1.Total knee arthroplasty analgesia:gabapentin combined with continuous femoral nerve block

BACKGROUND:Traditional analgesia method can relieve the pain after total knee arthroplasty, but the prognosis is poor and drug dependence is strong. Therefore, it is of great significance to study the analgesic drugs in perioperative period of total knee arthroplasty. OBJECTIVE:To explore the analgesic effect of gabapentin combined with continuous femoral nerve block on total knee arthroplasty. METHODS: A total of 48 patients with knee osteoarthritis receiving total knee arthroplasty in the Department of Orthopedics, Affiliated Hospital of Inner Mongolia Medical University from October 2013 to October 2014 were enroled in this study. Using general anesthesia, femoral nerve block was conducted before anesthesia. Analgesia pump was connected after arthroplasty. Patients were randomized to two groups. The control group received multimodal analgesia, and the experimental group received gabapentin combined with continuous femoral nerve block analgesia. Patient’s pain was scored by using resting, activity visual analog scale. Postoperative quality of life, range of motion of knee joint and complications were observed. RESULTS AND CONCLUSION:No significant difference in preoperative resting pain and activity pain was detected between the two groups (P > 0.05). Visual analog scale scores were decreased with time prolonged after arthroplasty in both groups. Visual analog scale scores of resting pain and activity pain were significantly lower in the experimental group than in the control group at 1, 3 and 7 days and 1 month (P < 0.05). Range of motion was significantly larger in the experimental group than in the control group at 3-7 days after arthroplasty (P< 0.05). Activity of daily living score, physical function score, mental function score and social function score were significantly higher in the experimental group than in the control group (P < 0.05). The complication rate was significantly lower in the experimental group than in the control group (17%, 46%,P < 0.05). These data indicate that during perioperative period of total knee arthroplasty, analgesic effect of gabapentin combined with continuous femoral nerve block is ideal. In particular, in patients with acute pain within 48 hours, their combination can promote early rehabilitation of the patient’s knee, and few side effects are found.
2.Effect of hypoxia on exosomes in renal tubular epithelial cells

Objective To explore the effect of hypoxia on exosomes secreted by renal tubular epithelial cells and the function of exosomes in chronic kidney diseases.Methods (1) The supernatant of renal tubular epithelial cells which were cultured in normoxia (21% O2) or hypoxia(1% O2) for 48 h was collected and centrifuged gradiently to harvest exosomes.Exosomes were identified and compared by transmission electron microscope, nanoparticle tracking analysis, Western blotting and measurement of the protein concentration.(2) Primary peritoneal macrophages of rats were co-cultured with exosomes in different concentrations (1, 10, 50, 100, 300 mg/L).The expression of interleukin-6(IL-6), tumor necrosis factor-α (TNF-α), inducible nitric oxide synthase (iNOS) in cells and supernatant were separately detected by quantitative real-time PCR (qRT-PCR) and ELISA, and the expression of phospho (p)-STAT/STAT and suppressors of cytokine signaling 1 (SOCS1) in macrophages was detected by Western blotting.At last, the expression of inflammatory microRNAs (miR) in exosomes was measured by qRT-PCR.Results (1) The vesicles harvested by gradient centrifugation were less than 150 nm and expressed CD63 which was characteristic of exosomes.Hypoxia had no effect on the morphology of exosomes, but stimulated their secretion.(2) Hypoxic exosomes dose-dependently improved the expression of IL-6, TNF-α, iNOS in macrophages polarized by lipopolysaccharide (LPS) and increased the expression of p-STAT while decreased the expression of SOCS1 (P < 0.01).MicroRNAs referred to inflammation such as miR-155 and miR-27a increased in hypoxic exosomes compared to that in normoxic exosomes (P < 0.05).Conclusions Hypoxia makes exosomes promoted the polarization of macrophages to M1, which may account for the microinflammation in chronic kidney diseases.
3.The study on cytotoxic effects and mechanisms of unsaturated fatty acid from Maoeryan of gansu origin on human BEL-7402 strain cells

Aim To study cytotoxic effects and mechanisms of unsaturated fatty acid from mao er yan of gansu origin on BEL-7402 strain cells.Methods The essential components of unsaturated fatty acid from mao er yan of gansu origin were determined by gas chromatography-mass spectrometry and cytotoxic effects of un-saturated fatty acid on BEL-7402 strain cells were determined by using flow cytometry,methods of MTT [3-(4,5-dimethyl thiazol-2yl)-2,5-diphenyltetrazolium bromide] and electron microscope scanning after 48 hours of the unsaturated fatty acid from mao er yan of gansu origin treatment at concentrations of 0.2,0.8 and 3.2 mg?L-1,respectively.Results The unsaturated fatty acid from maoer yan of gansu origin had significantly cytotoxic effects on BEL-7402 strain cells and inhibitory rates for proliferation of BEL-7402 strain cells were 34.6%、44.8% and 70.7% at those three concentrations,respectively.50% inhibiting concentration(IC_ 50)was 0.812 mg?L-1.Apoptosis rates were 18.6%,22.7% and 24.2%,respectively,and occurred cell membrane microvilli breakdown and cell rupture,compared to those of the control group.Conclusion The unsaturated fatty acid from mao er yan of gansu origin has significantly cytotoxic effects on BEL-7402 strain cells and their mechanisms may be related to inhibit cell division,destroy the structure of cell membrane and mitochondria.
4.Determination of Lead in Cosmetics by Potential Stripping Voltammetry

Objective To establish a method for determination of lead in cosmetics. Methods Samples were digested. The lead content was detected by isochronous mercury-plating method with the condition of lead cation being enriched for 40 s at -1.20V(vs. SCE). Results The stripping potential peak of lead appeared at - 0. 20 V( vs. SCE) . The detection limit was 0.03 jug Pb. Six parallel determinations of a sample were done with the mean of 3.574mg/ kg and the relative standard deviation of 4. 1% . Recovery rates obtained by standard addition test ranged from 90% -100.5% . Conclusion The method is suitable for detecting lead in cosmetics.
5.Application and nursing care of persistent balloon dilatation for anastomotic stricture after choledochojejunostomy

Objective To investigate the value and nursing of persistent balloon dilatation for anastomotic stricture after choledochojejunostomy. Methods The clinical data of 14 cases of anastomotic strictures after choledochojejunostomy accepted the treatment of persistent balloon dilatation were analyzed retrospectively. The effect, adverse reactions and approriate nursing were evaluated. Results Five patients were performed with persistent balloon dilatation thorough the output loop of intestine after choledochojejunostomy and 9 patients through percutaneous transhepatic cholangiography. There was no hemobilia, bile leak or other serious complications. There were 2 cases of balloon dilatation catheter damage, 5 cases of pressure pump damage and 4 cases of balloon migration with 25.0% (7/28) instrument damage rate and 4 cases of balloon migration. After persistent balloon dilation for 6 to 8 months, no anastomotic stricture was found by choledochoscopic examination. Follow up for 6 to 18 months, 2 cases had recurrent anastomotic stricture. Conclusions Persistent balloon dilatation by percutaneous transhepatic cholangiography is a simple, safe and effective method for anastomotic stricture after choledochojejunostomy. In the course of nursing, the balloon catheter and pressure pump damage, and balloon migration should be noted.
6.Laser in situ keratomileusis for myopia

Objective To study the predictability, stability and safety of laser in situ keratomileusis (LASIK) for myopia.Methods Five hundred and Sixty-five eyes ( 293 patients ) were treated with the SCMD corneal shaper and laser sight Compak - 200 excimer laser and followed up for more than 6 months. These eyes were divided into 4 groups according to the degrees of myopia: Group Ⅰ:-1.50~-5.75D,120 eyes; Group Ⅱ:-6.00~-9.75D,230 eyes; Group Ⅲ:-10.00~-14.75D,148 eyes and croup Ⅳ:-15.00~-26.00D,67 eyes.Results Six months post-operatively, mean uncorrected visual acuity of each group was 1.15±0.20, 0.92±0.22, 0.69±0.24 and 0.44±0.22 respectively; The uncorrected visual acuity of 0.5 or better was 100%, 94.6%, 85.8% and 47.7% respectively; And 1.0 or better was 91.7%, 56.7%, 18.0% , 0% respectively. Mean spherical equivalent of each group was -0.14D±0.61D, -0.43D±1.20D, -0.57D±1.47D and -1.79D±2.57D respectively. There were no serious complications. Conclusion LASIK is an effective and safe method for reducing myopia. But the potential complications should be noticed.
7.Nursing of endoscopic full-covered self-expanding removable metal stents implantation for bile duct anastomotic strictures after liver transplantation

Objective To investigate the nursing points of endoscopic full-covered self-expanding removable metal stents (FCSERMS) implantation for bile duct anastomotic strictures after liver transplantation. Methods The clinical data of patients who were treated by endoscopic full-covered self-expanding removable metal stents implantation for bile duct anastomotic strictures after liver transplantation from January 2013 to July 2015 were retrospectively analyzed, and the nursing process were summarized. Results The group of 9 patients were successfully placed and removed with FCSERMS. There was no postoperative complication, such as stent migration, acute pancreatitis, biliary bleeding and intestinal leakage. All the bile duct strictures were relieved after FCSERMS removement. Followed up for 10-32 months, there was no symptom and sign of bile duct anastomotic stricture recurrent. Conclusions The key in nursing points of FCSERMS implantation for bile duct anastomotic strictures after liver transplantation are introducing the function of FCSERMS and therapeutic process to improve patient compliance, mastering the endoscopic operations, the placement and removal method of FCSERMS to short operation time, strengthening postoperative nasal bile duct care, paying attention to the observation, detection and treatment of postoperative complications after the metal stent placement and removement, as well as the continuing care during the period between placement and removment of FCSERMS.
8.Graded nursing to rectal cancer patients with radioactive proctitis induced by preoperative neoadjuvant chemoradiotherapy

Objective To explore the key points of graded nursing care to rectal cancer patients with radioactive proctitis induced by preoperative neoadjuvant chemoradiotherapy.Methods The clinical data of 162 rectal cancer patients undergoing neoadjuvant chemoradiotherapy were analyzed retrospectively. The experience of grading nursing care for patients with radioactive proctitis induced by neoadjuvant chemoradiotherapy was summarized.Results Of 162 patients,radioactive proctitis occurred in 110 patients(67.9%).Among the 110 patients with proctitis,62(38.3%)were grade I radioactive proctitis,29(17.9%)gradeⅡand 19(11.7%)gradeⅢ.The graded nursing was done to the patients with radioactive proctitis and achieved good effect.Conclusions Radioactive proctitis has a high incidence in rectal cancer patients with preoperative neoadjuvant chemoradiotherapy and graded nursing care can reduce the distress of patients with radioactive proctitis effectively and promote wound healing,thus to improve their quality of life.
9.Distribution of anaerobes infection in gingivitis and comparative in vitro activity of ?-lactamase

Objective To understand the distribution of anaerobes in gingivitis infection, and anaerobes sensitivity to ? lastams Methods 44 specimens were taken from the gingival crevicular fluid of gingivitis. The anaerobic bacteria were cultured and identified Minimal inhibitory concentrations (MIC) were measured by using agar dilution method Results Among all samples, the isolation of anaerobes was 100%, the isolation of P.melaninogenica was 86.4%, P.intermedia was 56.8%, B.forsythus was 52.3%, P.gingivalis was 65.9%, F.mucleatum was 63.6%, P.micros was 52.3%, Actinomyces ssp was 38.6% and Capnocytophaga ssp was 9.1% respectively. The anaerobes are sensitive to ? lastams. Conclusion In gingival crevicular fluid, the infection rates of P.melaninogenica, P.intermedia, B.forsythus, P.gingivalis, F.mucleatum, P.micros, and Actinomyces ssp were significantly higher than those of the other anaerobes. ? lastams can be used for the treatment of anaerobes infected gingivitis.
10.Determination of serum levels of visfatin and high-sensitivity C-reactive protein in patients with polycystic ovary syndrome

Objective To explore serum levels of visfatin and high-sensitivity C-reactive protein (Hs-CRP) in patients with polycystic ovary syndrome (PCOS), as well as their clinical significance. Methods Ninety patients with PCOS and 30 ovulating healthy women with body mass index (BMI) less than 25 as controls were recruited. PCOS patients were divided to two groups, 48 in obese group with BMI≥ 25 and 42 in non-obese group with BMI<25. Serum level of visfatin was measured by enzyme-linked immunosorbent assay (ELISA) and that of Hs-CRP was assayed by immunoturbidimetry in all participants. Oral glucose tolerance test (OGTT) and insulin sensitivity were assessed in PCOS patients only. Results ①Obese and non-obese patients with PCOS beth presented markedly higher serum levels of visfatin and Hs-CRP than those in controls (t=11.35, 8.46, 10.43 and 8.25, respectively, P<0.05), and levels of visfatin and Hs-CRP were higher in obese group than those in non-obese group (t=7.74 and 6.38, respectively, P< 0.05 ). ②Univariate analysis revealed that serum levels of visfatin and Hs-CRP positively correlated with BMI and homeostasis model assessment insulin resistance, respectively (P<0.05) and serum level of visfatin positively correlated with that of Hs-CRP (P<0.05). Conclusions Chronic inflammation possibly exists in patients with PCOS, which may cause their serum visfatin and Hs-CRP levels increased.
