1.Effect of exercise rehabilitation pathway on rehabilitation of elderly patients with chronic heart failure

Objective To investigate the effect of exercise rehabilitation on the rehabilitation of elderly patients with chronic heart failure.Methods One hundred elderly patients with chronic heart failure were randomly divided into study group and control group in equal number using random digits table.The patients in the two groups were given regular exercise and nursing and those of the study group rehabilitation training on the basis of conventional care.Results After 3 months care,the scores on LVESV,LVEDV, LVEF and NYHA in the study group were significantly improved compared to the control group (all P<0.05).After nursing,6 min walking distance of the study group was significantly longer than that of the control group (P<0.05).The rate of readimission in the study group was significantly lower than that of the control group (P<0.05).Conclusions For elderly patients with chronic heart failure, exercise rehabilitation is effective and safe,and can improve the cardiac function.The exercise performance and life quality also can shorten hospitalization time,decrease readmission rate and promote rehabilitation of patients.
2.Prevention of Empyema of Post-operative Esophagogastrostomy Anastomotic Fistula Inside Thorax

OBJECTIVE To investigate the cause,diagnosis and treatment of the empyema of the anastomotic fistula after esophageal cancer resection.METHODS The clinical data of 624 cases after esophageal cancer and cardia cancer resection from Jan 2000 to Dec 2006 were analyzed retrospectively which included 14 cases of the empyema of the anastomotic fistula.RESULTS All the cases were treated with the closed thoracic drainage,in which 9 cases were cured with closing double-tube chest douching technique,3 cases were succeeded to operate secondary thoracotomy for anastomosis,1 case died after being sent to the higher grade hospital,1 case died for toxic shock and respiratory failure without the chance of reoperation.CONCLUSIONS The key measures of prevention of empyema of post-operative esophagogastrostomy anastomotic fistula inside thorax are strictly distincting between indication and contraindication,carefully preparating before operation,commanding operative time and technical skill,strengthening nursing management,and fulfilling the oral manipulation and nursing prcedures about the drainage tube after operation.
3.A panel of monoclonal antibodies for distinguishing metastasis adenocarcinoma cells from reactive mesothelial cells in serous fluids

Objective To enhance the diagnosis rate for metastasis adenocarcinoma cells in serous fluids and detect a panel of a monoclonal antibodies for distinguishing adenocarcinoma cells from reactive mesothelial cells in serous fluids. Methods Three marks of low molecular weight cytokeratin (CK LMW ), carcinoembryonic antigens (CEA) and mesothelial cells (MC) were used to immunostain the cells in serous fluids from 50 patients and in peritoneal washing from 14 patients. Results Expression rate of three groups of cells positive from CK LMW , CEA and MC in adenocarcinoma cells was 95.83%, 70.83%, 16.67%, in suspicious cancer cells 100%, 50.00%, negative, in reactive mesothelial cells 33.33%, 33.33%, 66.66%. The four suspicious cases and two reactive cases in primary cytopathological classifiecation should be reclassified as adenocarcinoma cases. The sensitivity, specificity and reliability of CK LMW , CEA and MC were 96.30%, 80.00%, 77.10%; 70.37%, 90 00%, 61 60%; 90.00%, 85.20%, 75.30%, respectively. Conclusion Immunocytochemistry, using a panel of CK LMW , CEA and MC antibodies appears to be adjunct of important value for distinguishing adenocarcinoma cells from reactive mesothelial cells in serous fluids.
4.The influence of Chlamydia trachmatis and Ureaplasma urealyticum infection on perinatal out-come and nursing

Objective To study the influence of Chiamydia trachmatis (CT) and Ureaplasma ure-alyticum (UU) infection on pregnant outcome and neonates. Methods 562 pregnant women were ran-domly selected, their cervical secretion was collected and detected for CT and UU by culture method. Groups were divided according to the results. All of them were followed-up till delivery. Results The in-cidence of infection of group CT、group UU、group CT and UU were 22.1%、28.8% and 3.7%;Compared with negative group,the group with positive result of CT、UU、 CT and UU had significant higher incidence of preterm labor, premature rupture of membranes, low weight infant and neonates pneumonia. Conclusions The infection of CT、UU can result in adverse pregnant outcome, preterm labor, premature rupture of mem-branes, low weight infant and neonate pneumonia.
5.The Cytologic Diagnosis and Analysis of Canceration of Breast Intraductal Papillomas

Objective: The cytologic methods were used in the diagnosis of cancerous change of (intraductal papillomas,IP) and its significance was discussed. Methods: We studied the nipple discharge smear by using Wright-Gimsa staining. Meanwhile, infrared ray scanning B-ultrasound and X-rays were performed. Results: One hundred and twenty-eight patients were with IP, among them, 15 (11.7%)were confirmed as tumors by surgery and pathological examination. The cancerous change of cytomorphology was based on IP. The average age of patients with cancerous change was 48.1.The nipple discharge in patients with cancerous change was primarily bloody(66.7%). Conclusion: Introductal papillomas is carcerous. The possibility of canceration in persons with longer time of nipple discharge is larger than those with shorter time of nipple discharge. Women who are elder than 45 years old, with nipple discharge and post menopause, should be considered to have the possibility of cancerous change.
6.The relationship between gastroesophageal reflux disease and idiopathic pulmonary interstitial fibrosis

Objective To determine the prevalence of gastroesophageal reflux disease (GERD) in patients with idiopathic pulmonary interstitial fibrosis (IPIF). Methods From December 2006 to January 2008, 24 consecutive patients with IPIF admitted to Beijing Chaoyang Hospital underwent 24-hour esophageal pH monitoring and esophageal manometry. Meanwhile, 23 patients with diffuse parenchymal lung disease (DPLD) (excluding IPIF) admired to the hospital in the same period served as a control group. Comparison of the prevalence of pathologic esophageal acid exposure GERD symptoms, and ineffective esophageal motility (IEM) between the two groups was made. In this study, nocturnal acid exposure is defined as acid reflux episodes occurring from 10pro to 6am. Results (1) 16 out of the 24 (66. 7%) patients with IPIF were demonstrated to have pathologic esophageal acid exposure; the prevalence of GERD in IPIF patients was significantly higher than that in other DPLD patients, whose prevalence was 26. 1% (P<0.05); (2) 87.5% patients with IPIF and GERD (GERD-IPIF) had nocturnal acid exposure episodes; (3) only 37.5% of the GERD-IPIF patients was found to have typical GERD symptoms such as heartburn and regurgitation; (4) The prevalence of IEM was similar in IPIF and other DPLD patients, being 42.9% and 39. 1% respectively (P >0. 05). Conclusions IPIF patients have higher prevalence of GERD and most of them usually do not show typical reflux symptoms. It is hereby suggested that IPIF patients should be screened with pH monitoring for GERD.
7.The expression of interleukin-25 in blood and nasal tissue of patients with chronic rhinosinusitis with nasal polyps.

OBJECTIVE:
To explore the expression of interleukin-25 (IL-25) in chronic rhinosinusitis with nasal polyps (CRSwNP) and its potential significance in pathogenesis.
METHOD:
IL-25 expression in blood was detected by enzyme-linked immunosorbent assay (ELISA). IL-25 expression in tissue was detected by immunohistochemistry (LSAB method) from polyps (68 CRSwNP patients) and 55 inferior turbinate mucosa from patients with deviation of nasal septum served as control. Complete blood count and HE staining of blood and tissue eosinophil infiltration degree.
RESULT:
IL-25 expression in CRSwNP group were significantly higher than the control group, the difference was statistically significant (P < 0.01). IL-25 expression in local organizations was positively correlated with the number of eosinophil infiltration in CRSwNP group (r = 0.679, P < 0.01).
CONCLUSION
The expression of IL-25 in CRSwNP patients mutually reinforcing and might increase eosinophil infiltration and play an important role in the development of CRSwNP.

8.Screen the antioxidants in the preparation process of fish oil cream

Objective To investigate the effects of antioxidant on the fish oil cream and to enhance its stabil-ity.Methods The fish oil cream containing different antioxidant such as vitamin C,vitamin E,EDTA -2Na were prepared.The content of vitamin A with 6 months acceleration test and 12 months after long -term test were deter-mined by HPLC.Results Antioxidants had significant effect on the vitamin A content in fish oil cream,the sequence that antioxidants enhance the stability of the fish oil cream was as follows:VE +Na2 S2 O5 >VC +Na2 S2 O5 >VE +ED-TA -2Na >VC +EDTA -2Na >VE ﹥ Na2 S2 O5 >VC >EDTA -2Na.Conclusion The fish oil cream with antioxi-dant have favorable stability,composite antioxidant is better than single antioxidant.
9.Influence of different volumes of ropivacaine with the same solubility on diaphragmaticparalysis following ultrasound-guided supraclavicular brachial plexus block

Objective To investigate the effects of different volumes of ropivacaine with the same solubility in supraclavicular brachial plexus block(SCBPB) under ultrasound guidance on ipsilateral diaphragmaticparalysis.Methods Seventy two patients (32 males,40 females, aged 18-65 years, of ASA Ⅰ or Ⅱ, scheduled for the right upper limb fracture internal fixation removal were randomized into group A (0.375% ropivacaine 20 ml) and group B (0.375% ropivacaine 30 ml), 36 cases in each group.The onset and duration of sensory and motor block were observed as well as complication.Diaphragmatic excursion were evaluated by M type ultrasound before and 30 min after drug injection under eupnea and forced respiration through observing diaphragmatic excursion to study diaphragmatic paralysis.Results The onset time of sensory block and maintaining time of sensory block or motor block had no significant difference between the two groups.Compared with group A,the onset time of motor block in group B was significantly shorter (P<0.05).Twelve cases (33.3%) in group A and 22 cases (61.1%) in group B respectively showed complete or partial hemidiaphragmatic paralysis 30 min after drug injection.Compared with group A, the rate of diaphragmatic paralysis in group B was significantly higher (P<0.05).Conclusion Supraclavicular brachial plexus block under ultrasound guidance with 0.375% ropivacaine 20 ml or 30 ml both can provide excellent anesthetic effect.0.375% ropivacaine 20 ml is less likely to lead to diaphragmatic paralysis.
10.The expression of miR-155 and miR-146a in peripheral blood mononuclear cells and plasma of rheumatoid arthritis patients

ObjectiveTo investigate the expression of miR-155 and miR-146a in peripheral blood mononuclear cells (PBMC) and plasma of rheumatoid arthritis (RA) patients.MethodsPBMC and plasma were separated from the peripheral blood of 34 RA patients and 15 healthy individuals.Total RNAs were isolated and miRNAs were purified.The levels of miR-155 and miR-146a were determined by quantitative reverse transcription PCR(qRT-PCR).U6 was used as housekeeping control.The amount of target miRNA was normalized relative to the amount of U6(ΔCt=ΔCtmiRNA-ΔCtU6).Relative expression levels were expressed as 2 △-ΔCt.Data were analyzed using SPSS 13.0 software.The test of homogeneity of variance and unpaired t-test was used to compare between groups.P values(2-tailed) less than 0.05 were considered as statistically significant.ResultsThe expressions of PBMC and plasma miR-155 were higher in RA patients than those in the healthy control individuals(0.08±0.08 vs 0.05±0.03,t=-2.225,P<0.05; 5.9±6.7 vs 1.3±2.0,t=-3.677,P<0.05).The expression of miR-146a in PBMC and plasma of RA patients and controls were(1.3±1.2 vs 0.8±0.6,t=-2.154,P<0.05)and(741±1001 vs 300±295,t=-1.669,P>0.05).According to their DAS28 value,RA patients were divided into high activity group (23 cases,DAS28≥5.0) and low disease activity group( 11cases,DAS28<5.0).The plasma miR-155 and miR-146a expressions were significantly higher in high activity group than those in low activity group.There were no significant differences in the expression of PBMC miR-155 and miR-146a between the two groups.ConclusionThe expression of PBMC and plasma miR-155 and miR-146a are higher in RA patients.The expression of plasma miR-155 and miR-146a are associated with RA patients' activity.Plasma miR-155 and miR-146a may be potential non-invasive biomarkers for RA diagnosis anddisease activity assessment.
