, which can reduce the dose of analgesic drugs and have smal adverse reaction.
10.Study and clinical application of external fixator with casual pining fixation and biologic stress compression

Objective To remedy the general defects of external fixator such as poor stability,complexed operation,strong stress shielding and bad extension function. Methods A total of 102 patients with limb shaft fracture were treated with the self-made external fixation and the curative effect observed. There were 84 males and 18 females aged at 5-78 years. Of all, 76 cases had tibial and fibular fractures, 10 femoral shaft fractures (children), 4 intertrochanteric or subtrochanteric fractures, 7 ulnar or radial fractures and 5 humeral shaft fractures. Results All patients got good results through a all-course observation, among which 76 cases with tibial and fibular fractures were followed up for 3 months to one year. Furthermore, the mean substantial bone healing time was only 3.5 months. The joint functions of the knee and the ankle recovered synchronously, with an overall excellence rate of 93.4%. No displacement of the fractured part or further fracture occurred. Conclusions The structure and function of the self-made external fixation accord with the demanding of current external fixator in orthopaedics.
