ObjectiveTo investigate the clinical effect,suitable dose,and its safety of dezocine combined with propofol used in painless artificial abortion.MethodsWith a completely randomized double blind method,200 cases of ASA Ⅰ or Ⅱ early intrauterine pregnancy patients were divided into A,B,C,and D four groups ( n =50).Each patient in A,B,and C groups was infused dezocine in 0.10,0.15,and 0.20 mg/Kg,respectively.D group was administrated with intravenous fentanyl 1.5 μ g/kg,then 3min later,slowly intravenous injection of 1％ of propofol.MAP,HR,RR,and SpO2 were recorded at the before anesthesia induction (T0 ),loss of eyelash reflex time (T1 ),recovery time (T2),and recovery 10 -min time (T3 ).Meanwhile,the parameters including the anesthesia induction time,anesthesia recovery time,directional force recovery time,induction dosage and total dosage of propofol,and the intraoperative and postoperative analgesic efficacy and adverse reaction were compared among different groups.Results For the A and D groups,MAP,HR,RR,and SpO2 at the T1 were all lower than that at the T0 ( P ＜0.05).For the B group,MAP and HR at the T1 were lower than that at the T0 ( P ＜ 0.05).The anesthesia recovery time and orientation recovery time in the B and C groups were significantly shorter than A and D groups ( P ＜ 0.05 ),and significantly statistical difference was found between A and D groups,no statistical difference between B and C groups ( P ＞ 0.05).The propofol induction dosage and total dosage were different among groups ( groups C ＜ B ＜ D ＜ A) with a statistical significance ( P ＜ 0.05 ).The intraoperarive analgesia effect,postoperative pain score grading,and intraoperative body dynamic reaction rates in the B and C groups were significantly different from the A and D groups ( P ＜ 0.05 ),and no statistical significance was found between B and C groups ( P ＞ 0.05 ).The respiratory inhibition,postoperative nausea,vomiting and other adverse reaction rates in the B and C groups were significantly lower than those in A and D groups ( P ＜ 0.05),and those parameters in B group were lower than C group.ConclusionsDezocine combined with propofol is definitely effective and very safe in painless induced abortion with very less respiratory inhibition,nausea,vomiting,and other adverse reactions; and their optimal dosage is 0.15 mg/kg.

The diploid type of Paragonimus westermani in Heilongjiang Province and triploid type of P.westermani in Liaoning Province were examined for amino acids by high performance liquid chromatography (HPLC).Eleven kinds of amino-acids of diploid type and 15 kinds of triploid type were found in metacercaria; 15 kinds amino acids of diploid type and 16 kinds of triploid type were detected in larvae.The amino acids were more abundant in larvae than in metacercaria and more in the triploid than in the diploid type.The predominant amino acids were cystine and Rlutamic acid.

Objective To understand the effect of cerium on the activity of SOD and CAT in Drosophila melanogaster.Methods Drosophila melanogasters were treated with cerium by adding cerium sulfate [Ce(SO4)2] into the culture medium at different concentrations(0,1,4,16,64,256 and 1 024 mg/L).The activity of SOD and CAT of the third-instar larvae were analyzed.Result Compared with the control,at 64-1 024 mg/L,the activity of SOD and CAT in the third-instar larvae significantly decreased and at 1-16 mg/L,the activity of SOD and CAT significantly increased Conclusion It suggested that cerium sulfate [Ce(SO4)2] has an oxidant toxicity to Drosophila melanogaster.

Objective To establish a clinical diagnostic criterion(referred as criterion hereinafter) for patients with social dysfunction and abnormal mental behaviors caused by internet addiction,so as to provide guidance in future mechanism study,treatment and prevention for such disorder.Methods A rough draft of symptom parameters was used to evaluate the symptoms among the patients with a chief complaint of wallowing in internet which caused damned influence on study,work and interpersonal communication.The 1st draft of the criteria was drawn up according to the incidence and correlation of the symptoms.408 patients with internet addiction(IA) diagnosed according to the 1st draft were followed up for 12 months after being discharged from hospital,and the diagnostic coincidence and recurrence rate were calculated and analyzed,based on which the 1st draft was then revised and the 2nd draft was established.Clinical tests were performed on 150 patients with the same chief complaints by psychiatrists from many mental hospitals in China.Descriptions in the 2nd draft were not modified based on the test findings and the final draft was formulated.Results The proposed criterion for IA symptom included the following 7 items: ① craving for using internet;② withdrawal reactions after internet use reduced or stopped;③ tolerance;④ unable to control the intention of using internet;⑤ regardless of hazardous consequences;⑥ pay no attention to other activities;and ⑦ avoidance or alleviation of unhealthy emotions.Items ① and ② combined with any one of the remains made the final diagnosis of IA.The criterion for illness duration was suggested as greater than or equal to 6 hours of continuous internet use one day in average and 3 months at least after meeting the criteria for IA symptom.It was found by following-up that one year recurrence rate among the discharged patients from hospital was 70.6%,and the diagnostic coincidence was 96.1%.The diagnostic consistency on symptom evaluation between two psychiatric raters was 89.3%-98.0%(Kappa 0.73-0.87,P

Objective To summarize the effect of predictive nursing combined with high-quality nursing in patients with pulmonary embolism or having risk factors of pulmonary embolism (PE).Methods A retrospective analysis of nursing in 57 PE patients was camied out,Nursing flow chart was established.For patients with high risk factors of PE,predictive nursingwas administered,including analysis of risk factors,health education,general nursing and pertinent nursing.For PE patients,holistic nursing was administered,including emergeney nursing,basic nursing,observation of illness,nursing of thrombolytic therapy,psychological nursing and rehabilitation nursing, Results All patients accorded with thrombolysis indication and received thrombolysis treatment. Conclusions Predictive and high-quality nursing should be used in PE patients in order to improve treatment effect.

AIM: To evaluate the effect of mechanical periodontal treatment combined with tetracycline on per iodontal attachment level (AL) and the avidity of serum antibody against porphyromonas gingivalis( P. gingivalis) in patients with aggressive periodontitis(AgP). METHODS: Twenty- five patients with AgP and twenty periodontally healthy controls were studied (HS). Clinical examination and recordings of AL were performed before and 3,6 and 12 months after the periodontal treatment. The avidity of IgG antibody against P. gingivalis lipopolysaccharide(LPS) was measured by diethylamine dissociation ELISA. RESULTS: A significant improvement in AL was observed following treatment ( P ＜ 0. 01 ). The avidity of serum IgG antibody against P. gingivalis increased compared with controls, and was decreased significantly after mechanical periodontal treatment combined with tetracycline (P ＜ 0. 01 ). CONCLUSION: Ourresults demonstrate that mechanical periodontal treatment combined with tetracycline provides clinically favorable results in patients with AgP.

BACKGROUND: It has been proved that interleukin-18 (IL-18) involves in the development of acute cerebral infarction (ACI) and is positively correlated with the time of brain stroke onset, erythrocyte sedimentation rate (ESR), as well as neurological deficit and low-density value of brain CT.OBJECTIVE: To investigate the dynamic changes of serum IL-18 in the course of ACI.DESIGN: Verification analysis with patients as subjects and healthy volunteers as controls.SETTING: Department of neurology of a municipal hospital.PARTICIPANTS: Totally 46 inpatients (29 males and 17 females) with ACI were randomly selected from the Department of Neurology of People's Hospital, Maoming City, between December 2002 and January 2004.Meanwhile, 40 healthy controls (27 males and 13 females) were recruited from those who came to the hospital for routine physical examination. All the participants signed informed consent.METHODS: Fasting peripheral vein blood of 2 mL was collected from the patients on the 1st, 7rh, 14th and 21st days of ACI onset, and from healthy controls on the day of routine examination. The blood samples were centrifuged at 3 000 r/minute for 15 minutes at 4 ℃, and then the supernatant was collected for detecting IL-18 level by ELISA.MAIN OUTCOME MEASURES: The serum IL-18 level of patients on the 1st, 7th 14th and 21st days of ACI onset and that of healthy controls on the day of routine examination.RFSULTS: Totally 46 patients and 40 healthy controls were enrolled in this study and all data were statistically analyzed. The level of serum IL-18 was significantly higher in the patients with ACI than in normal controls on the 1st and 7th days of onset [(178±41) ng/L, (104±34) ng/L, (65±14) ng/L, P ＜ 0.01],but was similar on the 14th and 21st days [(88±36) ng/L, (72±33) ng/L,(65±14) ng/L, P＞ 0.05]. The level of serum IL-18 in ACI patients was significantly higher on the 1st day than on the 7th, 14th and 21 st days of the onset (P ＜ 0.05-0.01); moreover, it was also significantly higher on the 7th day than on the 21st day of the onset (P ＜ 0.05).CONCLUSION: The level of IL-18 increases obviously on the 1st day of ACI onset, and gradually decreases with the extended course of disease and time of treatment.

Aim To investigate whether genistein pro-tects paraoxon-induced vascular endothelial dysfunction through down-regulating p22phox and Nox4 expressions as well as inhibiting the generation of ROS.Methods In this study,thoracic aortas were isolated from the male Sprague-Dawley(SD)rats and were divided into the following groups:① control group,the thoracic a-ortas were incubated with dimethyl sulfoxide (DMSO, 0.1%)for 30 min;② genistein group,the thoracic a-ortas were incubated with genistein(100 μmol·L -1 ) for 30 min;③ paraoxon group,the thoracic aortas were incubated with paraoxon at the concentration of 40.5 μmol · L -1 for 30 min; ④ paraoxon plus genistein groups,the thoracic aortas were incubated with paraoxon (40.5 μmol·L -1 )plus genistein (100μmol·L -1 )for 30 min.The expressions of p22phox and Nox4 mRNA were detected by RT-PCR and the protein expressions ofp 2 2 phox and Nox4 were detected by Western blot.Results Compared with the control group,the expressions of p22phox and Nox4 were markedly increased in the paraoxon group. In the genistein group,the expressions of p22phox and Nox4 were significantly repressed. When treated with genistein plus paraoxon,there was a marked increase in the expression of Nox4(P <0.05),but no signifi-cant difference in the expression of p22phox.The ex-pression of p22phox in the paraoxon plus genistein group was significantly decreased(P <0.05)as com-pared with paraoxin group,but there was no significant difference in the expression of Nox4.Conclusion Paraoxon may result in oxidative damage of vascular endothelium through up-regulating p22phox and Nox4 expressions,genistein may down-regulate the expres-sions of both and protect vascular endothelium.

Objective To analyze the composing experience of Menghe physician Ma Peizhi for cough by TCM inheritance support system. Methods The prescriptions for cough of Ma Peizhi were collected, frequency and association of drugs were analyzed by using data mining methods such as revised mutual information, complex system entropy cluster and unsupervised hierarchical cluster. Results Based on the analysis of 57 prescriptions, the frequency of each herb and association rules among the herbs were computed, 18 core combinations and 9 new prescriptions were mined from the database. Conclusion Menghe physicians Ma Peizhi is well experienced in expelling wind and opening the inhibited lung, dissolving phlegm and relieving cough. TCM inheritance support system can be used to analyze clinical experience of old TCM doctor.
