1.Diagnostic value of procalcitonin in patients with infection complicated by heart failure

Objective To evaluate the diagnostic value of procalcitonin (PCT) in patients with infection complicated by heart failure.Methods A total 2454 patients,which with simple heart failure,simple bacterial infection and bacterial infection complicated with heart failure,from Sun Yat-sen University affiliated zhongshan hospital were enrolled in this case-control study.244 heathly controls were aslo evaluated at the same time.The serum PCT levels in different groups were analyzed by Cobas E601.ROC curve was employed to compare the diagnostic values of PCT between simple heart failure group and infection complicated with heart failure group,and the cutoff value of PCT testing for infection complicated by different grades heart failure was determined.The difference between test groups was analyzed using Kruskal-Walis H method.Results PCT levels in simple heart failure group (3.46 ± 3.07) were significantly higher than those in control group (0.04 ± 0.03) (t =4.262,P < 0.01).Besides,PCT levels in infection complicated by heart failure group(18.18 ± 10.33)were significantly higher than those in simple infection group(8.97 ± 6.20) (t =2.694,P < 0.01).Although PCT could still be a biomarker for diagnosis of bacterial infection in patients with or without heart failure (areas under the ROC curve were 80%,82%,85%,88%,respectively),the positive predictive values of PCT were declined in line with the heart failure degree increasing [z(1,2) =-6.24,P<0.01; z(l,3) =-4.35,P<0.01; z(1,4) =-5.19,P<0.01; z(2,3) =-5.33,P<0.01;z(2,4) =-2.86,P<0.05; z(3,4) =-2.46,P<0.05].There are significant differences between areas under ROC curve [z(1,2) =-2.55,P < 0.05 ; z(1,3) =-5.42,P < 0.01; z(2,3) =-2.90,P < 0.05],and the cutoff values of PCT to identify infection patients complicated with Ⅱ,Ⅲ,Ⅳ degrees heart failure were 0.086,0.192 and 0.657 μg/L,respectively.Conclusion PCT levels were increased in heart failure patients so we should pay more attention to the degree of heart failure and PCT level on patients which suffered both infection and heart failure.
2.The association between blood pressure variability and left ventricular hypertrophy in elderly isolated systolic hypertension

Objective To study the association between blood pressure variability and left ventricular hypertrophy in elderly isolated systolic hypertension.Methods Active blood pressure was applied to measure average BP,24 hour daytime and night BP variability,meanwhile,left ventricular interior diameter,the thickness of interventricular septum and left ventricular posterior were taken using echocardiagram so as to measure left ventricular weight( LVW) and left ventricular weight index (LVMI) in 31 elderly isolated systolic hypertension and 24 controls.Results BP variability, LVM and LVMI in 31 cases were higher than that of 24 controls.Conclusion BP variability and average BP are related to LVH.
3.Differential expression and interaction of β-catenin/ICAT proteins in NSC67657 induced monocytic differentiation of HL60 cells

Aim To analyze differential expression and interaction of β-catenin/ICAT proteins in HL60 cells when they were induced into monocytic differentiation, and to figure out the mechanism of NSC67657 in cellu-lar induction. Methods HL60 cells were treated by 10 μmol · L-1 NSC67657 , and cellular differentiation could be observed by cytochemical staining and flow cytometry. Then, RT-PCR and Western blot were em-ployed to determine the differential expression of β-catenin/ICAT genes and proteins. Co-immunoprecipi-tation assay was used to confirm the interaction of β-catenin/ICAT proteins, and laser co-focus light mi-croscopy technology was used to co-indentify proteins differential expression and intracellular location. Re-sults HL60 cells could be induced into monocytic dif-ferentiation after 5 days treatment using 10μM NSC67657 . The CD14 ( +)% cells could be up to o-ver 90%, and cytochemical staining reports were con-sistent with this result. The expressions of ICAT gene and protein were up-regulated significantly ( P <0. 01 ) , but the expressions ofβ-catenin gene and pro-tein, on the contrary, were down-regulated(P<0. 05) when HL60 cells were induced into monocytic differen-tiation. From co-immunoprecipitation assay findings, ICAT protein interacted with β-catenin protein, and the absorbance of protein electrophoresis bands in-creased in differentiated cells. From laser co-focus light microscopy assay findings, the fluorescence of ICAT and β-catenin protein could be both observed in cytoplasm and nucleus. In drug treated HL60 cells, the fluorescence of ICAT protein was enhanced both in cytoplasm and nucleus, however, the fluorescence ofβ-catenin protein, which looked like transferring into different organelles, decreased significantly in nucleus, but increased in cytoplasm. Conclusions HL60 cells could be induced into monocytic differentiation by NSC67657 and β-catenin/ICAT proteins differentially expressed during cellular differentiation. The enhanced interaction of β-catenin/ICAT proteins and β-catenin protein transferring from nucleus into cytoplasm indi-cates that NSC67657 probably induces HL60 cells into monocytic differentiation through down-regulating β-catenin protein and blockingβ-catenin protein from nu-cleus.
4.Effects of D-glucosamine hydrochloride on the cell-cycle and expression of proteins in gastric carcinoma cell SGC-7901

Objective To study the effects of D-glucosamine hydrochloride on the apoptosis of gastric carcinoma cell line SGC-7901 and its mechanism.Methods SRB assay was used to examine the effects of D-glucosamine hydrochloride on the growth inhibition of SGC-7901.The cell-cycle and expression of cytochrome-C were observed by flow cytometry,the concentrations of intracellular Ca2+ was determined by CLSM and the expressions of cyclinB1,calcinerin were investigated by Western-Blot.Results D-glucosamine hydrochloride could block SGC-7901 cell-cycle at S phase of cell division,increase the concentration of intracellular Ca2+ and decrease the expression of cyclinB1,the expressions of calcinerin and cytochrome-c were increased respectively.Conclusion Block of cell-cycle may be one of the mechanism of inducing cell apoptosis by D-glucosamine hydrochloride.
5.The application of proteomic technology in differentiation and early diagnosis of myelocytic leukemia

Objective To figure out the differentially expressed proteins using proteome technology in leukemia cells induced into different lineages and investigate the application value in early screening of leukemia.Methods With induction of ATRA and NSC67657, the differentiation models was constructed using HL60 cells which has the potentiality to be induced into different lineages by different inducers.Then the differentially expressed proteins in the process of differentiation was separated using two-dimensional electrophoresis and identified using MALDI-TOF MS.The expression of 3 proteins FE1A1, TLE1, NME3 were chosen to be verified in myeloid samples of 5 leukemia patients and 1 normal volunteer using RT-PCR and WB.Results WB showed that NME3 was differentially expressed after both granulocytic and monocytic differentiation( Normal A value = 0.227, NSC67657 A value= 0.079, ATRA A value = 0.064, P < 0.01 ).However, only in 4 of 5 tested patients' myeloid samples, the NME3 protein expression were differentially expressed compared to the normal myeloid sample( Normal A value = 0.082,2 acute leumia transferred from chronic granulocytic leumia A value = 0.274,0.269, acute monocytic leukemia A value = 0.297, one patient with chronic granulocytic leukemia A value = 0.258.There was significant difference between normal and leukemia group, P <0.05 ).A value was 0.121 for another patents with chronic granulocytic leukemia The NME3 protein expression was not differentially expressed compared to the normal myeloid sample,P >0.05.Conclusions It is still a long way to go for proteome technology from basic research to clinical application.However, the identification of NME3 protein related to differentiation in leukemia patients' myeloid samples had set the foundation for the early diagnosis of leukemia using proteome technology.
6.Comparison of target dosimetry and treatment outcome in patients with stage Ⅲ non-small cell lung cancer treated with three-dimensional conformal radiotherapy and intensity-modulated radiotherapy

Objective To compare the target dosimetric distribution and clinical outcome in patients with stage Ⅲ non-small cell lung cancer (NSCLC) treated with three-dimensional conformal radiotherapy (3DCRT) and intensity-modulated radiotherapy (IMRT).Methods The clinical data of 419 patients with stage Ⅲ NSCLC treated with either 3DCRT or IMRT were retrospectively analyzed.Among them,there were 338 male and 81 female patients,and the median age was 63 years (range:32-84 years).There were 340 patients treated with 3DCRT and 79 with IMRT,and the median prescribed dose was 60 Gy (range:50-76Gy).One hundred and forty patients were treated with radiotherapy alone and 279 were treated with chemoradiotherapy.The target dosimetric distribution was evaluated with dose-volume histogram (DVH)parameters.The overall survival (OS) rate was calculated using the Kaplan-Meier method and analyzed by the log-rank test.Results When comparing the clinical data,the patients treated with 3DCRT were in older ages,and had advanced N and clinical stages (P =0.01,0.00,and 0.00,respectively).When comparing the target DVH parameters,the patients treated with IMRT had larger planning target volume (PTV) (P =0.01),significantly lower clinical target volume (CTV) D CTV D90,PTV D and PTV V65-V60 (P =0.05-0.01),significantly higher V5-V20 in both lungs,higher esophagus D longer esophagus in the radiation field,higher linear energy transfer between 45 and 55 keV/μm (LET45-LET55),and higher spinal cord Dmean(P =0.03-0.00).The follow-up rate was 97.4%.After radiotherapy,the 1-,3-,and 5-year OS rates were 65.5%,26.1%,and 18.5%,respectively,and the median survival time was 20 months.There were no significant differences in OS rate and the incidence of acute radiation pneumonitis and radiation esophagitis between patients treated with IMRT and 3DCRT (P =0.06,0.73,0.13).Stratified analysis showed that,when comparing the patients treated with IMRT with those treated with 3DCRT,the survival rate was only lower in male patients,patients in stage T3-T4 or N0-N2,and those without chemotherapy (P =0.04,0.04,0.02,0.00).Conclusions The treatment outcomes of patients with stage Ⅲ NSCLC undergoing IMRT and 3DCRT are comparable.IMRT shows a potential dosimetric advantage,but the result needs further investigation.
8.Comparative study of NT-proBNP and BNP assays for heart failure diagnosis in patients complicated with acute cerebral infarction

Objective To compare the capabilities of NT-proBNP and BNP in diagnosis of heart failure when complicated with acute cerebral infarction.Methods EP15-A2 document,was employed to verify the precision and accuracy of NT-proBNP and BNP assays on chemiluminescence analyzer Cobas E601 and ADVIA Centaur respectively for 363 samples from patients with chronic heart failure,cerebral hemorrhage and acute cerebral infarction,cerebral infarction complicated with heart failure and normal controls were collected and analyzed by Cobas E601 and ADVIA Centaur,and then determined the changing trends of NT-proBNP and BNP.Moreover,ROC curve was employed in diagnostic value comparison of NT-proBNP and BNP in heart failure groups.Results Cobas E601 and ADVIA Centaur showed good repeatability and accuracy in the detection of NT-proBNP and BNP that both total non-precision were below 3.5% and the deviations to calibrator were below 3.91%.The level of NT-proBNP an BNP didn't elevate in normal people and cerebral hemorrhage patients.However,they significant elevated in heart failure,acute cerebral infarction and cerebral infarction complicated with heart failure patients (P < 0.01).Their levels were heart failure grade dependent.The levels of NT-proBNP in cerebral infarction complicated with grade Ⅰto Ⅲ heart failure patients were significantly higher than those in chronic heart failure patients,with same heart failure(P < 0.05).However,compared to chronic heart failure patients,the level of BNP in cerebral infarction complicated with heart failure patients didn't elevate significantly until grade Ⅲ heart failure happened.Areas under the ROC curve (AUCs) of NT-proBNP and BNP were botb above 93% in the diagnosis of chronic heart failure.But for patients who suffered cerebral infarction complicated with heart failure,the diagnostic value of NT-proBNP and BNP were decreased due to cerebral infarction interference.However,the ROC area of BNP was larger than that of NT-proBNP(P < 0.05).Conclusion In the diagnosis of cerebral infarction complicated with heart failure,BNP could be a better choice to determine the degree of heart failure.
9.The Influence of Catgut Implantation at Acupoint on Insulin Resistance of Simple Obesity

Objective To study the effect of catgut implantation at acupoint on insulin resistance of simple obesity.Methods All patients were separated into the treatment group(catgut implantation group)and the control group(acupuncture group)with 30 cases in each group.Acupoints of zhongwan,tianshu,qihai,shangjuxu,were selected and needled for 3 months.Such indexes as body weight,waistline,hip circumference,body mass index(BMI),fasting blood glucose(FBG),insulin and insulin resistance(IR) before and after treatment were observed.Results Both groups showed obvious anti-obesity effect to simple obesity,manifested by significant differences(P<0.05)at the indexes of body weight,waistline BMI and appetite before and after treatment.Self-comparison in each group showed such indexes as BFS,insulin and IR were significantly decreased after treatment than those before treatment (P<0.05)There was no significant difference at the above-mentioned indexes between the two groups(P>0.05).Conclusions Catgut implantation at acupoint can reduce fat for simple obesity patients by controlling the level of fasting blood sugar,fasting blood insulin and IR in patients.
10.A Meta-analysis of the dynamic changes of platelet parameter in neonatal hypoxic ischemic encephalopathy

Objective To investigate the relationship between the dynamic changes of platelet parameter and the severity of the neonatal hypoxia ischemic encephalopathy .Methods To search PubMed ,Ovid ,Springer Database ,Chinese Academic Journal ,VIP database and Wanfang database in the library literature and screen documents according to the inclusion criteria of diagnostic tests , then to extract the characteristic information .Using RevMan 5 .1 software to analyze the data ,and to carry out the Meta analysis se-lected the effects model according the results of tests for heterogeneity .Results According to the inclusion criteria ,it was won the 10 articles eventually .The Meta analysis results showed that the PLT in acute period of HIE patients was significantly 1 .65-2 .14 times the standard deviation lower than the control group .The MPV and PDW in acute period of HIE patients were significantly 1 .12-1 .61 and 1 .17-1 .99 times the standard deviation higher than the control group .The PLT in convalescent of HIE patients was 1 .85-2 .35 times the standard deviation higher than the acute period of HIE patients .The MPV and PDW in the convalescent of HIE patients were significantly 0 .68-1 .38 and 0 .81-1 .37 times the standard deviation lower than the acute period of HIE pa-tients .Obviously ,the PLT ,MPV and PDW in the convalescent of HIE patients were closed to the level of the control group .Conclu-sion Clinical observing HIE infant′s platelet parameter dynamically can be regarded as the index of HIE infant′s condition severity and monitoring condition change .
