3.Micro-invasive Surgery in Treatment of Varicose Vein in Lower Limbs

Objective To investigate the skills and effects of TriVex system and to discuss its indications and complications in the treatment of varicose vein in lower limbs.Methods The TriVex system operation was underwent in 106 lower limbs with varicose vein.Sixteen of them showed thrombophlebitis while 38 of them showed chronic ulcer and pigmentation.The lower limbs with perforating vein were treated with subfascial endoscopic perforator vein surgery(SEPS) and limbs with deep insufficient vein valve underwent endoscopic valve repair.Results The therapeutic effect of TriVex system was satisfactory and none of them had soft tissue infection.Hematoma occurred in 12 lower limbs(11.2%).Pigmentation and skin petechia disappeared in 3 weeks after operation.Chronic ulcer healed and ankle pigment reduced in 38 limbs.Hematoma,petechia and numbness were the common complications in the patients with varicose vein in lower limbs.Conclusion It was effective,cosmetic and micro invasive to use the TriVex system in the treatment of thrombophlebitis and varicose vein with chronic ulcer.Adept operation skill can minimize the postoperative complications but the long-term effects of this new technique still needed follow-up studies.

Using ovarian cancer cell lines, we studied relationship between C-erbB2 expression and lysis activity of lymphokine-activated killer cells (LAK) and in vitro regulation by ?-interferon (IFN-?). The results showed that C-erbB2 overexpression was associated with resistance to lysis activity of LAK in ovarian cancer; IFN-? could reduce the overexpression of C-erbB2 and increase lysis activity of LAK on ovarian cancer cells in which C-erbB2 were overexpressed.
5.Effects of different conserve time on physiological characteristics of allograft nerve transplantation

Objective To observe the effect of different conserve time in ultra deep cryopreservation (-196 ℃)on physiological characteristics of allograft nerve transplantation .Methods A total of 80 female Wistar rats were divide into three groups by random principle:derived group which 20 rats were sacrificed to get both sides of femoral nerves ;control groups which each group had 10 rats include group A for fresh autologous nerve transplantation ,group B for fresh allograft nerve transplantation ;experiment groups which each group had 10 rats including groups C ,D ,E ,F that transplanted after femoral nerves conserve in the -196 ℃ for 3 ,6 ,9 , 12 weeks respectively ,and the results of exterior appearance ,light microscope ,electron microscope were observed and electrophysi-ological test was conducted after transplantation .Results After 9 weeks transplantation :the physiological characteristics of group B was most affected ,followed by group C ,D ,E ,F ,group A was with minimal impact ;the result of electrophysiological test showed that groups A&B ,A&E ,A&F had significant difference(P<0 .05) .Conclusion The physiological characteristics of allograft nerve transplantation relate to the freezing time .
6.Correlation between activated circulating endothelial cells and efficacy of anti-angiogenic therapy in non-small cell lung cancer patients

Objective: This study aimed to examine the number of activated circulating endothelial cells (aCECs) in the peripheral blood of patients with non-small cell lung cancer (NSCLC), and investigate the relationship among aCECs, anti-angiogenic therapy, and prognosis of NSCLC patients. This study also aimed to identify novel predictive markers for anti-angiogenic therapy, and provide basic data and experimental basis for establishing an evaluation system for this therapy. Methods: A total of 142 NSCLC patients were randomly divided into the chemotherapy group (Group 1) and combined therapy group (i.e., chemotherapy plus endostatin, Group 2). The number of aCECs was measured using flow cytometry by detecting the expression status of CD105 and CD146 in the peripheral blood. The correlation between the changes in aCECs and efficacy of drug treatment was statistically analyzed using SPSS software. Results:The number of aCECs in Group 2 increased significantly at 8 and 29 d, two cycles, 50 and 71 d, and four cycles after treatment, respectively (P<0.05). In particular, aCECs amount in cases of progressive disease increased more significantly after combined therapy (P<0.05). A negative correlation was found between the treatment cycle and difference in aCECs amount before and after therapy (r=-0.970, P=0.001). A negative correlation was also observed between the difference in aCECs amount and time to tumor progression (TTP) (r=-0.351, P=0.039). Therefore, the difference in aCECs amount before and after therapy could serve as an important predictor for TTP in NSCLC patients. Conclusion:CD105 and CD146 reflected the activation status of endothelial cells, and responded to the drug treatment. Thus, CD105 and CD146 could act as ideal markers for aCECs. The number of aCECs increased during cancer progression, but significantly decreased after long-term treatment. Therefore, the change in aCECs amount may be a useful marker in predicting the efficacy of anti-angiogenic therapy.
7.The correlation analysis of self- management behavior with self- efficacy of patients with lower extremity arterial disease

Objective To investigate the relationships between patients′self- management behaviors and self- efficacy with lower extremity arterial disease, and supply evidence for effective management and intervention of lower extremity arterial disease for clinical medical workers. Methods In August 2014 to January 2015,a total of 110 cases of hospitalized patients with lower extremity arterial disease of two hospitals′vascular surgery from Guangzhou completed questionnaires, including the General Information Questionnaire, Chronic Disease Self- management Behavior - Chinese version, Chronic Disease Self- efficacy -Chinese version. Results The total score of self- management behavior of patients with lower extremity arterial disease ranged from 0 to 69, with an average score of 18.93 ± 6.79, the overall level was not high. Self- efficacy score ranged from 6 to 60, with an average score of 32.25 ± 9.65, the overall level was moderate. Self -management behavior was positively correlated with self- efficacy and its dimensions, and negatively correlated with the dimensions of communication with doctors and Common disease management. Conclusions The clinical staff should strengthen education and guidance of self- management behaviors in patients with lower extremity arterial disease, promote the level of self- management behaviors and self- efficacy, improve the confidence of disease treatment and prevention, in order to achieve the purpose of health promotion.
8.Clinical efficacy of comprehensive treatment for the patients undergoing choledochoscopic gallbladder-preserving operation

Objective To investigate the effect of choledochoscopic gallbladder-preserving cholecystolithotomy combined with traditional Chinese medicine treatment.Methods A randomized controlled clinical study was conducted to analyze the 91 patients who were treated with choledochoscopic gallbladder-preserving cholecystolithotomy and 92 patients who underwent the same operation combined with subsequent treatment of traditional Chinese medicine.Intraoperative,postoperative and follow-up data were compared between the 2 groups (including operation time,blood loss,the rate of biliary fistula and common bile duct injury during the operation,gallbladder contraction function,the recurrence rate of gallstone,etc).Results In the simple choledochoscopic gallbladder-preserving cholecystolithotomy group,rate of gallstone recurrence was 7.7% (7/91),the gallbladder wall was (3.5 ±0.6) mm,the gallbladder contraction function was (34.0 ± 3.6)%.However,the comprehensive treatment group,the rate of gallstone recurrence was 1.1% (1/92),the gallbladder wall was(2.5 ±0.5) mm,the gallbladder contraction function was(48.0 ±4.5)%.There were significant differences between the two groups respectively (P < 0.01,respectively).Conclusion Choledochoscopic gallbladder-preserving cholecystolithotomy combined with traditional Chinese medicine treatment is a safe,feasible,and minimal invasive approach for gallstone,and it can be considered as a alternative treatment of gallstone.
9.Early response and acute adverse effect after particle radiation therapy for adenoid cystic carcinoma of the head and neck

Objective To study the early response and acute/subacute adverse effects after particle radiation therapy for adenoid cystic carcinoma (ACC) of the head and neck.Methods Between May 2015 and March 2016,a total of 8 patients with ACC of the head and neck were treated using proton and/or carbon-ion radiation therapy.Three patients had early stage and 5 had locally advanced disease.Five patients had an R2 and three achieved an R1 resection.Results Seven patients received intensitymodulated proton therapy (IMPT) followed by intensity-modulated carbon-ion therapy (IMCT) boost.One patient received IMPT only.Among the five patients who had an R2 resection,2/3 patients had partial response (PR)/stable disease (SD) at the end of radiation,0/3 achieved PR/complete response (CR) at 1-month follow-up,and 1/2 achieved PR/CR at 3-months' follow-up after the completion of radiation,respectively.Two patients experienced Grade Ⅲ mucositis during radiation therapy.No patient experienced moderate or severe skin reactions.At the time of this analysis,all patients are alive and no patient had disease progression or recurrence.Conclusions The short-term outcomes indicated that particle therapy is safe and potentially efficacious in the management of head and neck ACC.However,longer follow up is needed to assess late toxicities and long-term efficacy.
10.Effects of small-dose lidocaine combined with ketamine on early postoperative cognitive function in elderly patients undergoing gastrointestinal tumor surgery.

OBJECTIVETo observe the effects of anesthetic intervention with small-dose lidocaine and ketamine on early postoperative cognitive function in elderly patients undergoing surgeries for gastrointestinal tumors.
METHODSSixty patients (ASA I-III, aged 63-82 years) scheduled for surgeries for gastrointestinal tumors were randomized into intervention group (n=30) and control group (n=30). After intravenous induction and tracheal intubation, the patients in the interventional group received intravenous infusion of 0.5 mg/kg lidocaine and 0.5 mg/kg ketamine, followed by continuous infusion of lidocaine at the rate of 0.5 mg·kg(-1)·h(-1) till the end of the operation; the patients in the control group received saline infusion only. The cognitive function of the patients was assessed at 3 day before and 2 day after the operation using comprehensive neuro-psychological tests. Peripheral venous blood was extracted before anesthesia induction (T0), at the end of the surgery (T1), and at 1 day (T2) and 2 days (T3) after the operation for measurement of serum S-100β protein, NSE and IL-6 levels using ELISA.
RESULTSThe difference between the test scores before and after the operation (X values) was significantly smaller in the intervention group than in the control group (P<0.05). The intervention group showed a significantly lower incidence rate of postoperative cognitive dysfunction (POCD) than the control group (6.7% vs 33.3%, P<0.05). Compared with the control group, the intervention group exhibited significantly lower serum levels of S-100β protein, NSE and IL-6 at T1 (P<0.05), significantly lower NSE and IL-6 levels at T2 (P<0.05) time point, and significantly lower IL-6 level at T3 (P<0.05).
CONCLUSIONIntravenous injection of small-dose lidocaine and ketamine during the operation can reduce the incidence of POCD in elderly patients undergoing surgeries for gastrointestinal tumors possibly in relation to decreased serum S-100β, NSE and IL-6 levels.
