1.Cardiovascular protective effects of soy isoflavone

OBJECTIVE: Estrogen has cardiovascular protective effects while its adverse effects restrain its application in the therapy and prevention of cardiovascular diseases. To find a more effective and safer estrogen replacement becomes a hotspot in cardiovascular pharmaceutical researches. This paper summarized the current research situation on the cardiovascular protective effects of phytoestrogen, soy isoflavone.DATA SOURCES: Relative articles between January 1993 and December 2001 were searched by computer on Medline with the searching words of "isoflavones, atherosclerosis, vasodilation" in English, the language limitation of the articles. Simultaneously, articles between January 1994 and February 2002 were searched by computer on Wangfang Database and Chinese Journal Full Text Database with the searching words of "isoflavones,artherosclerosis, vasodilation(Chinese charcters)" in Chinese, the language limitation of the articles.DATA SELECTION: Literatures with experiments including study group and control group were selected from the data through preliminary screening to eliminate obvious non-randomized experimental studies. The full texts of the residual literatures were searched afterwards for the further judgment of ranincluded in the study. Exclusive criteria: repetitive experimental studies.DATA EXTRACTION: A total of 31 randomized or non-randomized experimental articles regarding the cardiovascular protective effects of soy isoflavone were collected, of which 26 experiments were in accordance with the inclusive criteria and the rest 5 articles were exclude due to repetition of same study.DATA SYNTHESIS: Twenty-six experiments including clinical experiments and animal experiments, which employed in vivo or in vitro two experimental methods after the application of soy isoflavone to observe and evaluate its cardiovascular protective effects. The above two methods had its own merits and shortcomings, of which in vitro experiment was a more common method for the observation.CONCLUSION: There is no adequate evidence that can prove the definite effects of soy isoflavone on the prevention and therapy of cardiovascular diseases in menopausal women, as well as its side effects. Researchers should do more researches on how to master the appropriate dose to make isoflavone reach effective blood concentration, and how to make the therapeutic effects become more beneficial.
2.Relationship of CACNA1C gene polymorphism with schizophrenia

Objective To examine the relationship of calcium channel, voltage-dependent, L type, alpha 1C subunit (CACNAIC) gene polymorphism with schizophrenia. Method A total of 118 patients with schizophrenia and 122 healthy volunteers were selected in succession. Denaturing high performance liquid chromatography (DHPLC) was used to analyze the genotypes of CACNAIC gene (rs10848683, rs2238032 and rs2299661). Medical record data of patients with schizophre?nia and the clinical manifestation of the patients (sensation and perception disorder, thought disorder, affective disorder and behavior disorder) were collected. The clinical phenotype databases of all patients were established. Result There were statistical differences for genotype and allele distribution of the rs2238032 and rs2299661 between patient group and control group (P<0.05). The TT genotype of rs2238032 (OR=0.394) and the CG genotype of rs2299661 (OR=0.326) associ?ated with the risk of schizophrenia. The genotype distribution of rs2238032 was related to the thought disorder, the emotion disorder and the PANSS score of schizophrenia patients (all P<0.05). The genotype distribution of rs2299661 was related to the perceptual disorders and PANSS score of schizophrenia patients (all P<0.05). Haplotype analysis showed that CTC (OR=1.811), CTG (OR=0.432) and TGC (OR=1.771) were significantly associated with schizophrenia (P<0.05). Conclu?sion The CACNA1C gene polymorphism is associated with schizophrenia and its clinical manifestations.
3.Study on evaluation of early breastfeeding behaviors in preterm infants

Objective To understand and evaluate the early breastfeeding behaviors in preterm infants,and to analyze the evolution process of early breastfeeding behaviors in preterm infants,and therefore to provide a theoretical basis for preterm infant breastfeeding.Methods A continuous evaluation of early breastfeeding behaviors in 38 preterm infants was performed commencing on the beginning of breastfeeding and finishing on the attainment of breastfeeding.Results The earliest breastfeeding was initiated from a postmenstrual age (PMA) of 30 weeks.Obvious rooting,efficient areolar grasp,repeated short sucking bursts and irregular swallowing were noted from 31 weeks; The maximum number of consecutive sucks was limited to between 6 and 24 sucks,with a median of 18 sucks.Full breastfeeding was attained at a median of 36 weeks,between 33 and 38 weeks.Conclusions The early breastfeeding capacity in preterm infants is enhanced and becomes perfect gradually with the implementation of breastfeeding,and that is sufficient and practical for establishment of full breastfeeding at a low postmenstrual age of preterm infants.

5.Preparation and Quality Standard of Bifonazole Hollow Effervescent Suppository

OBJECTIVE: To prepare bifonazole hollow effervescent suppository and establish its quality standard. METHODS: Bifonazole hollow effervescent suppository was prepared by hot-melting method, 38-type semisynthetic glyceride as the base material, NaHCO3 and citric acid as the effervescent agent, bifonazole as main ingredient. UV spectrophotometry was used for the content determination of bifonazole. RESULTS: The weight of prepared suppository was about 2.3 g and mean size of maximal foaming capacity were all above 8 mL. The linear range of bifonazole were 1.015~10.15 ?g?mL-1(r=0.999 9) with an average recovery of 99.74% (RSD=1.28%). CONCLUSION: The established preparation is simple, practical and up to the standard. The content determination method is precise and accurate.

7.Effects of Genistein on The Expression of Estrogen Receptor ? and c-fos Gene in Aorta in Ovariectomized Rat

Objective To study the effects of genistein(GST) on the expression of estrogen receptor ?(ER?) and the expression of c-fos gene in the aorta of ovariectomized rats.Methods Forty female Wistar rats were randomly divided into four groups:sham-operated(control),ovariectomized(OVX),ovariectomized with 17?-E2 replacement(OVX+E2),ovariectomized with genistein replacement(OVX+GST) group.After 8 weeks' replacement therapy,the rats were sacrificed and the expression of ER? and c-fos in the aorta was studied by immunohistochemistry.Results Ovariectomy significantly decreased ER ? expression and increased c-fos expression in aorta;while replacement therapy,GST and E2,attenuated the effect of ovariectomy manifested by increasing ER expression and decreasing c-fos expression in aorta.Conclusion GST may modulate the expression of ER? in the arteries of ovariectomized rats and play a beneficial effects on cardiovascular system which was associated with decreases in expression of c-fos gene in aorta.
8.Distribution and Drug-resistance Analysis of 2916 Pathogens Strains

OBJECTIVE To investigate the distribution of 2916 strains and analyze the antimicrobial susceptibility of major pathogens and to provide evidences for clinical therapy.METHODS The distribution,and the antimicrobial susceptibility of pathogens,which were collected and isolated from all of the clinical specimens from Jan 2007 to Oct 2008 in our hospital,were studied restrospectively.RESULTS Totally 2916 strains(except fungi) of pathogens were isolated from patients,among which 1857 strains were Gram-negative bacteria(63.7%) and 1056 strains were Gram-positive cocci(36.2%).The five common bacteria isolated from the specimens were Escherichia coli(14.3%),Klebsiella pneumoniae(11.8%),Pseudomonas aeruginosa(10.7%),Staphylococcus aureus(10.5%),and Acinetobacter baumanii(8.7%).The ESBLs producing E.coli and K.pneumoniae accounted for 31.4% and 33.7%,respectively;meticillin-resistant S.aureus(MRSA) accounted for 53.3% and meticillin-resistant coagulase-negative staphylococci(MRCNS) accounted for 49.6%;vancomycin-resistant S.aureus was not isolated Gram-negative bacilli had the lowest resistance to carbapenems,and then to piperacillin/tazobactam(TZP) and cefoperazone/sulbactam(CFS),showing multi-resistantce.Gram-positive cocci were more sensitive to vancomycin,rifampicin,nitrofurantoin than to other antibacterials.CONCLUSIONS The drug resistance of the isolated bacteria is common.It is very important to monitor the drug resistance of the bacteria regularly,for guiding the clinic use of antibiotics rationally,and infection control.
9.Comparative study on influence of large platelet on platelet count difference of optical method and electrical impedance method

Objective To know how large platelets influnece the platelet count difference of the optical and impedance methods so as to provide the support for the reasonable application of optical method and effectively increasing the accuracy of platelet count.Methods 132 blood routine specimens with normal RBC parameters were anaylzed by both electrical impedance and optical methods with the Sysmex-XE5000 fully automativ hematology analyzer.The data were divided into the large platelet group and the normal platelet group according to the platlet-large cell ratio(P-LCR)results.The RBC and platelet parameters were compared be-tween the two groups with the unpaired t-test and the platelet count results detected by the optical and electrical impedance methods were compared within group with the paired t-test.Results The extremely significant differences of platelet parameters existed be-tween the two groups.The P-LCR,MPV,PDW and IPF % of the large platelet group were significantly higher than those of the normal platelet group,but the RBC parameters had no obvious difference.The significant difference of the platelet count also existed between the electrical impedance and optical methods in the large platelet group(P <0.05 ),the platelet count results detected by the electrical impedance method was on the low side,but the normal platelet group had no significant difference.Conclusion P-LCR increase is related with the increase of immature platelet.The electrical impadence method conducts the differentiation according to the granular size,which is easy to exclude the large platelel with larger volume and leads to reduce the platelet count results.There-fore,when P-LCR is increased,the optical method should be adopted in conducting the platelet count for avoiding the false reduce caused by the electrical impedance method.
10.Relevance between inflammatory factors and intradialytic hypertension in patients with maintenance hemodialysis

Objective To study the relationship between intradialytic hypertension (IDH) and inflammatory factors in patients with maintenance hemodialysis.Methods Forty maintenance hemodialysis patients were involved,including 20 patients with IDH (IDH group) and 20 patients without IDH(non-IDH group) during hemodialysis.All of them had no sign of acute infections or other progressive disease.The levels of C-reactive protein (CRP),interleukin (IL)-1 β,IL-6,tumor necrosis factor (TNF)-α were measured by enzyme-linked immunosorbent assay (ELISA) before hemodialysis and 1,2,4 h after hemodialysis.The correlation of hypertension to CRP,IL-1 β,IL-6,TNF-α was analyzed.Results The levels of CRP,IL-1β,IL-6,TNF-α were increased in IDH group with the progress of hemodialysis,but were stable innon-IDH group.Logistic regression analysis indicated that there were positive correlations between CRP (r =0.3652),IL-1β (r =0.3261 ),IL-6(r =03156),TNF- α (r =0.3324) and blood pressure during hemodialysis (P < 0.01 ).Conclusion The levels of CRP,IL-1β,TNF-α,IL-6 are positively related to the IDH in maintenance hemodialysis patients.
