1.Progress of the gut microbiota to affect the development and function of the brain

Gut microbiota is essential to human health,playing a major role in the bidirectional communication between the gastrointestinal tract and the central nervous system through the immunological,hormonal,neural and metabolic pathways.The colonization of the gut microbiota is influenced by multiple and diverse factors,such as the perinatal factors,dietary habits,diseases and antibiotics.The infant is the critical period for modifying the gut microbiota,disruptions during this period of microbiota-host interaction have the potential to affect the development and function of the brain.To regulate the homeostasis of the gut microbiota has its profound influences on improving the symptoms of gastrointestinal and behavior disorders.
2.Comparison of two gastric tube insertion methods in patients undergoing computed tomographic virtual endoscopy

Objective To compare the different effect of two gastric tube insertion methods in pa-dents undergoing computed tomographic virtual endoscopy. Methods 56 patients undergoing computed tomographic virtual endoscopy were randomly divided into group A and B according to gastric tube insertion methods, group A inserted gastric tube to descending duodenum by gastroscope, group B inserted by Jag precursor via nose. One- time success rate and the incidence rate of side effect were compared between the two groups. X2 test was adopted. Results One- time success rate of group A and B was 33.3%, 82.9% respectively, with obvious statistic difference. The incidence rate of side effect of group A was higher than that of group B. Conclusions Inserting gastric tube to descending duodenum by Jag precursor via nose is convenient, time- saving, causing less pain and resulting in higher success rate.
3.Two cases of neck region Kimura's disease.

Kimura's disease is a rare, benign, slow-growing chronic inflammatory swelling with a predilection for the head and neck region and is almost always with peripheral blood eosinophilia and elevated serum IgE levels. It is endemic in Asian males and rare in Western people. Surgical excision of the lesion is the first line therapy. Drug and radiation therapy have to be considered for the refractory lesions.

4.Occupational Hazard Situation among Medical Staff:A Survey

OBJECTIVE To investigate the occupational hazard situation among medical staff and conduct effective measure and enhance the medical care.METHODS A questionnaire survey on occupational hazard had been completed among the hospitals in Fengtai District in Beijing in three years.RESULTS The rate of sharp damage of medical staff was 72.13%,from them the nurses were with highest rate(81.49%).The higher rate of sharp damage was in the operating theater(88.32%)and the surgical department(82.49%).CONCLUSIONS To assure the medical safety,we should pay more attention to the occupational hazard management.
5.A study of the relationship between iodine intake levels and hypothyroidism of pregnant women in Qingdao

Objective To investigate the urinary iodine level and the thyroid function of early pregnant women in Qingdao,to explore the relationship between the iodine nutritional status and hypothyroidism.Methods 326 early pregnant women (<12 gestational weeks)were collected.The urinary iodine concentration and serum thyroid stimulating hormone (TSH),free thyroxine (FT4 ),thyroid peroxidase antibody (TPOAb)were detected. Results The median of urine iodine of pregnant women was 176.57μg/L,and the ratios of iodine deficient,iodine adequate,iodine more than adequate and iodine excessive groups were 3 8.65%,41.10%,15.95% and 4.30%.The total ratio of innormal urine iodine was 58.9%(192 /326).The differences between the ratios of the iodine deficient group,iodine excessive group and iodine adequate group both had statistically significant(χ2 =4.308,P <0.05;χ2 =4.826,P <0.05 ).There was no effect of iodine intake levels on serum thyroid peroxidase antibody.Conclusion There is no iodine deficiency among early pregnant women in Qingdao,but still individuals expose iodine deficiency.Iodine deficiency and iodine excessive are the risk factors of hypothyroidism during early pregnancy women,so we commend to detect the urinary iodine level.
6.Primary study on examination model in core curriculum of clinical medicine

Objective To investigate the application value of real clinical case centered situation simulation examination model in clinical medicine. Methods Two classes were randomly selected from 2008 grade clinical medicine majors, respectively as test group(n=158) and control group(n=156). Final examination of test group includes usual performance, case-oriented situation simulation examination (COSSE) and final written examination. Traditional methods were used in control group. The same paper was used in final written examination of the two groups. Scores of written examination and question-naires were used to evaluate the COSSE model. Data were processed using SPSS 16.0 statistical software;t and chi-square tests were used to analyze the data;P<0.05 signifies that the difference is statistically sig-nificant. Results Scores of final examination in test group were higher than those in control group(P<0.001). Times of independent completion of history taking, physical examinations and medical record writing were more in test group than in control group(P=0.002,0.017,0.048). Conclusions Contents and methods of COSSE is conducive to improving student's professional comprehensive ability.
7.Comparison of abnormal electrocardiography of patients with Keshan disease in endemic area and non-endemic urban area in Ju County of Shandong Province

Objective To provide a scientific basis for prevention of Keshan disease by observing the changes of electrocardiogram in residents from Keshan disease area and urban residents in Ju County.Method In 2013,four countries (towns) of Sangyuan,Anzhuang,Qishan and Longshan were selected as the investigation sites across the Keshan disease areas in Ju County of Shandong Province,one village was selected in each country (town),no less than 200 people were chosen from each village.There were 36 residential communities in urban area in Ju County,one residential community was selected from 4 positions (east,west,south and north) in urban area,no less than 200 non-endemic urban residents were chosen from each residential community as control,the electrocardiogram examination was conducted.According to the Keshan Disease Diagnostic Standard (WS/T 210-2011),Keshan disease was diagnosed.Results Among the 964 inhabitants investigated in Keshan disease area,143 cases showed abnormal electrocardiogram and the detection rate was 14.83%;in the 926 cases of urban electrocardiogram,78 cases showed abnormal electrocardiogram,abnormal rate was 8.42%,the difference was statistically significant (x2 =18.80,P < 0.01).There were 93 cases who met the 8 Keshan disease diagnostic criteria of abnormal electrocardiogram changes in the Keshan disease areas,abnormal rate was 9.65%;while there were 37 cases in urban residents,abnormal rate was 4.00%,the difference was statistically significant (x2 =23.55,P < 0.01).Conclusion These residents who have lived in Keshan disease endemic areas have a higher rate of abnormal electrocardiogram in Ju County of Shandong Province,they are still threatened by the causative factors of Keshan disease,and the authority should strengthen the prevention measures of Keshan disease.
8.Investigation of vasopressin response to increasing osmotic pressure in the late-phase of septic shock patients

Objective To investigate the vasopressin (VP) response to increasing osmotic pressure in the late-phase of septic shock patients.Methods Thirty-seven septic shock patients hospitalized in intensive care unit (ICU) of the First Hospital of Hebei Medical Unive~ity from January 2012 to September 2013 were enrolled.All patients received 3% hypertonic saline solution infusion.Serum concentrations of VP and sodium were measured before and after hypertonic saline solution infusion.Patients with ratio of difference in VP and sodium before and after infusion of 3% hypertonic saline (△VP/△Na) ≤0.5 pg/mmol were defined as nonresponders,and who >0.5 pg/mmol defined as responders.The age,acute physiological and chronic health evaluation Ⅱ (APACHE Ⅱ) score,blood pressure,albumin level,vasoactive drug between the two groups were also analyzed.Results VP level in the nonresponsive group (n=20,54.05%) was markedly lowered before (ng/L:10.41 ± 1.70 vs.18.25 ± 5.90,t=5.29,P<0.01) and after (ng/L:11.36 ± 1.90 vs.24.33 ± 5.46,t=9.33,P<0.01) 3% hypertonic saline solution infusion,compared with that in the responsive group (n =17,45.95%).All patients in the two groups were given dopamine (DA) or norepinephrine (NE) for maintaining blood pressure,and the dose in the nonresponsive group were higher than those in the responsive group [DA (μg· kg-1· min-1):14.91 ± 3.78 vs.8.64 ± 1.69,t =-5.02,P< 0.01 ; NE (μg· kg-1· min-1):1.03 ± 0.48 vs.0.38 ± 0.12,t=-3.12,P<0.01].Three patients were given DA plus NE in the nonresponsive group while patients in the responsive group received only single drug therapy.The age,APACHE Ⅱ score,blood pressure,albumin level,sodium level before and after hypertonic saline solution infusion between the two groups were not statistically different.Conclusion VP secretion to osmotic challenge was impaired and decreased in the late-phase of septic shock,prompting dysfunction in VP synthesis.
9.The application of nursing intervention in the prevention of psychiatric disorder in patients after coronary artery bypass grafting

Objective To discuss the effeet of nursing intervention in the prevention of psychiatric disorder in patients after coronary artery bypass grafting (CABG). Methods Patients with CABG(202 cases) were divided into the control group (100 cases) and the intervention group (102 cases).The control group received routine treatment and nursing while the intervention group was given systemic nursing intervention.The mental status was evaluated by mini-mental status examination (MMSE). Results The incidence of psychiatric disorder in the intervention group was 4 cases (3.92%),which was statistically lower than those in the control group(16 cases,16.00%). Conclusions Effective nursing intervention combined with effective treatment could prevent and control the psychiatric disorder of patients after CABG.
10.Value of serum CRISPLD2 levels for the diagnosis and prognosis evaluation of sepsis patients

Objective To investigate the value of cysteine-rich secretory protein LCCL domain-containing 2 (CRISPLD2) in diagnosis and prognosis in patients with sepsis.Methods Clinical data of patients admitted to intensive care unit (ICU) of the First Hospital of Hebei Medical University from December 2014 to December 2016 were retrospectively analyzed. According to the severity of sepsis, the patients were divided into three groups: sepsis patients, severe sepsis patients and septic shock patients, and 100 healthy persons were enrolled as control group. Levels of serum CRISPLD2, procalcitonin (PCT) and C-reactive protein (CRP), acute physiology and chronic health evaluation Ⅱ (APACHEⅡ) score, sequential organ failure assessment (SOFA) score, and 28-day prognosis were recorded. Analysis of the correlation between CRISPLD2 and PCT, CRP, APACHEⅡscore, SOFA score was done. The receiver operating characteristic (ROC) curve was plotted for the CRISPLD2 value for the diagnosis and prognosis in patients with sepsis.Results A total of 115 patients with sepsis were enrolled in this study, including 52 sepsis, 48 severe sepsis, and 15 septic shock; 29 patients died after 28 days, 28-days mortality rate was 25.2%. There was no significant difference in CRISPLD2 between sepsis and healthy control group (mg/L: 204.1±74.5 vs. 211.3±12.0, P > 0.05); the level of CRISPLD2 in septic shock group was significantly lower than that in sepsis group and severe sepsis group (mg/L: 139.0±55.0 vs. 240.2±89.6, 233.0±8.9, bothP < 0.05). The level of PCT, CRP and APACHE Ⅱ score, SOFA score in sepsis patients were significantly higher than those in healthy control group, and increased with the severity of sepsis. There was no statistically significant difference in CRISPLD2 level between the dead and the survival of sepsis, and the levels of PCT and CRP in death group were significantly higher. The levels of CRISPLD2 were significantly negative correlated with the levels of PCT, CRP, APACHE Ⅱ score and SOFA score (r values were -0.089,-0.431, -0.115, -0.201, respectively, allP < 0.05). It was shown by ROC curve analysis that the area under ROC curve (AUC) and 95% confidence interval (95%CI) of CRISPLD2, PCT, CRP for diagnosis of sepsis were 0.907 (0.871-0.944), 0.922 (0.886-0.958), 0.916 (0.878-0.954) respectively, allP = 0.000; when the cut-off value of CRISPLD2 > 216.0 mg/L, the sensitivity was 96.7%, and the specificity was 92.6%, which power lied between PCT and CRP. The AUC of CRISPLD2 for prognosis was significantly lower than that of PCT [0.617 (0.507-0.727) vs. 0.786 (0.668-0.903),P <0.01]; when the cut-off value of CRISPLD2 was 103.5 mg/L, the sensitivity was 100%, and the specificity was 25.6%. Conclusion CRISPLD2 is a potential biomarker in sepsis, but cannot predict the prognosis of patients with sepsis.
