1.Systemic family rehabilitation for children with behavioral problems

BACKGROUND: Family factor is the main reason for children's psychological and behavioral problems. Seen from the view of systemic family theory, in any circumstances, children's behavioral problems reflect various interpersonal systemic networks among society, school, family, relatives and friends and the relationship of complicated interactions among individuals.Family therapy is an important way to cure children's behavioral problems.OBJECTIVE: To observe whether systemic family rehabilitation is adequate for intervening in children's behavioral problems.DESIGN: Cluster sampling; Contrast observationSETTING: Department of Psychiatry, Kunming Medical College.PARTICIPANTS: A total of 276 students in grade four from Mingtong primary school of Kunming City were selected along with their parents from May 2004 to March 2005. A survey was conducted on students and their parents according to the principle of cluster sampling.METHODS: Self-rating family dynamics scale was used to investigate the characteristics of family dynamics of 276 students in grade four. Achenbach child behavior checklist for parents was adopted to investigate the situation of their children' s behaviors. 57 cases of children with behavioral problems were detected out, and took the families in which the whole family members voluntarily participated in the intervention as interventional group and the families in which any one of the family members did not want to participate in the intervention as abnormal control group. The students and their parents in interventional group were treated with 4-month systemic therapy. 4 months later, resent the above rating scales.MAIN OUTCOME MEASURES: Comparison of family dynamics and children's behaviors between interventional group and abnormal control group before and after intervention.RESULTS: After intervention, changes of family dynamics and children's behaviors in interventional group had taken place, which had statistical difference. The changes as follows: family atmosphere changed from "depressive and hostile" to "harmonious, pleasant, equal and exoteric"; the degree of individuation of family members changed from low level to high; disease concept changed from "regarding the patients as helpless victims" to "regarding them as helpful doers", which realize "softening of disease concept". The total scores of Achenbach child behavior checklist for parents decreased [The scores of questionnaire for fathers before and after intervention were (42.20±22.58) and (28.95±21.90) respectively, and scores of questionnaire for mothers before and after intervention were (50.95±14.60) and (36.40-±15.36) respectively, P < 0.05]. There was no significant difference between the changes of children's behaviors assessed by fathers and mothers in abnormal control group [The scores of questionnaire for fathers before and after intervention were (38.32±17.30) and (37.57±16.45) respectively, and scores of questionnaire for mothers before and after intervention were (45.54±16.02) and (40.14±17.95) respectively, P > 0.05].CONCLUSION: Using the idea and technique of systemic family therapy ,it is feasible and effective to intervene in children's behavioral problems by family therapy.
2.A New Method for Accurate Determination of Peroxidase Activity Based on Fluorescence Decrease of Guaiacol

At present the most common colorimetry for the activity determination of the peroxidase ( POD) is based on the detection of the absorbance of product at 470 nm in a reaction system of the H2 O2/POD/guaiacol ( GA) , but the shortcoming of the method is that the formed product is not stable and there is the serious adsorption phenomenon on the cell. To solve this problem, a new method was established for accurate determination of POD activity based on the fluorescent feature of GA. By using standard solutions of horseradish peroxidase as the test samples and under these optimum conditions such as 0. 5 mmol/L of GA, 0. 5 mmol/L of H2 O2 , pH 6, 0. 05 mol/L of phosphate buffer solution and the reaction temperature of 20℃, the sample volume was only consumed 20 microlitre at a time, the linear response range was 500-60000 U/L ( r=0 . 9993 ) , the detection limit was 385 U/L and relative standard deviation was ≤2 . 4% ( n=11 ) . The comparisons for the determination results of the POD activity in the white radish’s extraction solutions were conducted among our method (9714±132 U/L) and the colorimetric method (9926±352 U/L) as well as the recirculating-catalytic flow analysis ( 9608±456 U/L ) . The results showed that the mutual consistency is better.
3.The Dichotic Listening Research for Different Chinese Rhyming Words in Patients with Major Depressive Disorder by Focused Attention

Objective:To explore the brain function asymmetry characters of the patients with major depressive disorder (MDD).Methods:The study examined the ear advantage of 58 healthy subjects, 115 patients with MDD using two kinds of rhyming words by the method of focused attention. Results:1.In healthy group, no ear advantage was found in true words test, but appeared significant right ear (left-hemisphere) advantage in pseudo-words test. 2.In MDD group, there was also no ear advantage in true words test, but appeared enhanced right ear (left-hemisphere) advantage in pseudo-words test. There was a significant difference between the MDD and healthy subjects. Among patient group, 3. by true words test, no ear advantage was found in those with a comorbid anxiety disorder, but those without comorbid anxiety appeared right ear advantage. By pseudo-word test, both patients with or without comorbid anxiety appeared right ear (left-hemisphere) advantage, and those without had larger advantage. there was a significant difference between this two groups. 4.HAMD, HAMA and SCL-90 ratings had no relation to asymmetry scores of true words or that of pseudo-words.Conclusions:1. both healthy control and MDD patients have no ear advantage for process and cognition of true words in, but as for that of pseudo-words, both of them appear right ear (left-hemisphere) advantage, with the MDD patients have greater advantage. 2.The patients with or without a comorbid anxiety disorder were different in response, patients with comorbid anxiety appear more close to normal control. 4. Asymmetry scores don’t correlate significantly with the scores of HAMD, HAMA or SCL-90. 5.The pseudo-words is the better choice as stimuli for dichotic listening test.
4.Dynamic monitoring of fat emboli on transesophageal echocardiogram during total knee arthroplasty

Objective To detect fat emboli in cardiac chamber of the patients undergoing total knee arthroplasty(TKA) by transesophageal echocardiography(TEE), and to discuss the relevant clinical value.Methods Eleven female patients with 12 osteoarthritis(OA) knees were underwent TKA. According to whether using tourniquet or not,12 knees were divided into 2 groups at random:tourniquet-related group (6 knees) and control group(6 knees). Echo intensity and ultrasonic characteristics of fat emboli in cardiac chamber were studied dynamically by multiplane TEE in various periods of the whole operation. Results All of the patients were implanted knee prostheses successfully. In different periods of TKA, fat emboli appeared as isoechoic or hyperechoic particles, which were found in right atrium, but nothing could be found in left heart. The imaging of the particles was flowing with duration time of 10~600s, like dots, lines, "moving star" or "shower". While femur expanding(0~3mins),the particles in right atrium and ventricle became more and more, and the echo intensity became significantly higher than those in other periods. After releasing tourniquet(0~5mins),most particles with highest echo intensity were observed in right heart,showing like "snowstorm" in tourniquet-related group, and more emboli were still detected at the end of monitoring (10 min after releasing tourniquet) by TEE,compared to the control group. No patient suffered from fat embolism syndrome(FES) with clinical manifestation. Conclusions Fat emboli in cardiac chamber can be observed sensitively by real-time TEE dynamically during TKA. It should be encouraged and promoted to analyze ultrasonic characteristics to facilitate the early detection, early diagnosis and early clinical intervention for the subclinical type of FES.
5.Anxiety state and serum angiontensin Ⅱ levels in patients with vitiligo

Objective To evaluate the possible correlation between anxiety state and serum angiontensin Ⅱ levels in patients with vitiligo.Methods Sixty patients with progressive vitiligo and 30 normal human controls were included in this study.Self-rating anxiety scale (SAS) was used to evaluate the anxiety state,and enzyme linked immunosorbent assay was performed to determine the serum level of angiontensin Ⅱ,in these subjects.Results SAS score was significantly increased in patients compared with the controls (46.13 ±11.72 vs.36.73 ± 12.59,t =3.50,P < 0.01),and 48.3% of the patients were affected by anxiety.The SAS score and prevalence of anxiety were statistically higher in female than in male patients (t =2.47,x2 =4.58,both P < 0.05),in unmarried than in married patients (t =2.59,P < 0.01; x2 =6.17,P < 0.05),in patients with lesions on exposed areas than in those with lesions on unexposed areas (t =3.60,P < 0.01; x2 =5.84,P <0.05).In patients affected by anxiety,the level of serum angiontensin Ⅱ was significantly elevated compared with patients without anxiety (63.83 ± 10.92 vs.40.74 ± 8.70 ng/L,t =9.09,P < 0.01 ).Conclusions Patients with vitiligo are usually affected by anxiety.The serum level of angiontensin Ⅱ is closely correlated with anxiety state in patients with vitiligo as well as with the development of vitiligo,and may have an impact on the initiation and progression of vitiligo.
6.Determination methods for the content of methyl salicylate-2-O-beta-D-galactopyranoside (1-4)-beta-D-glucopyranoside.

The paper is to report the establishment of three methods for determination of methyl salicylate-2-O-beta-D-galactopyranoside (1-4)-beta-D-glucopyranoside (MSG) by HPLC, UV or potentiometric titration. The results determined by the three methods turned out to be of no significant difference (P>0.05). The method was chosen according to MSG difference test demands.
7.Bacteria Contens in Air of Tuberculosis Wads Before and After Use:A Comparative Study

OBJECTIVE To detect the condition of bacteria of the air in wards of our hospital in order to reduce the possibility of air spread. METHODS Ten tuberculosis wards were chosen at random for study.Detected the bacteria content of the air in wards used before and after respectively and analysed the results. RESULTS The average of backgroud bacteria was 164 CFU/m 3 before the wards were used.Two years later,the average of bacteria increased to 682 CFU/m 3.The ratio was 1∶3:2 on average.There were significant differences between them. CONCLUSIONS Humen are the main facters that make the air polluted in these wards.The bacteria content will decline by good environment cleaning sanitation,ventilation and strengthening steriling management.Infection will decline in the hospital.
8.A survey of the status quo of health resources for carrying out community nursing in shaoxing countryside

ObjectiveA survey of the status quo of community nursing health resources in Shao xing countryside was carry out in order to rationally allocate medical resource,optimize quality of medical employees and construct modern rural community nursing patterns.MethodsA survey for 19 health service organizations and 381 nurses in rural communities was carried out with questionnaires.Results There existed regional differences in medical resources.The medical resources per ten thousand people were limited,only 0.7 health education room,1.2 recovery rooms,0.4 women and children′s health center.There was 1 nurse for every 1877 rural residents.The average age of nurses was (30.84±6.88) years old.Nurses on professional health education,community arevention of diseases only accountel for 43.8%.Community nuses of different ages and with different degrees had different knowledge requirements.ConclusionsThe medical resources are limited and imbalanced.The employees are too young and they have only low level of education and are not professional enough.Rational allocation of resources,optimization of personnel structure and abundance of on-the-job training is urgently needed.
9.Feasibility and safety of laparoscopic radical gastrectomy:a Meta-analysis

Objective:To explore the feasibility and safety of laparoscopic radical gastrectomy for gastric cancer through analysising the laparoscopic radical gastrectomy and open radical gastrectomy.Methods:Retrieval the Cochrane Library,PubMed,EMBASE,CBM,VIP,CNKI,WANFANG DATA,the time is from January 2010 to August 2015.Bring into the comparative studies about laparosopic radical gastrectomy and laparotomye.Processing data using the revman 5.2 to reaserch the feasibility and safety of laparoscopic radical gastrectomy.Results:7 clinical comparative studies were included in this study.The observation group was the group of laparoscopic radical gastrectomy and the control group was the group of laparotomye.Results showed that the length of incision of the observation group was shorter[MD=-12.93,95% CI (-13.16,-12.70),P<0.00001];The observation group bleeding was lesser[MD=-129.98,95%C1(-220.54,-39.42),P=0.005];The number of lymph node of the two groups had no significant difference [MD=0.77,95% CI (-2.56,3.60),P=0.56];The operation time of the observation group was longer [MD=49.17,95% CI (25.32,73.02),P<0.0001];The hospitalization time of the observation group is shorter [MD=-2.27,95% CI (-3.77,0.77),P=0.003].Conclusion:Laparoscopic radical gastrectomy is feasible and it is superior to laparotomye within the thoroughness,minimally invasiving and safety.
10.Changes and clinical relevance of plasma interleukin-6 and high sensitive C-reactive protein in patients with coronary heart disease and angina

Objective To explore the roles of interleukin-6 (IL-6) and high sensitive C-reactive protein (hsCRP) in coronary heart disease (CHD). Methods One hundred and fifty patients diagnosed as angina pectoris by coronary angiography were enrolled and randomly divided into unstable angina pectoris group ( UAP group n =78) and stable angina group (SAP group n =72). At the same time,50 cases with normal coronary angiography were selected as control. The plasma level of IL-6 was measured by ELISA, and hsCRP was measured by immunonephelometry. Results The mean plasma levels of IL-6 and hsCRP were significantly higher in UAP group compared with SAP group ( IL-6 ( 36. 98 ± 9. 35 ) pg/L vs. ( 23.19 ± 7. 35 ) pg/L; hsCRP ( 2. 25 ± 0. 73 ) μg/Lvs 1.88 ± 0. 68 ) μg/L; P < 0. 01 respectively), both were significantly higher than those of control group ( IL-6 (16. 93 ± 6. 15 )pg/L and hsCRP (1.28 ± 0.40)μg/L,P <0.01 respectively). We also found significantly positive relationship between IL-6 and hsCRP plasma level ( r = 0. 875, P < 0. 05 ). Conclusions Inflammatory response might play an important role in CHD. IL-6 and hsCRP were of high value in prediction of angina pectoris and development of CHD.
