2.Study on relationship between expression of survivin, galectin-3 and TPO and BRAFV600E mutation in patients with papillary thyroid microcarcinoma and its clinical significance

Objective:To detect the expressions of survivin, galectin-3 and thyroid peroxidase in papillary thyroid microcarcinoma ( PTMC) and cancer adjacent normal tissue to explore the clinical significant.The correlations between the expressions of survivin,Gal-3 and TPO with BRAFV600E gene mutation in PTMC were analyzed.Methods: The expressions of survivin,Gal-3 and TPO were measured by immunohistochemical ( IHC ) method in 56 cases of PTMC tissue and adjacent normal tissue; BRAFV600E mutation was detected by PCR amplification and subsequently sequencing.Chi square test was used to analyse the relation in the expression rates of survivin,Gal-3 and TPO and BRAFV600E gene mutation.Results:The survivin and Gal-3 were strongly expressed but TPO was negatively expressed.The survivin and Gal-3 were negative in adjacent normal tissues but TPO was expressed.There were sig-nificant differences in the expression rates of survivin, Gal-3 and TPO between PTMC tissue and adjacent normal tissue (all P<0.001).The BRAFV600E mutation rate (32.1%) and the expression rates of survivin,Gal-3 and TPO in PTMC tissue were found to be positively related to lymph node metastases (P=0.009,P=0.025,P=0.007,P=0.008),and negatively related to gender and age (all P>0.05).There were no correlation was found between the expressions of survivin,Gal-3,TPO and the BRAFV600E gene mutation in PTMC(all P>0.05).Conclusion: The strong expressions of survivin and Gal-3,the mild expression of TPO and BRAF mutation may be important in the development of PTMC,and the detection of BRAFV600E gene mutation and the expressions of survivin, Gal-3 and TPO could be used to the judgment of pathogenetic condition and prognostic outcomes.
3.Adequate feeding of early enteral nutrition in critical patients with cancer

Objective To describe the adequate feeding of early enteral nutrition in critical patients with cancer, and explore its influencing factors. Methods A retrospective analysis study was performed and 161 patients were involved. Results Nearly 57.14%patients got adequate feeding. The independent risk factors included complications (P < 0.05,OR=2.312,OR 95%CI 1.078-4.958), Acute Physiology and Chronic Health Evaluation Ⅱ (APACHEⅡ) scores (P < 0.05,OR=1.111,OR 95%CI 1.017-1.214) and hypoalbuminemia (P<0.05,OR=2.252,OR 95%CI 1.036-4.892) , and the protective factor was previous enteral nutrition experience (P<0.01,OR=0.123,OR 95%CI 0.021-0.494). Patients who got adequate feeding showed more increase in albumin levels, more decrease in APACHEⅡscores and shorter ICU stays. Conclusions Part of patients got inadequate feeding, due to comprehensive effects of many factors. And adequate feeding may be beneficial to the increase of albumin levels, also helps patients rehabilitate early and shorten the ICU stays.

5.Resistance and serotype of Salmonella isolated from pig slaughtering process in Shandong Province

We investigated serotype and resistance of Salmonella during pig slaughtering in Shandong Province,China,providing basic data for the risk assessment of Salmonella and for guiding the clinical medication.We used rapid classification kit to identify Salmonella serotype,adopted broth microdilution method to detect the resistance of 13 kinds drugs belong to 8 categories.Result showed that the identified 9 kinds of serotype were mainly S.derby and S.typhimurium.The resistance to 13 kinds drugs of 298 Salmonella were different.The higher percentage of tetracycline drugs as Doxycycline(DOX) and Tetracycline(TE) were 97.99% and 80.20%,respectively,which was most sensitive to Colistine E.The resistant rate was only 2.01 %,following by Amoxicillin/Clavulanic acid and Ofloxacin which were 2.35 % and 4.03%,and the multiple resistant rate was 81.88%.TE-DOX was the regnant drug-resistant spectrum.In conclusion,the predominant serotype of Salmonella in links of pig slaughtering in Shandong Province is S.derby,and resistance is different to the different drugs.The drug resistance of different slaughter links exist significant differences,multiple drug resistance is serious,and drug-resistant spectrum are varied.
6.Research on psychological distress of psychiatric nurses and its influencing factors

Objective To investigate the present status and influencing factors of psychological distress among psychiatric nurses, and provide theoretical basis for intervention measures. Methods Totally, 812 psychiatric nurses in six psychiatric hospitals of Shandong province were selected to complete the Kessler Psychological Distress Scale (K10), Perceived Organizational Support(POS), Simplified Coping Style Questionnaire(SCSQ) and Emotion regulation questionnaire(ERQ). Results The mean scores of psychological distress among psychiatric nurse was (21.29±8.11) points,70.3%of psychiatric nurses had psychological distress. There were significant differences in psychological distress among psychiatric nurses with different age (F=6.886,P=0.001), working years (F=4.403,P=0.004), professional title (F=6.597,P=0.000) and experience of nurse-patient conflict (F=7.228,P=0.000).Psychological distress was negatively related to cognitive reappraisal(r =- 0.093), positive coping style (r =- 0.163) and perceived organizational support(r =- 0.379), while psychological distress was positively related to expressive suppression(r=0.160) and negative coping style(r=0.258)(all P<0.01). Regression analysis revealed that the factors impacting psychological distress were positive coping style, perceived organizational support, expressive suppression, negative coping style and nurse-patient conflict (R2=0.224,F=46.611,P=0.000). Conclusions Psychiatric nursing managers should pay attention to the cultivation of emotional management ability of nurses and the ability to deal with conflict,encouraging them apply positive thinking cognition and positive coping style to deal with negative events in the work,creating a supportive work environment in order to improve the level of mental health.

Objective To investigate the current status of knowledge-attitude-practice of cardiac rehabilitation in patients undergoing coronary artery bypass grafting(CABG)and analyze its influen-cing factors.Methods From July to November 2022,288 patients with CABG in the coronary heart disease surgery center of Beijing Anzhen Hospital Affiliated to Capital Medical University were con-veniently included,and general information questionnaire and cardiac rehabilitation knowledge and practice questionnaire were used to investigate the general situation and the level of knowledge-atti-tude-practice of cardiac rehabilitation in patients with coronary heart disease.Results A total of 290 questionnaires were sent out in this study,and 288 were effectively collected,with an effective recov-ery rate of 99.31%.The total score of the questionnaire was(106.81±13.35),and the total score rate was 62.83%,including the knowledge questionnaire scoring(39.34±9.13),belief question-naire scoring(32.86±3.39)and behavior questionnaire scoring(34.61±3.54).The scoring rates of knowledge,attitude and practice questionnaire were 46.28%,82.15%and 76.91%,respective-ly.Multiple linearregression analysis showed that patients who lived in rural areas,had education background of primary school or below,were farmers,had public health insurance,and had no previous coronary intervention,had lower levels of knowledge-attitude-practice of cardiac rehabilitation(P<0.05).Conclusion The knowledge-attitude-practice of cardiac rehabilitation,especially the level of knowledge,in patients with CABG is needed to be improved.The factors that affect the lev-el of knowledge-attitude-practice of cardiac rehabilitation in this population are diversified,sugges-ting that individual cardiac rehabilitation guidance should be implemented according to different peo-ple and local conditions,and diversified cardiac rehabilitation models should be established to im-prove the long-term and compliance of cardiac rehabilitation.

10.Effects of multidisciplinary rehabilitation nursing combined with "3C" intervention in patients after knee surgery with manipulation under anesthesia

Objective:To explore the effects of multidisciplinary rehabilitation nursing combined with "3C" intervention in the nursing of patients after knee surgery with manipulation under anesthesia (MUA) .Methods:From March 2018 to March 2020, convenience sampling was used to select 171 patients after knee surgery with MUA in a rehabilitation center in Shanghai, and patients were divided into experimental group (87 cases) and control group (84 cases) according to admission time. Experimental group implemented multidisciplinary rehabilitation nursing combined with "3C" intervention, and control group implemented conventional rehabilitation. The intervention effects of knee flexion angle, Hospital for Special Surgery (HSS) , rehabilitation knowledge, rehabilitation compliance were compared between the two groups of patients.Results:The passive knee flexion angle of experimental group in the first week of intervention and the active knee flexion angle in the second and tenth weeks of intervention were greater than those of control group, and the differences were statistically significant ( P<0.01) . The HSS score of experimental group after intervention was higher than that of control group, and the difference was statistically significant ( P<0.01) . The experimental group's mastery of rehabilitation knowledge and rehabilitation compliance were higher than those of control group, and the differences were statistically significant ( P<0.01) . Conclusions:The implementation of nurse-led multidisciplinary rehabilitation nursing combined with "3C" intervention for patients after knee surgery with MUA is conducive to improving the patient's grasp of rehabilitation knowledge, compliance with rehabilitation interventions, the patient's knee flexion angle and knee joint function, as well as increasing the patient's satisfaction with nursing work.
