1.Analysis the treatment effect of different ventilator parameters on acute left heart failure

Objective To investigate clinical efficacy of different ventilator parameters on acute left heart failure.Methods One hundred and nineteen patients with acute left heart failure were randomly divided into A,B,C,D group.Patients in A,B,C,D groups were treated separately with higher air pressure,high pressure,medium pressure or low pressure respectively.In each group,clinical efficacy,heart rate (HR),arterial partial pressure of oxygen (PaO2),pulse oxygen saturation (SpO2),partial pressure of carbon dioxide (PaCO2) and pH were measured.Results There were significant differences among the four groups in terms of HR,PaO2,SpO2,PaCO2 and pH (F =32.21,29.46,12.34,36.47,18.38 ; P < 0.05).Compared with that in A,C and D group,the levels of HR,PaO2,SpO2,PaCO2 and pH in B group were significant difference (P < 0.05).Meanwhile those indices in B group were difference among before and after treatment (t =12.25,13.46,8.75,9.83,2.58 ;P < 0.05).The efficacy rates of four groups were significant different (x2 =0.97,P =0.012),and in B group was 93.1%,higher than that of other 3 groups(71.0%,65.6%,51.8% ;P <0.05).Condusion The ventilator parameters(10-15 cmH2O,55%-75% FiO2 at beginning and later revised the FiO2 to 30%-40%) was proved to be worthy in treating acute left heart failure.
2.Nosocomial Infection in 2979 Cases

OBJECTIVE Based on data analysis of nosocomial infections in our hospital,main reasons of the infection,especially the relationship between infection risk and malignant conditions were investigated in detail. METHODS A retrospective study of 50 272 patients in the period form Jan 2000 to Dec 2007 showed 2979 patients(5.93%) had nosocomial infections.The reasons caused nosocomial infections were diverced. RESULTS The patients with severe blood disorder or tumors were at high risk of nosocomial infections.The longer the stay in hospital,the higher the risk they had.Moreover,the Department of Hematopoietic Stem Transplantation and the Department of Hematological Disease both accounted for the majority of nosocomial infections.Intervention therapy was another main reason of the infection.Respiratory tract was the most common infection site. CONCLUSIONS The infection rate of our hospital is lower among same kinds of hospitals.To reduce infection risk further,a new system with advanced statistical analysis software is being established.The system includes a monitor motif which could conquer the current weakness.
3.Effect of Rehabilitation Stroke Unit on Walking in Stroke Patients with Hemiplegia

Objective To investigate the effect of rehabilitation stroke unit on walking in stroke patients with hemiplegia. Methods 86 patients with walking impairment after stroke were randomly divided into control group (n=43), who accepted conventional treatment, and experimental group (n=43), who were incorporated into the rehabilitation stroke unit. They were assessed with Berg Balance Scale (BBS),10 m Maximum Walking Speed (MWS), Fugl-Meyer Assessment (FMA) and modified Barthel Index (MBI) before and 8 weeks after treatment.Results The scores of BBS, MWS, FMA and MBI improved after treatment in both groups (P<0.01), and improved more in the experimental group than in the control group (P<0.05). Conclusion Rehabilitation stroke unit is effective on walking ability in stroke patients with hemiplegia.
4.Preliminary Exploration on Treating Gallbladder for Viral Hepatitis,Clinical Observation on 60 Cases.

The viewpoint of treating gallbladder and combinedtreatment of liver and gallbladder for liver diseasse isproposed and the therapeutic project of intravenousdrip of Mixture of Radix Bupleuri plus Radix SalviaeMiltiorrhizae is formulated.Sixty eases of chronic hep-atitis of damp—heat of liver—gallbladder with block-age of collaterals by stagnant blood were thus treated.Results revealed the relief of jaundice,decrease of en-zyme and inhibition of viral replication in treatinggroup are all better than the western drug controlgroup.
5.Effects and adverse drug reactions of mtrisone in the treatment of patients with severe acute respiratory syndrome

AIM: To study effects and adverse drug reactions of mtrisone in the treatment of patients with severe acute respiratory syndrome. METHODS: The information of the medications in 680 patients with severe acute respiratory syndrome (SARS) in Xiaotangshan Hospital was collected by HIS system and the effects and ADRs of metrisone were staiated. RESULTS: The kinds of drugs of SARS patients who had been cured by metrisone were more than those which were not cured by metrisone. Condition of SARS patients who had been cured by metrisone was more serious than those which were not cured by metrisone. The ADRs rate, blood glucose and leukocyte of SARS patients who had been cured by metrisone are higher than those which were not cured by metrisone while blood K+ is lower. CONCLUSION: The utilization of metrison to SRAS patient should be more cautious to balance the effects and ADRs of metrisone.
