1.Bibliometrics research about nursing needs of the aged at home in China

Objective To summarize the main results and characters of the research literature by reviewing systematically and comprehensively,we get an overall awareness of the current status of the nursing needs of the aged at home and discover the deficiency.Consequently,we provide evidence to improve the nursing services for the aged at home.Methods We systematically searched CNKI (1979-2014.5),CBM (1978-2014.5)and VIP (1989-2014.5).According to the including criteria and excluding criteria,we screened the literature twice.Main results and characters of the literature were described.Results A total of 975 articles were included.The first literature were published in 1986.From 2006,the amount of the literature increased annually.42.36% (413/975)of the research types were descriptive research.The contents of the literature mainly focused on the need of comprehensive nursing services,such as community nursing services [37.44%(365/975)];home care [20.10%(196/975)];long-term care [10.26%(100/975)].The overall quality of the hterature was not high.Experimental research and descriptive research were implemented mostly in developed areas.The 46.97%(458/975) of the first authors worked in hospitals.Conclusions There is a small quantity of literature published with low quality.Questionnaire with comprehensive items describing needs demanded by the aged are required.We should put an eye on the aged living in undeveloped areas or with recognition impairment.Meanwhile,depending on the general hospital,we should carry out high-quality experimental research,as well as exploring multidisciplinary model.
2.Effect of retroviral vectors carried sense or antisense TGF?1 on human bladder cancer cells in vitro

Purpose:To investigate the effect of replication defective retroviral vectors carried sense or antisense TGF?1 fragment on the cell cycle regulation and proliferation of human bladder cancer. Methods:The replication defective retroviral vectors that integrated sense or antisense bioactive fragment of transforming growth factor?1 were constructed,and named as pRevT? and pRevT?-AS respectively. The influence of each vector on the cell proliferation,clone-formation and alteration of cell cycle of bladder cancer cell line EJ were observed in vitro.Results:The titre of pRevT? and pRevT?-AS were 0.84,0.88?10 5 CFU/ml respectively,the vectors integrated to EJ cells and expressed efficiently. Inhibition TGF?1 gene expression reduced proliferation and clone-formation rates of EJ cells. The G 0 /G 1 stage ratios in the antisense TGF?1-transfected EJ cells were increased,simultaneously,the S stage ratios were decreased. Conclusions:The antisense TGF?1 vector can reduce the expression of endogenous TGF?1 in EJ cells,induce G 1 stage arrest and inhibit proliferative growth in vitro.
3.The protective effect of prenatal hypoxia adaptation on the brain of newborn rat

Objective To examine whether the prenatal hypoxia adaptation has any protective effect on the brain of the newborn rat Methods 12 22 day pregnant Wistar rats were randomly divided into 2 groups: the control group and the treated group In the treated group the pregnant rats were placed in a tightly closed hypoxia adaptation chamber When the O 2% in the chamber dropped to 15%, the rat was taken out to breathe fresh air for 5 min then put back in the chamber This process was repeated until its natural delivery In control the chamber was not tightly closed (O 2%=21%) 40 newborn rats weighing 6 8g were selected and subjected to brain ischemia and hypoxia, Left common carotid artrey was ligated under ether anesthesia 2h after recovery from surgery the newborn rats were placed in hypoxia chamber (T=36℃?1℃,O 2%=9%)for 1 5h 24h later they were sacrificed and brain was removed for microscopic examination (optical and electron) and flow cytometry measurement Results In the treated group most newborns were normal There were a few apoptosis cells in early stage The rate of apoptosis was 2 9%, necrosis cells could hardly be seen In the control group, although most neurons were also normal but there were apoptosis cells in early, middle and late stage and even necrosis cells The rate of apoptosis was 9 51%, which was significantly different from that in the treated group Conclusions Prenatal hypoxia adaptation has protective effect on the brain of newborn rat
4.Effects of clod plasma cardioplegia on the function and structure of immature myocardium in rabbits

Objective To investigate the effects of clod plasma cardioplegia on the function and structure of immature myocardium. Methods Thirty-two new Zealand white rabbits of 17-26 days weighing (512 ?56) g were anesthetized and heparinized. Their hearts were excised and perfused in a Langendorff apparatus with warm (37℃) oxygenated (95%O2, 5%CO2) Krebs-Henseleit buffer(KHB) at 60 cm H2O perfusion pressure. After the isolated heart had been beating regularly for 10 min, the heart was perfused with clod (10℃) St. Thomas cardioplegic solution (group C, n = 16) or cold cardioplegic solution mixed with plasma (St. Thomas solution: plasma = 1:4) (group P, n=16). The K+ concentration of the cardioplegic solution in both groups was the same. After 30 min cardiac arrest the heart was again perfused with KHB. The following observations were made: (1) the interval from the beginning of cardioplegic solution infusion to cardiac arrest and the amount of cardioplegic solution used; (2) the duration from KHB reperfusion after 30 min cardiac arrest to the recovery of regular heart beat; (3) heat rate after recovery from cardioplegia; (4) coronary flow; (5 ) myocardial water content; (6) myocardial structural changes shown by microscopic examination. Results (1) There was no significant difference in the time needed to induce cardiac arrest and for recovery of regular heart beat from cardioplegia. 6.Effects of different doses of dexmedetomidine on the recovery quality from general anesthesia undergoing thyroidectomy

Objective To investigate effects of two doses of dexmedetomidine (Dex) on the recovery quality from general anesthesia undergoing thyroidectomy.Methods A prospective randomized controlled double-blind trial was conducted in 90 patients admitted to Shengjing Hospital of China Medical University who were scheduled for thyroidectomy.They were randomly divided into three groups:group D0.4 received Dex 0.4 μg/kg intravenously,group D0.8 received Dex 0.8 μg/kg intravenously,and control group with same volume of normal saline.There were 30 patients in each group,and all the patients received the above drug or saline 30 minutes after intubation.The heart rate (HR),systolic blood pressure (SBP) and diastolic blood pressure (DBP) were recorded before Dex administration,at the end of surgery,time of eye opening and extubation,and 10 minutes after extubation.Time of eye opening after surgery and time of extubation after eye opening were recorded.End-tidal partial pressure of carbon dioxide (PETCO2) before and after operation as well as restlessness score (RS),Ramsay sedative score,and visual analogue pain scale (VAS) scores at 10 minutes after extubation were recorded.Events during extubation including cough,nausea and vomiting,respiratory depression were also recorded.Results ① Compared with control group,in D0.4 and D0.8 groups incidence of lowering of HR,SBP and DBP was lower at time of eye opening and extubation,and 10 minutes after extubation.Compared with D0.4 group,D0.8 group had lower HR at the time of extubation and 10 minutes after extubation,SBP was lower at the time of eye opening and extubation,and lower DBP at the time of eye opening.② D0.4 and D0.8 groups showed lower RS and VAS scores than those of control group,Ramsay sedative score in groups D0.4 and D0.8 was higher than that in control group (RS:1.40±0.51,1.20±0.42 vs.1.90±0.56; VAS:1.50±0.52,0.80±0.63 vs.2.50 ± 0.52; Ramsay:2.10 ± 0.56,2.40 ± 0.51 vs.1.60 ± 0.51,P<0.05 or P<0.01),and VAS score in group D0.8 was lower than that in D0.4 group (P<0.01).The time of eye opening and extubation were longer in group D0.8 as compared with those in control and D0.4 groups (minutes:12.50 ± 1.08 vs.10.50 ± 1.58,10.40 ± 1.26; 15.00 ± 0.94 vs.13.00 ± 1.63,12.80 ± 1.13,P<0.05 or P<0.01),but there was no significant difference between the latter two groups.No significant difference in PETCO2 was found among three groups before and after surgery.③ The incidence rate of cough,nausea and vomiting in control group (16.7%,13.3%) were significantly higher than those in groups D0.4 (3.3%,0) and D0.8 (0,0).There was no respiratory depression in the three groups.Conclusion Adjunctive infusion of Dex 0.4 μg/kg at 30 minutes after anesthesia induction was recommended as it may result in more steady hemodynamics,with shorter recovery time and extubation time after thyroidectomy.
7.Review on primary intraventricular hemorrhage

Primary intraventricular hemorrhage is a rare type of non-traumatic cerebral hemorrhage neurological disorder.Not only has it higher mortality and morbidity,but also complicated etiologies.However,there is still lack of standard diagnostic techniques and treatment methods for decreasing mortality rate and improving prognosis of primary,intraventricular hemorrhage.Rational use of ultra-early hemostatic therapy and acute surgery therapy are considered as clinical treatment strategies to increase survival rate and improve the quality of life for primary intraventricular hemorrhage patients.This paper aims to give review on some etiology,diagnosis and therapy methods of primary intraventricular hemorrhage,and to provide new ideas for the treatment.
8.Evaluation of reliability and validity of the serf-assessed questionnaire of the elderly health care needs

Objective To test the reliability and validity of the self-assessed questionnaire of the elderly health care needs.Methods Based on the Zuluaga-Raysmith model, the initial questionnaire was formulated by literature reviewing and interviews.Through Delphi experts consultation, a questionnaire containing 12 factors, 61 items was constructed.A total of 750 elderly residents were investigated to test the reliability and validity of the questionnaire.Results The effective rate of questionnaires was 88.67% (665/750).The total Cronbach α was 0.871.The test-retest reliability of all the items were all greater than 0.75.The item-level content validity index were all greater than 0.80.The scale-level content validity index was 0.95.14 factors were extracted by exploratory factor analysis, explaining 66.653% of the total variance.Conclusions The elderly health care needs self-assessed questionnaire is comprehensive with good stability, which can be used in a multi-center, large sample research.The research may help to get a deep understanding of health care needs of the elderly as well as influencing factors.
9.Correlation between plasma homocysteine levels and behavioral and psychological symptoms in patients with mild to moderate vascular dementia

Objective To explore the correlation between serum homocysteine(Hcy) levels and behavioral and psychological symptoms dementia (BPSD) in patients with mild to moderate vascular dementia (VaD).Methods Two hundred and ninety-three patients with mild to moderate vascular dementia (aged 40 or over) admitted to the department of neurology of the First Affiliated Hospital of Liaoning Medical College from January 2012 to January 2015.Patients were diagnosed with dementia by NINDS-AIREN criteria,MMSE scores ≤26,Hachinski ischemic scale(≥4) and clinical dementia rating(1≤ C DR ≤ 2).VaD patients were divided into high Hcy(HHcy) group (Hcy≥ 15 μ,mol/L,n=188) and control group(Hcy<15 μmol/L,n=105).The total scores and the scores of 12 functional domains of behavioral and psychological symptoms in NPI were analyzed by using comparative statistical methods.Results Prevalence of high homocysteine was 64.16% (n=188) among mild to moderate VaD (n=293).The incidence of BPSD in HHcy group(80.9%) was significantly higher than that in the control group (57.1%),the difference was statistically significant (x2=18.932,P<0.01).HHcy patients (27.95±5.04) had a significantly higher total score of NPI compared with control patients (16.87± 1.87),the difference was statistically significant (t=3.753,P<0.01).In terms of scores in 12 functional domains in NPI,the scores in sleep disorders (2.99± 1.40),high (2.10±0.53),irritability/mood swings (2.64± 1.43),abnormal behavior (1.74±0.52),disinhibition (1.40±0.43),agitation (2.02±0.74) were higher than those in control groups (1.85±0.37,0.21±0.05,1.80 ±0.56,0.36±0.09,0.45±0.07,0.68±0.23),all the difference were significant(t=2.327,t=2.012,t=2.136,t=2.066,t=2.050,t=2.007,all P<0.05).Binary logistic regression analysis showed that there were positive correlation between scores of NPI and plasma Hcy levels (OR=1.164,95% CI:1.052-1.288,P=0.003).Compared with HHcy group and control group,there were no statistical significance in regard to infarcted brain focus including the frontal lobe,parietal lobe,temporal lobe,occipital lobe,basal ganglia,brain stem and cerebellum (x2=0.528,x2=0.043,x2=0.630,x2=0.166,x2=0.657,x2=1.010,x2=0.019,allP>0.05).Conclusion High homocysteine levels are correlated with behavioral and psychological symptoms in patients with mild to moderate VaD,including performance of sleep disorders,high,irritability/mood swings,abnormal behavior,disinhibition and agitation.Moreover,the severity of BPSD is positively associated with homocysteine levels.The higher the homocysteine level,the more severe the BPSD.The serum homocysteine levels are no correlated with infarcted brain focus.
10.Extracranial and intracranial hemodynamic changes before and after carotid artery stenting

Objective To study the extracranial and intracranial arterial hemodynamic changes before and after carotid artery stenting(CAS). Methods Thirty-eight patients with extracranial internal carotid arterial(EICA) stenosis were selected.Before and after CAS, the vessel diameter,the peak systolic velocity(PSV) in bilateral EICA and the PSV, pulsatility index(PI) in bilateral middle cerebral artery(MCA),ophthalmic artery(OA) were measured respectively by color Doppler flow imaging(CDFI) and transcranial color-coded duplex sonography(TCCD).Results Compared with pre-CAS,the diameter of ipsilateral EICA showed obviously increase post-CAS(P 0.05 ).The established collateral circulation were closed. Conclusions The extracranial and intracranial arterial hemodynamic data measured by CDFI and TCCD are valuable in evaluation of CAS.CAS is an effective and safe treatment for carotid stenosis,and the long time follow-up remains to be.
