2.E-Flow imaging on detection of the fingertip's microvascular changes of hemodynamics in patients with essential hypertension

objective To assess relationship between microvascular changes of hemodynamics of the fingertip and evolution of hypertension in patients with essential hypertension(EH)by enhanced power Doppler imaging(E-Flow).Methods The right ring fingertip'S blood flow peak systolic velocity(PSV),end-diastolic velocity(EDV),resistance index(RI)of finger ventral arteriole and nail bed arteriole in thirty-nine patients with EH were separately observed and were made contrast with 35 age-matched healthy subjects by E-Flow imaging.Results Compared with traditional colour Doppler flow imaging and power Doppler imaging,E-Flow imaging was clearer to demonstrate the distribution and movement of the fingertip'S microvascular.It was shown that the PSV and EDV in the ring finger ventral arteriole and nail bed arteriole of EH were decreased(P<0.05),but the RI increased(P<0.05),along with the evolution of EH.Conclusions Fingertip'S microvascular changes of hemodynamics could be a criterion to detect obstruction alternations of essential hypertension.E-Flow imaging iS superior to traditional color Doppler flow imaging and power Doppler imaging in differentiating microvascular and could provide a new method in assessing microvacular changes of patients with EH for clinic.
3.Advances in the study of tuberculosis live vector vaccines

Currently,the BCG is used to prevent tuberculosis,but the immune effect is not ideal due to varied reasons.The existence of drug-resistant strains of tuberculosis and the increased prevalence and incidence of AIDS have leaded to the increased incidence of TB year by year.Therefore,the development of new tuberculosis vaccine is imminent In this paper,the latest research results in recent years for tuberculosis live vector vaccines were summarized,which provide a theoretical reference for further research and development of new TB vaccines.
4.Biomechanical properties of thoracic spine with various locations of metastatic defects:three-dimensional finite element analysis

BACKGROUND:The spinal column is the most common site of cancer metastases. Most of the previous biomechanical experiments utilized models with defects only in the vertebral body or posterior elements, but the biomechanical changes of the thoracic vertebrae and posterior part with various locations of metastasis deserve further research. OBJECTIVE:To set up the three-dimensional (3D) finite element model to investigate biomechanical effects by simulating combined destruction of vertebral body and other posterior components. METHODS:Based on CT data, we constructed the 3D geometric models of the thoracic vertebrae (T9-11), including intervertebral discs, ligaments and ribs using the Mimics software. The 3D models of T9 vertebra and different parts of the posterior thoracic vertebrae related with the metastasis could be simulated, including the control group with the intact vertebrae, the group of the T10 vertebrae with the right defective hemi-vertebrae, the group of the defective hemi-vertebrae with the defective ipsilateral pedicle, the group of the defective hemi-vertebrae with the defective ipsilateral costovertebral joint, the group of the defective hemi-vertebrae with the defective ipsilateral pedicle and costovertebral joint, the group of the defective hemi-vertebrae with the defective ipsilateral pedicle, costovertebral joint and transverse process. The corresponding 3D finite element models were established using the Abaqus software. The displacement and Von Mises stress distribution of the models were analyzed when the anterior compressive flexure load was applied. RESULTS AND CONCLUSION:When the anterior compressive flexure load was applied, the entire stiffness was proportionaly decreased when the more posterior parts destroyed, especialy destruction of vertebral body and pedicle significantly decreased. The destruction of posterior structures such as the thoracic rib joints and transverse processes was not great. However, the maximal Von Mises stress increased significantly when the vertebral body and pedicle were destructed, but additional costovertebral joint destruction slightly decreased the maximal Von Mises stress because of the stress was re-distributed.
5.Subarachnoid adrenal medullary transplants reduce neuropathic pain in rats

Objective:To investigate the efficacy and characteristics of subarachnoid xenogenetic adrenal medullary transplants on neuropathic pain. Method: Eighty rats were assigned to 4 different groups before their right sciatic nerves were tied according to the method described by Bennet. Each rat received subarachnoidly xenogenetic medullary pieces or isolated chromaffin cells according to the groups. The basal pain thresholds to thermal and electrical stimuli were determined before nerve ligations,and the analgesiometric tests were repeated at weekly intervals following transplantation for 10 weeks. Behavioral indications of spontaneous pain were recorded concomitantly. The effects of naloxone, phentolamine and fentanyl on the antinociception of chromaffin cells were evaluated. Result:The pain thresholds to noxious thermal and electrical stimuli increased after transplantation of medullary pieces or isolated chromaffin cells subarachnoidly. The Behavioral indications of spontaneous pain and hyperalgesia to thermal and electrial were also eliminated after the transplantation. These antinociceptive effects can be reversed by naloxone and phentolamine. Conclusion:Transplanting xenogenetic chromaffin cells into subarachnoid space can reduce neuropathic pain effectively,and this antinociception is conducted via adrenoreceptors and opiate receptors.
6.Quantitative Analysis of Longitudinal Strain and Strain Rate of Left Ventricle in Normal Infants and Young Children

Purpose To quantitatively evaluate longitudinal strain and strain rate (SR) of left ventricular (LV) in normal infants and young children using velocity vector imaging (VVI). Materials and Methods A total of 50 normal infants and young children were enrolled in the study. The longitudinal strain peaks and SR peaks in three sections of LV walls during systole period and relaxation period were measured by VVI from apical four-chamber view, apical two-chamber view and long axis view, respectively. Results Forty-one cases (82%) presented a smooth strain curve without incisures and a double peak in SR curve (systolic peak and single diastole peak). The strain in anterior septum and posterior septum, the systolic SRs and diastolic SRs decreased from basal to apical segments. However, only the strain in basal segment and that in apical segment in anterior septum showed statistic difference (t=1.79, P<0.05);the systolic SRs of the same level in different walls and those in the same wall at different levels had no statistic difference (P>0.05). Conclusion Most normal infants and young children present a smooth diastolic strain curve and a double peak SR curve of LV, which can provide clinical guidance in the early diagnosis of myocardial abnormality for infants and young children.
7.Effect of short-term insulin pump therapy on left ventricular systolic function in type 2 diabetes mellitus patients evaluated by three-dimensional speckle tracking imaging

Objective To assess the improvements of left ventricular systolic function by three‐dimensional speckle tracking imaging ( 3D‐STI) in type 2 diabetes mellitus ( T2DM ) patients after short‐term insulin pump intensive therapy . Methods Thirty‐five T2DM patients complicated with microangiopathy and thirty‐two healthy volunteers were studied ,underwent the dynamic image of the four‐chamber view ,three‐dimensional images of left ventricle were obtained for all the individuals . The left ventricular global longitudinal strain ( LVGLS) ,left ventricular global circumferential strain ( LVGCS) ,left ventricular ejection fraction (LVEF) ,peak basal and apical rotation (LV‐ProtB ,LV‐ProtA) ,peak LV twist ( LV‐tw ) were calculated using TomTec software .After insulin pump intensive therapy for two weeks ,all the indexes were reexamined in T2DM patients . Results Compared with control group ,the LVGLS , LVGCS ,LV‐tw and LV‐ProtA were significantly decreased in diabetes mellitus group before and after treatment ( P < 0 .01 or P < 0 .05) . Compared with diabetes mellitus patients before treatment ,the LVGLS ,LVGCS had significant higher level after treatment( P <0 .05) . The LVGLS ,LVGCS ,LV‐tw and LV‐ProtA were significantly correlated with LVEF in type 2 diabetes mellitus patients and normal controls . Conclusions Insulin pump intensive treatment could improve left ventricular systolic function in type 2 diabetes patients complicated with microangiopathy . 3D‐STI can be sensitive to accurately assess the therapeutic effect and has the important clinical value .
8.Chronic granulomatous disease and Mcleod syndrome caused by continuous X chromosome deletion:a report of two cases and literature review

Objective To explore the clinical features of chronic granulomatous diseases and Mcleod syndrome caused by continuous X chromosome deletion. Methods The clinical data of two children diagnosed as chronic granulomatous disease and Mcleod syndrome by gene detection were retrospectively analyzed. Results Two males, 4 year 1 month and 1 year 9 month old, were both hospitalized due to persistent pulmonary infections. Both of them had a history of repeated severe infections and BCG vaccine associated lymphadenitis, and were diagnosed as X-linked chronic granulomatous disease for respiratory burst defects and deletion of all CYBB exons. Both of them had retarded motor development, and were diagnosed as DMD for detection of DMD gene exons and muscle speciifc promoter region and exon 1-2 deletion by MLPA. One case was found with obvious echinocytes, the other case showed whole exons deletion of XK gene. Both of them were diagnosed as Mcleod syndrome. Conclusion Continuous X chromosome deletion could lead to combination of Mcleod syndrome, DMD, and X-CGD, which may complicate the condition. Due to the lack of Kx antigen, repeated common blood transfusion can produce relative antibody, which lead to severe hemolytic crisis.
9.Evaluation of the carotid artery elasticity in patients with type 2 diabetes mellitus using two-dimensional strain imaging

Objective Three parameters include circumferential strain (CS),circumferential strain rate (CSr) and stiffness parameter (β) were measured to evaluate the elasticity of carotid artery using two-dimensional(2D) strain imaging and evaluate its clinical value in patients with type 2 diabetes mellitus (T2DM).Methods 60 patients with T2DM were collected and divided into carotid intima-midia thickness (CIMT) thickeness group(34 cases,1.0 mm<CIMT≤1.2 mm) and plaque group(26 case,CIMT> 1.2 mm).33 normal people were supplied as control group.The systolic global peak CS and CSr of the carotid artery were obtained from short-axis view of the common carotid artery and the stiffness parameters β was measured using 2D strain imaging.All the parameters were compared among the three groups.Results The CS and CSr were decreased and β was increased in patients with T2DM (P < 0.01),The CS and CSr obtained from the plaque group were less than that of the CIMT thickeness group(P <0.05),but the β was higher than that of the CIMT thickeness group (P <0.01).The CS and β were correlated significantly with CIMT respectively (r =-0.79,P < 0.01 ; r =0.72,P < 0.01).Conclusions 2D strain imaging is a technique for early assessing the carotid artery elasticity in patients with T2DM,the thicker the CIMT is,the smaller the deformation is and the larger the β is.
10.Application of echocardiography in the diagnosis of neonatal ductus arteriosus aneurysm

Objective To investigate the application of echocardiography in the diagnosis of neonatal ductus arteriosus aneurysm (DAA).Methods A total of 27 neonates with DAA were detected by echocardiography.Echocardiographic features in these cases were explored and outcome of these neonates were followed up.Results There were four crucial features in DAA detected by echocardiography:① A vessel bump was shown on the left side of the origination of left pulmonary artery(LPA) in the high parasternal short-axis view.②A horizontal shunt jet from the bump to the origination of LPA was shown when interrogate the top of the bump.③The bump joined the descending arota when interrogate it down and back orientately.A ‘V' shaped sign was shown when the aortic arch appeared on the right side and the bump on the left side.④A slow red flow signal was found in bump when the scale of color Doppler was cut down.The follow-up echocardiography showed spontaneous closure of the DAA when 3 months of age except one case lost.There were no abnormality in the figure and function in heart.Conclusions Echocardiography can be a valuable method for the diagnosis of neonatal ductus arteriosus aneurysm.Neonate with DAA have a good outcome.
