1.Clinical observation of progesterone combined with psychological intervention on threatened abortion caused by luteal insufficiency

Objective To investigate the clinical effect of progesterone combined with psychological intervention on threatened abortion due to luteal insufficiency.Methods The control group was treated with progesterone alone, and the study group was given psychological intervention on the basis of oral progesterone.Two groups of corpus luteum dysfunction in patients with threatened abortion clinical effects and bad mood improvement effect.Results The study group of corpus luteum dysfunction in patients with threatened abortion success rate (93.75%) was significantly higher than the control group (77.08%), data comparison(P<0.05);study group of SDS, SAS scale score improved better than the control group, comparison of the data(P<0.05).Conclusion Application of progesterone combined with mental intervention in the treatment of corpus luteum dysfunction aura the abortion effect, help to protect patients' quality of life, physical and mental health.
2.Utility of full-field digital mammography and localization in breast biopsy in diagnosis of nonpalpable breast cancer

Objective To investigate the utility of full-field digital mammography (FFDM) and localization and biopsy in diagnosis of nonpalpable breast cancer. Methods 42 cases with suspect breast cancer by routine film reading but negative clinical palpation were performed localization and biopsy under X-ray orientation and diagnosed with breast cancer by pathology. The characters of those cases were studied.Results Among 42 cases, 5 cases showed micronodules, 8 cases demonstrated local dense infiltration, 11cases had architecture distortion, 18 cases demonstrated micro-calcifications. The type of pathology comprised of intraductal carcinoma (23 cases), tabular carcinoma in situ (2 cases), early infiltration of ductal carcinoma (12 cases), infiltrating ductal carcinoma (3 cases), papilloma carcinoma (2 cases). Conclusion FFDM can clearly show direct and indirect X-ray signs of breast cancer, combined with localization and biopsy the nonpalpable breast cancer can be easily diagnosed.
3.The clinical value of arterial chemotherapy and embolization in the treatment of advanced uterinecarcinoma

Objective To evaluate the clinical value of arterial chemotherapy and embolization in the treatment of advanced uterine carcinoma.Methods In this study,52 cases of uterine carcinoma(including 43 carcinoma of cervix and 9 cases carcinoma of endonetrium)were treated interventional therapy.In these cases,after one or two times interventional therapy and after 10-14 days,45 cases followed by uterectomy and pelvic lymphadenectomy. 7 cases that can't operated continue followed by interventional therapy. Results Curative effect was obvious,the clinical symptoms were alleviated,the bleed in operation was decreased and the tumor was divided easily.In 45 cases followed by operation,the one-year and three-year survival rate are 100% and 92%.In 7 cases treated by interventional therapy alone,the one-year and three-year survival rate are 100% and 85%.Conlusion The interventional therapy preoperation can reduce the clinical stage and metastasis of lymph,improve the ectomy by operation and survival rate of patients obviously,so it can be used as a routine treatment in uterine carcinoma.
4.Effects of Shengji Huayu Recipe and its decomposed formulas on synthesis of collagen types I and III in granulation tissue of rats in early wound healing

OBJECTIVE: To study the effects of Shengji Huayu Recipe (a traditional Chinese medicine compound recipe for resolving stagnation and promoting granulation) and its decomposed formulas (Huayu Recipe for resolving stagnation and Shengji Recipe for promoting granulation) on the synthesis of collagen types I and III in granulation tissue of rats in early wound healing. METHODS: Twenty-four male Sprague-Dawley (SD) rats with full-thickness skin lesion were randomized into 4 groups: Shengji Huayu Recipe-treated group, Shengji Recipe-treated group, Huayu Recipe-treated group and untreated group. Collagen types I and III in granulation tissue of the rats were tested with immunohistochemical methods and image analysis. RESULTS: On the third day of wound healing, collagen I of the rats in both Shengji Huayu Recipe-treated group and Shengji Recipe-treated group was higher than that in the untreated group, and collagen I of the rats in Huayu Recipe-treated group was lower than that in the untreated group (P<0.05). Collagen III of the rats in the three treated groups were lower than that in the untreated group (P<0.05). On the seventh day of wound healing, Collagen I of the rats in both Shengji Huayu Recipe-treated group and Shengji Recipe-treated group was higher than that in the untreated group (P<0.05), and collagen III of the rats in both Shengji Recipe-treated group and Huayu Recipe-treated group was higher than that in the untreated group (P<0.05). CONCLUSION: Resolving stagnation and promoting granulation therapy can promote the wound healing in rats.
5.Distribution and Drug Resistance of Isolates from Blood Culture Samples

OBJECTIVE To enhance the positive rate of the blood culture in order to make pathogenic diagnosis actually and quickly and conducting usage of antimicrobial agents in clinic.To value the clinical applied circumstance of BacT/Alert 3D automated blood culture system.METHODS The 3728 blood cultures were detected by BacT/Alert 3D automated blood culture system,and bacterial susceptibility test was conducted on all isolates using Kirby-Bauer methods with CLSI standards.To statistically analyze the examined time,the positive rate and variety and drug resistance for all kinds of pathogens.RESULTS The blood culture positive rate was 6.0% in the 3728 blood cultures with 222 strains of bacteria.The false positive rate was 0.2% and the false negative rate was 0.4%.The positive rate of blood culture was 0.89% in 12 hours,2.01% in 18 hours,and 3.51% in 24 hours.Among all the 222 isolates,29.7% were Enterobacteriaceae,the drug resistant rates to amikacin,ceftazidime,ciprofloxacin,and imipenem were 13.6%,36.3%,42.4%,and 3.0%,respectively;20.3% were Staphylococcus,the drug resistant rates to erythromycin,levofloxacin,cefoxitin,and vancomycin were 75.6%,33.3%,68.9%,and 0,respectively;11.7% were Enterococcus,the drug resistant rates to errythromycin,levofloxacin,fosfomycin,and vancomycin were 96.2%,80.8%,23.1%,and 0,respectively;11.3% were non-fermented bacilli,the drug resistant rates to amikacin,ceftazidime,ciprofloxacin,and imipenem were 32.0%,52.0%,32.0%,and 32.0%,respectively;12.6% were fungi.CONCLUSIONS The pathogens in the blood specimens are more wider in distribution and more higher in drug resistance rates than before.BacT/Alert 3D automated blood culture system can be an important tool for the blood culture,it can provide the diagnostic help quickly for the clinic and increase the positive rates in blood culture.It can not only shorten the check-up time,but also be more quick and more exact.
6.Bacteria Flora in Nosocomial Infection over the Past Five Years:Analysis on Distribution Tendency and Drug Resistance

OBJECTIVE To understand the distribution or change and drug resistance of the bacteria flora in nosocomial infection,in order to provide laboratory evidence for controlling nosocomial infection and to indicate clinical antimicrobial agents usage.METHODS All isolates were identified by routine procedure and VITEK microbe automatic system and API identified system.Drug susceptibility test was used K-B paper disk diffusion method in accordance with the CLSI/NCCLS standards.RESULTS There were 11 464 strains of bacteria which were obtained from 2002 to 2006.Sputum,secretion and middle urine were the major specimens.The major bacteria floras were Pseudomonas aeruginosa,Escherichia coli,Acinetobacter,Enterococcus,Staphylococcus and Candida albicans,whose isolating rates were 57.02% in 2002,64.46% in 2003,57.83% in 2004,57.49% in 2005,and 60.73% in 2006.E.coli and Klebsiella pneumoniae produced ESBLs rates were from 39.87% to 61.98% that drug resistance rates to cefoperazone/sulbactam and imipenem were 14.06% and 0.64%.The drug resistance rates of nonferment Gram-negtive bacteria to cefoperazone/sulbactam were below 33.22%,Gram-positive cocci to vancomycin was below 0.34%.CONCLUSIONS Now,the important bacteria flora is nonferment Gram-negative bacteria in nosocomial infection.The P.aeruginosa,E.coli,Acinetobacter baumannii,Enterococcus faecalis,C.albicans,and meticillin-resistant staphylococcus(MRS) were prevalent important bacteria in nosocomial infection.The situation of producing ESBLs in Enterobacteriaceae is very severe.Glycopeptides are the best choice to MRS.To zymogenic bacteria and the nonferment Gram-negtive bacteria that cause severe infection,combining-drug treatment can be chosen according to drug susceptibility test results.
7.Deep Fungi Infection:Flora Distribution and Drug Resistance

OBJECTIVE To understand flora distribution and four antifungal drugs′in vitro antifungal activity of deep fungi in nosocomial infection in order to provide help to clinics.METHODS Fungi were cultured and isolated by routine procedure which identified by VITEK microbe automatic system.Drug susceptibility test used Rosco paper disk diffusion and broth dilution method with NCCLS M27-A.RESULTS Totally 156 strains with 9 species of deep fungi that main fungi were Candida albicans,and C.tropicalis with 57.69%,and 31.41%,respectively,were isolated from nosocomial infection.The major isolating rates of clinical infection specimens were from respiratory,cardiovascular surgery,and neurological departments with 26.28%,12.18%,and 9.62%,respectively.The main infection specimens were from respiratory tract and urinary tract with 71.15% and 16.67%,respectively.Drug resistance rates to fluconazole,amphotericin B,itraconazole,and ketoconazole with Rosco paper disk diffusion were 23.08%,2.56%,12.18%,and 17.36%,MIC90 were 64.0,2.0,8.0,and 16.0mg/L,respectively.CONCLUSIONS The main deep fungi are C.albicans and C.tropicalis.Antifungal activity of amphotericin B is the highest than others.The drug resistance rate to fluconazole is more and more higher.Clinics should use antifungal drug rationally in accordance with drug susceptibility test results.
8.Analysis of 278 breast biopsy cases guided by mammography

Objective To analyze 278 breast biopsy cases guided by mammography and find the characteristic of the most suitable cases.Methods The initial diagnosis of breast cancer of 278 cases were divided into mass type (197 cases) and non-mass type (81 cases) according to the imaging features,the biopsy was performed in 14G core needle guided by mammography and contrasted to paraffin pathology of post operation.Results The cases of biopsy conformed by paraffin pathology were 260,accuracy rate is 93.52 %(260/278).192 cases were diagnosed pathologically in 197 cases of mass type,and 68 cases in 81 cases of non-mass type.The accuracy rates were 97.46 % (192/197) and 83.95 % (68/81) respectively.Conclusion The mass type with clinical and imaging features is suited for biopsy.To avoid missed diagnosis,it is important to frozen during operation for the cases with the negative result in non-mass type.
9.Evaluation of image quality and radiation dose in maramography: a comparison among digital radiography, computed radiography and film-screen systems

Objective To compare the image quality, radiation dose and diagnostic accuracy on film-screen, digital radiography (DR) and computed radiography (CR) mammography. Methods Three different kinds of mammography machines (film-screen, DR and CR) were tested to make sure they were qualified and comparable. Radiographies were taken on Mammo-152 phantom and ACR phantom using filmscreen, DR and CR systems with the same radiation dose. Next, radiographies were taken on two phantoms using DR and CR systems with classic mode or AEC and the radiation dose was recorded. The images were numbered and scored by eight independent experienced radiologists under the same reading condition according to ACR method. Statistics was performed with the randomized complete-block design variance analysis. The diagnostic accuracy of three kinds of mammography methods were compared with Kappa test in clinical cases who had pathologic results. Results The spacial resolution of film-screen system was the highest (7.0-8.0 Lp/mm) and that of CR was the lowest (2.5 Lp/mm). When the radiation dose was increased (27. 0%-30. 0% ), the resolution was improved with DR ( +11. 0% ) but almost no change with CR ( +1.5%). The radiation dose of DR was the lowest on ACR phantom. The delineation of mass was better on film-screen than on CR, but the delineation of fibers and speck was better on CR. When the radiation dose of CR was increased (25. 0% ), the delineation of fibers and speck on CR was similar to that on DR and the delineation of mass was similar to that on film-screen. There was a good correlation between the mammography diagnosis and pathological results (film-screen is 50/56, DR is 83/90 and CR is 61/69,P < 0. 01 and Kappa value > 0. 75) . The diagnostic accuracy of three mammography systems had no statistical difference [film-screen 89. 3% (50/56), DR 92. 2% (83/90) and CR 88. 4% (61/69),Pearson X 2=0. 722 ,P =0. 697]. Conclusion Film/screen system has the highest space resolution and DR has the lowest radiation dose. Three mammography systems have the same diagnostic accuracy for breast disease.
10.Impact of Xuebijing injection on the expression of tumor necrosis factor α and high mobility group box 1 in rat peritoneal mesotheliai cells induced by lipopolysaccharide

Objective To observe the effect of Xuebijing injection on the expression of tumor necrosis factor-alpha (TNF-α) and high mobility group box-1 protein (HMGB-1) in rat peritoneal mesothelial cells (PMCs) induced by lipopolysaccharide (LPS). Methods PMCs were isolated from rat colic omentum and the 3rd generation cells were used in the experiment. PMCs were incubated with LPS at different concentrations (1,10,100 mg/L);with LPS (10 mg/L) for 2, 6, 12, 18, 21, 24, 36 h;with Xuebijing injection at different concentrations (2,10,20 g/L) after incubation with LPS (10 mg/L) for 2 h. PMCs in the control group were incubated with medium. HMGB-1 mRNA was detected by RT-PCR. TNF-α and HMGB-1 protein in supernatants was detected by ELISA. Results Compared to the control group, the expression of HMGB-1 mRNA and protein was significantly increased in groups stimulated by LPS in a time- and dose-dependent manner (all P<0.05);the expression of TNF-α was increased in the groups stimulated by LPS in a dose-dependent manner (P<0.05). In the groups stimulated by LPS (10 mg/L), the expression of TNF-α appeared double hump within 36 hours. Compared to LPS (10 mg/L) group, Xuebijing injection significantly inhibited the expression of HMGB-1 and TNF-α (all P<0.05 ) in a dose-dependent manner. Conclusions HMGB-1 as a late mediator of inflammatory responses may play a role in the pathogenesis of peritoneal dialysis related peritonitis. Xuebijing injection can reduce peritoneal inflammatory impairment by inhibiting the up-regulation of TNF-α and HMGB-1 induced by LPS.
