1.Analysis of drug resistance of Psendomonas aeruginosa in ICU during 2006-2007

Objective To discuss the drug resistance and the vicissitude of Pseudomonas aerugi-nosa in Intensive Care Unit (ICU), in order to provide basis for rational use of antibiotics in clinic. Methods Identification of bacteria was performed by applying ATB Expression Identification System produced by BioMerieux Company; drug susceptibility tesr was done by using K-B method. The drug resistance of Pseudomonas aeruginosa was compared between ICU and general wards; and then the difference of drug resistance was investigated between 2006 and 2007. Resnlts The more sensitive an-tibiotics to Pseudomonas aeruginosa in ICU of our hospital were as follows, amikacin (resistance rate 6.7%-9.4%), eiprofloxacin (resistance rate 7.7%-24.7%), cefoperazone/sulbactam (resistance rate 11.5%-34%), ceftazidime (resistance rate 11.5%-37.7%), aztreonam (resistance rate 20.2%-40.6%). The drug resistance rate of ICU was higher than that of the general wards, and the drug re-sistance rates to ciprofloxacin, ceftazidime, cefoperazone/sutbaetam,meropenem, imipenem and aztreo-nam in 2007 were significantly higher than those in 2006 (P<0.01). Conclusion The drug resistance rate of Pseudomonas aeruginosa in ICU of our hospital was very high,and it is rising year by year. So it is important to retrospect the results of the drug resistance periodically and provide the up-to-the-mi-nute data of epidemiology and vicissitude of drug resistance.
2.Mechanisms of oxaliplatin neurotoxicity and its predictions

As the main drug in colorectal cancer chemotherapy,oxaliplatin is gradually infiltrated into other malignancies therapy.But oxaliplatin-induced peripheral neuropathy limits its clinical application.The mechanisms of oxaliplatin neurotoxicity are not yet clear.Recent researches show that the acute neurotoxicity of oxaliplatin is mediated through changes in Na + transient conductances.And the function of the mitogen-activated protein kinases in chronic neuropathy has already been demonstrated in vitro.Furthermore,numerous indicators can be used to predict oxaliplatin-induced peripheral neuropathy,which bring the hope for improving the continuity of chemotherapy and realizing personalized medicine therapy.
3.Stress distribution in zygapophyseal joint following two level artificial disc replacement under axial loading

BACKGROUND:There is no study about adjacent zygapophyseal joint stress distribution following two level artificial disc replacement (ADR) in China.OBJECTIVE:To explore cervical adjacent zygapophyseal joint stress distribution following two level ADR and anterior cervical discectomy and fusion (ACDF),so as to provide biomechanical reference for clinical application of two level ADR.DESIGN,TIME AND SETTING:In vitro contrast observation and biomechanical detection were performed at the National Key Laboratory of College of Material Science and Engineering,Central South University from January to February 2006.MATERIALS:A total of 11 fresh adult corpse cervical samples with intact cervical disc and ligament and the facet joint capsule ranged from C_3 to T_1 segment.METHODS:A total of 11 intact adult fresh-frozen multi-segmental cervical spine segments were utilized and biomechanically evaluated under the following C_(4~5),C_(5~6) cervical disc conditions:intact spine (intact group),Bryan disc prosthesis implantation (ACDR group) and intervertebral fusion (ACDF group).The testing was performed under axial load (25 N-150 N) on each group.The stress changes of the caudad (C_(5~6),C_(6~7)) zygapophyseal joint was tested.MAIN OUTCOME MEASURES:The pressure of the caudad (C_(5~6),C_(6~7)) zygapophyseal joint.RESULTS:Under axial loading,stress of the caudad (C_(5~6),C_(6~7)) zygapophyseal joint increased with the load and showed no difference between the intact group and ACDR group (P>0.05).Under axial loading,the ACDF group resulted in significant increase of the stress of the caudad (C_(5~6),C_(6~7)) zygapophyseal joint compared with intact group and ACDR group (P<0.05).CONCLUSION:Two level artificial cervical disc replacement can maintain stress in the caudad (C_(5~6),C_(6~7)) zygapophyseal joint,thus reconstruct biomechanical property of cervical spine.Two level cervical discectomy and fusion can increase stress in the caudad (C_(5~6),C_(6~7)) zygapophyseal joint,which may be one of the causes for adjacent cervical spondyiosis postoperatively.
4.Risk factors of diagnostic delay of Crohn's disease and their impact on prognosis

With the rise of the incidence of Crohn's disease,clinicians began to recognize and pay attention to this disease.There is no effective method to cure the Crohn's disease currently,and the complications of the Crohn's disease are numerous and complex.Therefore,early diagnosis and treatment are very important.Due to many factors,the problen of diagnostic delay is widespread.Female patients diagnosed younger than 40 years old,the presence of ileal disease,use of non-steroidal anti-inflammatory agents,smoking history,parental symptoms are risk factors of diagnostic delay of Crohn's disease.Delayed diagnosis might increase the risk of complications,especially intestinal stenosis and operation-related complications.Crohn's disease needs to be early diagnosed and treated to improve the prognosis of patients.
5.Effects of noxious coldness and non-noxious warmth on the magnitude of cerebral cortex activation during intraoral stimulation with water.

OBJECTIVEWe used functional magnetic resonance imaging (fMRI) to explore the effects of noxious coldness and non-noxious warmth on the magnitude of cerebral cortex activation during intraoral stimulation with water.
METHODSSix male and female subjects were subjected to whole-brain fMRI during the phasic delivery of non-noxious hot (23 °C) and no- xious cold (4 °C) water intraoral stimulation. A block-design blood oxygenation level-dependent fMRI scan covering the entire brain was also carried out.
RESULTSThe activated cortical areas were as follows: left pre-/post-central gyrus, insula/operculum, anterior cingulate cortex (ACC), orbital frontal cortex (OFC), midbrain red nucleus, and thalamus. The activated cortical areas under cold condition were as follows: left occipital lobe, premotor cortex/Brodmann area (BA) 6, right motor language area BA44, lingual gyrus, parietal lobule (BA7, 40), and primary somatosensory cortex S I. Comparisons of the regional cerebral blood flow response magnitude were made among stereotactically concordant brain regions that showed significant responses under the two conditions of this study. Compared with non-noxious warmth, more regions were activated in noxious coldness, and the magnitude of activation in areas produced after non-noxious warm stimulation significantly increased. However, ACC only significantly increased the magnitude of activation under noxious coldness stimulation.
CONCLUSIONResults suggested that a similar network of regions was activated common to the perception of pain and no-pain produced by either non-noxious warmth or noxious coldness stimulation. Non-noxious warmth also activated more brain regions and significantly increased the response magnitude of cerebral-cortex activation compared with noxious coldness. Noxious coldness stimulation further significantly increased the magnitude of activation in ACC areas compared with noxious warmth.

6.Clinical features of immunocompetent patients with Penicilliosis marneffei

Objective To discuss the clinical features of immunocompetent patients with Penicilliosis marneffei.Methods To analyze and summarize the clinical data of 4 immunocompetent patients with Penicilliosis marneffei.Results Clinical manifestation of patients centered on intermittent slight or mild fever,lymphadenectasis,skin eruptions,osteolytic destruction,anaemia,and leukocytosis.The cultural Penicillium marneffei in specimen such as blood,pus showed a higher positive rate.Amphotericin B and itraconazole was available in treatment.Conclusion Immunocompetent people can be infected by Penicillium marneffei,and they appear nonspecific in clinical manifestation after infection.A timely and effective antifungal therapy can improve the prognosis of patients.
7.Paclitaxel-octreotide conjugates inhibit growth of human non-small cell lung cancer cells

Objective: To evaluate the cytotoxicity and targeting ability of self-developed paclitaxel-octreotide (PTX-OCT) conjugates on A549 non-small cell lung cancer (NSCLC) cells and Calu-6 NSCLC cells. Methods: Conjugates PTX-OCT and 2PTX-OCT were synthesized by our school. Reverse transcription-polymerase chain reaction was used to detect mRNA of human somatostatin receptor subtypes (SSTRs) using specific primers. The cells were treated with different concentrations (1, 100 nmol/L and 1 ?mol/L) of paclitaxel and the conjugates for different time periods (24-72 h); we also set up a control group. MTT assay was used to evaluate the cell viability after treatment; cell cycle perturbations were determined by FAC Scan flow cytometer 24 h after treatment with 1 ?mol/L paclitaxel, PTX-OCT, and 2PTX-OCT. Results: Both A549 cell and Calu-6 cell expressed the mRNA of SSTR2 and SSTR5; no SSTR mRNA was detected in the fibroblasts. The conjugates had a similar cytotoxicity to paclitaxel; they both effectively inhibited the growth of A549 cells and Calu-6 cells in a concentration-and time-dependent manner. After 72 h treatment with 1 ?mol/L paclitaxel, PTX-OCT and 2PTX-OCT, the survival rates of A549 cells were (26.9?7.3)%, (26.6?9.2)% and (35.7?4.3)%, respectively; the survival rates of Calu-6 cells were (29.5?5.0)%, (28.2?9.7)% and (26.5?4.9)%, respectively. 8.Comparing the differences between domestic and imported EIA-HIV screening results for voluntary blood donors

Objective To compare the differences of two kinds of EIA reagents for HIV-1/2 (Ab/Ag) screening results of voluntary blood donors,in addition to find out the feasibility of reducing 1 times of EIA detection.Methods To collect data of HIV 1/2 screening positive results and confirmatory test for voluntary blood donors from 2009 to 2014 in Jiaxing area,and to compare the relationship of screeing test results with that of the confirmatory test,and then to analyze the relevance between S/CO values of screening test and confirmatory test.Results Screening positive rates of domestic and imported reagents,which were 9.58/10 000 and 12.43/10 000,respectively;and the confirmatory coincidence rates were 11.84% and 9.12%,respectively.There was no significant difference (x2 =1.11,P>0.05).The double-reagent joint detection positive rate was 1.37/10 000,and its positive predictive value was 82.86%.Single-reagent test result compared with that of double-reagent test,which had significant differences (x2domestic =94.04,P<0.05 and x2ximported =124.86,P<0.05).When the S/CO value was more than 6,domestic and imported reagents positive predictive values were 93.55% (29/31) and 87.50% (28/32),respectively.Conclusion There is no difference between domestic and imported reagents EIA-HIV1/2.
9.Myeloid-derived suppressor cells-immunotherapy targets in hematological malignancies

In recent years, the research hot in the field of solid tumor and blood tumor focuses on the myeloid-derived suppressor cells (MDSCs).In tumors, MDSCs not only exert immunosuppression by inhibiting T cell proliferation, destroying the functions of natural killer cells and recruiting regulatory T cells, but also play non-immunosuppression roles in the promotion of angiogenesis and tumor metastasis.All of these hinder the anti-tumor therapy, and particularly affect the curative effect, which are related with a poor clinical prognosis.MDSCs can be used as prognostic markers, which provide new targets for immunotherapy.
10.Culturing and Identification of Penicillium marneffei

OBJECTIVE To improve the diagnosis level of Penicillium marneffei culture identification in the grass-roots laboratories.METHODS The patient′s blood or bone marrow were innoculated to BacT/Alert 3D blood culture system.when positive sample was found,the culture fluid and node tissues were smeared to take,Gram stain.The pathogens suchas small long branch,septate hyphae,sausage-shaped cell,were transferred to Colombia blood plate,sand Paul agar and CHROMagar Candida chromogenic culture medium to observe the 25 ℃ and 35 ℃,to observe the colony morphology and cell characteristics.The relevant biochemical reaction tests,were cutler taken.RESULTS The foundation of identifying P.marneffei culture:it is a dimorphic fungus,cultured for fungal phase at 25℃,and for yeast phase at 35℃.Both could produce wine red pigment.Morphology under the microscope:culture had typical broom-shaped sticks at 25℃,and round,oval,blunt and slightly curved at both ends,and sausage-shaped thallus that had horizontal septum.Biochemical results:both could assimilate and ferment glucose to produce acid,urea luciferase positive.Both could be inhibited by Actidione,and couldn′t assimilate fermenting lactose.CONCLUSIONS The appropriate medium for the P.marneffei is Sabourand′s medium.The appropriate stains of the cell morphology and structure are lactic acid cotton blue staining and Gram staining which are suitable for the grass-root level laboratories.
