1.Clinical significance of the defermination of serum leptin in patients with post -hepatitis liver cirrhosis

Objective To determine serum leptin levels and investigate its relatio nship with liver function, insulin resistance and nutritional parameters in pati ents with post-hepatitis liver cirrhosis. Methods Thirty-two ma le patients with post-hepatitis cirrhosis and fourteen male control subjects were enrolled in thi s study. Fasting blood samples were obtained, and serum leptin levels, fasting g lucose and insulin levels were determined in all subjects, and nutritional param e ters were determined in cirrhosis patients. The cirrhosis patients were divided into three groups according to Child-Pugh classification. Results Serum leptin l evels in male hepatic cirrhosis were significantly higher than those of healthy control. There was no significant difference in serum leptin levels among the th ree groups of patients with cirrhosis according to Child-Pugh classification. Se rum insulin levels in male cirrhosis were significantly higher than that of heal thy control. The insulin sensitivity index in cirrhotic patients was decreased c ompared with normal control group. 2.Prognosis of 64 cases of myelodysplastic syndrome patients with abnormal karyotypes

ObjectiveTo investigate the correlation between the chromosomal abnormalities and prognosis of the myelodysplastic syndrome(MDS)patients, and analyze the effects of treatment. Methods Karyotype analysis of 122 patients according to the international human cytogenetics(ISCN) criteria.Treatment of RA and RAS were mainly dependent on agents to induce differentiation of hematopoietic cells and drugs based.RAEB,RAEB-t,CMML treatment were dependent on low-dose chemotherapy and low-dose combination chemotherapy regimens.The treatments of 64 MDS patients with abnormal karyotype were analyzed and compared with control group, and 58 normal karyotype MDS patients were hospitalized in the same period.ResultsAfter treatments,17 cases gained complete remission among 64 patients with abnormal karyotype MDS patients.The CR rate was 26.6 %.While in control group,30 gained CR in 58 MDS patients with normal karyotype. The CR rate was 51.7 %. Comparing with the CR patients of normal karyotype, the number of patients with abnormal karyotype of CR was significantly lower (x 2 =8.1 3,P < 0.05).Conclusion Karyotype analysis shows important significance in the diagnosis and prognosis of MDS.Karyotype transformation demonstrates differently in the risk of leukemia progress.
3.Nosocomial infection characteristics and control strategies of initial treatment acute leukemia patients

Objective To investigate the characteristics of co-infection in initial treatment acute leukemia induction chemotherapy.Methods The clinical features of 179 untreated acute leukemia patients with nosocomial infection were analyzed after combined chemotherapy.Results In the 179 patients,151cases achieved complete remission,the complete remission rate was 84.4 %,82 cases suffered from nosocomial infections,the incidence of infection was 45.8 %.The sites of infection were oral,anal,lung,as well as primary foci was not clear bacteremia.In 428 specimens,the isolated bacterial colony counted a total of 66,the number of fungal colonies was 9,the bacterial colony was G-bacteria-based.G-bacteria had different degrees of resistance to many antibiotics.Extended-spectrum β-lactamases strains had not been detected in these specimens.Conclusion Acute leukemia patients is easy to co-infection after chemotherapy.Control and prevention of nosocomial infections should run throughout the entire treatment process,application of laminar flow bed helps reduce the newly diagnosed acute leukemia patients with nosocomial infection incidence.
4.Analysis of the risk factors of central nervous system complications after allogeneic hematopoictic stem cell transplantation

Objective To analyse the etiology,clinical characteristics and risk factors of central nervous system (CNS) complications after allogeneic hematopoietic stem cell transplantation (allo-HSCT).Methods The clinical features of CNS complications in patients who underwent allo-HSCT were observed,and analysis its causes and risk factors.Results 8 cases of CNS complications occured in 69 patients within 6 months after allo-HSCT and the incidence was 11.6 %,the occurrence rate of CNS complications was 21.4 % (6/28) in HLA mismatched group,higher than HLA matehed group [49 % (2/41)] (P < 0.05).Analogously,the incidence was 44.4 % (4/60) in patients with graft-versus-host disease (GVIID) (>grade 2),which was significantly higher than patients with 2 or below grade 2 GVHD [6.7 % (4/9)] (P < 0.01).But there was no significant difference in the incidence of CNS complications between ≤14 years old and >14 years old,with or without ATG,different stages of diseases,whether pretreatment with maryland respectively (P >0.05),either.Epilepsy and intracranial infection were the most common CNS complications in allo-HSCT,followed by intracranial hemorrhage.Conclusion HLA mismatched and above grade 2 GVHD are the risk factors of CNS complications in allo-HSCT.Epilepsy,intracranial infection and bleeding are common CNS complications in allo-HSCT.
5.A prospective comparison cohort study between baseline serum uric acid level and coronary artery disease in first-degree relatives and non-first-degree relatives of type 2 diabetes males

Objective To investigate the relationship between baseline serum uric acid and the severity of coronary artery disease ( CAD ) in the first-degree relatives or non-first-degree relatives of men with type 2 diabetes. Methods Three hundred and eighty-one men with negative coronary angiography for the first time were divided into diabetes and non-diabetes groups and followed-up for 5 years. The primary outcome was acute coronary syndrome suspected during subsequent 5 years, and the coronary angiography was conducted simultaneously. The severity of CAD was assessed by the coronary stenosis index ( CSI) and the number of coronary lesion vessels. Results In normal blood glucose group, serum uric acid was higher in the first-degree relatives of diabetics compared with non-first-degree relatives(P<0. 01), along with higher morbidity of CAD, CSI, and coronary lesion vessels (all P<0.01). Correlation analysis showed that CSI(r=0. 250, P=0. 041) and coronary lesion vessels(r=0. 252, P=0. 040) in non-diabetics group were associated with baseline levels of serum uric acid. Conclusion The elevation of serum uric acid was closely related to subsequent CAD, especially in first-degree relatives of male with type 2 diabetes, which could be used as an early indicator for CAD prediction.
6.An automatic 3D brain segmentation based on improved level-set method in micro-CT rat/mouse images.

OBJECTIVEIn vivo Micro-PETICT imaging of mouse/rat brain has been widely used to non-invasively monitor brain and provides researchers a better understanding of therapeutic effects in models of human neurological disease. For the need of further processing, extraction of brain tissue from head is required and vital.
METHODSAn automatic multistep combination methods was proposed based on an improved level set framework, which includes (1) Use Fuzzy-C-Means method together with threshold and morphology methods to get the initial level-set surface automatically. (2) Combine gradient vector flow to enhance the gradient contrast and enforce the surface move toward to the object's surface much faster, especially obtain a significantly improvement in the regions of forehead and the joint between brain and neck. (3) introduce an automatic stop condition based on average bandwidth energy maximization to overcome the leakage problem.
RESULTS3 Micro-CT images of rat and 3 of mouse have been tested using the proposed methods and the average accuracy has increased by 33% for rat and 6.7% for mouse. The average processing duration for rat and mouse are about 8 minutes and 4 minutes, respectively.
CONCLUSIONSThe proposed methods were proved that it can be effectively used for Micro-PET/CT imaging of mouse/rat brain segmentation and have a great improvement on accuracy and convenience.

7.Validity and reliability of the Chinese version of the Kutcher Adolescent Depression Scale

Objective:To assess the validity and reliability of the Chinese version of the eleven-item Kutcher Adolescent Depression Scale (KADS-11)in Chinese adolescents,calculate its optimal cut-off value and the sensi-tivity and specificity,and explore the possibility of providing a useful tool to assess the severity of adolescent de-pressive symptoms.Methods:Totally 3180 students aged 11 -17 years were selected from schools in 6 provinces and Shanghai.All of them were asked to complete the KADS-11 and Children Depression Inventory (CDI). Students whose CDI scores were above 19 (including 19)were diagnosed with the DSM-IV criteria of depressive disorder,73 students from Shanghai sample were assessed with KADS-11 and CDI to analyze the test-retest reliabil-ity 1 month later.Results:Exploratory factor analysis showed that KADS-11 had 2 factors,and confirmatory factor analysis tested proved the 2-factor model fit better than the one-factor model.The KADS-11 total scores were posi-tively correlated with CDI total scores (r =0.74,P <0.01 ),and the KADS-11 scores were higher in depressive group than those in non-depressive group.The mean area under the curve (AUC)of KADS-11was 0.94,the mean area under the curve of each item ranged from 0.7 to 0.9.The optimal cut-off point of KADS-11 was total score≥9,sensitivity and specificity were 89% and 90% respectively.The Cronbach's alpha coefficient of the KADS-11 was 0.84,the spilt-half reliability coefficient was 0.71 (P <0.01),and the test-retest coefficient was 0.77 (P <0.01).Conclusion:The KADS-11 is appropriate for Chinese adolescents because of its good validity,reliability and diagnosis accuracy,it could be used to assess depressive symptoms for adolescents.
8.The study of NSE,ProGRP,Ki-67,TTF-1 in the diagnosis of small cell lung cancer

Objective To evaluate the application value of serum tumor markers in small cell lung Cancer, and analyse the relationship between serum tumor markers and immunohistochemistry indicators.Methods The electrochemical luminescence technology was used to detect the concentration of carcino-embryonic anti-gen(CEA),neuronspecific enolase(NSE),cyto-keratin fragment 19(CYFRA21-1),the immunological lumines-cence technology was used to detect the concentration of ProGRP,the expression of Ki-67,TTF-1 were detec-ted by MaxVision immunohistochemistry methods,the date was analyzed by two independent sample rank sum test,rank correlation methods,chi-square test and ROC curve.Results The concentration of CEA,NSE,pro-gastrin-releasing peptide(ProGRP),CYFRA21-1 in SCLC were higher than that in benign disease,the concen-tration of CEA,NSE,ProGRP,CYFRA21-1 in extensive stage were higher than that in limited stage(P<0. 05),the difference was statistically significant.The ROC area of NSE,ProGRP were large,they were 0.972 and 0.965,the Kappa value of the NSE+ProGRP was 0.860,optimstic data consistency with "the gold stand-ard".There was a negative relationship between Ki-67 and CEA,NSE,ProGRP,the positive rate of TTF-1 was higher than CYFRA21-1,while lower than ProGRP,But NSE,CEA and TTF-1 have same results(P>0.05). Conclusion Combined detection NSE and ProGRP is of great value in the diagnosis and periods of SCLC,the sensibility and specificity of NSE+ProGRP was high in SCLC,he high expression of Ki-67 was not responsed the concentration of serum tumor markers,TTF-1 had the same results with NSE,while inferior to ProGRP in detect of SCLC.
9.Identification of immune hemolytic transfusion reaction and exploration of transfusion compatibility testing

【Objective】 To analyze the causes of immune hemolytic transfusion reaction in one case, identify related antibodies, and explore transfusion compatibility testing. 【Methods】 ABO/Rh blood group identification, unexpected antibody identification of serum and diffusion fluid, direct antiglobulin test(DAT) and cross matching were conducted by saline method and/or microcolumn gel method. 【Results】 The patient′s blood group was O, and Rh phenotype was identified as DCCee. The DAT was negative, with strong anti-E antibody and weak anti-c antibody detected. Acute hemolytic transfusion reaction occurred in the patient after the last transfusion. 【Conclusion】 Currently, immune hemolytic transfusion reaction in China are mainly caused by Rh blood group system antibodies. The absence of unexpected antibody screening before blood transfusion and the weak anti-c antibody which resulted in missed detection of non compatibility in cross matching led to acute hemolytic transfusion reaction. It is recommended to conduct unexpected antibody screening before blood transfusion, and to collect blood sample for testing as soon as possible to improve the accuracy of DAT when acute hemolytic transfusion reaction is suspected.
10.Development of a real time PCR assay for rapid detection of Vibrio parahaemolyticus from seafood.

A real time PCR assay for the detection of Vibrio parahaemolyticus in seafood samples was developed using a novel specific target and a competitive internal amplification control (IAC). The specificity of this assay was evaluated using 390 bacterial strains including V. parahaemolyticus, and other strains belonging to Vibrio and non-Vibrio species. The real time PCR assay unambiguously distinguished V. parahaemolyticus with a detection sensitivity of 4.8 fg per PCR with purified genomic DNA or 1 CFU per reaction by counting V. parahaemolyticus colonies. The assays of avoiding interference demonstrated that, even in the presence of 2.1 μg genomic DNA or 10(7) CFU background bacteria, V. parahaemolyticus could still be accurately detected. In addition, the IAC was used to indicate false-negative results, and lower than 94 copies of IAC per reaction had no influence on the detection limit. Ninety-six seafood samples were tested, of which 58 (60.4%) were positive, including 3 false negative results. Consequently, the real time PCR assay is effective for the rapid detection of V. parahaemotyticus contaminants in seafood.
