5.Effects of Naikan cognitive therapy on improving clinical symptoms in patients with convalescent schizophrenia

Objective To explore effects of Naikan cognitive therapy on improving clinical symptoms in patients with convalescent schizophrenia. Methods The 69 convalescent schizophrenic patients with convalescent clinical state were consecutively recruited. All the patients were divided into Naikan cognitive therapy ( NCT) group an control group at random and were pretreated with antipsychotic agent therapy. In NCT group,the patients received NCT for successive 7 days. In control group,the patients only received antipsychotic agent therapy. Pre-and post-treatment positive and negative syndrome scale( PANSS) , Nurses'observation scale for inpatient evaluation (NOSIE) were administered to all subjects. Results ① A significant decrease occurred in PANSS total score, negative symptom score, positive symptom score, compound scale score, general psychopathology score, reaction retardation score and paranoid score in NCT group ( t = 2. 672~7. 370, P < 0. 05). In the post-treatment, PANSS total score, negative symptom score, positive symptom score, compound scale score, reaction retardation score and thought disorder score were significantly lower in NCT group than those in control group ( t ' = 2. 696, P = 0. 009; t = 5. 186, P=0.000; t = 3.757, P = 0.001; t = 2.634,P = 0.011; t ' =2.376, P = 0.021). ②A significant decrease occurred in NOSIE total negative score( 10.43 ± 9. 24 vs 13. 87 ± 8. 03, t = 3. 463 , P = 0. 002) , irritation score(3. 13 ±0.43 vs8.53 ±4.98, t = 6. 139, P=0.000) and retardation score(1.07 ± 1.64 vs 2. 20 ±2.85, t = 2.067, P = 0.048) in NCT group. Conclusion NCT can possibly improve part clinical symptoms of patients with convalescent schizophrenia to a certain extent,especially negative symptom,but need to further prove the effect of NaiKan cognitive therapy.

7.Application value of biochemical and immunological indicators in Lee′s classification of IgA ;nephropathy

Objective To analyze the differences of biochemical and immunological indicators in gender or Lee′s classification of IgA nephropathy ( IgAN) to provide laboratory evidence for clinical diagnosis and treatment of IgAN.Methods Retrospective cohort study.The information of biochemical and immunological indicators of 213 in-hospital patients which were admitted in Chinese PLA General Hospital in June to December, 2012.The data were collected and analyzed with t-test, non-parameter analysis, Pearson correlation analysis, ROC curves analysis and Logistic regression analysis according to gender or Lee′s classification ( Lee′s≤3 group and Lee′s>3 group).Results In this study, the average age and sex ratio of patients with IgAN was ( 35.0 ±10.6 ) years old and 2.04∶1.00.T-test and non-parameter analysis indicated homocysteine(HCY), immunoglobulin E (IgE), immunoglobulin M (IgM), prealbumin (PA), ceruloplasmin ( CP) ,α1-acidoglycoprotein(α1-AGP) , albumin ( Alb) ,α2-globulin(α2-G) andγ-globulin (γ-G) had significant difference in gender (P<0.05).And cystantin-C (CYS-C), HCY, complement 4 (C4),β2-microglobulin(β2-MG), PA, α1-AGP, α1-globulin(α1-G), immunoglobulin G (IgG), IgM and transferrin ( TF ) had significantly different in Lee′s classification ( P<0.05 ).ROC curves analysis showed that the areas under curves of CYS-C andβ2-MG were 0.891 and 0.839 respectively;the sensitivity was 90.2%and specificity was 75.3% when cutoff value of CYS-C was 1.56 mg/L; the sensitivity was 80.3%and specificity was 78.8% when cutoff value of β2-MG was 0.275 mg/dl.Logistic regression analysis showed that CYS-C ( OR: 31.380, 95% CI: 10.808 -91.113, P=0.000 ) and HCY ( OR:1.035, 95%CI:1.002-1.069, P=0.040) were introduced into the model after twice variable selections.ROC curve of CYS-C combined with HCY for classifying Lee′s classification showed that the sensitivity, specificity, Jorden index and AUC were 0.843, 0.862, 0.705 and 0.894 ( P=0.000 ) , respectively.Conclusions CYS-C,β2-MG, PA and HCY were valuable indicators for Lee′s classification.CYS-C andβ2-MG had high correlation with Lee′s classification, which indicated that these two tests were related to the severity of IgAN.CYS-C combined with HCY could improve the diagnostic efficiency of IgAN Lee′s classification.Combination of biochemical and immunological indicators could improve the accuracy of Lee′s classification and provide effective laboratory evidence for clinical treatment of IgAN.

BACKGROUND:Apoptosis of islet cel s is closely related to the long-term hyperglycemia-and hyperlipemia-induced injuries. OBJECTIVE:To observe the effect of Shizidaiping formula on the apoptosis and insulin secretion in MIN6 cel s under the high glucose and lipid environment, and to explore the protective effect of Shizidaiping formula and the related apoptosis mechanism. METHODS:MIN6 cel s were divided into normal, model, melbine, low-, medium-and high-dose Shizidaiping formula groups. The cel activity was examined by cel counting kit-8, the insulin secretion was measured by ELISA, the rate of apoptosis was measured by Annexin V-FITC&PI and the expression levels of MEK1/2, ERK1/2 and p-ERK1/2 were examined by western blot assay. RESULTS AND CONCLUSION:Shizidaiping formula significantly improved MIN6 cel activity under high glucose and lipid condition (P<0.05), decreased early cel apoptosis, increased the level of insulin stimulated by low glucose in cel supernatant (P<0.05), and improved the expression levels of MEK1/2, ERK1/2 and p-ERK1/2 (P<0.05). These results suggest that Shizidaiping formula can protect islet cel s from hyperglycemia and hyperlipemia damage by improving the activity of MIN6 cel s, reducing the insulin secretion and inhibiting the apoptosis of pancreaticβcel s in MIN6 cel s.
9.Sucrose for analgesia in preterm infants:a clinical randomized controlled trial

Objective To explore the analgesic efficacy and stability of oral sucrose for the treatment of procedural pain in preterm infants,then to evaluate the effect of repetitive oral sucrose on glucose metabolism.Methods Eighty-two preterm infants (gestational age ≤34 weeks,birth weight ≤2 000 g)who were admitted to Neonatal Intensive Care Unit in Shenzhen Nanshan Maternity and Child Health Care Hospital between January 2014 and October 2016 were randomized into an intervention group (n =42) and a control group (n =40).The intervention group received a pretreatment with 0.3-0.5 mL of 120 g/L sucrose solution bemore a painful procedure was executed.But the control group only received usual care.The two groups were compared for the changes in heart rate (HR),blood pressure (BP),percutaneous oxygen saturation(SPO2) in pre-and post-painful procedure and the duration of the HR recovering to pre-procedure level.The variations of Neonatal Infant Pain Scale (NIPS) of pre-and post-painful procedure were compared in each group and between 2 groups during the first 4 weeks of life.Finally,the concentrations of fasting blood glucose,the level of insulin,and glucose-to-insulin ratio were compared between the 2 groups on the day of discharge.Results The variations in HR,BP,NIPS and SPO2 between pre-and post-painful procedure were lower in the intervention group than those in the control group [(10.59 ± 5.50) times/min vs.(20.75 ± 14.18) times/min;(1.86 ± 2.45) mmHg vs.(3.64 ± 1.78) mmHg (1 mmHg =0.133 kPa);(2.11 ± 0.93) scores vs.(3.25 ± 1.21)scores;(0.71 ± 0.53) % vs.(1.03 ± 0.54) %],and the differences were statistically significant (t =-3.298,-3.008,-4.084,-2.195;P =0.002,0.004,0.001,0.033).Meanwhile,there was a shorter duration of the HR recovery to pre-procedure level in intervention group compared with the control group[(36.27 ±9.86) s vs.(54.72 ±26.43) s,t =-3.093,P =0.004].The variations in NIPS of pre-and post-procedures showed no significant difference in the 2 groups during the first 4 weeks (F =0.188,P =0.904).The concentrations of fasting blood glucose,the level of insulin,and glucose-to-insulin ratio were similar between the 2 groups when the infants were discharged [(4.73 ± 0.85) mmol/L vs.(4.96 ± 0.83) mmol/L,P =0.411;(6.30 ± 13.65) mU/L vs.(6.44 ± 8.60) mU/L,P =0.969;0.069 ± 0.135 vs.0.096 ± 0.108,P =0.567].Conclusions Oral sucrose (120 g/L) has an effective and stable analgesic effect on preterm neonates pain stimulation,but no significant effect on glucose metabolism.
10.WT1 downregulation during K562 cell differentiation and apoptosis induced by bufalin.

OBJECTIVETo study the change of WT1 gene expression during human leukemic K562 cell differentiation and apoptosis induced by bufalin.
METHODSCell viability was determined by trypan blue exclusion, cell differentiation and apoptosis by nitro blue tetrazolium (NBT) reduction test, morphology and flow cytometry, expression of WT1 protein by Western blot analysis, and expression of WT1 mRNA by reverse transcriptase-polymerase chain reaction (RT-PCR).
RESULTS(1) The cell proliferation was inhibited by bufalin and the IC(50) at 24, 48, 72 h were 0.026, 0.032 and 0.006 micro mol/L, respectively. (2) Bufalin induced K562 cell differentiation towards macrophage/monocyte within concentration from 0.01 to 0.05 micro mol/L and apoptosis at higher than 0.026 micro mol/L. (3) The expression of WT1 protein and mRNA were downregulated by bufalin in the initial stage of differentiation and apoptosis induced by bufalin.
CONCLUSIONK562 cell differentiation and apoptosis induced by bufalin might relate to the downregulation of WT1 expression.
