Objective To evaluate the inhibitory effect of highly selective M4 receptor antagonist MT3 on the form deprivation myopia in guinea pigs and its potential mechanism. Methods Thirty?two healthy male guinea pigs were ran?domly divided into three groups:control group, form deprivation group, and form deprivation + MT3 group, 8 animals in each group. Refraction was measured by retinoscopy after cycloplegia before and after the experiment. The ocular biological dimensions were measured by A?scan ultrasound. RT?PCR was used to detect the relative expression of TGF?β2 mRNA in the retina and choroid. Results Compared with the right eyes of control group, the right eyes of form deprivation + MT3 group developed relative myopia of -1?44 ± 0?50 D (right?left eye) (P =0?001). The vitreous chamber depth and axial length of the right eyes were significantly prolonged by 0?10 ± 0?02 mm and 0?14 ± 0?07 mm (P<0?001, P<0?001), respectively, but the increases of myopia and axial length were significantly smaller than that of the form deprivation group (P<0?001, P<0?001, P<0?001). Down?regulation of relative mRNA expression of TGF?β2 in retina and choroid was found in the form deprivation group (P<0?001, P =0?014) compared with the right eyes of the control group, while up?regulation of relative mRNA expression of TGF?β2 in retina and choroid was found in the form deprivation + MT3 group ( P<0?001, P<0?001). Conclusions MT3 can inhibit the development of form deprivation myopia in guinea pigs, which may play an important role by the regulation of TGF?β2 mRNA level in the retina and choroid.

Objective To study liver regeneration of the non-ligated liver lobes following portal branch ligation (PBL). Methods Sixty male Wistar rats were randomly divided into PBL group and sham operation (SO) group. Under ether anesthesia, the rats were subjected to PBL and sham operation, respectively. The animals were sacrificed on the 1st, 2nd, 3rd, 7th and 14th day respectively. The blood sample was collected from heart and the livers were harvested to determine serum alanine aminotransferase (ALT) levels and total liver weight, respectively. The hepatic histopathology was studied through light microscopy. The number of liver cell nuclear mitosis index was counted. The number of proliferative cell nuclear antigen (PCNA) index was counted by immunohistochemistry. The hepatic ultrastructural changes were studied under electron microscope. Results Elevated serum ALT level was observed in the first postoperative day in PBL group compared with SO group (P

OBJECTIVE:To provide reference for rational use of antibiotics to treat Pseudomonas aeruginosa (PA) infection in the clinic. METHODS:Resistant rate of PA in our hospital during 2011-2015 were analyzed retrospectively. Antibiotics use densi-ty(AUD)of 10 commonly used antibiotics were analyzed statistically, and the correlation of resistant rate with AUD was investi-gated by Spearman correlation analysis. RESULTS:One thousaud and eleven strains of PA were isolated in our hospital during 2011-2014,detection rate of PA always occupied the top 5 place. Top 3 antibiotics in the list of AUD were levofloxacin,ceftazi-dime,cefoperazone sodium and tazobactam sodium. AUD of piperacillin sodium and tazobactam sodium,levofloxacin,ciprofloxa-cin and meropenem were positively correlated with resistant rate of PA(r were 1.000,0.900,1.000,1.000,P<0.05). AUD of ce-foperazone sodium and tazobactam sodium were negatively correlated with resistant rate of PA(r=-0.900,P<0.05). AUD of imi-penem and cilastatin sodium, ceftazidime, gentamicin, aztreonam and amikacin had no correlation with resistant rate of PA(P>0.05). CONCLUSIONS:There is correlation between AUD of antibiotics and resistant rate of PA. It is of important significance to detect resistant rate of PA and the use of antibiotics regularly. Antibiotics should be selected cautiously in accordance with bacterial monitoring data, results of drug sensitivity tests, the amount and resistant rate of antibiotics, etc,in order to reduce resistant PA.

Objective To outline the research fronts of ten major sub-specialties in internal medicine. Methods Based on their impact factor scores and the proportion of the journals of 10 subspecialties (endocrinology & metabolism, cardiac & cardiovascular systems, hematology, infectious diseases, nephrology, gastroenterology & hepatology, respiratory system, rheumatology, critical care medicine, clinical neurology) in Journal Citation Report (JCR),and careful consulation of expert clinicians,we identified 50 journals.Their bibliographic records (including references) published in 2011 were downloaded, and the frequency of the references (citations) in each sub-specialty was counted and the highly cited records were extracted. We performed a clustering analysis according to the co-cited times among any pairs of the highly cited records. To tag each cluster of highly cited records,we browsed the titles and abstracts of all highly cited records in the same cluster, and concluded the main topics of each cluster. Finally,we extracted the current published papers devoted to particular cluster by some clustering analysis indicators. The clusters of highly cited records were considered as the intelligence base, and the main topics in current papers which citing these highly cited papers were considered as research fronts. Results Totally 50 journals on 10 sub-specialties in internal medicine were identified.A total of 202 highly cited papers,38 clusters (knowledge bases),and 152 corresponding current citing papers presenting the research fronts were selected. Conclusions We confirm and present research fronts in 10 major sub-specialties of internal medicine. This study provides a synchronic structure of contemporary research activities in internal medicine sub-specialties.

Objective:To summarize the best evidence for extubation management of adult patients with mechanical ventilation in the intensive care unit (ICU), which will provide practical guidelines for medical staffs. Methods:BMJ best clinical practice, UpToDate clinical consultants, Guidelines International Network (GIN), National Guideline Clearinghouse (NGC), National Institute for Health and Care Excellence (NICE), Scottish Intercollegiate Guidelines Network (SIGN), Registered Nurses' Association of Ontario (RNAO), Medlive guide, Cochrane Library, JBI evidence-based Health Care Center Database, Web of Science, PubMed, Cumulative Index to Nursing and Allied Health Literature (CINAHL), Ebsco, SinoMed, CNKI, Wanfang Database, etc., were systematically searched. Clinical guidelines, systematic reviews, expert consensus, and randomized controlled trial (RCT) studies were searched from the above database unit August 31st, 2020. Literature quality was evaluated using Appraisal of Guidelines for Research and Evaluation Ⅱ (AGREEⅡ), JBI quality evaluation tools, and Cochrane risk bias assessment. Two researchers evaluated the quality of the included literature respectively, and then the best evidence of endotracheal intubation and extubation management in ICU adult patients with mechanical ventilation was extracted and summarized. Results:A total of 12 articles were collected, including 2 guidelines, 5 systematic reviews, 2 expert consensus, and 3 RCTs. This paper summarizes 17 best evidences on extubation management of adult patients with mechanical ventilation in ICU, including accurate pre-extubation assessment, personnel and equipment, medication, posture, oxygen therapy, airway management, and post-extubation monitoring. Conclusion:Medical staff should choose the best evidence that meets the requirements of the clinical settings and standardize the management of patients after extubation to reduce the incidence of complications and re-intubation rate to ensure the safety of patients, which will be very important for the management of ICU adult patients with mechanical ventilation after extubation.
