OBJECTIVE To investigate the therapeutic effect of modified uvulopaoatopharyngoplasty(UPPP)on obstructive sleep apnea hypopnea syndrome(OSAHS).METHODS Sixty eight cases of OSAHS were treated with modified UPPP.During operations,the uvula was preserved,the fat tissues of space veli palatine were dissected,palatoplasty and pharyngoplasty were performed.All the patients were followed up for over 6 months.Polysomnography(PSG)was monitored again one year after operation.RESULTS The symptoms of dyspnea,snoring,sleepiness were disappeared or improved obviously after 6 months.The effective rate was 95.6% at one year after operation.CONCLUSION The modified UPPP not only extends the oropharyngeal cavity,but also avoids postoperative complications and improve the effectiveness of UPPP.

Objective To investigate the effects of special siRNA targeting TLR4 gene (TLR4-siRNA) on hyperoxia-induced secretion of inflammatory factors from human alveolar epithelial cell line A549.Methods Human alveolar epithelial cell line A549 was purchased from Chongqing Children Hospital and cultured in six-well tissue culture plates and randomly divided into 4 gorups:A549 cells + ambient air group (group C) ; A549 cell + hyperoxia group (group H); A549 cells transfected with TLR4-siRNA + hyperoxia group (group TR) and A549 cells transfected with control-siRNA + hyperoxia group (group NR).TLR4-siRNA and control-siRNA were transfected into A549 cells through lipofectamine 2000.The efficiency of transfection was assessed by detection of FAM expression using flow cytometry.The cells were exposed to 95％ O2-5％ CO2 delivered at 1 L/min for 30 min in groups H,TR and NR.TLR4 mRNA and protein expression was detected by RT-PCR and cytometry respectively.The NF-κB activity in A549 cells was measured by EMSA and the IL-6 and IL-8 levels in supernatant were determined by ELISA.Restults Hyperoxia significantly up-regulated TLR4 mRNA and protein expression and increased NF-κB activity in cells and IL-8 and IL-6 concentrations in supematant in group H as compared with group C.Transfection with TLR4-siRNA significantly attenuated hyperoxia-induced up-regulation of TLR4 mRNA and protein expression and increase in NF-κB activity in the cells and IL-6 and IL-8 concentrations in supernatant in group TR as compared with group H,but transfection with control-siRNA did not.Conclusion TLR4-siRNA could significanly inhibit hyperoxia-induced secretion of inflammatory factors from human alveolar epithelial cell line A549.

Objective To investigate the effect of magnesium sulfate on epidural labor analgesia with ropivacaine in the patients with preeclampsia. Methods Seventy nulliparous parturients with pre?eclampsia, aged 23-34 yr, of American Society of Anesthesiologists physical status Ⅱ or Ⅲ, weighing 63-81 kg, with a singleton fetus in vertex presentation, without contradications to neuraxial anesthesia, waiting for vaginal delivery, received epidural analgesia for labor. The patients were divided into magnesi?um sulfate group and control group using a random number table, with 35 patients in each group. In magne?sium sulfate group, magnesium sulfate 50 mg∕kg ( 30 ml) was infused intravenously over 15 min when their cervical dilation was 3 cm, while the equal volume of normal saline was given in control group. Epi?dural labor analgesia was performed with ropivacaine. The up?and?down sequential allocation was used to determine the median effective concentration of epidural ropivacaine ( EC50 ) . The severity of pain was as?sessed with visual analogue scale score. Effective analgesia was defined as visual analogue scale score of≤1. The initial concentration of ropivacaine was 0.15%. Each time the concentration was increased∕decreased according to whether or not analgesia was effective, and the ratio between the two successive concentrations was 0.9. The EC50 and 95% confidence interval ( CI) of ropivacaine for epidural labor analgesia was calcu?lated. Results The EC50 (95% CI) of ropivacaine for epidural labor analgesia was 0.066% (0.062%-0.071%) in magnesium sulfate group. The EC50 (95% CI) of ropivacaine for epidural labor analgesia was 0.078% (0.072%-0.085%) in control group. The EC50 of ropivacaine was significantly lower in magnesi?um sulfate group than in control group ( P<0.05) . Conclusion Magnesium sulfate can enhance the effica?cy of ropivacaine for epidural labor analgesia in the patients with preeclampsia.

OBJECTIVE To investigate the role of brain-derived neurotrophic factor(BDNF) and its receptor TrkB in the development of allergic rhinitis(AR). METHODS SD rats were randomly divided into allergic rhinitis group(AR group, n=20) and control group (Con group, n=10) . A model of allergic rhinitis in health SD rat was established by using ovalbumin intraperitoneal immunization and nasal antigen challenge. The nasal mucosa obtained from 18 out of 20 AR models as well as 10 normal controls were studied with immunohistochemistry (Envision System) method to observe the expression and distribution of BDNF and TrkB. RESULTS Immunohistochemical staining showed that the distribution of BDNF and TrkB in nasal mucosa from normal rats was in accordance with that in the nasal mucosa from rats with allergic rhinitis on the whole. The study showed that immunoreactivity to BDNF (or to TrkB) was present both on the surface and in the cytoplasm of epithelial cells, glandular cells and their duct cells. The statistical analysis of imumunohistochemical staining showed that the IOD of BDNF in rat nasal mucosa with allergic rhinitis (77344.17?8567.02) was obviously higher than that in the normal rat nasal mucosa (54124.82? 18515). Objective To study the effect of growth hormone (GH) and PRL (prolactin) on the secretion of Th1/Th2 type cytokines in the patients with systemic lupus erythematosus (SLE). Methods The secretion of cytokines in PBMCs stimulated by GH and PRL was detected by ELISA in vitro. Results The results showed that the secretion levels of IL-6 and IL-10 from PBMCs of the SLE patients in active stage were higher than those of the normal controls, and the secretion level of IFN-? was lower than that of SLE patients in resting stage and the normal controls, but it is of no statistical significance. The secretion levels of IL-6 and IL-10 were significantly increased in PBMC stimulated by GH and PRL, but GH and PRL had no effect on the secretion of IFN-?. There was no difference on the secretion level of IL-6, IL-10 and IFN-? when the PBMC was stimulated by GH and PRL. Conclusion GH and PRL might play an important role in the pathogenesis of SLE.

Recent studies show that SPE also has some therapeutic effects on chronic prostatitis, prostate cancer, sexual dysfunction, and so on. This article presents an overview on the application of SPE in the treatment of BPH, prostate cancer, and chronic prostatitis/chronic pelvic pain syndrome, with a discussion on its action mechanisms.
