1.Progress in diagnosis and treatment of bronchiolitis

Bronchiolitis is the most common lower respiratory tract disease in infants younger than 2 years of age.Many viruses can cause bronchiolitis,however,respiratory syncytial virus is most common etiology.The pathological characters are acute inflammation,edema,necrosis of epithelial cells and mucus productions increased.Diaguosis of bronchiolitis is based on clinical signs and symptoms and a thorough history and physical exam.The disease severity and risk factors for severe disease need to be assessed.The treatment of bronchiolitis has been largely supportive.Continuous positive airway pressure treatment has been accepted for severe cases.Palivizumab prophylaxis is probable in high risk infants.Hand hygiene,decreasing tobacco smoke and breastfeeding are considered as efficient preventive methods.
2.Prevention of Empyema of Post-operative Esophagogastrostomy Anastomotic Fistula Inside Thorax

OBJECTIVE To investigate the cause,diagnosis and treatment of the empyema of the anastomotic fistula after esophageal cancer resection.METHODS The clinical data of 624 cases after esophageal cancer and cardia cancer resection from Jan 2000 to Dec 2006 were analyzed retrospectively which included 14 cases of the empyema of the anastomotic fistula.RESULTS All the cases were treated with the closed thoracic drainage,in which 9 cases were cured with closing double-tube chest douching technique,3 cases were succeeded to operate secondary thoracotomy for anastomosis,1 case died after being sent to the higher grade hospital,1 case died for toxic shock and respiratory failure without the chance of reoperation.CONCLUSIONS The key measures of prevention of empyema of post-operative esophagogastrostomy anastomotic fistula inside thorax are strictly distincting between indication and contraindication,carefully preparating before operation,commanding operative time and technical skill,strengthening nursing management,and fulfilling the oral manipulation and nursing prcedures about the drainage tube after operation.
3.Compound Skin Disinfectants:Clinical Application and Observation

OBJECTIVE To observe the clinical efficacy of two kinds of compound skin disinfectants(compound iodine and Anerdian).METHODS One hundred and twenty recruited patients were divided into three groups randomly,group A(compound iodine group),group B(Anerdian group) and control group(iodine tincture group),40 patients per group.The groups A and B were carried on with one-step skin greasing disinfection.The control group was carried on with routine two-steps skin greasing disinfection.The sampling and detection were carried on by The Technological Specification of Hospital Disinfection published by the Ministry of Health.RESULTS The kill ratio of three groups was(95.44?9.45)%,(95.29?7.21)% and(96.24?11.12)%,respectively.Their disinfection qualification ratio was 97.50%,95.00% and 97.50%,respectively.Comparing the kill ratio and disinfection qualification ratio,there was no significant difference among three groups.CONCLUSIONS The disinfection efficacy of two kinds of the compound skin disinfectants are definite,there is no difference comparing with the traditional iodine tincture and ethanol disinfectants,but they are easy to carry on,safe and practical and the time is shorter,saving the time and the labor.It is helpful to increase the work efficiency of the nurse and deserve to be extended in the clinical application.

OBJECTIVE To analyze the contamination situation in the oxygen aspiration facility and the measure for infection control.METHODS The first half of the year 2006 was determined as a traditional disinfection supervision group,the second half of the year 2006 was an improvement disinfection group,65 and 62 unused damping bottles in traditional disinfection and improvement disinfection groups were detected.Sixty three and 62 samples of continuously used damping liquid in the two disinfection groups were detected daily.The damping liquid detection of d1 and d6 in above two groups were compared each with other.The detection result was statistically analyzed.RESULTS The statistical analysis of the unused damping bottle qualification ratio between the two groups was with significant differences,P
5.Analysis of cause and the countermeasures for the conflicts between accompanies and nurses in pediatric transfusion clinic

Objective To analyze the incidence factors between accompanies and mwses in pediatric transfusion clinic, and find out the countermeasures. Methods 75 conflicts occurred between August 2004 to August 2008 were analyzed and made a contrast. Results The elements for the conflict were as follows: nurses, accompanies and patients. Conclusions The nurses should improve the emergency cop-ing abilities, communication skills, technical skills, supporting psychological nursing for the patients and the accompanies. All these are effective measures to avoid conflicts.
6.The effects of frequency in treating neuropathic pain using transcranial magnetic stimulation

Objective:To compare the effects of repetitive transcranial magnetic stimulation (TMS) at three different relatively high frequencies on neuropathic pain so as to find the best frequency.Methods:One hundred Sprague-Dawley rats were randomly divided into a sham operation group, a model group, a TMS group and a control stimulation group. The TMS group was further divided into a 5Hz group, a 10Hz group, and a 20Hz group. The rats in the model, control stimulation and TMS groups received constriction injury of the sciatic nerve, while the rats in the sham group were given a sham version of the operation. On the third day after the operation the rats in the TMS group and the control stimulation group began to receive TMS treatment. Neuropatic pain was evaluated on the day before the operation, and on the 3rd, 5th, 7th, 10th and 12th days after the operation. The evaluations included the paw withdrawal thermal latency (PWTL) and the paw withdrawal mechanical threshold (PWMT).Results:The average PWTLs and PWMTs in all of the TMS groups increased with the TMS treatment. Those of the 10Hz and 20Hz groups were significantly higher than the 5Hz group′s average, while there were no significant differences between the 10Hz group and 20Hz group.Conclusions:High-frequency TMS at different frequencies has different effects on neuropathic pain, at least in rats. The treatment efficacy at 10 and 20Hz is superior to that at 5Hz.
7.Comparison of glycosylated hemoglobin levels detected by 3 kinds of analytic instruments

Objective To compare levels of glycosylated hemoglobin detected by 3 kinds of analytic instruments .Methods 75 samples were measured by D-10 glycosylated hemoglobin automatic analyzer(ionexchange chromatography) ,HA-8160 glycosylated hemoglobin automatic analyzer(affinity chromatography) and 7170A automatic analyzer(immune turbidimetry) .Results were tested by the homogeneity of variance ,the one-way analysis of variance and correlation analysis .Results There was a positive correlation between D-10 glycosylated hemoglobin automatic analyzer and HA-8160 glycosylated hemoglobin automatic analyzer(r2 =0 .996) , the linear equation was Y=0 .953X+0 .519 .There was a positive correlation between D-10 glycosylated hemoglobin automatic ana-lyzer and 7170A automatic analyzer(r2 =0 .996) ,the linear equation was Y=0 .925X+0 .576 .There was a positive correlation be-tween HA-8160 glycosylated hemoglobin automatic analyzer and 7170A automatic analyzer(r2 =0 .998) ,the linear equation was Y=0 .969 X+0 .081 .Conclusion In the premise of quality control in laboratory ,three different instrument can use to detect the level of glycosylated hemoglobin .
8.Study on Medication Regularity of Compound Herbal Formulae for Treating Advanced ;Non-small Lung Cancer Based on Data Mining

Objective To study the medication regularity of compound herbal formulae in treating patients with advanced non-small lung cancer (NSCLC). Methods Compound herbal formulae published on journals for the treatment of NSCLC were searched and collected from CBM, CNKI, and Wanfang Data for database establishment. Frequency analysis, cluster analysis, and association rule analysis were used for data mining, and regularity of main Chinese medicinal herbs and herbal pairs (combinations) were investigated. Results There were 107 compound herbal formulae (involving 193 Chinese medicinal herbs and 1298 frequency) for NSCLC. 42 Chinese medicinal herbs were used for more than 9 times. Cluster analysis on the tastes and properties of the 42 Chinese medicinal herbs found that, among the four qi, cold showed the highest frequency (409);among the five tastes, sweet showed the highest frequency (627);among the meridians, lung meridian showed the highest frequency (546). Cluster analysis divided main Chinese medicinal herbs into 9 types, including yin-nourishing herbs, heat-clearing herbs, qi-tonifying herbs, blood-enriching herbs, blood circulation activating herbs, hemostasis herbs, phlegm-reducing herbs, qi-descending herbs, lung-astringing herbs. Associate rule analysis obtained 26 rules for Chinese medicinal herbal pairs, 18 rules for Chinese medicinal herbal combination, most of which were combinations for tonifying qi, nourishing yin, and clearing heat. Conclusion Chinese compound herbal formulae for the treatment of NSCLC mainly use Chinese medicinal herbs with properties of cold and mildness, with the tastes of sweet and bitter, with meridians of lung, spleen, and stomach, with the treatment of tonifying qi, nourishing yin, and detoxication.
9.Use of Antiallergic Drugs in Inpatient Pharmacy of Our Hospital During 2004~2006

OBJECTIVE:To evaluate the status quo of the use of antiallergic drugs.METHODS:The antiallergic drugs used during 2004~ 2006 in the inpatient pharmacy including the non-surgery department,surgery department and pediatric department of our hospital were analyzed statistically in respect of kinds,consumption sum and DDDs.RESULTS:The antiallergic drugs had a fluctuation in consumption sum and DDDs,showing a reduction in 2006.No significant change was noted in the structure of drug use.CONCLUSION:More and more new antiallergic drugs went on the market,but the application still gives priority to the cheaper kinds.

OBJECTIVETo study the change of WT1 gene expression during human leukemic K562 cell differentiation and apoptosis induced by bufalin.
METHODSCell viability was determined by trypan blue exclusion, cell differentiation and apoptosis by nitro blue tetrazolium (NBT) reduction test, morphology and flow cytometry, expression of WT1 protein by Western blot analysis, and expression of WT1 mRNA by reverse transcriptase-polymerase chain reaction (RT-PCR).
RESULTS(1) The cell proliferation was inhibited by bufalin and the IC(50) at 24, 48, 72 h were 0.026, 0.032 and 0.006 micro mol/L, respectively. (2) Bufalin induced K562 cell differentiation towards macrophage/monocyte within concentration from 0.01 to 0.05 micro mol/L and apoptosis at higher than 0.026 micro mol/L. (3) The expression of WT1 protein and mRNA were downregulated by bufalin in the initial stage of differentiation and apoptosis induced by bufalin.
CONCLUSIONK562 cell differentiation and apoptosis induced by bufalin might relate to the downregulation of WT1 expression.
