1.SYNTHESIS AND BIOLOGICAL ACTIVITY OF 2-[(SUBSTITUTED PHENYL)VINYL] INDOLE DERIVATIVES

AIM To synthesize derivatives of 2-[substituted phenyl)vinyl] indole and find compounds with biological activities by screening in vitro. METHODS Twenty-one compounds of 2-[(substituted phenyl)vinyl] indole were synthesized by reduction, oxidation and Witting reaction. MS, 1HNMR and elemental analysis were used to determine the structures of the new compounds. RESULTS These compounds are new ones. CONCLUSION Six compounds (3,9,11,13,18 and 20) showed effects on some different receptors, such as alpha2, D2 and H1, and is worth further studying.
2.Determination of baicalin, rhein and berberine in Yiqing Capsule by HPLC

AIM: To determine the contents of baicalin, berberine, rhein in Yiqing Capsule(Radix et Rhizoma Rhei, Radix Scutellariae, Rhizoma Coptidis) by HPLC. METHODS: The HPLC system consisted of Kromasil C 18 column (150 mm?4.6 mm, 5 ?m), 0.02 mol?L -1 phosphoric acid (TEA adjusted to pH=6.82)-acetonitrile(40∶11) mixture as mobile phase, detection wavelength at 254 nm, flow rate of 1.0 mL?min -1. RESULTS: The calibration curves of three ingredients were linear within the range of 3.666-73.32 mg?L -1(r=0.9998, baicalin), 0.3112-15.56 mg?L -1(r=0.9998, rhein), 1.221-50.04 mg?L -1(r=0.9999,berberine), respectively. The average recovery of baicalin is 102.3%(RSD=2.5%), berberine for 101.8%(RSD=1.8%), and rhein for 98.3%(RSD=2.9%). CONCLUSIONS: The method is rapid, sensitive and accurate.

4.The influence of stress hyperglycemia on fracture healing of patients with fracture of shaft of femur pro-operation

ObjectiveTo investigate the influence of stress hyperglycemia on fracture healing of patients with fracture of shaft of femur pro-operation.MethodsTo retrospectively analyze the clinical data of 65 patients with fracture of shaft of femur.Then according to the blood sugar level of the initial admission,all patients were divided into high glucose group 41 cases and no high glucose group 24 cases.The difference of fracture healing time was compared between both groups.ResultsThe mean clinical fracture healing time in high glucose group was ( 11.5 ± 1.3 ) weeks.ConclusionOur study found that there was relationship between blood glucose level and fracture healing in patients with fracture of shaft of femur pro-operation,patients with stress hyperglycemia pre-operation seemed to have longer fracture healing time than the ones without.
5.Drug-induced Edema Caused by Traditional Chinese Medicine Injections:Literature Analysis of 61 Cases

OBJECTIVE: To discuss the general regularity and the characteristics of the drug-induced edema caused by traditional Chinese medicine injections (TCMI) so as to provide basis for rational drugs use in the clinic. METHODS: 61 cases of drug-induced edema caused by TCMI publicly reported in domestic journal from 1994 to 2007 were analyzed statistically. RESULTS: The drug-induced edema caused by TCMI was not linked to sex and age. The symptom appeared in a short period and edema was induced in 44 cases within 30 min (72.1%). Laryngeal edema accounted for 54.1% and ocular angioedema 34.4%. 23 kinds of TCMI were involved, among which top 5 drugs in the list of incidence were Qingkailing injection, Shuanghuanglian injection, Yuxingcao injection, etc. CONCLUSION: Great importance should be attached to the monitoring of the clinical use of TCMI to promote the safety of TCMI.
6.Reconstruction of soft and hard tissue around the tooth in aesthetic areas: a case report

Gingival recession or root exposure is associated with the loss of soft and hard tissue around the root. Gingival recession not only leads to aesthetic problem, but also affects tooth function, and even causes the loss of tooth. A case was diagnosed as gingival recession of upper incisor and periodontal-endodontic combined lesion. This report included the history of disease, clinical examination, X-ray film, treatment planning, root canal treatment and sequence of periodontal disease treatment (initial therapy, local drug therapy, bone graft and guided tissue regeneration combined with double laterally positioned flaps, subepithelial connective tissue graft and coronal positioned flap). The reasons of gingival recession, the methods of root coverage and the reconstruction of soft and hard tissue were also discussed on the basis of this case.
7.Comparison of surgical treatment and conservative treatment for mandibular condylar fractures

Objective To compare the therapeutic effect and complications of surgical treatment and conser-vative treatment for mandibular condylar fractures.Methods 100 cases of mandibular condylar fractures were selected as the research subjects.According to the different treatment method,the patients were divided into the observation group and the control group,50 cases in each group.The observation group was treated by operation,and the control group was treated with conservative treatment.The therapeutic effect,mouth opening of before and after treatment,the change of movement and lateral movement,and complications were observed and compared between the two groups. Results The excellent and good rate of the observation group was 88%,compared with the control group of 86%,the difference was not statistically significant (P >0.05).12 months after treatment,mouth opening,protrusion movement and lateral movement of two groups were significantly improved than treatment after 6 months[(38.23 ±9.65)vs (35.98 ±9.65),(37.98 ±9.65)vs (35.43 ±8.76),(7.98 ±1.66)vs (7.21 ±1.03),(7.87 ±1.43)vs (6.98 ±1.06),(8.96 ±1.32)vs (8.06 ±1.06),(8.86 ±1.25)vs (8.86 ±1.25)],the differences were statisti-cally significant (t =2.87,3.43,3.43,3.43,4.17,4.21,all P <0.05).But there were no significant differences between the two groups (P >0.05).The incidence rate of postoperative complications including limitation of mouth opening,opening deflexion,joint symptoms,injury of facial nerve,parotid salivary fistula and bite axis disorder between two groups had no significant difference (16.0% vs 14.0%,P >0.05).Conclusion Surgical treatment and conser-vative treatment are the common methods for treatment of mandibular condylar fractures,and the effect of surgical treatment is better than conservative treatment.
8.Several problems demanding prompt solution in the construction of the nutrition department

Clinical nutrition, medical treatment and nursing constitute the three pillars in searching for the best curative effect. In view of the fact that the management and overall construction of the nutrition department seriously lags behind medical and nursing work, which impairs the quality and image of the hospital and hinders its overall development, the authors, basing themselves on the realities and having their eyes on the future, explore the overall construction of the nutrition department from the perspectives of disciplinary orientation identification, management system, personnel training, and hardware construction. They argue that the key to the construction of the nutrition department lies in identifying disciplinary orientations and implementing policy favor; carrying out the manning quotas and laying solid organizational foundations; strengthening personnel training and grasping key development opportunities; streamlining the management system and safeguarding a healthy development.
9.Clinical study on the sedation and analgesia effect of dexmedetomidine combined with sufentanil in severe acute pancreatitis patients with mechanical ventilation

Objective To investigate the sedation and analgesia clinical effects of dexmedetomidine and midazolam sufentanil in severe acute pancreatitis patients with mechanical ventilation.Methods Sixty four patients with mechanical ventilation in severe acute pancreatitis were randomly divided into dexmedetomidine and midazolam groups,and 32 cases in each group.All patients were routinely given sufentanil continuous intravenous analgesia.The patients in dexmedetomidine group received dexmedetomidine 1.0 μg/kg loading dose by intravenous injection and followed with 0.2 ~ 0.7 μg/(kg · h) continuous intravenous infusion.The patients in midazolam group received midazolam:0.05 mg/kg loading dose by intravenous injection and followed with 0.02 ~0.08 mg/(kg · h) continuous intravenous infusion.The sedation and analgesia effects and inflammation index changes in two groups were observed.Results The analgesia and sedation onset time,awake time and time of live in intensive care unit (ICU) in dexmedetomidine group were significantly superior to the midazolam group (P < 0.05).Before weaning from mechanical ventilation the white blood cell count,C-reaction protein (CRP),heart rate,and respiratory frequency index in dexmedetomidine group were significantly better than those in midazolam group (P < 0.05).The adverse event's incidence rate in dexmedetomidine group (9.38%) was ignificantly lower s than that in midazolam group (31.25%) (P <0.05).Conclusions The application of dexmedetomidine combined with sufentanil in severe acute pancreatitis patients with mechanical ventilation may play a good role of sedative and analgesic effects,and alleviating the inflammatory reaction of patients,as well as fewer adverse events.
10.Relationship between heart rate variability and prognosis in patients with systemic inflammatory response syndrome

Objective To study the autonomic nervous regulation function,and to study relationship between heart rate variability and severity,organ dysfunction and prognosis in patients with systemic inflammatory response syndrome (SIRS).Methods All the patients were divided into two groups:SIRS and non-SIRS according to the criteria of SIRS. The SIRS group included 100 patients and the non-SIRS group 30 patients,and the control group included 30 heathy adults. SIRS group was further divided into two subgroups:survivor and succumbed.Heart rate variability(HRV)was analyzed with the two methods of time-domain and frequency-domain.Results The SIRS group had significantly higher APACHEll scores and MODS scores(P<0.01)and lower HRV when compared with the non-SIRS group(P<0.05).In the SIRS group,the succumbed groupallad statistically higher APACHEII and MODS scores and lower HRV than the survivor group( P<0.01).There was a negative correlation between the prognosis of the patients and SDNN(r=-4.68,P<0.01), SDNN<55 ms was the most sensitive(76.2%),and specific(76.7%),with positive predictive value(90.4%)and negative predictive value(52.6%).Conlusion The SIRS patients had lower HRV,and there was a negative correlations with the severity of disease,organ dysfunction and prognosis.HRV(SDNN)were the useful index to evaluate the prognosis of critically ill patients.
