2.Investigation on the caring behaviors of family caregivers of patients of indwelling catheter

Objective To investigate the family caregivers of patients with indwelling catheter care behavior .By analyzed its in‐fluencing factors to help caregivers improve the quality of life for patients .Methods Convenience sampling method by visit 70 pri‐mary caregivers of family indwelling catheter for the patient′s .They were investigated with gathering general information and Gen‐eral Self‐Efficacy Scale(GSES) ,following with caregivers awareness questionnaire indwelling catheter .Indwelling catheter care be‐havior questionnaire survey analyze behavior and other items to take care of the relationship .Results The general self‐efficacy and caregiver indwelling catheter cognition ,which account for 73 .9% of the total variance of caring behaviors ,were the important fac‐tors affecting care behaviors .Conclusion There is a big shortage in indwelling catheter home care caregiver knowledge and care be‐haviors .According to above professionals can provide targeted guidance for caregivers .

4.The aspergillus laryngotracheobronchitis. A case report and literature review.

The patient presented with dry cough, lending, fever and progressive dyspnea for two weeks. The patient had a prior respiratory infection history and the symptoms were not obvious, Early X-ray showed lung infection. Under the fibrolaryngoscope, the lingual surfaces of the epiglottis, epiglottic vallecula, and bilateral vocal cords were covered by yellow pseudomembrane. The motion of vocal cords was normal with poor glottic closure, and no ulcer was noted. Endotracheal mucosa was swelling and congested with an uneven surface, and purulent discharge and pseudomembrane was formed. Pathological examination revealed Aspergillus. The disease was diagnosed as Aspergillus laryngotracheobronchitis.

5.The factors which have influenced the quality of life of the patients with cancer and the corresponding nursing measures

Objective Analyze the factors which have influenced the quality of life (QOL) of the patients with cancer, and then put up the corresponding nursing measures. Method Using the EROTCQOLQ- C30 and the social support scale to analyze 600 patients with cancer. Results The value which can indicate the QOL of patients with cancer was 83.00 ? 22.00. The factors which have influenced the QOL of patients with cancer were age, gender, occupation, marriage, cultural degree, social support, disease, and so on. Conclusion Using the positive methods and the social support to raise the QOL of cancer patients according to the concreted condition of them.
6.Efficacy of 32P and pinyangmycin injection in combination on maxillofacial hemangioma in infants and children

Objective To explore and evaluate the effect of 32P and pinyangmycin injected in combination for the treatment of maxillofacial hemangioma. Methods The patients were randomly divided into three groups. The first group of patients was treated with 32P; 100 patients were enrolled in this group, of which 37 were male and 63 female. 32P dosage between 0.37 and 0. 74 MBq per cm3 was prepared together with 0.5 ml of 2 %lidocaine, and appropriate normal saline, and then the mixture was injected into hemangioma every two weeks. The second group was given pinyangmycin, and the third group received both 32P and pinyangmycin. The second group included 30 male and 60 female, and the third group 64 male and 136 female. The dose and frequency were given as the same to the first group. Results 88.5 % of patients were cured using both 32P and pinyangmycin, and the cure rate was 77.0 % and 71.7 % with either 32P or pinyangmycin. Conclusion These results prove that 32P and pinyangmycin injected in combination for the treatment of maxillofacial hemangioma is more effective than either 32P or pinyangmycin alone, and furthermore, the method is easy, safe and has less complications.
7.Efficacy of 3LL/GM-CSF tumor vaccine combined with pacilitaxel chemotherapy in treatment of mice bearing transplanted Lewis lung carcinoma

Objective: To evaluate the efficacy of 3LL/GM-CSF tumor vaccine combined with pacilitaxel chemotherapy in treatment of mice bearing transplanted Lewis lung carcinoma. Methods: The tumor vaccine 3LL/GM-CSF was prepared by infecting Lewis lung carcinoma cell line 3LL with adenovirus encoding GM-GSF. Mice model of Lewis lung carcinoma was established by subcutaneous injection of 2?104 3LL cells into C57BL/6(H-2b)mice. The sensitivity of Lewis lung carcinoma cell line-3LL to the treatment of pacilitaxel was detected in vivo and in vitro. The mice tumor model was first treated with pacilitaxel chemotherapy and then with 3LL/GM-CSF, or first with 3LL/GM-CSF and then with pacilitaxel. Tumor growth and the long-term survival of mice were observed after treatment. The immune memory and cytotoxicity against target cells were studied in the mice. Results: Pacilitaxel at 100 nmol/L killed 32.10% 3LL cells after 24 hour in vitro; but pacilitaxel at 5-25 mg/kg only had a poor effect on growth of 3LL cells in vivo. The tumors rebated in 70% of mice treated with pacilitaxel chemotherapy and 3LL/GM-CSF vaccination successively, and the survival of these mice was obviously longer than that of pure pacilitaxel chemotherapy group (70.0 days vs 27.5 days). The killing rate of 3LL/GM-CSF after pacilitaxel chemotherapy was 41.35% on day 3. Meanwhile, the survival mice could resist the re-attack of 3LL cells (2?104). We also noticed that first treatment with 3LL/GM-CSF and then pacilitaxel chemotherapy had no effect on tumors. Conclusion: Application of tumor vaccine shortly after pacilitaxel chemotherapy can induce specific immune responses and prolong the survival of experimental mice, which provide a basis for future clinical practice.
8.Efficacy of intensity-modulated radiotherapy combined with chemotherapy for nasopharyngeal carcinoma in children and adolescents

Objective To evaluate the treatment outcomes and toxicities of intensity-modulated radiation therapy (IMRT) combined with chemotherapy for children and adolescents with nasopharyngeal carcinoma.Methods Forty-three nasopharyngeal carcinoma patients less than 19 years old were recruited between April 2010 and April 2016.All patients were treated with IMRT (total dose 61.2-76 Gy) combined with cisplatin based chemotherapy.The Kaplan-Meier test was used to calculate overall survival (OS) and progression-free survival (PFS).The patient's clinical characteristics,side effects and longterm effects of treatment were retrospectively analyzed.Results Among 43 patients,there were 29 (67.4%) male and 14 (32.6%) female,and the median age was 14 years old (range,6-18 years).According to AJCC 7thstaging system,2 patients were in stage Ⅱ,26 in stage Ⅲ,7 in stage ⅣA and 8 in stage Ⅳ B.All patients were confirmed as non-keratinizing carcinoma.The positive rates of EB virus VCAIgA was 53.8% (7/13),and Rta-IgG was 60.0% (6/10) before treatment.The median radiation dose was 70 Gy (range,61.2-76 Gy) to the primary tumor.Thirty-three (76.7%) patients received neoadjuvant chemotherapy,with 20 (46.5%) and 36 (83.7%) patients treated by concurrent and adjuvant chemotherapy,respectively.With a median follow-up of 24 months (range,3-76 months),the 5-year OS and PFS ratios were 75.3% and 64.7%,respectively.There were 5 patients (11.6%) occurred to bone metastasis within 2 years after treatment.Hypothyroidism was reported in 47.4% (9/19) patients after IMRT.Conclusions Nasopharyngeal carcinoma in childhood and adolescence is mostly locally advanced diseases with poor differentiation.IMRT combined with chemotherapy produce a well treatment outcome with good tolerance in children and adolescents patients.The most common treatment failure bone metastasis.Radiation-induced hypothyroidism is common.

Objective Rapid elevation of the IgG antibody against Candida Enolase ( Eno ) has been observed in patients with invasive candidosis in an early stage .The present study was to confirm the association of Candida colonization with humoral im-mune anamnestic response of the dominant antigen . Methods Twenty-four mice were randomized into group 1 treated by oral Candi-da colonization plus intraperitoneal infection ( immunocompetent , n=8) , 2 treated with immunosuppressant in addition to the treatment of group 1 ( immunocompromised , n=8) , 3 treated by oral Candida colonization only ( immunocompetent , n=4) and 4 treated by in-traperitoneal injection only( immunocompetent, n=4).The number of Eno-specific memory B-cells in the spleen and the levels of IgG , IgM and IgA antibodies were determined in the peripheral blood of the immunocompetent and immunocompromised invasive candidiasis mice . Results At 7 days after invasive infection , there were significantly more Eno-specific memory B-cells in the mice of groups1 ( 47.25 ± 13.81) and 2 (43.14±15.95) than in groups 3 (8.00±3.74) and 4(8.50±2.38) (P<0.01), with no statistically significant differences between either groups 1 and 2 or groups 3 and 4 (P >0.05).Eno-IgG antibodies were detected in the serum of the mice of the first two groups in the early stage of invasive infection and positively corre -lated with antigen-specific memory B-cells (r=0.737,P <00.1 ). Conclusion Rapid elevation of the Eno-IgG antibody level in the early stage of invasive infection after Candida colonization may be attributed to the rapid proliferation of humoral immune memory cells.
10.Value of serum procalcitonin determination for the etiological diagnosis and prognosis of systemic inflam-matory response syndrome in surgical ICU

Objective The systemic inflammatory response syndrome ( SIRS) can be caused by infection and non-infection factors, which have similar clinical features but differ in treatment and prognosis .Rapid synthesis of procalcitonin ( PCT) during infec-tion can be used as a biomarker for the early diagnosis of sepsis .The present study aims to assess the value of the serum PCT level in the etiological diagnosis and prognosis of SIRS in the surgical ICU . Methods We retrospectively analyzed the data on 166 cases of SIRS from the surgical ICU in Jinling Hospital between June 2014 and June 2015 .The data obtained were associated with the patients'demograph-ics, primary diseases, laboratory results, and clinical outcomes.We analyzed the serum PCT values , blood culture results , and clinical outcomes. Results Totally, 131 of the patients were diagnosed with sepsis, with a median value of serum PCT of 2.43 (0.81-10.51) ng/mL, of whom 109 were PCT-positive (≥0.47 ng/mL), with a positive rate of 83.2%.Among the 35 non-infection SIRS patients, the mean level of serum PCT was 0.23 (0.1 -0.39) ng/mL, with a positive rate of 17.14%(6/35).There were statistically significant differences between the two groups in both the ser-um PCT level and positive rate (P<0.05).The PCT-positive rate was significantly higher in the bacteria-infected than in the fungi-in-fected group (86.5%[83/96] vs 74.3%[26/35], P<0.05), with a median value of 4.28 (1.05-14.59) ng/mL and 0.89 (0.37-1.59) ng/mL, respectively, so was it in the survivors than in the non-survivors (94.4%[34/36] vs 78.9%[75/95], P<0.05), with a median value of 12.89 (4.76-47.73) ng/mL and 1.41 (0.54-4.00) ng/mL, respectively. Conclusion The se-rum PCT level might be used to distinguish between sepsis and non-infection SIRS, significantly higher in bacteria-infected and survival groups than in fungi-infected and non-survival groups .Serum PCT determination contributes to the etiological diagnosis and prognosis of SIRS.
