1.Clinical value of emergency gastroscopy in diagnosis and treatment for upper gastrointestinal hemorrhage

Emergency gastroscopy was performed for 146 patients with upper gastrointestinal hemorrhage admitted to Tieying Hospital, Beijing to identify causes of bleeding and establish diagnosis. One hundred and thirty-nine of them were cured with endoscopic treatment, but two failed so to change to surgical operation. It suggested that early gastroscopy is a convenient, easy and safe method in diagnosis and treatment of upper gastrointestinal hemorrhage.
2.Retrovirus-mediated HSV_1-tk gene therapy for androgen-independent prostate cancer

Objective To use HSV 1 tk (herpes simplex virus type Ⅰthymidine kinase)/GCV (ganciclovir) in androgen independent prostate cancer cells(C4 2,PC3) in vitro in order to provide useful basis for clinical use. Methods HSV 1 tk gene was ligated to a pN 2A retroviral vector. Recombinant DNA molecules being introduced into a packaging cell line PA317,the high titer virus producer cells (VPC) were screened.The integration and expression of HSV 1 tk gene in VPC was observed by PCR and RT PCR. VPC was co cultured with these cancer cells in the light of 1∶1,1∶2,1∶4,1∶8.Cell viability (cytotoxicity) was assessed by SRB (sulforhodamine B protein dye binding)after the first day,the third day,the fifth and the seventh day. Results The highest titer VPC producing HSV 1 tk gene was isolated. Retrovirus mediated HSV 1 tk gene therapy was effective and active against such prostate cancer cells.The best one was co culture of VPC and cancer cells at 1∶1 and the fifth day followed by GCV. Compared with C4 2, PC3 decreased remarkably.The activation of apoptosis and other ways failed to be found. Conclusions Retrovirus mediated HSV 1 tk gene therapy in Vitro directly killed the tumor cells by cytolytic activity.
3.Relation between antithrombin Ⅲ,protein C,protein S with cerebral infarction

Objective To study the relation between the levels of protein C(PC),protein S(PS),antithrombin Ⅲ(AT-Ⅲ)with cerebral infarction.Methods 126 patients with cerebral infarction were divided into the groups according to the age,onset time and infarction area and contemporaneous 30 individuals with healthy physical examination were selected as the control group.The levels of PC,PS and AT-Ⅲ were detected.The relation between the change of these indicator levels with the age,onset time and infarction area was analyzed Results The levels of AT-Ⅲ,PC and PS in the acute stage youth group were lower than those in the acute stage middle age and elderly group with statistical differences between them (P <0.05).The level of the AT-Ⅲ,PC and PS in the acute stage group were lower than those in the recovery stage group and the control group,difference had statistical significance(P <0.05).The levels of AT-Ⅲ and PS in the recovery stage group were lower than those in the control group with statistical difference (P <0.05),but the level of PC had no statistical difference between the recovery stage group and the control group.The further study found that the level of AT-Ⅲ,PC and PS in the small infarction group,middle infarction group and large infarction group showed the gradually decreasing trend,the differences had statistical significance (P <0.05).Conclusion The decrease of AT-Ⅲ, PC and PS levels is the importan factor of cerebral infarction occurrence and is closely correlated with cerebral infarction ocurrence especially in the patients less than 45 years old.Observing the change of PC,PS and AT-Ⅲ levels has the important significance for judging the cerebral infarction progression.
4.Determination of Deoxynivalenol in Four Traditional Chinese Herbal Medicines by LC-MS-MS

Objective:To develop a method for determining deoxynivalenol in four kinds of traditional Chinese herbal medicines. Methods:After being extracted by water, purified by an immunoaffinity column, deoxynivalenol was analyzed by LC-MS-MS. Results:The calibration curve was linear within the range of 2-50 ng·ml-1 for deoxynivalenol. The recovery was 68. 7%-88. 3%. Conclusion:The method is simple, sensitive and accurate in the determination of deoxynivalenol in Semen Ziziphi Spinosae, Semen Sojae Praepara-tum, Semen Coicis and Fructus Psoraleae.
5.Clinical study of laparoscopic hepatectomy for hepatolithiasis

Objective To explore the feaibility of laparoscopic hepatectomy(LH) for hepatolithiasis.Methods Eight patients with hepatolithiasis were treated by laparoscopic cholecystectomy(LC) and common bile duct exploration(LCBDE) and LH.Laparoscopic resection of left lateral lobe of liver was performed in 7 cases,and left hemihepatectomy in 1 case.Results Procedures were all successful with operation time of(285.00?37.42) minutes,and bleeding volume(306.25?29.73)mL.The postoperative hospital stay was(7.88?1.36) days.No complications occurred.No residual stone was found in any patient.Conclusions LH was safe and effective for hepatolithiasis,and gives better results when combined with choledoscopic stone removal.
6.Effect of intermittent pneumatic pressure pump on the formation of deep venous thrombosis of lower extremity in elderly patients undergoing thoracotomy

Objective To investigate the effect of intermittent pneumatic pressure pump on the formation of deep venous thrombosis of lower extremity in elderly patients undergoing thoracotomy. Methods Fifty patients undergoing thoracic surgery were managed with intermittent pressure inflation pump besides general preventive measures. The pre-and postoperative blood flow in the lower extremity and incidences of deep venous thrombosis were compared. Result The differences in blood flow in the lower extremity and incidences of deep venous thrombosis were not statistically significant (P>0.05). Conclusion Intermittent pneumatic pressure pump can promote pulsatile blood flow in the lower extremity so as to effectively prevent formation of deep venous thrombosis in elderly patients after thoracotomy.
7.Expression of the minichromosome maintenance 2 protein and the cell surface molecule CD24 and their prognostic significance in patients with non-small cell lung cancer

Objective To explore expression of the minichromosome maintenance 2 (MCM2)protein and the mucin-like cell surface adhesion molecule CD24 in non-small cell lung cancer (NSCLC) and their relationship with its prognosis. Methods Seventy-three patients of NSCLC diagnosed for the first time and received surgical treatment in Xuanwu Hospital, Beijing were selected for the study. Expression of the MCM2 and CD24 in pathological specimens of the patients was measured by immunohistochemistry and their relationship with its prognosis was analyzed retrospectively. Results High-level expression of the MCM2 and CD24 was seen in 42 and 54 of 73 NSCLC patients, accounting for 57. 5 percent and 74. 0 percent,respectively. Risk of death for the patients with high-level expression of the MCM2 or the CD4 was significantly higher as compared to those with low-level expression ( P < 0. 05 ). Risk of death for patients with both high-level expression of the MCM2 and CD24 was significantly higher than that in those with only high-level expression of the MCM2 or the CD24 (HR =2. 59, 95%CI 1.40 -4. 80, P=0. 002) and in those with both low-level expression of them ( HR = 15.32, 95 % CI = 2.07 - 113.41, P = 0. 008 ). But there was no significant difference in risk of death between patients with high-level expression of the MCM2 or CD24 and those with low-level expression of both of them ( HR = 5. 60, 95% CI 0. 79 - 44. 82, P = 0. 083 ), and cumulative survival rate of patients with both high-level expression of the MCM2 and CD24 was significantly lower than those with only high-level expression of the MCM2 or the CD24 ( P = 0. 001 ). Conclusions Both expression of the MCM2 and the CD24 are independent prognostic factors for NSCLC and combined detection of the two markers have higher prognostic value for it.
8.Effect of social support and coping style an attachment influencing on mental health

Objective To explore the direct effect of attachment and the indirect effect through coping style and social support on mental health in technical school students. Methods 372 technical school students were assessed by Experiences in Close Relationship' s Inventory ( ECR), Symptom Check Scale List-90 ( SCL90) ,Coping Style Scale For Secondary School Students( CSS)and Perceived Social Support Scale(PSSS). Results ( 1 ) Total score of SCL-90 had significant correlations with all index of attachment anxiety, parents attachment avoidance( r=0. 165 ~0.370, P<0.01 ) ,with coping style focus on problem ( r=0.291 ~0.552, P<0. 01 ) and perceived social support( r = -0. 245, P < 0.01 ). (2)Coping style focus on emotion had significant correlations with attachment anxiety( r=0.237 ~0.383, P<0.01 ) and coping style focus on problem had significant correlations with attachment avoidance( r= -0. 267 ~ -0. 403, P<0. 01 ). (3) Perceived social support had significant negative correlations with attachment avoidance ( r= - 0. 425 ~ - 0. 459, P < 0.01 ). (4) The coping style focus on emotion had 65.2% mediation effect between attachment anxiety and mental health and perceived social support was moderated by attachment avoidance. Conclusions ( 1 ) Attachment anxiety and attachment avoidance have direct effect on mental health. ( 2 ) The indirect effect of attachment anxiety on mental health is a lot mediating effect performed by the coping style focus on emotion. Attachment avoidance also indirectly influences on mental health by moderating perceived social support.
9.Risk Factors of Professional Exposure among Medical Staff:Analysis and Preventive Strategies

OBJECTIVE To investigate the risk factors of professional exposure among medical staff and explore the preventive strategies.METHODS The influence of factors including profession,exposure process,exposed materials,treatment process,and follow-up among 110 medical staff with professional exposure was analyzed.RESULTS The general incidence of professional exposure was 52.73%,35.45% and 6.36% for clinical doctors,nures,and workers,respectively.Among all exposure cases,we found the incidence was 62.73% for pricking wound,20% for various liquid contamination,4.55% for glass injury,2.73% for scalpel cutting,and 4.55% for other instrument related damage.The reasons leading to exposure were treatment and nursing care,surgery assistance,and instrument and materials manipulation,which accounted for 40%,31.82%,and 18.18% of total incidence,respectively.All of the exposure cases had been properly treated.None of the investigated cases had been found illed with blood derived transmitted disease caused by professional exposure.CONCLUSIONS It is critical to intensify the training of medical staff for the knowledge of professional exposure and to improve their personal protective awareness,so that the incidence of professional exposure could be reduced.
