1.Application of blood flow resistance index plus the risk of malignant index in preoperative assessment of ovarian neoplasm

Objective To investigate the possibility of blood flow resistance index (RI),pulse index (PI) plus the risk of malignant index (RMI) in preoperative diagnosis of ovarian neoplasm.Methods Four hundred and sixty-eight women with ovarian tumor,30 years or older,were enrolled in current study from January 2007 to December 2012 in the People's Hospital of Tianjin.Among them,127 were with malignant ovarian tumors and 341 with benign ovarian tumors.They were retrospectively prospective assessed by RI + PI,RMI,and RI + PI combine with RMI.Results The value of RI,PI of malignant ovarian tumors group were lower than those of benign ovarian tumors group ((0.41 ± 0.08) vs (0.68 ± 0.15),t =28.83,P < 0.01 ; (0.82 ± 0.24) vs (1.67 ±0.71),t =22.26,P <0.01).RMI of malignant ovarian tumors group was (269.7 ±41.2),higher than that of benign ovarian tumors group(25.6 ± 11.4,t =107.55,P <0.01).The indices of RI <0.40 and/or PI <1.00,RMI > 200 were served as the critical biomarker respectively.Histopathological examination result was conducted and considered as gold standard for identifying patients with ovarian cancer pelvic masses.The sensitivity of RI,PI plus RMI was 92.57%,higher than that used by RI and PI (81.02%) or RMI only (84.21%),and the difference was significant (P < 0.05).The sensitivity,specificity,positive and negative predictive values of the combined index of RI,PI adding to RMI were all higher than 90%.Conclusion The combined index of RI,PI plus RMI was proved to enhance the diagnostic accuracy regarding of the preoperative diagnosis of benign or malignant ovarian tumors.
3.Changes of Markers of Prethrombotic State and Their Clinical Significance with Heart Failure

Objective To investigate changes of markers of prethrombotic state(PTS)with heart failure and to explore their clinical significances.Methods Plasma levels of t-PA、PAI-1、Fg、PT、APTT、ET-1、MPV were measured in 52 patients with heart failure and in 38 normal controls.Results Compared with the healthy control group,patients with heart failure had significant increase of plasma PAI-1、Fg and ET-1level.Conclusions PTS is present in heart failure,especially in those with more severe Nyha class.
4.Efficacy of Venlafaxine and Reboxitine in treatment of selective serotonin reuptake inhibitors non-responder with major depressive disorder

Objective The purpose of this study was to access the efficacy of Venlafaxine and Reboxitine on treatment of depressive patients who had not responded to selective serotonin reuptake inhibitors for eight weeks. Methods 117 cases depressive patients who had not responded to selective serotonin reuptake inhibitors for eight weeks were divided into three Venlafaxine group(n =41),Reboxitine group(n =39),Original medicine group(n =37) by randomly and were treated by corresponding drugs for 8 weeks. The therapeutic response and safety were evaluated by scale of HAMD and TESS at before and after treatment the end of 1,2,4,8 week. Results The score of HAMD in Venlafaxine and Reboxitine groups were significantly lower than Original medicine group at the end of 4,8 weeks after treatment and the score of HAMD in Venlafaxine group were significantly lower than Reboxitine group at the end of 8 weeks after treatment. The score of HAMD in Venlafaxine and Original medicine groups were marked descendanter than before treatment from after treatment second weekend(P <0.01),while reboxitine group did not marked descent until the 4th weekend after treatment. The reduce score of factors in cognition,retardition and desperation of Venlafaxine and Reboxitine groups were significantly higher than Original medicine group at the end of treatment. The curative effect of Venlafaxine group were supper than Original medicine group. The side reaction of three groups were not difference. Conclusions There are obviously curative effect using Venlafaxine to treat the depressive patients who had not responded to selective serotonin reuptake inhibitors. The time of initial effect in Venlafaxine were rapider than Reboxitine group.

Objective To explore the effect of hemeoxygenease/carbon monoxide system on hypoxic pulumonary vascular structural remodeling. 6.The investigation of geriatric syndromes in hospitalized patients

Objective To understand the incidence of common geriatric syndromes in hospitalized patients,and analyze the relationship between different kinds of geriatric syndromes.Methods A crosssectional study was conducted by five well-trained investigators-300 patients were chosen from geriatric department and medical department within 3 top three hospitals in Chengdu.Results 300 copies of questionnaires were distributed in total,and 300 questionnaires were returned (response rate 100%).Among those,277 questionnaires were considered valid (valid response rate 92.3%).Among those elderly which were above 65 years old,having risk of falling accounted for 91.3%,feeling pain accounted for 79.1%,chronic constipation accounted for 59.2%,having risk of malnutrition accounted for 49.1%,malnutrition accounted for 28.2%,having risk of pressure ulcers accounted for 32.5%,incontinence accounted for 25.3%,and dysphagia accounted for 17%.The correlation among these syndromes was signifcant.Conclusions The overall incidence of geriatric syndromes in hospitalized patients is higher than that in the community.These syndromes are correlated with each other.One can be a risk factor of other geriatric syndromes and also could affect each other.
7.Expression and significance of MHC class Ⅰ chain-related gene and mMIC in acute leukemia

Objective To detect and determine the expression and significance of MHC class Ⅰ chain-related gene A/B (MICA/B) and membrane MIC molecules (mMIC) on the bone marrow mononuclear cells (MNC) of patients with acute leukemia (AL). Methods Expression of MICA/B gene was detected by semi-quantitative reverse transcriptaso polymerase chain reaction (RT-PCR) in MIC-pesitive K562 cell line, bone marrow MNC from 10 healthy people and 69 cases of acute leukemia (AL). Expression of mMIC was detected by Western blotting. The differences of the expression of MIC gene and mMIC between AML and ALL were compared. The prognosis was determined by chromosome type between patients with mMIC+ and mMIC-. Results The expression of MIC gene and mMIC could not be detected in healthy people. The expression rate of MICA gene was 49.28% and the MICB gene was 42.03% and the mMIC was 34.78% in patients with AL. In AML group, the expression rate of MICA gene was 60.00%, and the expression rate of MICB gene was 53.33%, and the expression rate of mMIC was 44.44%. But in ALL group, the expression rate of MICA gene was 29.17%, of MICB gene 20.83%, and of mMIC 16.67%. The expression of MICA/B gene and mMIC in AML group were higher than that in ALL group (P<0.05). The prognosis of patients with mMIC+ is better than the ones with mMIC-. Conclusion The up-regnlation of MIC gene and mMIC in bone marrow MNC from patients of AL may have some relationship with the occurrence of AL The expression of MIC gene and mMIC is high in AML and low or devoid in ALL, which would be an possible mechanism that ALL cells were easy to escape killing from NK and CTL cells. Determined by chromosome type, the prognosis of AL with mMIC positive was better than the ones with mMIC negative. MIC might be one of the factors to determine the prognosis of AL.

9.High-throughput screening of single Chinese plant extracts on specific anti-hepatoma drugs

OBJECTIVE To evaluate the effect of single traditional Chinese medicine(TCM) herb extracts on hepatoma and normal fibroblast cells using high-throughput screening in order to obtain extracts with specific anti-hepatoma effect. METHODS 242 commonly used TCM herbs were extracted by petroleum ether,ethanol and water,respectively. The total number of TCM extracts was 554. The cyto?toxicity of samples was evaluated by MTT in human hepatoma cells Bel7402 and mice normal fibroblasts NIH3T3. RESULTS 7.4%of the total extracts had an inhibitory effect greater than 50%for Bel7402,but 14.8% for fibroblasts NIH3T3 cells. Extracts with an inhibitory effect above 50% on both Bel7402 and NIH3T3 cells accounted for 4.4%of the total extracts. Our results showed that the sample DF173 had preferable cytotoxicity effect on hepatoma carcinoma cells in a good dose-effect relationship. DF173 is an ethanol extract from Stephania tetrandra,which is a commonly used herb in TCM. The cytotoxic IC50 of DF173 against Bel7402 was 8.27 mg·L-1,and 19.48 mg·L-1 on NIH3T3. CONCLUSION The components of TCM herbs are highly complicated. The combination of tumor cells with normal fibroblast cells to evaluate the cytotoxicity effect during anti-tumor drug screening will contribute much to the discovery of TCM drugs with high anti-tumor efficiency and lower toxicity.
10.Alteration of pulmonary vascular structure and gaseous molecules in rats with pulmonary hypertension induced by high pulmonary blood flow

AIM: To examine the alteration of pathologic structure and gaseous molecules in rats with pulmonary hypertension induced by high pulmonary blood flow.METHODS: Aortocaval shunting was produced for 11 weeks in rats, and pulmonary hemodynamics was evaluated.Pulmonary vascular micro- and ultra- structure was also examined.Meanwhile,the concentration of plasma nitric oxide (NO) and carbon monoxide (CO) was measured by spectrophotometry.The expression of endothelial nitric oxide synthase (eNOS) and heme oxygenase-1 (HO-1) in pulmonary arteries was detected by immunohistochemistry.RESULTS: After 11- week aortocaval shunting,pulmonary artery mean pressure was significantly increased.Muscularization of small pulmonary vessels and relative medial thickness and area of pulmonary arteries were obviously increased in shunting rats compared with controls.Ultrastructure of intrapulmonary arteries changed obviously in shunting rats.Meanwhile,plasma NO concentration was increased and eNOS expression in pulmonary artery endothelial cells was significantly augmented in rats of shunting group.Plasma carbon monoxide level and HO-1 expression in puomonary artery smooth muscle cells,however,were not altered in shunting rats.CONCLUSIONS: Pulmonary vascular structural remodeling is the important pathologic basis of pulmonary hypertension induced by a left-to-right shunt,and NO other than CO might play an important regulating role in the development of high pulmonary blood flow-induced pulmonary hypertension.
