1.The diagnostic value of VILIP-1 and NR2 peptide combined detection in the early ischemic stroke

Objective To Investigate the concentration of VILIP-1 and NR2 peptide in the serum of patients with ischemic stroke , and to explore their clinic value in early diagnostic of ischemic stroke patients.Methods The levels of VILIP-1 and NR2 peptide were examined by ELISA ( enzyme linked immunosorbent assay ,ELISA) with suspicious TIA ( defined as a neurological deficit that resolved within 24 hours) or acute ischemic stroke patients ( within 72 hours of onset of symptoms ) 340 cases,102 healthy controls,98 patients with vascular risk factors and 35 patients with hemorrhagic stroke.Among all the groups , VILIP-1 and NR2 peptide level were analyzed using the nonparametric Wilcoxon test.Diagnostic performance were analyzed among the groups with the two biomarkers independently and combinedly .Results Serum levels of VILIP-1 and NR2 peptide in patients with ischemic stroke (IS) were 9.80 (1.90-14.22) μg/L, 14.40 (5.60-27.91) μg/L respectively,which was higher than that of the healthy control group [VILIP-1:0.02 (0.01-0.09),NR2:0.33 (0.02-1.15),χ2 were 5.61 and 9.54,P<0.001],the group with vascular risk factors [VILIP-1:0.03 (0.02-0.16),NR2:0.27 (0.01-1.54),χ2 were 6.74 and 10.62,P<0.001], the group of patients non-stoke [VILIP-1:0.04 (0.03-0.19),NR2:0.53 (0.45-1.21),χ2 were 3.78 and 7.63, P <0.001 ].The levels of VILIP-1 and NR2 peptide was significantly increased in IS patients presenting within 3 h of symptom onset.When differentiating IS from patients with hemorrhagic stroke ,NR2 had a AUC of 0.934,showing a strong distinguishing effectiveness.Differentiating IS from healthy controls , patients with vascular risk factors and non-stroke patients,the AUC of combination of VILIP-1 and NR2 was 0.974,which was higher than the AUC of either VILIP-1(0.849) or NR2(0.862) alone(P <0.05). Conclusions VILIP-1 and NR2 peptide are very sensitive and specific biomarkers to the early diagnosis of IS.The combination of VILIP-1 and NR2 peptide has higher value of clinical applications than one of them independently.
2.Endoscopic resection of gastrointestinal submucosal tumors

Objective To study on the endoscopic resection of submucosal tumors (SMT) in gastro-intestinal tract with especial regards to the effectiveness and safety, and the value of endoscopic ultrasonogra phy ( EUS) in SMT before endoscopic resection. Methods Endoscopic therapy was performed in 71 patients with gastrointestinal SMT diagnosed by endoscopy, and of them 64 patients (90. 1 % ) were examined by EUS before the procedure. The SMT located in the esophagus 36 cases, stomach 29 cases, duodenum 3 cases and rectum 3 cases. The median tumor diameter was 14. 2 mm ( ranged 6-20 mm). Endoscopic mucosal resection was performed using a two-channel endoscope after submucosal injection of saline solution in 55 cases, then the SMT resected. In 16 patients, the SMT was resected completely with one-step standard procedure using cap-fitted endoscope ( 10 cases, the SMT less than 10 mm in diameter) or two-channel endoscope (6 cases). Results Sixty-eight of seventy-one solid SMT were removed completely under endoscopy and 2 SMT (one ectopic pancreas and one gastric leiomyoma) were resected incompletely; the findings were showed at 4th week by repeated endoscopy. One patient with rectal leiomyoma required surgical resection after unsuccessful endoscopic resection. The follow up period was 18. 7 months on average, no recurrences were found in 67 cases. Histological results showed 51 (71. 8% ) leiomyomas, one granular cell tumor, 4 fibromas, 3 ectopic pancreas, 3 lipomas, 2 stromal tumors and 2 carcinoids in the SMT. Five tumors of mesenchymal tissue , their histological origin cannot be firmly defined due to lack of immunoassayed specimen. Bleeding occurred in only 9 of 71 cases and was easily managed with endoscopic haemostatic measures. One patient with stromal tumor suffered from gastric perforation after the procedure. Conclusions Endoscopic resection of gastrointestinal SMT is a safe and effective procedure and obtains the histological diagnosis simultaneously. EUS is helpful in selecting indicated cases for endoscopic resection.
3.Optimizing the construction of school doctor team and improve the capacity of school health services

Abstract
The capacity of school health services is an important guarantee for ensuring physical and mental health of students and faculty, and is an important component of healthy schools. With the development of society, health status of children and adolescents presents new characteristics, which puts forward higher requirements for school health service capabilities. The allocation of health personnel, whether in universities or primary and secondary schools, still faces a common problem of being extremely mismatched with educational needs. To comprehensively build a high quality school health system that meets the current goal of building a strong education country, optimizing the construction of school doctors has become a key part of it. The article summarizes the main problems and challenges in the construction of school medical teams in China, proposes potential development paths and suggestions, in order to provide the reference for improving and developing high quality school health work systems, enhancing overall school health service capabilities, and promoting the comprehensive healthy development of children and adolescents.
4.Current status of standardized construction of health system in colleges and universities of Guangdong Province

Objective:
To understand the current status of the standardized construction of the health system in colleges and universities in Guangdong Province, so as to provide recommendations for promoting its standardized construction.
Methods:
From October to November 2023, a random digital table method combined with a questionnaire survey and one site investigation were used to investigate the basic information of school medical institutions, the construction situation of school medical institutions, the service provision of school medical institutions, the work of infectious disease prevention and control, and health education in 281 branches of 169 colleges and universities across 21 cities in Guangdong Province.
Results:
Among the 281 branches of colleges in Guangdong Province, 264 (94.0%) had school medical institutions. Among those, 88 (33.3%) had a preventive health care department, and 35 (13.5%) had one office for health education research. There were a total of 2 950 health professionals in school medical institutions, with 988 being regular staff members, 372 (12.6%) of whom hold senior titles. A total of 190 (73.4%) branches conducted annual infectious disease prevention and control health education for faculty, 219 (84.6%) branches conducted annual health education for students with at least 1 hour of instruction, 109 (42.1%) branches offered health education courses, and 106 (40.9%) branches established health education resources.
Conclusion
The construction of health system in colleges and universities in Guangdong Province is becoming more complete, but there are still issues such as unclear functional positioning of school health related departments and personnel, a weak team of school health professionals, and insufficient investment in health education.
5.Targeted therapies for lupus nephritis: Current perspectives and future directions

Lupus nephritis (LN), a severe manifestation of systemic lupus erythematosus, poses a substantial risk of progression to end-stage renal disease, with increased mortality. Conventional therapy for LN relies on broad-spectrum immunosuppressants such as glucocorticoids, mycophenolate mofetil, and calcineurin inhibitors. Although therapeutic regimens have evolved over the years, they have inherent limitations, including non-specific targeting, substantial adverse effects, high relapse rates, and prolonged maintenance and remission courses. These drawbacks underscore the need for targeted therapeutic strategies for LN. Recent advancements in our understanding of LN pathogenesis have led to the identification of novel therapeutic targets and the emergence of biological agents and small-molecule inhibitors with improved specificity and reduced toxicity. This review provides an overview of the current evidence on targeted therapies for LN, elucidates the biological mechanisms of responses and failure, highlights the challenges ahead, and outlines strategies for subsequent clinical trials and integrated immunomodulatory approaches.
6.Hematoma density heterogeneity scale scores predict early hematoma expansion in patients with spontaneous intracerebral hemorrhage

Objective To evaluate the predictive performance of hematoma density heterogeneity scale (HDHS) to hematoma expansion (HE) in patients with spontaneous intracerebral hemorrhage (sICH).Methods Sixty-two patients with sICH,received therapy in our hospital form November 2011 to December 2014,were enrolled.HDHS scores were assessed on non-contrast CT (NCCT) image source,and HE progresses were assessed based on the follow up of NCCT at 24 h.Binary Logistic-regression analysis was used to determine independent risk factors of HE.Receiver operating characteristic (ROC) curve was adopted to identify the predictive performance of HDHS for HE.Results There were 37% patients (23/62) in the cohort underwent HE.Binary Logistic-regression analysis showed that HDHS scores were independently associated with HE (OR=2.451,95% CI:1.345-4.468,P=0.003).ROC curve analysis showed that the area under curve (AUC) was 0.745 (95%CI:0.625-0.865,P=0.001),the diagnostic critical value was 3,the sensitivity was 87% and the specificity was 59%.Conclusion HDHS is an efficient tool that independently predict early HE in patients with sICH,and provides reliable evidence for prognosis and treatment.
7.Study on Quality Evaluation of Notopterygii Rhizoma et Radix through UPLC Fingerprint Combined with Chemometrics Analysis and Multi Index Component Content Determination

Objective To establish the ultra-high performance liquid chromatography(UPLC)chromatographic fingerprint of Notopterygii Rhizoma et Radix;To determine the contents of ferulic acid,nodakenin,ammijin,notopterol,isoimperatorin and volatile oil of Notopterygii Rhizoma et Radix from different producing areas;To provide reference for quality evaluation of Notopterygii Rhizoma et Radix.Methods Waters BEH C18 chromatographic column(2.1 mm×150 mm,1.7 μm)was used,with mobile phase acetonitrile-0.02%formic acid aqueous solution gradient elution,flow rate 0.25 mL/min,column temperature 25℃,detection wavelength 330 nm,injection volume 2 μL.UPLC fingerprints of 25 batches of Notopterygii Rhizoma et Radix were established,and the similarity analysis and chemometrics analysis were carried out.The contents of ferulic acid,nodakenin,ammijin,notopterol and isoimperatorin were determined simultaneously,and the contents of volatile oil was determined by steam distillation method.Results Totally 23 common fingerprint peaks were calibrated,11 known components were identified.According to the results of the cluster analysis and principal component analysis,25 batches of Notopterygii Rhizoma et Radix samples were divided into 3 categories,and the 6 potential differential components were screened out by orthogonal partial least squares-discriminant analysis(OPLS-DA).The results showed that the contents of notopterol and volatile oil from Sichuan Province were higher than those from Gansu Province and Qinghai Province.Conclusion The method established in the study is accurate and reliable,which can provide scientific basis and reference for the quality evaluation and control of Notopterygii Rhizoma et Radix.

Objectives:This study aimed to assess the efficacy and safety of bendamustine in combination with rituximab (BR regimen) for the treatment of newly diagnosed indolent B-cell non-Hodgkin's lymphoma (B-iNHL) and elderly mantle cell lymphoma (eMCL) .Methods:From December 1, 2020 to September 10, 2022, a multi-center prospective study was conducted across ten Grade A tertiary hospitals in Shandong Province, China. The BR regimen was administered to evaluate its efficacy and safety in newly diagnosed B-iNHL and eMCL patients, and all completed at least four cycles of induction therapy.Results:The 72 enrolled patients with B-iNHL or MCL were aged 24-74 years, with a median age of 55 years. Eastern Cooperative Oncology Group (ECOG) performance status scores of 0-1 were observed in 76.4% of patients, while 23.6% had scores of 2. Disease distribution included follicular lymphoma (FL) (51.4% ), marginal zone lymphoma (MZL) (33.3% ), eMCL (11.1% ), and the unknown subtype (4.2% ). According to the Ann Arbor staging system, 16.7% and 65.3% of patients were diagnosed with stage Ⅲ and stage Ⅳ lymphomas, respectively. Following four cycles of BR induction therapy, the overall response rate was 98.6%, with a complete response (CR) rate of 83.3% and a partial response (PR) rate of 15.3%. Only one eMCL patient experienced disease progression during treatment, and only one FL patient experienced a relapse. Even when evaluated using CT alone, the CR rate was 63.9%, considering the differences between PET/CT and CT assessments. The median follow-up duration was 11 months (range: 4-22), with a PFS rate of 96.8% and an OS rate of 100.0%. The main hematologic adverse reactions included grade 3-4 leukopenia (27.8%, with febrile neutropenia observed in 8.3% of patients), grade 3-4 lymphopenia (23.6% ), grade 3-4 anemia (5.6% ), and grade 3-4 thrombocytopenia (4.2% ). The main non-hematologic adverse reactions such as fatigue, nausea/vomiting, rash, and infections occurred in less than 20.0% of patients.Conclusion:Within the scope of this clinical trial conducted in China, the BR regimen demonstrated efficacy and safety in treating newly diagnosed B-iNHL and eMCL patients.
9.Comparison of Component Differences in Chebulae Fructus and Chebulae Fructus Immaturus Based on Chemical Pattern Recognition and Multi-index Quantitative Analysis

OBJECTIVE:To provide reference for the identification of Chebulae Fructus and Chebulae Fructus Immaturus . METHODS:UPLC method was adopted. The determination was performed on Waters Cortecs T 3 C18 column with mobile phase consisted of acetonitrile- 0.2% phosphoric acid solution (gradient elution )at the flow rate of 0.35 mL/min. The column temperature was 30 ℃,and the detection wavelength was set at 270 nm. The sample size was 1 μL. Using gallic acid as reference,UPLC fingerprints of 17 batches of Chebulae Fructus and 14 batches of Chebulae Fructus Immaturus were established and their similarity was evaluated by TCM Chromatographic Fingerprint Similarity Evaluation System (2012 edition). By comparing substance control , UV absorption spectrum and related literaturs ,common peaks were identified. PCA and PLS-DA were performed by using SPSS 20.0 and SIMCA 14.1 software. The contents of main difference components in Chebulae Fructus and Chebulae Fructus Immaturus were determined by above UPLC method and compared. RESULTS :There were 8 common peaks in UPLC fingerprint of Chebulae Fructus and Chebulae Fructus Immaturus ,i.e. chebulic acid (peak 1),gallic acid (peak 2),punicalagin A (peak 3),punicalagin B (peak 4),corilagin(peak 6),chebulagic acid (peak 7)and chebulinic acid (peak 8). The similarities of 17 batches of Chebulae Fructus were from 0.92 to 0.99,while 14 batches of Chebulae Fructus Immaturus were all above 0.99. The similarity of control fingerprint between Chebulae Fructus and Chebulae Fructus Immaturus was 0.909. PCA demonstrated the differences between Chebulae Fructus and Chebulae Fructus Immaturus . The results of PLS-DA were consistent with those of PCA ,and the variable importance in projection (VIP)values of peak 5,4,7,3 and 2 were above 1 in the PLS-DA model. In 31 batches of samples ,the contents of gallic acid (peak 2),punicalagin A(peak 3),punicalagin B (peak 4)and chebulagic acid (peak 7)were 2.63-10.31, 5.37-44.63,8.02-60.77,44.07-162.98 mg/g;RSDs were 40.14%, 47.91% ,53.97% ,36.22%(n=31). There was statistical significance in the differences of the mentioned 4 components between Chebulae Fructus and Chebulae Fructus Immaturus 719412818@qq.com (P<0.05). CONCLUSIONS :There are significant differences between Chebulae Fructus and Chebulae Fructus Immaturus gallic acid ,punicalagin A ,punicalagin B and chebulagic acid are the main difference components for identification.
10.Comprehensive Quality Evaluation of Lysimachia christinae Based on the Entropy Weight TOPSIS Method Combined with Multiple Indicator Components

OBJECTIVE:To e stablish UPLC characteristics fingerprints of Lysimachia christinae ,and to simultaneously determine 3 effective components and to comprehensively evaluate the quality of L. christinae from different production areas. METHODS:UPLC method was adopted to establish characteristics fingerprint of the whole plant ,stem and leaves of 10 batches of L. christinae ,and determine the contents of kaemperfol- 3-O-rutinoside,quercetin,kaemperfol. The determination was performed on Waters CORTECS UPLC T 3 column with mobile phase consisted of acetonitrile- 0.2% phosphoric acid aqueous solution (gradient elution )at the flow rate of 0.2 mL/min. The detection wavelength was set at 364 nm,and column temperature was 30 ℃. The sample size was 1 µL. Similarity Evaluation System for TCM Chromatographic Fingerprint (2012 edition)was adopted to evaluate its similarity , and common peaks were confirmed. Using the contents of kaemperfol- 3-O-rutinoside, quercetin, kaemperfol,total ash ,acid-insoluble ash and sulfur dioxide residue ,the ethanol-soluble extract as index ,entropy weight TOPSIS was used to evaluate the overall quality of L. christinae comprehensively. RESULTS :There were 7 common peaks in the whole plant,stem and leaves of 10 batches of L. christinae ,among which 3 peaks were identified as kaemperfol- 3-O-rutinoside, quercetin and kaemperfol. The similarity of same part in the whole plant of L. christinae from different batches were not lower than 0.830. The similarity between stem and leaves of L. christinae in same batch was 0.504-0.859; the similarity between whole plant and stem was 0.593-0.904;the similarity between whole plant and leaves was 0.885-0.995. The linear ranges were 0.392 0-39.197 0 μg/mL for kaempferol-3-O-rutinoside, 0.397 0- 39.703 4 μg/mL for quercetin,0.380 9-38.093 0 μg/mL for kaempferol(r>0.999 0). RSDs of precision ,stability and repeatability tests were all lower than 2%. The recoveries were 96.43%(RSD=0.63%,n=9),100.32%(RSD=0.46%,n=9), 101.80%(RSD=0.32%,n=9),respectively. The content range of above components in L. christinae were 0.006 3%-0.041 1%, 0.002 9%-0.008 6%,0.004 4%-0.017 5%(stem);0.024 8%-0.290 5%,0.000 9%-0.009 0%,0.001 3%-0.012 4%(leaves); 0.007 9%-0.118 0%,0.001 5%-0.008 8%,0.002 8%-0.012 5%(whole plant ). There was no significant difference in the contents of 3 components in L. christinae among different producing areas (P>0.05). The order of the contents of kaempferol- 3-O- rutinoside in different parts of L. christinae was leaves >whole plant >stem. The contents of quercetin and kaempferol were high relatively in the stem. Results of entropy weight TOPSIS method showed that mean values of Ci for L. christinae from Zhongjiang county and Shuangliu county of Sichuan province ,Shizhu county of Chongqing city were 0.446,0.512,0.287. CONCLUSIONS : Established fingerprint and content determination method are stable and feasible ,and multi-index evaluation model constructed by characteristic chromatogram combined with entropy weight TOPSIS analysis method can be used for comprehensive quality evaluation of L. christinae . The quality of L. christinae from Sichuan province is better.
