The research of the secondary damage remote from middle cerebral artery territory infarction has made significant progress in recent years.More animal experiments from the cellular,biochemical and molecular levels have been performed for in-depth and detailed research on remote site damage. The injury mechanisms such as oxidative damage and β amyloid deposition have been found.The new imaging detection technologies,such as magnetic resonance diffusion tensor imaging(DTI),have gradually been applied to the diagnosis of remote site damage.

ObjectiveTo investigate the clinical efficacy of Yaotongxue(Ex-UE7)finger manipulation plus ultrashort waves in treating acute lumbar sprain.MethodForty-two patients with acute lumbar sprain were enrolled and randomized into groups. The control group received lumbar ultrashort waves treatment and the observation group, Yaotongxue finger manipulation in addition. Pain relief and activities of daily living were assessed using the Visual Analogue Scale (VAS) and the Barthel Index before and after one and five sessions of treatment.ResultThere was no statistically significant pre-treatment difference in the VAS score between the two groups (P>0.05). After one and five sessions of treatment, the VAS score was significantly lower in the observation group than in the control group and there was a statistically significant difference (P<0.05); the Barthel Index score was significantly higher in the observation group than in the control group and there was a statistically significant difference (P<0.05). at three months of follow-up, the VAS score and the Barthel Index score were basically normal; there were no statistically significant differences between the two groups (P>0.05). No adverse events occurred during the study.ConclusionYaotongxue finger manipulation plus ultrashort waves is safe and highly effective in treating acute lumbar sprain.

Objective To investigate the influence of psychological intervention on the anxiety of patients with obstructive jaundice and supply theoretical reference for psychological evaluation and nutsing.Methods 129 patients with obstructive jaundice was divided into the intervention group(64 cases)and the control group(65 cases).The control group adopted routine treatraent method such as anti-infection,liver protection treatment,enhancement of coagulation mechanism and supporting treatment.The intervention group received psychological nursing,relaxation training and distraction of attention based upon routine nursing.The degree of anxiety and pruritus was obseved and recorded in the two groups.Results The degree of anxiety in the intervention group was evidently lower than that of the control group(P＜0.01).The degree of pruritus in the intervention group was 18.75%,which was statistically different from that of the control group(35.38%),P＜0.05.Condusion Adoption of psychological intervention could effectively reduce the degree of anxiety and pruritus of patients with obstructive jaundice.

Objective:To investigate the culture technique and secretory function of parathyroid cells in rats in order to establish experimental basis of parathyroid cell transplantation.Methods:After parathyroid glands were digested by collagenase and trypsin,isolated parathyroid cells were cultured.The cells cultured for 5 days were observed by transmission electron microscopy.Parathyroid hormone obtained from the cell culture fluid on day 1,3,5 and 7 was measured respectively.Results:Parathyroid cells cultured for 5 days had abundant rough surfaced endoplasmic reticula、Golgi bodies、stab mitochondria and secretory granules.Cultured parathyroid cells could secrete parathyroid hormone normally,especially on the 5th day.Conclusion:Parathyroid cells cultured for 5 days still possess original morphologic structure and have maximum secreting function,so it is an optimal donor for transplantation.

Objective To summarize our experiences in the long-term continuous maintenance of the peripherally inserted central catheter (PICC) in a patient with intestinal fistula.Method The systematic holistic nursing care has been provided according to the nursing practices to a patient with intestinal fistula since 2006,during which eitht PICCs were used successively.Results The eight PICCs were placed for 3-14 months.The 1 st,3rd,5th,and 7th cathers were retained for 12,13,11,and 14 months.The second catheter was retained for 6 months,and then removed due to increased body temperature (38.7℃).The 4th cather was retained for 3 months,then catheter-related bloodstream infection was suspected due to high body temperature (39℃) ; culture for fungi at the tip showed positive results.The 6th catheter was retained for 7 months,inserting from jugular vein to the superior vena cava; however,it was removed due to puncture point local purulent changes at the puncture site.The 8th catheter was applied for 8 months,but was rmoved due to tube occlusion,which responded poorly to urokinase.Conclusion The application of PICC in patients with fistula can relieve the pain and improve the quality of life.

Objective To investigate the early period cesarean scars pregnancy diagnosis and its influence on prognosis. Methods Clincal data of 42 cases diagnosed as cesarean scar pregnancy from May 2006 to February 2011 treated in our hospital were retrospective analysed. Results All cases underwent B ultrasound examination,which showed gestational sacs or inhomogeneous echo-enclosed mass in the lower segment of anterior wall of uterus. Thirty-nine cases were successfully eonservatively treated,3 cases underwent cleanrance of focal lesion and neoplasty. Conclusion The cesarean scar pregnancy diagnosis depends on a B ultra-sound examination, and the color Doppler ultra sound is reliable for the diagnosis. Floss plants in the crater of scar and developed to uterus wall. Curettage guided by B ultra-sound after uterine artery embolization is a safe and efficient treatment method.

Objective To investigate the effect of mindfulness on anxiety and depression of gynecological oncology patients.Methods Using objective methods,80 patients with gynecological oncology were divided to the usual care group and mindfulness training group according to random number table,40 cases in each group.The self -rated anxiety scale and self -rated depression scale were used to compare the effects at three times.Results There were 3 patients lost and 77 patients were analyzed finally.The ANOVA showed that there were significant differences on anxiety and depression scores of SAS (F =4.614,P =0.035) and SDS (F =4.070,P =0.044) between the two groups,and the scores of SAS (F =15.115,P <0.001) and SDS (F =7.034,P =0.002) had interaction effects. Conclusion Mindfulness can adjust the psychology state of gynecological oncology patients and decreased the level of anxiety and depression.

Objective To explore the value of contrast-enhanced ultrasonography (CEUS) in diagnosis of esophagogastric varices (EGV) compared with color Doppler flow imaging (CDFI). Methods CDFI and CEUS were performed on 23 variceal patients based on the B mode ultrasound examination. The blood flow of esophageal varices (EV) or gastric varices (GV) displayed with CDFI and CEUS were compared. Results The flow signal demonstration rate of EV with CDFI and CEUS was 42.11% (8/19) and 73.68% (14/19), while that of GV with CDFI and CEUS was 76.47% (13/17) and 94.12% (16/17), respectively. CEUS displayed the flow signal of EV better than CDFI did (χ~2=4.17, P＜0.05), but the flow signal demonstration rate of GV was not significantly different (χ~2=1.33, P＞0.05). Conclusion CEUS can be used more extensively in clinic practice for diagnosis of EGV.
