1.Clinical study of concurrent chemoradiotherapy combined with gamma knife therapy for cervical cancer with positive pelvic lymph node

Objective:To evaluate therapeutic efficacy and adverse reactions of synchronous chemoradiotherapy combined with gamma knife therapy for pelvic lymph node metastasis of cervical cancers. Methods:Data of 42 cervical cancer patients who suffered from residual pelvic lymph node metastasis and received concurrent chemoradiotherapy were retrospectively analyzed. Intensity-modu-lated radiotherapy was used in the treatment. The prescribed doses of planning target volume and pelvic metastasized lymph node of the planned gross tumor volume were 50.4 Gy/28 F and 59.92 Gy/28 F, respectively. The combined internal irradiation dose was 6 Gy/6 F. Concurrent chemotherapy was administered with 40 mg/m2·w cisplatin. Three months after chemoradiotherapy was completed, the pa-tients with residual pelvic positive lymph node received additional dose ranging from 10 Gy to 15 Gy at three or four fractions by using a gamma knife. Results:Near-term efficacy was 83.3%(35/42) in three months. Local control rates were 88.1%(37/42), 83.3%(35/42), and 76.2%(32/42) in 6, 9, and 12 months, respectively. The 1-and 2-year survival rates were 77.5%(31/40) and 70.0%(28/40), re-spectively. The incidence rates of radiation enteritis, proctitis, cystitis, gut toxicity, and neutrocytopenia were 11.9%(5/42), 38.1%(16/42), 7.1%(3/42), 90.5%(38/42), and 85.7%(36/42), respectively, and the majority of these conditions were classified as grades I and II. Conclusion:Synchronous chemoradiotherapy combined with gamma knife therapy is an effective and feasible treatment method for pelvic lymph node metastasis of cervical cancer;this method exhibits a minimal adverse reaction.
2.Expression and significance of PTEN, P-ERK and P-AKT in the middle ear cholesteatoma.

OBJECTIVE:
Detect the expressions of the protein tyrosine phosphatase gene (PTEN), phosphorylated protein kinase B (P-AKT) and phosphorylated extracellular signal-regulated kinase (P-ERK) in human middle ear cholesteatoma tissue and its correlation to explore their important role in the mechanism of the formation of cholesteatoma.
METHOD:
Use immunohistochemical SABC method (SABC immunohistochemical method) to detect the expressions and location of PTEN, P-AKT and P-ERK proteins in 40 cases of middle ear cholesteatoma tissue samples and 15 cases of normal ear skin specimens. Use Western blot to detect the expression levels of PTEN, P-AKT, P-ERK proteins and the internal reference GAPDH in 20 cases of cholesteatoma tissue and 10 cases in nor mal ear skin specimens.
RESULT:
(1) Immunohistochemistry showed coloring of PTEN both in the nucleus and cytoplasm of cholesteatoma and normal skin . Nuclear PTEN positive expression rates in the cholesteatoma was significantly lower than normal skin, and the difference was significant (P < 0.01); cytoplasm PTEN positive expression rates in the cholesteatoma was significantly lower than normal skin, and the difference was significant (P < 0.01); P-AKT mainly expresses in the cytoplasm of cholesteatoma and normal skin. The positive expression rates in the cholesteatoma was significantly higher than normal skin,and the difference was significant (P < 0.01); the P-ERK mainly colors in cholesteatoma and normal skin cell nucleus. the positive expression rates in the cholesteatoma was significantly higher than normal skin, and the difference was significant (P < 0.01). In cholesteatoma specimens, there was a significantly negative relationship (P < 0.01) between PTEN, respectively, and P-AKT, P-ERK protein. (2) Western blot discovered: the expression of PTEN in cholesteatoma was significantly less than the amount of expression in normal skin; P-AKT and P-ERK expression in cholesteatoma was significantly more than the level in normal skin.
CONCLUSION
Abnormal expression of PTEN, P-AKT and P-ERK protein in cholesteatoma may be closely related to antiapoptosis and high degree of proliferation in cholesteatoma. Expression of PTEN deletion leads to the weakening of the inhibition. Excessive expression of P-AKT gives rise to cholesteatoma epithelial cell apoptosis inhibited; excessive PERK expression result to cholesteatoma epithelial cell proliferation strengthened.

3.Clinical study on chemotherapy combined radioactive seed intersti-tial brachytherapy for locally advanced non-small cell lung cancer

Objective: To evaluate the clinical effect and adverse reactions of radioactive seed interstitial brachytherapy com-bined with etoposide (EP) regimen concurrent chemoradiotherapy treatment for locally advanced non-small cell lung cancer. Methods:All 24 cases of locally advanced non-small cell lung cancer received three-dimensional conformal intensity modulated radiation therapy by using 6 MV X-ray to obtain 95%60-66 Gy/30-33 F planning target volume. All cases received radiation therapy five times a week. EP regimen chemotherapy concurrent with radiotherapy was given to 24 patients. The therapy included VP-16, 60 mg/m2 intravenous in-fusion for 1 d to 5 d, and DDP 50 mg/m2 intravenous infusion on the 1st, 8th, and 28th therapy day. Chemotherapy with EP regimen was given for four cycles, in which two cycles were given during radiotherapy and the remaining cycles were completed after radiotherapy. The patients were reexamined three months after concurrent chemoradiotherapy was completed. Patients with residual tumor, con-firmed via positron emission tomography/computed tomography, underwent 125I radioactive seed implantation interstitial brachytherapy to complement the dose of tumor. Results: The response rate was 83.3% (20/24); the local control rates of 3, 6, 9, 12, 18, and 24 months were 87.5% (21/24), 83.3% (20/24), 75.0% (18/24), 70.8% (17/24), 58.3% (14/24), and 50.0% (12/24). The median survival was 20.2 months. The one-year survival rate was 62.5%, and the two-year survival rate was 37.5%. The following main toxicities were observed:the incidence of radiation-induced lung injury was 25%;the incidence of radiation esophagitis was 33.3%;the incidence of grades Ⅰ to Ⅱ gastrointestinal reactions was 82.3%; the incidence of reducing neutropenia was 87.5%, in which the incidence of gradesⅠtoⅡwas 75.0%, gradeⅢwas 12.5%, and gradeⅣwas 0%. Conclusion:EP regimen concurrent radiotherapy and chemo-therapy combined with radioactive seed interstitial brachytherapy for locally advanced non-small cell lung cancer is effective and has few serious adverse reactions, thus making this approach worthy of promotion.
4.Nursing for patients with ovarian cysts cured by abdominoscope

Objective Summarize the nursing experiences of abdominoscope for patients with ovarian cysts. Methods Enough preoperative evaluation, careful perioperative nursing and observation were used in 150 patietns with ovarian cysts when cured by abdominoscope. Results All the patients had accepted succesful operation, there were no intensive complication among the patients. Conclusions Proper nursing measures should be developed with therapeutic laparoscopy, nursing cares according to the operation can effetive shorten the course of rehabilitation for patients.
5.Family intervention on patients with essential hypertension

Two hundred and thirty two patients with essential hypertension were divided into intervention and control groups (116 in each) ,by the end of study period (from January 2005 to January 2006) 106 patients in intervention group (91.4%) and 102 in control group (87.9%) completed observation.The main findings were as follows: the blood pressure control rate in intervention group was higher than that in controls (X2=23.92,P <0.01) ;the family adaptability(X2 =31.12,P <0.01),family caring(X2=37.90 ,P<0.01),family communication (X2= 17.91,P<0.01) and family function(X2 = 8.26,P<0.01)in intervention group were better improved than those in control group.The results suggest that family can be effective resources for improvement of patients with essential hypertension.

7.Determining of Cyanogen Chloride in Drinking Water

Objective To develop a method for determining cyanogen chloride (CNCl) in drinking water. Methods Pyridine-barbituric acid spectrophotometry was employed and some parameters including the pH value of the phosphate buffer, the time of color reaction, temperature, the concentration of chloramine T, the proportion of the phosphate buffer, the proportion of color agent and the maximum absorption wavelength were optimized. The linear range, limit of detection, precision and recovery rate were studied. Results Trace quantity of CNCl obeyed the Law of Beer. In the range of 0-0.1 ?g/ml CN-, the correlation coefficient was 0.999 4, the regression equation was y=0.194x-0.033, the limit of detection was 0.003 ?g/L, the lower limit of detection was 0.01 mg/L, the relative standard deviations were 1.5%-6.2% and the recovery rates were 92.9%-107.1%. Conclusion The method has the characters of accurate, sensitive, simple and rapid, it can be used in monitoring of drinking water quality.

[Objective]To explore the change and its significance of the expression of vascular endothelial growth factor(VEGF) messenger RNA and bone morphogenic protein-2(BMP-2) of the local femoral head in the nontraumatic osteonecrosis of femoral head(NONFH).[Method]Sixteen samples of femoral heads of NONFH were collected as the experimental group and fresh 10 samples of femoral heads of femoral neck fracture as control group,overall examples were collected from total articular replacement arthroplasty.The samples were splitted in coronal plane and get one bone block from necrosis area and another one from healthy area,make them into microtome section after the process of immobility and decalcification.Their pathological change was observed by using optical microscope and electron microscope and detect the expression of BMP-2 and VEGFmRNA in femoral head through making use of immunohistochemistry and in-situ hybridization technique.[Result]The organization structure of experimental group was disorganized,cracked and the bone trabecula was rarefactive and non-intact and there was a great number of empty lacuna in bone trabecula.While there was the reverse situation in the control group.The intensity and area of positive expression of BMP-2 and VEGFmRNA in femoral head of the experimental group were lower than that of control group obviously.The result showed statistical significance(P
