1.Analysis of Drug Resistance for 227 Strains Enterococci in Our Hospital

OBJECTIVE:To study the enterococcal distribution in clinical samples and their drug resistance to commonly use antibiotic and provide the guidance for rational administration.METHODS : Adopting paper diffusion method to observe the drug resistance of enterococci isolated from clinical samples in our hospital and analyzing their distribution in the samples.RESULTS :Enterococcus faecalis accounts for 44.1% in 227 strains and Enterococcus faecium accounts for 37.9%,the other kinds of enterococci were rare.There was no statistical difference between drug resistance of Enterococcus faecium and that of enterococcus faecalis. Vancomycin was the most potent antimicrobial.Penicillin,ampicillin,and nitrofurantion showed good antibacterial activity against enterococcus faecalis. Enterococcus faecium displayed high-level resistance against antibiotics in addition to glycopeptides.CONCLUSION:Doctor should pay attention to the drug resistance about clinically common pathogenic bacterium. Continual surveillance of resistance rate of enterococcus is important to direct the treatment of enterococcus infections.
2.Current diagnosis and therapy of anterior mandibular fracture associated with condyle fractures.

Anterior mandibular fracture, which includes symphyseal and parasymphyseal mandibular fractures, is one of the most common types of fracture in clinical work, and is usually associated with condyle fracture. This type of fracture predisposes the patients to facial widening, malocclusion, and ankylosis when not properly and timely treated because of the influence of its anatomic structure. This article reviews the characteristics, complication, diagnosis, and therapy of this type of fracture.

3.Clinical efficacy on neurogenic bladder after spinal cord injury treated with rehabilitation training and acupuncture-moxibustion.

OBJECTIVETo compare the differences in the clinical efficacy on neurogenic bladder after spinal cord injury (SCD between the matrix needling technique combined with rehabilitation training and rehabilitation training in the patients.
METHODSSixty patients of SCI neurogenic bladder were randomized into an observation group (29 cases) and a control group (31 cases). In the control group, the conventional rehabilitation therapy, the intermittent catheterization and bladder function training were adopted, once every day, for 4 weeks totally. In the observation group, on the basic treatment of the rehabilitation as the control group, the matrix needling technique and acupuncture at sanyin points were applied. The treatment of 8 days made one session, at the interval of 2 days, continuously for 3 sessions, meaning 4 weeks totally. The bladder function score, residual urine volume and bladder capacity were compared before and after treatment in the two groups.
RESULTSThe bladder function score was reduced after treatment as compared with that before treatment in either group (both P<0. 05). The score in the observation group was lower than that in the control group after treatment (P<0. 05). No matter which spinal segment was injured, the residual urine volume after treatment was reduced as compared with that before treatment and the bladder capacity was increased (P<0. 05). The residual urine volume in the observation group was less than that in the control group after treatment and the bladder capacity was more increased (P< 0. 05).
CONCLUSIONThe conventional rehabilitation program in combination with the matrix needling technique and acupuncture at sanyin points achieve the significant efficacy on SCI neurogenic bladder.

4.A literature review on acupuncture and moxibustion for prevention and treatment of lung cancer in SCI iournals (2003-2013).

To explore the status of acupuncture and moxibustion for prevention and treatment of lung cancer in current years; literature regarding acupuncture and moxibustion for prevention and treatment of lung cancer between 2003 and 2013 from SCI journals was retrieved and analyzed. As a result, 20 papers were included, which were published in 17 journals including Journal of Clinical Oncology, Chest, Respirology and Lung Cancer, etc. Of them, 3 papers discussed the effects of acupuncture on progressing of lung cancer; 4 articles confirmed that acupuncture could reduce myelosuppression and digestive tract reactions induced by radiotherapy and chemotherapy; 6 papers showed that acupuncture could relieve pain or fatigue of lung cancer; 3 papers indicated that acupuncture could palliate dyspnea in lung cancer patients. It is concluded by domestic and overseas researches that acupuncture and moxibustion are effective and safe for symptoms of lung cancer, which is worthy of further study.

5.Application of enhanced recovery after surgery in laparoscopy-assisted distal gastrectomy

Objective To evaluate the feasibility and efficacy of enhanced recovery after surgery (ERAS) combined with laparoscopy-assisted distal gastrectomy (LADG) for gastric cancer. Methods 60 patients with distal gastric cancer from April 2016 to September 2016 in Shanxi Cancer Hospital were enrolled. According to the admission time, the patients were numbered randomly by odd and even. 30 odd patients received conventional treatment (traditional group), and were even 30 incorporated into ERAS group. Record and comparison of two groups were conducted, including age, sex, weight, body mass index (BMI), 1-day preoperative serum albumin (ALB) level, operation time, anastomosis type, total intraoperative blood loss, postoperative TNM staging, bowel movement recovery time, postoperative hospital stay, total cost of hospitalization, complications, and serum ALB level after operation. Results The level of ALB in ERAS group was (36.16 ± 2.46) g/L at 4th day after operation, and (39.61±2.03) g/L at 7th day after operation, which were higher than those in traditional group [(34.38 ±2.31) g/L and (37.98 ±1.96) g/L, respectively], with stable changes and statistical difference (P= 0.006, P= 0.003). The recovery time of intestinal peristalsis in ERAS group was shorter than that in traditional group [(51.23±9.05) h vs. (58.97±9.61) h, P= 0.003], and the total hospitalization cost in ERAS group was less than that in traditional group [(7.23±0.34) × 104 yuan vs. (7.58± 0.37) × 104 yuan, P< 0.001]. There were no significant differences in age, sex, weight, BMI, 1-day preoperative serum ALB level, operative time, anastomosis type, total intraoperative blood loss, postoperative TNM staging, complication and the postoperative hospital stay between the two groups (all P > 0.05). Conclusion The combination of ERAS and laparoscopic-assisted distal gastrectomy is safe and effective compared with traditional ways, which can relieve the stress reaction, promote the recovery of gastrointestinal function, reduce the hospitalization cost, and accelerate the rehabilitation of patients.
6.Discussion on the strategies for maintenance management of medical devices of hospital based on the risk analysis

Objective:To discuss the management and maintenance of medical equipment in hospital, the department of biomedical and engineering should develop a reasonable management plan to meet the needs of using safety management of medical equipment based on the risk analysis.Methods: Make up a strategy based on the analysis of application risk according to the clinical function, physical risk, fault avoidable probability and the adverse events recorded, and conduct the preventative maintenance and safety performance inspection. Results: The focus of clinical engineering professionals change from passive acceptance to repair equipment to preventive equipment maintenance, and carry out safety performance inspection to reduce the risk caused by the failure of equipment which is harmful to the patients.Conclusion: The clinical engineering professionals need to pay attention to the application risk and improve the management strategy while they improve the technical level.
7.Effect of cerulenin combined with gemcitabine on proliferation and apoptosis in human pancreatic cell line BxPC3

Objective To investigate the effect and mechanism of action in pancreatic cancer BxPC3 cell treated by cerulenin in combination with gemcitabine.Methods BxPC3 cells were cultivated with 10 μg/ml cerulenin,20 μmol/L gemcitabine or 10 μg/ml cerulenin + 20 μmol/L gemcitabine for 48 h,and cells without treatment were control.Cell proliferation was detected by CCK-8 assay,and early apoptosis was detected by AnnexinV/PI double staining method.The expression of Bcl-2 mRNA and Bax mRNA were detected by RT-PCR,and the protein level of Bcl-2,Bax were detected by Western Blot.Results The inhibition rate of BxPC3 cells were 0,(51.28 ± 1.84) %,(53.59 ± 1.62) %,(84.57 ± 1.01) % in control,cerulenin group,gemcitabine group,combination group; and the rate of early apoptosis was (0.83 ± 0.31) %,(31.37 ± 1.04) %,(38.33 ± 0.75) %,(69.43 ± 0.83) %,and the expression of Bcl-2 mRNA was 0.67 ± 0.01,0.44 ±0.01,0.36 ±0.08,0.27 ±0.07,and the expression of Bax mRNA was 0.14 ±0.01,0.31 ± 0.02,0.32 ± 0.03,0.91 ± 0.06 ; while the ratio of Bcl-2 mRNA/Bax mRNA was 4.78 ± 0.13,1.39 ± 0.04,1.15 ± 0.02,0.30 ± 0.02 ; the expression of Bcl-2 protein was 1.24 ± 0.04,0.51 ± 0.02,0.42 ± 0.02,0.13 ±0.01 ; and the expression of Bax protein was 0.20 ± 0.05,0.47 ± 0.01,0.54 ± 0.01,1.21 ± 0.03 ; while the ratio of Bcl-2/Bax was 6.00 ± 0.11,1.11 ± 0.01,0.77 ± 0.03,0.10 ± 0.06.And the difference among the groups was statistically significant (P < 0.01).Conclusions Cerulenin combined with gemcitabin has synergistic effect on the inhibition of BxPC cells proliferation.Its mechanisms may be up-regulation of Bax,down-regulation of Bcl-2,and promoting apoptosis of cells through the decrease of Bcl-2/Bax ratio.
8.Distribution and clinical significance of genotypes and subtypes of hepatitis B virus in HBV-infected patients

Objective To study the distribution and its clinical significance of hepatitis B virus genotypes and subtypes in HBV-infected patients in Hefei. Methods 192 serum samples from HBV-infected patients were collected to extract HBV DNA. HBV the genotypes and subtypes of HBV were detected by PCR. Results Among 192 HBV-infected patients, 82 had genotype B, 99 had genotype C, and 11 had B/C. In genotype B, 57 were subtype Ba, 8 were subtype Bj, and the remaining 17 were neither Ba nor Bj. In genotype C, 8 were subtype C1, 56 were subtype C2, and the other 35 were neither C1 nor C2. In genotype B/C, 6 were Ba/C2, 2 Bj/C2, 2 Ba/neither C1 nor C2, and one neither Ba nor Bj/C2. Tthe distribution of HBV genotypes and subtypes exhibited significant differences in the various clinical types; and it showed that genotype C was predominant among patients with liver cirrhosis and hepatocellular carcinoma , while genotype B was predominant in asymptomatic carriers. In addition, there were no differences in HBV genotype distribution among patient wih different gender; but the genotype distribution differed significantly between patients positive with HBeAg and those negative with HBeAg. Conclusions Subtypes Ba and C2 are predominant in HBV-infected patients in Hefei. Genotype C may be associated with the development of liver cirrhosis and hepatocellular carcinoma.
9.Clinical application of combined detection of tumor molecular markers in lung cancer

Objective To explore the values of combined detection of carcino embryonic antigen (CEA),carcinoma antigen 125 (CA 125),cytokerain 19 fragment 21-1 (CYFRA2 1-1),carcinoma antigen Scc-Ag (Scc-Ag) and neuron specific enolase (NSE) for diagnosis and treatment of lung cancer.Methods The concentrations of CEA,CA125,CYFRA21-1,Scc-Ag and NSE in 198 patients with lung cancer (lung cancer group),50 with benign pulmonary disease (benign disease group) and 50 healthies (health control group) were detected by electrochemiluminescence immunoassay (ECLIA).The concentration of Pro-GRP31-98 was detected by enzyme linked immunoassay.Results Six tumor markers in lung cancer group showed significant diffcrence compared with those of benign disease group and health control group (P < 0.01).The concentrations of serum tumor molecular markers were significantly higher in Ⅲ,Ⅳ stage group than those in [,Ⅱ stage group.Before the treatment,the concentrations of serum tumor molecular markers in lymph node metastasis group and lymph node recurrence group were significantly higher than those in no lymph node metastasis group and no lymph node recurrence group after the treatment (P < 0.01).The positive rate of CEA,CA125 in lung adenocarcinoma group,CYFRA21-1 and Scc-Ag in lung squamous cell carcinomas group and NSE and Pro-GRP31-98 in small cell lung carcinoma group were significantly different from the other two kinds of pathological types (P < 0.01).The positive rates of CEA and CA125 combinated detection in lung adenocareinoma,CYFRA21-1 and Scc-Ag combinated detection in lung squamous cell carcinomas,NSE and Pro-GRP31-98 combinated detection in small cell lung carcinoma was significantly higher than the individual detection respectively.The sensitivitv rates of CEA,CA125,CYFRA21-1,Scc-Ag,NSE and Pro-GRP31-98 were 52.52 %,51.01%,48.99 %,53.03 %,49.49 % and 50.50 %,respectively.And the combinated detection sensitivity rate of the six tumor markers was 96.46 %,which significantly higher than that of a single marker (P < 0.01).Conelusions The tumor molecular markers of CEA,CA125,CYFRA21-1,Scc-Ag,NSE and Pro-GRP31-98 may be the indicators in diagnosis of lung cancer.The combined detection of serum tumor molecular markers would significantly improve the sensitivity of diagnosis for lung cancers.At the same time,it should be valuablc for determination of clinical stage,histological type and follow-up studies of lung cancer.
10.Clinical effect of pedicle screw single segment fixation through paravertebral muscles on treating patients with thoracolumbar fracture

Objective To investigate the methods and clinical outcome of the operative treatment of thoracolumbar fractures with pedicle screw single segment fixation and the approach through paravertebral muscles.Methods One hundred and twenty patients were selected as our subjects,who were suffered from thoracolumbar fracture without neurological deficit and underwent surgical treatment in the Central Hospital of Zibo from Jan.2009 to Dec.2010.All cases were randomly divided into mini-invasion group and control group.Patients in mini-invasion group were received the single segment fixation of the spatium intermusculare of longissimus muscle and multifidus,and in control group were received the traditional approach with short segment fixation group.The muscle gap approach along the longest muscle and multifidus muscle gap enters,preserving the integrity of the paraspinal muscles.Results The operation periods of mini-invasion group and control group were (53.8 ± 12.3) min and (88.5 ± 18.5) min (t =1.731,P < 0.05),the intraoperative bleeding volume were (95.7 ±21.5) ml and (280.3 ±83.1) ml(t =2.414,P <0.05),the postoperative drainage volume were (20.4±8.2) ml and (132.9 ±50.3) ml(t =2.068,P <0.05) and inpatient time were (13.8 ±2.2)d and (20.4 ±5.5)d(t =1.853,P <0.05).However,there was no significant difference in terms of fracture healing time,visual analogue scale,Oswestry disability index and radiology(P >0.05).All cases had union of fractures and no reduction loss and fixation failure occurred.Condusion The treatment of pedicle screw single segment fixation and the approach through paravertebral muscles on thoracolumbar fracture is proved to be an effective method with minimally invasive,less bleeding volume,simple procedure and fast recovery method.
