Objective To investigate the role of mitochondrial apoptotic pathway in intrathecal doxepin?induced apoptosis in rat neurons. Methods Twenty?four adult male Wistar rats, weighing 250-300 g, in which intrathecal catheters were successfully implanted without complications, were randomly divided into 4 groups ( n=6 each) using a random number table: control group ( group C) and different concentrations of doxepin groups (D5, D10 and D20 groups). In D5, D10 and D20 groups, 5, 10, and 20 mmol∕L doxepin 0. 2 μl∕g were injected intrathecally, respectively. In group C, the equal volume of normal saline was given instead. At 6 h after intrathecal administration, the animals were sacrificed, and the lumbar segments of the spinal cords were obtained for detection of the cell apoptosis ( by TUNEL assay) , expression of Bax and Bcl?2 ( by immunohistochemistry) , release of cytochrome c ( Cyt c) , and expression of caspase?3, caspase?8 and caspase?9 mRNA ( using real?time reverse transcriptase polymerase chain reaction) . Apoptosis rate and Bcl?2∕Bax ratio were calculated. Results Compared with group C, the expression of Bax was significantly up?regulated, the expression of Bcl?2 was down?regulated, Bcl?2∕Bax ratio was decreased, the release of Cyt c and apoptosis rate were increased, and the expression of caspase?3 and caspase?9 mRNA was up?regulated in D10 and D20 groups, the expression of Bcl?2 was down?regulated in group D20 ( P<0.05 or 0.01) , and no significant change was found in the parameters mentioned above in group D5 (P>0.05). The expression of Bax was significantly up?regulated, Bcl?2∕Bax ratio was decreased, the release of Cyt c and apoptosis rate were increased, and the expression of caspase?3 and caspase?9 mRNA was up?regulated in D10 and D20 groups, and the expression of Bcl?2 was down?regulated in group D20 as compared with group D5 ( P<0.05 or 0.01) , and in group D20 as compared with group D10 ( P<0.05) . There was no significant difference in the expression of caspase?8 mRNA among the four groups ( P>0.05) . Conclusion Mitochondrial apoptotic pathways, but not death receptor pathway, is involved in intrathecal doxepin?induced apoptosis in rat neurons.

The aim of the present research was to investigate and analyze the hunting intentions of medical students for rural grassroots in Gansu and to provide a reference for decision-making on rural health human resource . A questionnaire is the research tool which was used to investigate the views of the senior students from six medical schools , and we studied their employment willingness to rural healthcare institutions .Chi-square test and non-condi-tional logistic regression analyses were used for data analysis .A sample of 600 medical graduates was involved in this investigation. The results of this study show that more than a half (55.6%) of the students would like to work in ru-ral, but only 13.8%of which were “very willing” to work at the rural healthcare institutions .According to the out-comes of this investigation , the number of junior college students who would like to work at the grassroots was 2.3 times the number of the undergraduate students .Medical students whose monthly salary expectations were not high ,with the household registration in rural areas , and having a better understanding of the policy of rural grass-roots em-ployment were more likely to work at the rural healthcare institutions .Based on the findings of this investigation , it can be concluded that the grassroots medical career intention of medical students is not only influenced by its own fac -tors, such as education and household registration , but also the policy of the government , school employment guid-ance and social atmosphere play an important role .The above-mentioned factors should be considered by decision-making and management departments of public healthcare .The departments should introduce a reasonable recruit-ment policy and strengthen the construction of grass-roots medical technology , equipment , and infrastructure , im-prove the grass-roots of employment environment and career development opportunities and strengthen the medical students'employment guidance work to make medical students more willing to work at rural healthcare institutions .

Objective To explore the effect of motor imagery therapy and its combination with counterbalance exercise on the fall efficacy of elderly people. Methods Totals of 156 eligible elderly people with fear of falling ( FOF) were selected as objects and randomly divided into three groups. The first group was the control group, treated with routine rehabilitation nursing. People in the second group were treated with routine rehabilitation nursing plus motor imagery therapy. People in the third group were treated with routine nursing plus motor imagery therapy, combined with counterbalance exercise. The intervention lasted for 2 weeks. People in the three groups were assessed by fall efficacy and depression scales before and after the intervention. Results Regarding fall efficacy before and after the intervention, there were not significant differences in the control group [(3.79±1.46) points vs. (4.01±1.35)points, t=0.798, P>0.05)], or in the second group [(3.88±1.86) vs. (4.38±1.14), t=1.653, P>0.05)], but there was significant difference in the third group [(3.91±1.54) vs. (5.75±1.43), t=6.314, P<0.01)]. As for depression scores before and after the intervention, there was not significant difference in the control group [(56.68±8.64) vs. (54.21±8.28), t=1.489, P>0.05)], but there were significant differences in the second group [(55.58±7.42) vs. (47.45± 6.89), t=5.790, P<0.01)], and in the third group [(56.23±6.54) vs. (46.23±7.21), t=7.408, P<0.01)].Conclusions The motor imagine therapy alone can not significantly improve fall efficacy of the elderly with FOF, but can significantly reduce their depression. Combined use of motor imagery therapy and counterbalance exercise can significantly increase their fall efficacy and reduce their depression.

This study aims to prepare altrenogest extended-release tablets, evaluate their quality and establish a content determination method. The hydrophilic gel skeleton type, dosage and core thick-ness of altrenogest extended-release tablets were used as the investigating factors, and the release degree of the tablets was used as the investigating index, the prescription process of altrenogest ex-tended-release tablets was optimized by one-factor screening and central combinatorial design re-sponse surface method, and quality evaluation was carried out,the in vitro release model was es-tablished,and a high-performance liquid chromatography(HPLC)assay method was set up for the determination of altrenogest extended-release tablets. The results showed that the optimal pre-scription of altrenogest extended-release tablets was 2%as the main drug,70%as the solubilizer,0.5%as the lubricant,19.1%as the filler,8.4%as the hydrophilic gel skeleton material, and the thickness of the tablets was 3.8 mm.The in vitro drug release conformed to the Higuchi model, and the altrenogest showed a good linear relationship with the R2=0.999 98 in the range of 10-80 mg/L.The optimized process for the extended-release tablets was stable and had a good quality. The extended-release tablets were stable and had significant slow-release effect. The HPLC method is accurate and reliable and can be used for the determination of altrenogest in extended-release tablets.
