A multi-channel central monitoring system based on B/S mode is designed. It is consistent of the central monitoring server software, the Internet Explorer in the client PC, and a physiological signal collector. The server software can accept two client's request of data transfer, and real-time monitor several patients through Internet. The client uses the B/S mode, and just need to install an ActiveX control. The only thing users should to do is opening the Intemet Explorer, from which the waveform and signals are shown and transferred. The system extends the central monitoring service from hospital to the community, and is benefit for patient's long-term observation and cutting fees. Additionally, it can be a new solution of network based health care devices.

Objective To evaluate the application value of PCR-reverse dot blot hybridization in the identification of Candida and the detection of Candida albicans drug-resistant genes.Methods The vaginal secretion samples from 285 patients with candidal vaginitis and 50 healthy women were collected.The identification of Candida species and their drug susceptibility were detected by the bioMérieux Yeast identification cards and MIC method(Zhengzhou Antu kit),respectively.The identification of Candida species and the mutation of Candida albicans,drug-resistant genes were also detected by the Shenzheng Yaneng test kit(PCR-reverse dot blot hybridization).The drug-resistant genes were also identified by PCR and nucleic acid sequencing.Based on the culture identification,MIC method and nucleic acid sequencing as the contrast methods,the sensitivity,specificity and accuracy of PCR-reverse dot blot hybridization in the identification of Candida species and the mutation detection of Candida albicans drug-resistant genes were evaluated.Results Compared with the bioMérieux Yeast identification method,the sensitivity,specificity,positive predictive value,negative predictive value and total coincidence rate of PCR-reverse dot blot hybridization for detecting six kinds of Candida species,including Candida albicans,Candida glabrata,Candida tropicalis,Candida parapsilosis,Candida krusei and Candida guilliermondii,were above 95％,96％,96％,98％ and 97％,respectively.There was no significant difference in detecting six kinds of Candida species between the two methods (x2 =0.44,0,0,0,0 and 0,respectively,P ＞ 0.05),and there was good consistency between them (Kappa ＞ 0.9).Compared with the MIC method,the sensitivity,specificity,positive predictive value,negative predictive value and total coincidence rate of PCR-reverse dot blot hybridization for detecting the drug resistance of Candida albicans were 98％,88％,98％,88％ and 96％,respectively.There was no significant difference in detecting the drug resistance of Candida albicans between the two methods (x2 =0.17,P ＞ 0.05),and there was good consistency between them (Kappa ＞ 0.8).The results of PCR-reverse dot blot hybridization in detecting the mutation sites of six kinds of Candida albicans drug-resistant genes were 100％ of coincidence with that of the nucleic acid sequencing method.Conclusion The PCR-reverse dot blot hybridization has high consistency with the culture method and the nucleic acid sequencing method in the identification of Candida species and the mutation detection of Candida albicans drug-resistant genes,which is more early and rapid than the traditional detection methods,and may be applied to the auxiliary diagnosis of vulvovaginal candidiasis (VVC).

Objective To explore the application of scene simulation teaching combined with research design in the teaching of Community Nursing and evaluate its effects.Methods The scene simulation teaching combined with research design was used in the teaching of Community Health Promotion and Health Education in 68 nursing undergraduate students.The student groups were required to do the scenario case preparation and presentation,and conduct the research design according to specific situations of community health education.The mastery of related knowledge of students before and after the class was compared.Results All the student groups completed the course requirements.After class,the students better grasped the community health education,research design and other related knowledge compared with before class.Sixty-one students (89.71％) regarded the scene simulation teaching combined with research design was suitable for the teaching of Community Health Promotion and Health Education,and over 80％ students regarded this teaching was benefit for the development of their ability of self-learning,teamwork and flexible application of knowledge.Conclusions The scene simulation teaching combined with research design can not only help students master related knowledge,but also focuses on students' ability training,which is suitable for comprehensive application of knowledge.

Objective To explore the effects of reoperation on treatment of recurrent pelvic endometriosis(RPEM).Methods The clinical data of 47 cases of RPEM reoperation in our hospital from April 2005 to October 2010 was investigated,and the efficacy was analyzed compared with the first operation data.Results The cases of painful nodules was significantly different between reoperation group and the first operation group(28 vs 14,x2 =8.436,P =0.004 ).There was significant difference on laparoscopic surgery cases between reoperation group and the first operation group (25 vs 40,x2 =7.259,P =0.007 ).The operation time in reoperation group was significantly longer than that in the first operation group( [ 106.4 ±41.0] min vs [ 78.4 ± 26.4 ] min,t=3.995,P ＜ 0.01 ),and the amout of intraoperative hemorrhage in reoperation group was more than that in the first operation group ( [ 143.2 ± 118.3 ] ml vs [ 70.6 ± 68.1 ] ml,t =3.660,P ＜ 0.01 ).However,there was no significant difference on symptoms,cyst location and clinical stage between these two groups(P ＞0.05).Conclusion Due to the pelvic adhesion would be dense and extensive in RPEM,it should be carefully dissected during reoperation.At the same time,the operator should pay attention to the anatomical location and try to restore the normal anatomy of the pelvic organs and physiological state,and try to reduce postoperative adhesions.Complete removal of the lesions is the key to improve the treatment effect and prevent recurrence and reoperation.

Objective To investigate the relationship between the biological characteristics and the expres-sion of stromlysin (MMP-7) and microvessel density (MVD) in breast cancer tissue and the effect of MMP-7 on MVD of breast cancer. Methods 60 fresh samples were obtained from patients with breast cancer,and then the ex-pression of MMP-7 and vascular endothelial cell CD34 were studied by means of immunohistochemical assay (SP method). The relationship between MMP-7, MVD and age, tumor size, histological type, lymph node metastasis and expression of ER, PR, PCNA, p53 and CerbB2 was studied. The relationship between the biological characteristics and the expression of MMP-7 and MVD in breast cancer were evaluated. Results The positive expression rote of MMP-7 in the d>2 cm group,the group with lymph node metastasis,the group with positive expression of PCNA, p53 and C-erbB-2 (78.1%,74.2%,71.8%,67.6% and 72.2%)were all higher than that in the d≤2 cm group, the group without lymph node metastasis,the group without positive expression of PCNA,p53 and C-erbB-2(32.1%, 41.3%,38.1%,38.5% and 33.3%) (P<0.05). MVD was higher in the positive breast cancer tissue of the group of d>2 cm(34.61±6.97), the group with lymph node metastasis (34.37±7.50), PCNA ( 33.24±8.39), p53 (33.28±8.94), C-erbB-2 (33.55±8.57) than in the negative breast cancer tissue of the d≤2 cm group ( 28.60±9.82), the group without lymph node metastasis (27.48±8.66), PCNA (26.88±7.89), p53 (21.71± 7.59),C-erbB-2(27.42±27.69) (P<0.05). MVD(33.62±7.36)/ high power lens in the MMP-7 positive group was higher than that (27.86±9.45)/high power lens in the MMP-7 of negative group(P<0.05). The posi-tive expression of MMP-7 was correlated with MVD (r=0.380, P<0.05). Conclusion The high expression of MMP-7 and MVD is associated with the development and metastasis of breast cancer. MMP-7 could promote the mi-crovessel development in mammary cancer.

Objective To investigate the association of T help (Th) lymphocyte and heart rate variability (HRV) in congestive heart failure (CHF) patients. Methods Ninety-six patients with CHF and thirty healthy persons were enrolled in the study. Time-domain HRV analysis was performed based on 24 hour Holter Electrocardiogram (ECG) monitoring. Interferon-γ (IFN-γ) was used as markers for the differentiation of Th1 subsets and interleukin-10 (IL-10) for the Th2 subsets. IFN-γand IL-10 in CD4 + T lymphocytes were quantified by 3-color flow cytometry. Results The frequency of IL-10-Producing T Cells in the CHF group was significantly lower than those in the healthy control ( ( 16.4 ± 5.8 ) % vs. ( 26.8 ± 3.7 ) %, t = 9. 243, P ＜ 0. 001 ). The frequency of IFN-γ in the CHF group ( ( 18.4 ± 7.3 )% ) was significantly lower than that in the healthy controls ( (7.3 ±4.6) % ,t =7. 917, P ＜ 0. 001 ). The following index of HRV in the CHF groups were all significantly lower than those in the healthy controls: (98. 6 ± 21.3) ms vs. ( 145. 1 ± 42. 6) ms for SDNN, (83. 9 ± 22.4) ms vs.(136.5 ±39.6)ms for SDANN, (40.6 ± 14.5) ms vs. (55.8 ± 17.9) ms for SDNNI, (20. 7 ± 12.9) ms vs.(29.1 ± 12.6) ms for RMSSD, (5.6 ± 3.7 ) % vs. ( 11.8 ± 4.4) % for PNN50 ( Ps ＜ 0.05 ). In CHF patients, the frequency of IL-10 were positively associated with SDNN, SDANN, SDNNI, RMSSD and PNN50 ( r = 0. 49,0. 57,0. 58,0.47 and 0. 52 ,respectively,Ps ＜ 0.01 ). In the CHF patients, the frequency of IFN-γand IFN-γ/IL-10 ratio were negatively associated with SDNN ,SDANN ,SDNNI, RMSSD and PNN50 ( r = - 0. 49, - 0. 54, - 0. 57, - 0.52,- 0.53, - 0. 52, - 0.64, - 0.57, - 0. 58, - 0. 67, Ps ＜ 0. 01 ). Conclusions Autonomic nervous system is involved in the regulation of the balance of Th1/Th2 in patients with CHF. Sympathetic nerve system enhances the effect of Th1 ,whereas parasympathetic nervous system enhances the effect of Th2.

Objective To describe fatigue among children with acute lymphoblastic leukemia (ALL)receiving chemotherapy in hospital and explore its related factors.Methods A total of 100 hospitalized children with ALL receiving chemotherapy and their main caregivers were surveyed on demographic information,treatment data,Wong-Baker facial scale and the PedsQLTMMFS Multidimensional Fatigue Scale.Results The incidence of fatigue was 99％ (99/100).The total score of fatigue scale was (56.71±17.32) on average.Multiple linear regression modeling showed that the factors related with fatigue included the time of dexamethasone use,childen's sleep quality,hemoglobin content,pain score,the time for the present chemotherapy,age and main caregivers' self-rating health status (P＜ 0.05 or 0.01).Conclusions Fatigue is serious among children with ALL receiving chemotherapy and is related with demographic factors,diseases,treatment condition and health status of the caregivers.

Objective To explore the application of Chinese version of PedsQLTM Multidimensional Fatigue Scale(PedsQLTM MFS) in children with leukemia.Methods 141 children with leukemia and their main caretakers (one parent) were surveyed with the Chinese version of PedsQLTMMFS.Results The Cronbach α was 0.836-0.949 and 0.761-0.930 for parent-proxy scale and children's self-report scale respectively.The correlation coefficients between each item and its subscale were higher than those between each item and other subscales.The scores of parent-proxy scale and children's self-report scale were positively correlated with the correlation coefficient as 0.373-0.738,P＜0.01.Conclusions The Chinese version of PedsQLTMMFS has good reliability and validity.It is a reliable scale in measuring fatigue in children with leukemia.
