1.NOTCH3 gene diagnosis in cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy

Objective To analyse the mutation types of the NOTCH3 gene with cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy(CADASIL). Methods Including 4 probands of CADASIL,10 cases of CADASIL and 4 normal members from 3 CADASIL families and 100 healthy controls were recruited. Genomic DNA was extracted from white blood cell. The amplicons were analyzed by the denaturing high-performance liquid chromatography (DHPLC) technique. The positive samples which identified by the DHPLC were sequenced to determine the specific mutation or polymorphism, respectively. Results Three heterozygous missense mutations including Cys90Arg, Arg141Cys, Cys134Tyr located in the exon3 and exon4 were found in the 4 probands and 10 cases of CADASIL among the 3 families. 15 polymorphisms were also found. 2 members individual from family 1 and 2 were found to carry the same pathological mutations as in their proband but without clinical symptoms. They were identified as preclinical patients. Conclusions Mutation detection of NOTCH3 is the molecular genetic mechanical for CADASIL. The exon3 and exon4 are possible hot mutation spots in Chinese patients. The mutation of Cys134Tyr in exon4 is a novel mutation which has not been reported previously in China.

The DNA microarray study showed that in STZ-induced diabetic rats the elevatedexpression of 42 genes in artery were depressed markedly after atorvastatin treat ment .This suggeststhat atorvastatin may have the protective effects on the diabetic vascular lesion.
3.Effect of Intensive Pedaling Training on Lower Limbs Recovery from Hemiplegia

Objective To explore the effect of intensive pedaling training on lower limbs recovery from hemiplegia. Methods 60 hemiplegics after stroke were divided into treatment group (n=30) and control group (n=30). Both groups accepted routine rehabilitation, including physiotherapy, occupational therapy, acupuncture and electrical stimulation, while the treatment group accepted intensive pedaling training in addition. They were evaluated with Berg Balance Scale (BBS), Fugl-Meyer Assessment of lower limb (FMA-L), modified Barthel index (MBI), Functional Ambulation Category (FAC), modified Ashworth scale (MAS) before and after 8 weeks of treatment. Results The scores of BBS, FMA-L, MBI, FAC, and MAS improved in both groups after treatment (P<0.01), and improved more in the treatment group than in the control group (P<0.05). Conclusion Intensive pedaling training can further improve the lower limbs function of stroke patients with hemiplegia
4.Clinical application of genechip based detection system in diagnosing mycobacterium infection and TB drug resistance gene mutation

Objective To investigate the clinical application value of genechip detection system in the mycobacterial species iden-tification and drug resistance analysis .Methods The specimens of sputum ,punctured pus ,pleural and abdominal ascites ,cerebro-spinal fluid and so on were performed the Mycobacterium DNA detection by using the gene chip technique .Then Mycobacterium tuberculosis positive samples were further performed the drug-resistant analysis .Meanwhile the Ziehl-Neelsen acid-fast staining was adopted to detect the sample .The positive rates were compared between the two groups .And TB-IGRA was used to examine the tubercle bacillus infection in partial patients .Results In 4402 samples ,137 cases (3 .36% ) of M ycobacterium tuberculosis (MTB) and 11 cases(7 .4% ) of non-tuberculosis mycobacterium(NTM) were detected .Puncture solution ,bronchoalveolar lavage fluid and tissue specimen had higher positive rate .In the 137 positive M TB samples by rifampicin-resistant gene rpoB ,isoniazide-re-sistant gene katG and inhA detection ,22 cases of resistance gene mutations were detected ;the positive rate of genechip for detecting sputum ,cerebrospinal fluid ,hydrothorax and ascites was higher than that of acid-fast staining .TB-IGRA detection had higher pos-itive rate of TB infection than genechip .Conclusion The genechip detection system can directly conduct Mycobacterium identifica-tion and drug resistance analysis ,which is especially suitable for sputum ,cerebrospinal fluid ,hydrothorax and ascites samples ,and which is simple and rapid with higher sensitivity and good specificity .
5.Comparison of effects of ELISPOTs using pleural fluid and peripheral blood in the diagnosis of tuberculous pleurisy

Objective To evaluate the effects of ELISPOT (enzyme-link immunospot) test using different samples in diagnosing tuberculous pleurisy. Methods Using T-Spot-TB kit to detect interferon-γlevel in pleural effusion and periph?eral blood from 164 patients with tuberculous pleural effusion and 102 patients without tuberculous pleural effusion. Number of spot forming cells (SFCs) as well as the specificity and sensitivity of the tests were compared between these two methods (ELISPOT using leural effusion or peripheral blood). Results The area under the ROC curve was 0.947 in pleural effusion Elispot test while it was 0.905 in peripheral blood Elispot test. The sensitivity of pleural effusion ELISPOT test in diagnosis of tuberculous pleurisy (95.1%) was significantly higher than that of peripheral blood ELISPOT test (89.0%). What’s more, the specificity of pleural effusion ELISPOT test in diagnosis of tuberculous pleurisy (90.2%) was higher than that in diagno?sis of peripheral blood ELISPOT test (88.2%). Conclusion The pleural effusion ELISPOT test is more valuable than periph?eral blood ELISPOT in the diagnosis of tuberculous pleuritis.
6.Effect of intermedin pretreatment on repair and regeneration process of renal ischemia reperfusion injury

ObjectiveTo explore the effect of intermedin pretreatment on repair and regeneration process of renal ischemia reperfusion (IR) injury.Methods Male Wistar rats were randomly divided into four groups: sham group,IR group,empty plasmid group and IMD group.One week after removing the right kidney,eukaryotic expression vector encoding rat IMD gene was transfected into the left kidney using an ultrasound-microbubble-mediated system,and transfection effect was detected by Western blotting and RT-PCR.After successful transfection,renal IR injury model was induced by clamping left renal arteries for 45 minutes followed by reperfusion.BUN and Scr were tested.Renal pathology was observed by PAS and HE staining.The proliferation of tubular epithelial cells was detected by SABC immunohistochemical method.Results (1)The expression of IMD protein and mRNA in IMD group was significantly up-regulated compared to empty plasmid group(P<0.05),reaching the peak at day 7,and the difference was not significant with that at day 4.(2)Compared with sham group,BUN and Scr were obviously increased at day 1and 2 in IR group(P<0.05).However,compared with IR group,BUN and Scr in IMD group decreased obviously(P<0.05).Differences between IR group and empty plasmid group were not significant(P>0.05).(3)Kidney tubules in IR group,IMD group and empty plasmid group were injuryed,but the injury in IMD group was less serious compared with that in IR group and empty plasmid group.The injury in three groups was the most severe at day 2 after reperfusion.(4)The number of PCNA positive cells evidently increased at day 7 in IR group and empty plasmid group,but the number of PCNA positive cell evidently increased at day 3 in IMD group.Compared with IR group 1 to 4 days after perfusion,PCNA positive cells in IMD group increased obviously (P<0.05).Compared with IR group day 7,PCNA expression in IMD group 7 d was obviously decreased(P< 0.05).Conclusion IMD pretreatment can induce tubular epithelial cells proliferative and accelerate the repair and regeneration progress in IR injury kidneys.
7.Evaluation study on nursing safety management indicators for skilled nursing facilities

Objective To check the maneuverability of the nursing safety management indicators for skilled nursing facilities.Methods A hospital based skilled nursing facility in Chongqing was chosen as the main study target while a Red Cross nursing hospital in Shanghai which operated earlier was also included as a comparison object.Combination of files reference,on-the-scene observation,manager interviews,sampling survey and institution's self-assessment,scores of indicators were calculated,and the total scores for both institutions were calculated as well.Results The institution from Chongqing marked 81 points while the other one got 86 points.Conclusions The indicator system had good maneuverability and the weight of indexes was consistent with the practical requirement of safety management.This indicator system could provide reference for standardize skilled nursing facilities' management,but still needed further revision and consummation.
8.Expression of signal transduction pathway genes in ARDS transcription profiles of rats

Objective To study the signal transduction pathway genes in acute respiratory distress syndrome(ARDS) of rats by gene chip. Methods Total RNAs were isolated from lungs of normal and ARDS rats respectively. The RNAs were purified by oligotex. Both mRNAs from two kinds of tissues were reversely transcribed to cDNA with incorporation of fluorescent-labelled dUTP to prepare the hybridization probes. The mixed probes were hybridized to cDNA microarray. Picture signals of fluorescence in gene array were scanned and compared by CapitlBio Molecule Annotation System V4.0. Quantitative fluorescence RT-PCR was used to validate the results of genechip. Results The results showed that there were 2 genes up-regulated and 9 genes down-regulated in lung tissue of ARDS rats,and these genes were involved in 11 signal transduction pathways. Conclusion Many genes of signal transduction pathways expressed in ARDS transcription profiles of rats.
9.Effect of Eating on Pharmacokinetics of Ethanesulfonic Acid Levofloxacin Tablets

OBJECTIVE: To investigate the effect of eating on pharmacokinetics of ethanesulfonic acid levofloxacin tablets.METHODS: The blood concentration of 10 healthy male subjects were determined by HPLC after receiving single oral dose of 200mg ethanesulfonic acid levofloxacin both before and after eating by randomized crossover way.The data processing was conducted with 3p97 software so as to figure out the pharmacokinetic parameters.RESULTS: The plasma concentration-time curves of the subjects after administration of drugs were in conformity with two-compartment model.The respective main pharmacokinetic parameters of the eating group and the empty stomach group were as follows: Cmax were (1.91?0.36)mg/L and(2.16?0.69) mg/L; AUC0~t were (14.14?2.32)(mg?h)/L and(14.40?3.11)(mg?h)/L; t1/2? were(6.59?1.66)h and(6.94?0.81 )h.CONCLUSION: Neither eating nor empty stomach has effect on the pharmacokinetics of ethanesulfonic acid levofloxacin tablets.
10.Analysis of Antibiotics Use and Bacterial Resistant before and after Intervention in Our Hospital

Objective:To analyze the use status of antibiotics and the resistance of clinic isolate bacteria against the commonly used antibiotics before and after the intervention. Methods:Using the information retrieval systems, the consumption of antibiotics in the inpatients during the 1st quarter of 2012(before the intervention) and the same period of 2013(after the intervention) was com-pared. According to the defined daily dose ( DDDs) , the antibiotics were ranked, and the resistance rate against the commonly used antibiotics was analyzed. Results:Compared with the top ten before the intervention, the top ten after the intervention was changed sig-nificantly, however, cephalosporins was still the main species. After the intervention, the overall decline in DDDs was significant, the separation rate and distribution of bacteria remained stable. ESBLs enzyme production rate of Enterobacteriaceae Escherichia coli was re-duced by 7. 61%, and that of Klebsiella pneumoniae was reduced by 1. 34%, and the resistance rate against the commonly used antibi-otics was in an overall downward trend. The resistance rate of Gram-positive staphylococci against the commonly used antibiotics was de-creased, while that of Gram-positive enterococci showed notable difference. Conclusion:The DDDs of antibiotics and bacterial resist-ance rate are in an overall downward trend in our hospital after the intervention;however, there is still exception. Therefore, the clini-cal antimicrobial susceptibility tests should be performed as soon as possible to help the choice of antibiotics.
