1.GENE EXPRESSION OF COLLAGEN TYPE Ⅰ AND TYPE Ⅲ IN TISSUE OF LOW COMPLIANT BLADDER

Objective To investigate the alteration in gene expressions of collagen type Ⅰ and type Ⅲ in tissue of obstructive low compliant bladder of rats. Methods Gene expressions of collagen type Ⅰ and type Ⅲ in the rat bladder tissues were evaluated with RT-PCR. Results The gene expressions of collagen type Ⅰ and type Ⅲ in smooth muscle of low compliant bladder were up regulated, but the level of type Ⅲ gene expression was stronger than that of the type I. Conclusion Collagen type Ⅲ and Ⅰ gene expression was upregulated in tissue of obstructive low compliant bladder of rats. Up regulation of collagen gene expression was related to low compliant bladder after obstruction.
2.The characteristics of affective intensity and cognitive emotion regulation in borderline personality disorder

Objective To examine the characteristics of affective intensity and cognitive emotion regulation in subjects with borderline personality disorder ( BPD) . Methods The BPD subscale of personal-ity disorder questionnaire( PDQ-4+) was used to assess the BPD symptoms,the short affective intensity scale (SAIS) and cognitive emotion regulation questionnaire (CERQ) were used to measure affect intensity and cognitive emotion regulation strategy,respectively. 765 subjects with BPD and 776 healthy controls were se-lected. The independent-samples t test was used to analyze the differences between BPD group and controls and logistic regression analysis was used to examine the related factors affecting the BPD. Results Com-pared with the control group(negative intensity:(3.08±0.66),negative coping dimension:(37.20±5.94), BPD group got higher scores in negative intensity((3.88±0.74), t=22.29, P<0.01,Cohen''s d>0.8)and negative coping dimension((44.77±6.36), t=24.16, P<0.01,Cohen''s d>0.8). The logistic regression anal-ysis showed that negative intensity(B=1.38,Exp(B)=3.97,95%CI for EXP(B):3.15~5.00, P<0.01) and negative cognitive regulations strategy(B=0.19,Exp(B)=1.21,95% CI:1.18-1.25, P<0.01) could affect the prevalence of BPD. Conclusion Subjects with BPD traits have more significant negative affective inten-sity and tend to use negative cognitive regulations strategy.
3.Astragalus injection' effect on the expression of hypoxia-inducible factor-1α and p53 in the retina of rats under hypoxia environment

Objective To observe the expression and mechanism of hypoxia-inducible factor-1α (HIF-1α) and p53 protein at the altitude of 5000 meter plateau hypoxia environment in rats,as well as the effect of Astragalus injection.Methods Sixty Sprague Dawley rats were randomly divided into the Astragalus injection intervention group and normal saline control group,30 rats in each group.Astragalus injection group rats were intraperitoneal injected of Astragalus injection (15 ml/kg) before 30 minutes into the plateau environment simulation cabin,normal saline group rats were intraperitoneal injected with the same volume of saline.30 minutes after injection,rats in each group were reared in the plateau experiment cabin which simulated altitude of 5000 m (oxygen partial pressure 11.3 kPa) for 2,6,8,12,24 hours,each time period of 6 rats.When get out,the rats were executed immediately and eyes were harvested.Retinal sections were studied by hematoxylin eosin stain,and immunohistochemical method for HIF-1α and p53 expression.Results For control rats,after 2 hours in the cabin,there was edema in retinal layers.HIF-1α and p53 were expressed mainly in the cytoplasm of retinal layers.When the periods in cabin extended,there was atrophy of retinal nerve fiber layer,swelling and degeneration of ganglion cells.The expression of HIF-1α and p53 was increased.Compared with the control group,the intervention group rat had similar but less severe retinal changes,and the expression of HIF-1α and p53 was significantly decreased (P <0.05).Conclusion Astragalus injection can reduce pathological retinal damage in rats at high altitude environment,and its mechanism may be associated with reduced HIF-1α,p53 expression.
4.Effect of Ultra-early Hyperbaric Oxygenation on Bone Calcium, Biomechanical Properties and Bone Collagen of Femur in Rats with Complete Spinal Cord Transection Injury

Objective To explore the effect of ultra-early hyperbaric oxygenation (HBO) on bone calcium, biomechanical properties and bone collagen of femur in rats with complete spinal cord transaction. Methods A total of 75 Sprague-Dawley rats were randomly divided in-to sham group (n=15), model group (n=20) and HBO group (n=40). HBO group was divided into three hours group (HBO1 group, n=20) and twelve hours group (HBO2 group, n=20). All groups underwent laminectomy at T10, while the model group, HBO1 group and HBO2 group underwent complete spinal cord transection at the same level. Three hours and twelve hours after surgery, HBO1 group and HBO2 group received HBO, respectively, for three courses with ten days in a course. After treatment, the femoral biomechanical properties, bone calcium and hydroxyproline (Hyp) were determined. The morphology of bone trabecula and the bone collagen was observed with HE stain-ing and Masson triad color staining, respectively. Results After treatment, compared with the sham group, the femoral biomechanical proper-ties, the content of bone calcium and Hyp decreased in the model group (P<0.05);compared with the model group and HBO2 group, they in-creased in HBO1 group (P<0.05). The number of bone trabecula and the bone collagen decreased, and derangement and sparseness were ob-served in the model group;however, the changes were substantially mild in HBO1 group. Conclusion Ultra-early HBO could increase the content of bone calcium and Hyp of femur, improve the morphology of the femur bone collagen, and improve the femoral biomechanical properties in rats with complete spinal cord transection.
5.The gene expressions of collagen types Ⅰ and Ⅲ in obstructive low compliant bladder in rats

Objective To detect the gene expressions of collagen types Ⅰ and Ⅲ in obstructive low compliant bladder in rats. Methods The expressions of collagen types Ⅰ and Ⅲ mRNA in rat bladder tissues were detected by in situ hybridization. Results Compared with those in the normal control, the gene expressions of collagen types Ⅰ and Ⅲ in smooth muscle of low compliant bladder were up regulated. Type Ⅲ gene expression was more obvious than type Ⅰ. Conclusion The expressions of collagen types Ⅲ and Ⅰ in obstructive low compliant bladder in rats are up regulated, which is related with low compliant bladder after obstruction.
6.Inhibitory effect of SIN-1 on ICAM-1 expression in rat kidney after ischemia/reperfusion injury

Objective To explore the therapeutic mechanisms of nitric oxide donor (SIN 1) in ischemic acute renal failure (IARF). Methods The effects of SIN 1 on intercellular adhesion molecule 1 (ICAM 1) expression and inflammatory cell infiltration in rat kidney after ischemia/reperfusion (I/R) injury were examined using ICAM 1, ?2 integrin polyclonal antibody, and immunohistochemistry. The renal functions were measured simultaneously. Results Progressive increased expression of ICAM 1 was found in kidneys, especially in vasa recta, after IR injury. ?2 integrin positive cells increased simultaneously in the outer medulla. The two indexes reached the peak values at 24 h after I/R injury. SIN 1 infusion at the beginning of reperfusion could remarkably inhibit the enhanced ICAM 1 expression, reduce the local infiltration of inflammatory cells, and ameliorate the renal functions. Conclusion NO donor can inhibit the renal expression of ICAM 1 and the infiltration of leukocytes after ischemia reperfusion injury, which may play an important role in the treatment of ischemic acute renal failure.
7.Antibiotic Drug Use in Inpatients:Survey,Analysis and Management

OBJECTIVE To improve the management of the use of antibiotics in the hospital through investigating and analyzing the cases of inpatients.METHODS From Jan 2005 to Dec 2006,totally 810 cases of operated patients and 1220 cases of non-operated patients were investigated retrospectively,and the usage of antibiotic was analyzed.RESULTS The need of using antibiotic drugs was proven in all cases in which antibiotic drugs were used.However,only 56 cases in the total 810 operated cases used drugs were completely reasonable,which accounted for 6.91%.Unreasonableness was found in the other 754 operated cases(ccounted for 93.01%),who had used inappropriate antibiotic drugs or those drugs in an improper time during the preoperative period.The average time of using prevention drugs for Type Ⅰand Type Ⅱ incisions were 6.59 and 6.44 days after the operation,respectively.In the 1220 non-operated cases,only 730 cases used antibiotic drugs,which accounted for 59.83%.From them,432 cases(accounted for 59.18%) used the antibiotic drugs were completely reasonable and other 298 cases(accounted for 40.82%) were found have used the antibiotic drugs unreasonably.The unreasonableness mainly included in the dose and method used and a lack of examination in the pathogens in some adaptive cases.CONCLUSIONS Enhancing the training and encouraging the study of rational use of antibiotic drugs,establishing and optimizing the management system on the use of antibiotic drugs are the key tasks involved to ensure the reasonable use of the antibiotic drugs.
8.Effect of Raw Atractylodes macrocephala on Motility and 5-HT4 Receptor in Small Intestine of Model Rats with Plateau Hypoxia

Background:Gastrointestinal dysmotility is commonly seen in individuals exposed to acute plateau hypoxia. Its pathogenic mechanism is still not clear and intervention study is rarely performed. Aims:To investigate the influence of plateau hypoxia on small intestinal motility of rats,its possible mechanism,and the intervention effect of raw Atractylodes macrocephala. Methods:Seventy Wistar rats were randomly divided into 7 groups:control group,two high altitude model groups(3 500 m and 5 000 m),trimebutine and raw Atractylodes macrocephala groups at 3 500 m or 5 000 m altitude. Rats in six experiment groups were placed in a hypobaric chamber mimicking 3 500 m or 5 000 m altitude and given trimebutine/ raw Atractylodes macrocephala/ saline intragastrically for 3 days. Then all the rats were lavaged with 2 mL ink and sacrificed 30 minutes later. The propulsion rate of ink in small intestine was measured,the pathological changes of small intestine tissue were examined,and the expression of 5-HT4 receptor was determined by immunohistochemistry. Results:Compared with control group,the propulsion rate of small intestine and immunopositive area of 5-HT4 receptor were reduced in model group at 3 500 m altitude,while those in model group at 5 000 m altitude were increased(P ﹤0. 05). Moderate-to-severe mucosal injury was observed in two model groups. In model rats treated with raw Atractylodes macrocephala,the abnormalities in small intestine propulsion and 5-HT4 receptor induced by high altitude(3 500 m and 5 000 m)returned to the control level(P ﹤ 0. 05),and the mucosal injury ameliorated simultaneously. Efficacy of raw Atractylodes macrocephala was prior to trimebutine,a positive control. Conclusions:Acute plateau hypoxia may induce small intestine dysmotility with diverse manifestations,and altitude is a crucial determinant in this process. Raw Atractylodes macrocephala can exert therapeutic effect on this dysmotility by modulating 5-HT4 receptor,and it is also effective in repairing mucosal injury.
9.Establishment of three-dimensional simulation model of acetabular fracture and its significance

Objective To establish three-dimensional (3D) simulation model of acetabular fracture based on Letournel-Judet classification and analyze its significance. Methods Pelvis of one healthy female volunteer was scanned with 16 slice spiral CT. Dicom format data were processed by software Mimics 10.01 to reconstruct 3D model of hip bone. Different fracture types were simulated based on Letournel-Judet classification of acetabular fracture. The fracture fragments were displayed with different color. Screen-captures of fracture model from different directions were saved and video mode of fracture model was exported. Five orthopedic doctors and 10 medical students were employed to compare 3D fracture model with two-dimensional planar schematic diagram and made preliminary evaluation. Results The 3D simulation model of acetabular fracture had realistic form and stereoscopic sense, with satisfactory visual effect. The fracture could be observed and charted from optional direction and angle. Fracture model could rotate for 360° in the corresponding video mode. Four orthopedic doctors and nine medical students considered that 3D simulation model of acetabular fracture was helpful for understanding fracture classification. Conclusion The 3D simulation model of acetabular fracture is intuitive, realistic and dynamic and is helpful for clinical work and medical teaching.
10.Analysis for low scores in patients with reflux gastroesophageal disease using reflux diagnostic questionnaire

Objective To analyse the clinical features of patients with gastroesophageal reflux disease(GERD)who had scores<12 using reflux diagnostic questionnaires(RDQ).Methods A comparative and retrospective study was carried out in 148 GERD patients.Among whom 22 patients had low RDQ score(<12)and 126 patients had high RDQ score(≥12).The differences in heart burn,substantial chest pain,regurgitation,severity and frequency of food reflux between two groups were compared.Nevertheless,the atypical symptoms such as chest distress.abnormal sensation of regurgitation or food reflux was significantly lower in patients with RDQ<12 than those with RDQ≥accounted for 60.28%of the total RDQ score,whereas the heart burn,regurgitation and food reflux atypical symptom of chest distress was found in 13 patients(59.09%),abnormal sensation of throat in 8 patients(36.36%).cough and asthma in 3 patients(13.63%),headache in 5 patients(22.72%),belching in 9 patients(40.90%),aypnia in 8 patients(36.36%),and anxiety in 6 patients(27.27%).Conclusion RDQ is insensitive to those who have chest pain with no obvious symptom of heart burn,regurgitation or food reflux as well as those with atypical symptom beyond the RDQ.
